Abstract

Simple Summary A novel active release system magnetic sphingomyelin-containing liposome encapsulated with indocyanine green, fluorescent marker, or the anticancer drug cisplatin was evaluated. The liposomal sphingomyelin is a target for the sphingomyelinase enzyme, which is released by stressed cells. Thus, sphingomyelin containing liposomes behave as a sensitizer for biological stress situations. In addition, the liposomes were engineered by adding paramagnetic beads to act as a receiver of outside given magnetic energy. The enzymatic activity towards liposomes and destruction caused by the applied magnetic field caused the release of the content from the liposomes. By using these novel liposomes, we could improve the drug release feature of liposomes. The improved targeting and drug-release were shown in vitro and the orthotopic tongue cancer model in mice optical imaging. The increased delivery of cisplatin prolonged the survival of the targeted delivery group versus free cisplatin. Most available cancer chemotherapies are based on systemically administered small organic molecules, and only a tiny fraction of the drug reaches the disease site. The approach causes significant side effects and limits the outcome of the therapy. Targeted drug delivery provides an alternative to improve the situation. However, due to the poor release characteristics of the delivery systems, limitations remain. This report presents a new approach to address the challenges using two fundamentally different mechanisms to trigger the release from the liposomal carrier. We use an endogenous disease marker, an enzyme, combined with an externally applied magnetic field, to open the delivery system at the correct time only in the disease site. This site-activated release system is a novel two-switch nanomachine that can be regulated by a cell stress-induced enzyme at the cellular level and be remotely controlled using an applied magnetic field. We tested the concept using sphingomyelin-containing liposomes encapsulated with indocyanine green, fluorescent marker, or the anticancer drug cisplatin. We engineered the liposomes by adding paramagnetic beads to act as a receiver of outside magnetic energy. The developed multifunctional liposomes were characterized in vitro in leakage studies and cell internalization studies. The release system was further studied in vivo in imaging and therapy trials using a squamous cell carcinoma tumor in the mouse as a disease model. In vitro studies showed an increased release of loaded material when stress-related enzyme and magnetic field was applied to the carrier liposomes. The theranostic liposomes were found in tumors, and the improved therapeutic effect was shown in the survival studies.

Abstract

Information on the pathophysiology of glucocorticoid-induced osteoporosis (GIO) is limited, since its clinical picture often reflects a combined effect of glucocorticoids (GC) and the treated systemic disease (i.e., inflammation and immobility). In 50 healthy adult (30-mo-old) primiparous Gottingen mini-pigs, we studied the short-term (8 mo, n = 30) and long-term (15 mo, n = 10) effect of GC on bone and mineral metabolism longitudinally and cross-sectionally compared with a control group (n = 10). All animals on GC treatment received prednisolone orally at a dose of 1.0 mg x kg body [wt.sup.-1] x [day.sup.-1] for 8 wk and thereafter at 0.5 mg/kg body [wt.sup.-1] x [day.sup.-1]. In the short term, GC reduced bone mineral density (BMD) at the lumbar spine by -47.5 [+ or -] 5.1 mg/[cm.sup.3] from baseline (P < 0.001), which was greater (P < 0.05) than the loss [not significant (NS)] in the control group of -11.8 [+ or -] 12.6 mg/[cm.sup.3]. Calcium absorption decreased from baseline by -2,488 [+ or -] 688 mg/7 days (P < 0.001) compared with -1,380 [+ or -] 1,297 mg/7 days (NS) in the control group. Plasma bone alkaline phosphatase (BAP) decreased from baseline by -17.8 [+ or -] 2.2 U/l (P < 0.000), which was significantly different (P < 0.05) from the value of the control group of -1.43 [+ or -] 4.8 U/l. In the long term, the loss of BMD became more pronounced and bone mineral content (BMC), trabecular thickness, mechanical stability, calcium absorption, 25-hydroxyvitamin [D.sub.3], 1,25-dihydroxyvitamin [D.sub.3], and parathyroid hormone tended to be lower compared with the control group. There was a negative association between the cumulative dose of GC and BMD, which was associated with impaired osteoblastogenesis. In conclusion, the main outcomes after GC treatment are comparable to symptoms of GC-induced osteoporosis in human subjects. Abstract

Methoden des quantitativen Ultraschalls (QUS) sind parametrische Verfahren zur Bestimmung von Knocheneigenschaften mittels Analyse von Schallwellen nach Durchquerung des Knochens oder eines Teils davon. Dabei existieren verschiedene Verfahren, je nach Art des Knochens und der Durchschallungsrichtung. Die Quertransmission durch den Kalkaneus hat sich als bestes Verfahren zur Prädiktion osteoporotischer Frakturen erwiesen, mit einer mit der DXA vergleichbaren Prädiktionskraft. Solange es aber noch keine Studien über die medikamentöse Frakturreduzierung in einer Patientengruppe gibt, die ihre Therapieempfehlung anhand von QUS-Kriterien erhalten hat, kann keine Therapie anhand einer QUS-Messung ohne DXA-Messung empfohlen werden. QUS am Kalkaneus korreliert stark mit der Knochendichte am selben Messort, aber schwach mit der Dichte an anderen Messorten wie dem Femur oder der Wirbelsäule, weshalb auch keine Diagnose der Osteoporose mit QUS möglich ist. Neuere Entwicklungen zielen auf die Messung des proximalen Femurs, insbesondere des Schenkelhalses, und auf die Bestimmung von Knochenfestigkeitsparametern, die mit einer Knochendichtemessung nicht erfassbar sind.

Abstract

Trabecular bone score (TBS) is a gray-level textural index of bone microarchitecture derived from lumbar spine dual-energy X-ray absorptiometry (DXA) images. TBS is a bone mineral density (BMD)-independent predictor of fracture risk. The objective of this meta-analysis was to determine whether TBS predicted fracture risk independently of FRAX probability and to examine their combined performance by adjusting the FRAX probability for TBS. We utilized individual-level data from 17,809 men and women in 14 prospective population-based cohorts. Baseline evaluation included TBS and the FRAX risk variables, and outcomes during follow-up (mean 6.7 years) comprised major osteoporotic fractures. The association between TBS, FRAX probabilities, and the risk of fracture was examined using an extension of the Poisson regression model in each cohort and for each sex and expressed as the gradient of risk (GR; hazard ratio per 1 SD change in risk variable in direction of increased risk). FRAX probabilities were adjusted for TBS using an adjustment factor derived from an independent cohort (the Manitoba Bone Density Cohort). Overall, the GR of TBS for major osteoporotic fracture was 1.44 (95% confidence interval [CI] 1.35-1.53) when adjusted for age and time since baseline and was similar in men and women (p > 0.10). When additionally adjusted for FRAX 10-year probability of major osteoporotic fracture, TBS remained a significant, independent predictor for fracture (GR = 1.32, 95% CI 1.24-1.41). The adjustment of FRAX probability for TBS resulted in a small increase in the GR (1.76, 95% CI 1.65-1.87 versus 1.70, 95% CI 1.60-1.81). A smaller change in GR for hip fracture was observed (FRAX hip fracture probability GR 2.25 vs. 2.22). TBS is a significant predictor of fracture risk independently of FRAX. The findings support the use of TBS as a potential adjustment for FRAX probability, though the impact of the adjustment remains to be determined in the context of clinical assessment guidelines. Abstract

OBJECTIVE—Periarticular osteopenia is an early radiological sign of rheumatoid arthritis (RA). Quantitative ultrasound (QUS) devices have recently been shown to be useful for assessing osteoporosis. In this study the capability of a transportable and easy to use QUS device to detect skeletal impairment of the finger phalanges in patients with RA was investigated. METHODS—In a cross sectional study 83 women (30 controls, 29 with glucocorticosteroid (GC) treated RA, and 24 with GC treated vasculitis) were examined. QUS measurements were obtained at the metaphyses of the proximal phalanges II-V and directly at the proximal interphalangeal joints II-IV with a DBM Sonic 1200 (IGEA, Italy) QUS device. Amplitude dependent speed of sound (AD-SoS) was evaluated. In 23 of the patients with RA, hand radiographs were evaluated. RESULTS—Significant differences between patients with RA and the other groups were found for AD-SoS at both measurement sites. Compared with age matched controls, the AD-SoS of patients with RA was lowered by two and three standard deviations at the metaphysis and joint, respectively. Fingers of patients with RA without erosions (Larsen score 0-I) already had significantly decreased QUS values, which deteriorated further with the development of erosions (Larsen II-V). CONCLUSION—This study indicates that QUS is sensitive to phalangeal periarticular bone loss in RA. QUS is a quick, simple, and inexpensive method free of ionising radiation that appears to be suited to detection of early stages of periarticular bone loss. Its clinical use in the assessment of early RA should be further evaluated in prospective studies.

Abstract

Quantitativer Ultraschall (QUS) am Skelett ist eine relativ neue Methode zur Bestimmung des menschlichen Skelettzustandes. Ultraschall ist eng mit den mechanischen Eigenschaften des durchschallten Mediums verknüpft, frei von ionisierender Strahlung und kostengünstig zu implementieren. Allerdings ist die Ausbreitung von Ultraschallwellen im anisotropen Knochen komplex und nicht einfach beschreibbar, weshalb sich Ultraschall im Knochen noch weitgehend der Vorstellung entzieht. Um das Innere dieser "Black Box" etwas zu erhellen, wird die Schallausbreitung im Knochen mit Hilfe einer neuen Simulationssoftware sichtbar dargestellt. Die unterschiedlichen Meßprinzipien kommerzieller Geräte werden erläutert und die Meßparameter beschrieben.

Abstract

Quantitative parametrische Bildgebungsverfahren eröffnen qualitativ neue Perspektiven für die radiologische Bildgebung. Quantitative 2D-, 3D- und 4D- Darstellungsmöglichkeiten ergänzen sich mit parametrischen Darstellungen biologischer Gewebeeigenschaften und -funktionen und damit von Organeigenschaften. Dies ermöglicht die Interpretation radiologischer Daten aus einer biochemischen, biomechanischen oder physiologischen Perspektive. Durch die Quantifizierung werden Veränderungen nachweisbar, die visuell noch nicht erkennbar sind, was Anwendungsmöglichkeiten in der Früherkennung von Erkrankungen und bei einer empfindlicheren Therapiekontrolle eröffnet. In dieser Übersichtsarbeit werden die Möglichkeiten, welche die quantitative parametrische Bildgebung bietet, dargestellt und an beispielhaften Anwendungen erläutert. Ein besonders attraktives Anwendungsfeld, der Einsatz der Verfahren bei pharmakologischen Zulassungsstudien, wird dargestellt. Bewertungskriterien zur Qualität quantitativer Bildgebungsverfahren werden im Zusammenhang mit deren Grenzen und Möglichkeiten aufgeführt. Wenngleich quantitative parametrische Bildgebungsverfahren die etablierte qualitative Bildinterpretation nicht ersetzen, sondern ergänzen, so eröffnen sie doch damit potenziell verbesserte Möglichkeiten in der Diagnostik und Prognostik sowie insbesondere in der Verlaufskontrolle von Erkrankungen und ihrer Therapien.
