Abstract

Summary Rectal motility was assessed in three groups of geriatric patients (faecally incontinent, continent faecally impacted and control patients) to determine whether 'uninhibited' rectal contractions are a cause of faecal incontinence. The incidence of rectal contractions in response to rectal distension did not differ between the three study groups. Two-thirds of the incontinent patients were unable to retain a condom distended with water (soft-stool model) during a proctometrogram. Involuntary expulsion of this device was correlated with the presence of rectal contractions and low anal resting pressure. Involuntary expulsion of an air-filled balloon firm-stool model) from the rectum occurred less frequently and was correlated with low resting pressure but not with rectal contractions. The contribution of 'uninhibited' rectal contractions to faecal incontinence is insignificant except for a minor role in the expulsion of liquid stool.

8. Early serum creatinine accurately predicts acute kidney injury post cardiac surgery.

Abstract

Background: Acute Kidney Injury (AKI) is a well recognized complication of cardiac surgery. It is associated with significant morbidity and mortality. The aims of our study are twofold; 1. To define the incidence of AKI post cardiac surgery. 2. To identify pre-morbid and operative risk factors for developing AKI and to determine if immediate post operative serum creatinine (IPOsCr) accurately predicts the development of AKI. Method(s): We prospectively studied 196 consecutive patients undergoing elective (on-pump) cardiac surgery. Baseline patient characteristics, including medical co-morbidities, proteinuria, procedural data and kidney function (serum creatinine (sCr) were collected. Internationally standardised criteria for AKI were used (sCr >1.5 times baseline, elevation in sCr >26.4 mummol/L (0.3 mg/dl). Measurements were collected pre-operatively, within 2 h of surgical completion (IPOsCr) and daily for two days. Logistic regression was used to assess predictive factors for AKI including IPOsCr. Model discrimination was assessed using ROC AUC curves. Result(s): Forty (20.4%) patients developed AKI postoperatively. Hypertension (OR 2.64, p = 0.02), diabetes (OR 2.25, p = 0.04), proteinuria (OR 2.48, p = 0.02) and a lower baseline eGFR (OR 0.74, p = 0.002) were associated with AKI in univariate analysis. A multivariate logistic model with preoperative and surgical factors (age, gender, eGFR, proteinuria, hypertension, diabetes and type of cardiac surgery) demonstrated moderate discrimination for AKI (ROC AUC 0.76). The addition of IPOsCr improved model discrimination for AKI (AUC 0.82, p = 0.07 versus baseline AUC) and was independently associated with AKI (OR 7.17; 95% CI 1.27-40.32; p = 0.025). Conclusion(s): One in 5 patients developed AKI post cardiac surgery. These patients have significantly increased morbidity and mortality. IPOsCr is significantly associated with the development of AKI, providing a cheap readily available prognostic marker.Copyright © 20

Item does not contain fulltext PURPOSE: Infective endocarditis (IE) is characterized by aggregation of activated platelets, fibrin, and bacteria. DMP444, a high-affinity glycoprotein IIb/IIIa receptor antagonist, binds to the fibrinogen-binding domain of activated platelets, depicting a key feature of IE. Tc-99m DMP444 scintigraphy was studied in a group of patients with possible IE. METHODS: Tc-99m DMP444 (600 MBq; 16 mCi) planar and SPECT images of the heart were recorded in patients with possible IE for as long as 6 hours after injection. Results were compared to echocardiography and the Duke classification. RESULTS: Sixteen patients (age range, 37 to 78 years) participated. DMP444 imaging was positive on SPECT in five patients, and all had definite endocarditis (affecting both prosthetic and native valves). Eleven patients were DMP444 negative, seven with no proof of IE. The remaining four patients were classified as having IE, but three had been receiving adequate intravenous antibiotic regimens for > or = 2 weeks at the time of scintigraphy and one had Q-fever endocarditis. CONCLUSIONS: DMP444 SPECT allows in vivo visualization of IE if it is performed within 1 to 2 weeks after the start of antibiotic treatment. Given the high affinity of DMP444 for activated platelets, the results indicate the involvement of activated platelets in early IE.

16. Predictors of acute kidney injury following on pump cardiac surgery.

Abstract

Aim: (1) To define the incidence of acute kidney injury (AKI) post cardiac surgery. (2) To identify pre-morbid and operative risk factors for developing AKI. (3) To accurately predict patients at risk of AKI using routine biochemical tests. Background(s): AKI post cardiac surgery is a poor prognostic indicator, which independently predicts mortality. Newer published biomarkers are expensive and are not routinely available in clinical practice. Method(s): We prospectively studied 93 consecutive patients undergoing elective (on pump) cardiac surgery. Baseline patient characteristics, including presence of medical co-morbidities, proteinuria, procedural data and kidney function (serum creatinine (sCr) were collected. Internationally standardised criteria for AKI was used, (sCr > 1.5 times baseline, elevation in sCr > 26 ummol/L, or urine output <0.5 ml/kg/hr for >6 hours within 48 hours). Measurements were collected pre-operatively, within 2 hours of surgical completion and daily for two days. Logistic regression was used to assess predictive factors for AKI including immediate post operative (OPsCr). Model discrimination was assessed using ROC AUC curves. Result(s): 18 (19%) patients developed AKI in the post operative period. Inotrope use (OR 3.90, p < 0.026), preoperative eGFR (OR0.96, p = 0.013), but not proteinuria (OR 2.58, p = 0.11) were associated with AKI in univariate analysis. A multivariate logistic model with preoperative and surgical factors (age, gender, eGFR, proteinuria, cardiac bypass time & inotropes) demonstrated moderate discrimination for AKI (ROC AUC 0.76). Addition of OPsCr improved model discrimination for AKI (AUC 0.83) and was independently associated with AKI (OR 1.39/umol increase, 1.05-1.82, p = 0.018). Abstract

The relationship between genotypes at two red flower colour loci (R1/rl and R2/r2) and four basic flower colour forms (red, salmon, rose-pink and soft-pink) in zonal pelargoniums (Pelargonium x hortorum Bailey L.H.) was studied using complementation tests. The results confirmed that red flower colour is due to complementary gene interaction at the two loci hence R1-R2 plants with rose-pink and salmon colored flowers always complemented each other to produce red-flowered plants among theprogenies. The two flower colour forms were also the only ones that complemented each other to produce red-flowered progeny. The genotypes r1r1R2 and r1r1r2r2 always corresponded to salmon, rose-pick and soft-pink flower colours respectively. Thus genotypes at the two red flower colour loci (R1/r1 and R2/r2) were also directly responsible for salmon, rose-pink and soft-pink flower colours in zonal pelargoniums. Abstract

Objective: It is uncertain whether self-propulsion in a wheelchair should be encouraged or discouraged in the early stages of stroke rehabilitation. Design: A two-centre pilot study to assess the feasibility of performing a multicentre randomized controlled trial on this subject. Setting: Clatterbridge and Aintree Stroke Rehabilitation Units, Merseyside, UK. Subjects: Forty early stroke patients (mean age 67 years) in whom it was uncertain whether self-propulsion in a wheelchair should be encouraged were studied. Intervention: A central randomization service at Newcastle University was used to determine the policy about wheelchair provision and use for each patient. They were allocated to either an ‘encouraged to self-propel’ or a ‘discouraged from self-propulsion group’. Outcome measures used: Independent outcome assessment was performed by postal questionnaire and telephone interview using the Barthel ADL Scale, Nottingham Extended ADL Scales and the shortened General Health Questionnaire (GHQ-12) at 3 and 12 months. Patient’s length of stay and their Ashworth tone score were also measured either at three months or when they were discharged from hospital. Results: After considerable preparation time it was possible to conduct a trial on self-propulsion in early stroke rehabilitation in the two-pilot centres. No major differences were found between the pilot groups for any of the outcome measures. Conclusions: A multicentre randomized controlled trial to assess this question is feasible but further work is being conducted before proceeding, to satisfy the concerns expressed to our group regarding the appropriateness of the intervention and the outcome measures.
