Abstract

Oral anticancer medications (OAMs) are increasingly utilized. We evaluated the representativeness and completeness of IQVIA, a large aggregator of pharmacy data, for breast cancer, colon cancer, chronic myeloid leukemia, and myeloma cases diagnosed in six Surveillance, Epidemiology, and End Results Program (SEER) registries between 2007 and 2011. Patient's SEER and SEER-Medicare data were linked and compared with IQVIA pharmacy data from 2006 to 2012 for specific OAMs. Overall, 67.6% of SEER cases had a pharmacy claim in IQVIA during the treatment assessment window. This varied by location, race and ethnicity, and insurance status. IQVIA consistently identified fewer cases who received an OAM of interest than SEER-Medicare. The difference was least pronounced for breast cancer agents and most pronounced for myeloma agents. The IQVIA pharmacy database included a large portion of persons in the SEER areas. Future studies should assess receipt of OAMs for other cancer sites and in different SEER registries.

Abstract

PurposeEnd-of-life (EOL) cancer care is costly, with challenges regarding intensity and place of care. We described EOL care and costs for patients with colorectal cancer (CRC) in the United States and the province of Ontario, Canada, to inform better care delivery.MethodsPatients diagnosed with CRC from 2007 to 2013, who died of any cancer from 2007 to 2013 at age ≥ 66 years, were selected from the US SEER cancer registries linked to Medicare claims (n = 16,565) and the Ontario Cancer Registry linked to administrative health data (n = 6,587). We estimated total and resource-specific costs (2015 US dollars) from public payer perspectives over the last 360 days of life by 30-day periods, by stage at diagnosis (0-II, III, IV).ResultsIn all months, especially 30 days before death, higher percentages of SEER-Medicare than Ontario patients received chemotherapy (15.7% v 8.0%), and imaging tests (39.4% v 31.1%). A higher percentage of Ontario patients were hospitalized (62.5% v 51.0%), but 43.2% of hospitalized SEER-Medicare patients had intensive care unit (ICU) admissions versus 17.9% of hospitalized Ontario patients. Cost differences between cohorts were greater for patients with stage IV disease. In the last 30 days, mean total costs for patients with stage IV disease were $15,881 (SEER-Medicare) and $12,034 (Ontario) versus $19,354 and $17,312 for stage 0-II. Hospitalization costs were higher for SEER-Medicare patients ($11,180 v $9,434), with lower daily hospital costs in Ontario ($1,067 v $2,004).ConclusionThese findings suggest opportunities for reducing chemotherapy and ICU use in the United States and hospitalizations in Ontario.

11. Standard-Setting Study of College Board Mathematics Level I and Mathematics Level II Achievement Tests for Use in Course Placement and Credit by Examination in Mathematics 305G at the University of Texas at Austin--Fall 1991.

Abstract

The Measurement and Evaluation Center of the University of Texas at Austin conducted a standard-setting study in 1991 to reassess the use of the College Board's Achievement Tests in mathematics, Level I and Level II for course placement and credit by examination in an elementary functions and coordinate geometry course. Students in 20 sections of the course were given either the Level I or the Level II test as part of the final examination. Test scores were obtained for 363 students who took the Level I math test and for 342 students who took the Level II math test. Test results were correlated with preliminary course grades obtained from the instructors. For each of the tests, score values indicated by six guidelines about decision scores in the professional literature were presented to the mathematics department to consider for selecting credit by examination scores and course placement scores. There were five possible decision scores for Level I and four for Level II. The prerequisite requirement for enrollment in the course was not changed, but the scores for receiving credit by examination were changed. Eight tables present study findings. (SLD)

Abstract

Advances in solar array and electric thruster technologies now offer the promise of new, very capable space transportation systems that will allow us to cost effectively explore the solar system. NASA has developed numerous solar electric propulsion spacecraft concepts with power levels ranging from tens to hundreds of kilowatts for robotic and piloted missions to asteroids and Mars. This paper describes nine electric and hybrid solar electric/chemical propulsion concepts developed over the last 5 years and discusses how they might be used for human exploration of the inner solar system.

Abstract

This randomized clinical trial compares the need for re-treatment of respiratory infection following hospitalization for community-acquired pneumonia among children receiving amoxicillin at high doses vs low doses and at 3-days vs 7-days duration. Question For children with community-acquired pneumonia discharged from an emergency department, observational unit, or inpatient ward (within 48 hours), is subsequent outpatient treatment with oral amoxicillin at a dose of 35 to 50 mg/kg per day noninferior to 70 to 90 mg/kg per day, and is a 3-day course noninferior to 7 days, with regard to the need for antibiotic re-treatment? Findings In this 2 x 2 factorial randomized clinical trial of 814 children requiring amoxicillin for community-acquired pneumonia at hospital discharge, antibiotic re-treatment within 28 days occurred in 12.6% vs 12.4% of those randomized to lower vs higher doses, and in 12.5% vs 12.5% of those randomized to 3-day vs 7-day amoxicillin duration. Both comparisons met the prespecified 8% noninferiority margin. Meaning Among children with community-acquired pneumonia discharged from an emergency department, observational unit, or inpatient ward, further outpatient treatment with oral amoxicillin at a dose of 35 to 50 mg/kg per day was noninferior to a dose of 70 to 90 mg/kg per day and 3 days was noninferior to 7 days with regard to the need for later antibiotic re-treatment. Importance The optimal dose and duration of oral amoxicillin for children with community-acquired pneumonia (CAP) are unclear. Objective To determine whether lower-dose amoxicillin is noninferior to higher dose and whether 3-day treatment is noninferior to 7 days. Design, Setting, and Participants Multicenter, randomized, 2 x 2 factorial noninferiority trial enrolling 824 children, aged 6 months and older, with clinically diagnosed CAP, treated with amoxicillin on discharge from emergency departments and inpatient wards of 28 hospitals in the UK and 1 in Ireland between February 2017 and April 2019, with last trial visit on May 21, 2019. Interventions Children were randomized 1:1 to receive oral amoxicillin at a lower dose (35-50 mg/kg/d; n = 410) or higher dose (70-90 mg/kg/d; n = 404), for a shorter duration (3 days; n = 413) or a longer duration (7 days; n = 401). Main Outcomes and Measures The primary outcome was clinically indicated antibiotic re-treatment for respiratory infection within 28 days after randomization. The noninferiority margin was 8%. Secondary outcomes included severity/duration of 9 parent-reported CAP symptoms, 3 antibiotic-related adverse events, and phenotypic resistance in colonizing Streptococcus pneumoniae isolates. Results Of 824 participants randomized into 1 of the 4 groups, 814 received at least 1 dose of trial medication (median [IQR] age, 2.5 years [1.6-2.7]; 421 [52%] males and 393 [48%] females), and the primary outcome was available for 789 (97%). For lower vs higher dose, the primary outcome occurred in 12.6% with lower dose vs 12.4% with higher dose (difference, 0.2% [1-sided 95% CI -infinity to 4.0%]), and in 12.5% with 3-day treatment vs 12.5% with 7-day treatment (difference, 0.1% [1-sided 95% CI -infinity to 3.9]). Both groups demonstrated noninferiority with no significant interaction between dose and duration (P = .63). Of the 14 prespecified secondary end points, the only significant differences were 3-day vs 7-day treatment for cough duration (median 12 days vs 10 days; hazard ratio [HR], 1.2 [95% CI, 1.0 to 1.4]; P = .04) and sleep disturbed by cough (median, 4 days vs 4 days; HR, 1.2 [95% CI, 1.0 to 1.4]; P = .03). Among the subgroup of children with severe CAP, the primary end point occurred in 17.3% of lower-dose recipients vs 13.5% of higher-dose recipients (difference, 3.8% [1-sided 95% CI, -infinity to10%]; P value for interaction = .18) and in 16.0% with 3-day treatment vs 14.8% with 7-day treatment (difference, 1.2% [1-sided 95% CI, -infinity to 7.4%]; P value for interaction = .73). Conclusions and Relevance Among children with CAP discharged from an emergency department or hospital ward (within 48 hours), lower-dose outpatient oral amoxicillin was noninferior to higher dose, and 3-day duration was noninferior to 7 days, with regard to need for antibiotic re-treatment. However, disease severity, treatment setting, prior antibiotics received, and acceptability of the noninferiority margin require consideration when interpreting the findings.

Abstract

Pancreatic cancer (PC) is highly fatal, and its incidence is increasing in the United States. Population‐based registry studies suggest associations between a few autoimmune conditions and PC risk, albeit based on a relatively small number of cases. We conducted a population‐based, nested case‐control study to examine the associations between autoimmune conditions and PC risk within the Surveillance, Epidemiology, and End Results Program (SEER)‐Medicare population. Incident primary malignant PC cases (n = 80 074) were adults ≥66 years and diagnosed between 1992 and 2015. Controls (n = 320 296) were alive at the time cases were diagnosed and frequency‐matched to cases (4:1 ratio) by age, sex, and year of diagnosis. We used multivariable‐adjusted, unconditional logistic regression to calculate odds ratios (ORs) and 95% confidence intervals (CIs) for 45 autoimmune conditions identified from Medicare claims. Eight autoimmune conditions including ankylosing spondylitis (OR = 1.45; 95% CI: 1.14‐1.84), Graves' disease (OR = 1.18; 95% CI: 1.03‐1.34), localized scleroderma (OR = 1.27; 95% CI: 1.06‐1.52), pernicious anemia (OR = 1.08; 95% CI: 1.02‐1.14), primary sclerosing cholangitis (OR = 1.37; 95% CI: 1.18‐1.59), pure red cell aplasia (OR = 1.31; 95% CI: 1.16‐1.47), type 1 diabetes (OR = 1.11; 95% CI: 1.07‐1.15), and ulcerative colitis (OR = 1.18; 95% CI: 1.07‐1.31) were associated with increased PC risk (false discovery rate‐adjusted P values <.10). In subtype analyses, these conditions were associated with pancreatic ductal adenocarcinoma, whereas only ulcerative colitis was associated with pancreatic neuroendocrine tumors. Abstract

Advances in solar array and electric thruster technologies now offer the promise of new, very capable space transportation systems that will allow us to cost effectively explore the solar system. NASA has developed numerous solar electric propulsion spacecraft concepts with power levels ranging from tens to hundreds of kilowatts for robotic and piloted missions to asteroids and Mars. This paper describes nine electric and hybrid solar electric/chemical propulsion concepts developed over the last 5 years and discusses how they might be used for human exploration of the inner solar system.

Abstract

BACKGROUND: The National Cancer Institute’s Surveillance, Epidemiology, and End Results (SEER)-Medicare–linked database was first created almost 30 years ago. Over time, additional data have been added to the SEER-Medicare database, allowing for expanded insights into the delivery of health care across the cancer continuum from screening to end of life. METHODS: This article includes an overview of the current SEER-Medicare database, presenting potential users with an introduction to how the data can facilitate innovative epidemiologic and health services research studies. With a focus on the population 65 years and older, this article presents descriptive data on beneficiary demographics, cancer characteristics, service settings, Medicare coverage (eg, Parts A, B, C, and D), and use (number of services or bills) from 2011 to 2015. RESULTS: From 2011 to 2015, 857 056 cancer patients and 601 470 population-based noncancer controls were added to the database. The database includes detailed tumor characteristics and clinical assessments for cancer cases, and demographics and health-care use (eg, hospitals, outpatient facilities, individual providers, hospice, home health-care providers, and pharmacies) for both cases and controls. Although characteristics varied overall between cases and controls, sufficient cancer-specific matched controls are available. Roughly 60% of cases were enrolled in fee for service at cancer diagnosis. The annual average number of claims per case was 60.7 and 92.3 during the year before and after cancer diagnosis, respectively, and 127.5 during the year before death. CONCLUSIONS: The large sample size and diverse array of data on cancer patients and noncancer controls in the SEER-Medicare database make it a unique resource for conducting cancer health services research.

46. Correlating friction velocity in turbulent boundary layers subjected to freestream turbulence

Abstract

A turbulent boundary layer was subjected to grid-generated freestream turbulence to study the effects of turbulent length scale and intensity on skin friction. The study emphasized the importance of the turbulent-to-viscous length scale ratio and focused on correlating skin friction without the use of conventional boundary-layer Reynolds numbers. The experimental data were characterized by small-scale freestream turbulence above boundary layers with momentum-thickness Reynolds numbers ranging from 225 to 2700. Freestream turbulence intensities varied from 0.1 to 8.0 %. The turbulent-to-viscous length scale ratios spanned from 100 to 1000, and ratios of freestream turbulent length scale to boundary-layer momentum thickness ranged from 4 to 32. A new boundary-layer model was used to establish a skin-friction correlation that does not require knowledge of the momentum-thickness Reynolds number. The correlation was evaluated using the new small-scale data and other data from the literature. When only freestream information (length scale, mean velocity, and turbulence intensity) was employed, the correlation provided an estimate of the friction velocity to within 8% (20:1 odds). The correlation was evaluated using data with momentum-thickness Reynolds numbers up to 6300, freestream intensities as large as 29%, turbulent-to-viscous length scales reaching 11,000, and length scale-to-boundary-layer momentum-thickness values as high as 190.

47. Heat transfer in turbulent boundary layers subjected to free-stream turbulence--Part II: analysis and correlation

Abstract

A new heat transfer correlation for turbulent boundary layers subjected to free-stream turbulence was developed. The new correlation estimates dimensionless heat transfer coefficients without the use of conventional boundary-layer thickness measures and the associated Reynolds numbers. Using only free-stream parameters (mean velocity, turbulence intensity and length scale), the new correlation collected many authors' elevated-turbulence, flat-plate Stanton number data to within [+ or -] 11%. The level of scatter around the new correlation compared well to previous correlations that require additional flow information as input parameters. For a common subset of data, scatter using earlier correlation methods ranged from 5-10%; scatter around the new correlation varied from 6-9% over the same data subset. A length-scale dependence was identified in a Stanton number previously defined using a near-wall streamwise velocity fluctuation, St'. A new near-wall Stanton number was introduced; this parameter was regarded as a constant in a two-region boundary layer model on which the new correlation is based. [DOI: 10.1115/1.1538623]

Abstract

Turbulent boundary layers were subjected to grid-generated free-stream turbulence to study the effects of length scale and intensity on heat transfer. Relative to conventional boundary layer thickness measures, test conditions included very small-scale free-stream turbulence. The boundary layers studied ranged from 400-2700 in momentum-thickness Reynolds number and from 450-1900 in enthalpy-thickness Reynolds number. Freestream turbulence intensities varied from 0.1-8.0%. Ratios of free-stream length scale to boundary-layer momentum thickness ranged from 4.4-32.5. The turbulent-to-viscous length-scale ratios presented are the smallest found in the heat-transfer literature; the ratios spanned from 115-1020. The turbulent-to-thermal ratios (using enthalpy thickness as the thermal scale) are also the smallest reported; the ratios ranged from 3.2-12.3. Relative to clean-free-stream expectations based on the momentum- and enthalpy-thickness Reynolds numbers, the skin friction coefficient increased by up to 16%, and the Stanton number increased by up to 46%.
