6. Ethnic differences in the use of peritoneal dialysis as initial treatment for end-stage renal disease

Abstract

Cultural differences between ethnic groups could influence treatment decisions even when barriers to health care are removed. Researchers analyzed data reported to the Medicare End-Stage Renal Disease (ESRD) program from North and South Carolina and Georgia between Jan 1989 and Dec 1991 to determine differences in the use of peritoneal dialysis (PD) by patients with chronic kidney failure. During that time, 10,726 white or African-American patients began receiving treatment at an ESRD center. A total of 1,337 white patients began PD, which involves placing a permanent catheter in the abdomen. Only 996 African-American patients did so. African-American patients were only half as likely to receive PD as whites. Patients with little education and no job were less likely to receive PD. Renters and those who lived alone were also less likely to receive PD., Objective.--To evaluate the influence of ethnicity on the use of peritoneal dialysis (PD) as initial treatment for end-stage renal disease (ESRD) after controlling for other patient characteristics. Design.--Inception cohort analysis of incident ESRD patients. Patients.--All African-American and white patients (N=10726) who began treatment for ESRD at dialysis center's in North Carolina, South Carolina, and Georgia and reported to ESRD Network 6 between January 1, 1989, and December 31, 1991. Main Outcome Measure.--Odds ratios (ORs) of the association between ethnicity and PD as initial treatment modality. Results.--African-American patients were 56% less likely than whites to use PD (OR, 0.44; 95% confidence interval [CI], 0.40 to 0.49). This difference persisted (OR, 0.45; 95% CI, 0.38 to 0.52) after multivariable adjustment for age, education, social support, home ownership, functional status, albumin level, hypertension, history of myocardial infarction, peripheral neuropathy, and comorbid diabetes. Conclusions.--Ethnic differences in initial PD use cannot be explained by many demographic, socioeconomic, and comorbid factors associated with the use of PD as initial treatment for ESRD. Abstract

Little research has been conducted on behavioral characteristics of children with autism spectrum disorder (ASD) from diverse cultures within the US, or from countries outside of the US or Europe, with little reliable information yet reported from developing countries. We describe the process used to engage diverse communities in ASD research in two community-based research projects-an epidemiologic investigation of 7- to 12-year olds in South Korea and the Early Autism Project, an ASD detection program for 18- to 36-month-old Zulu-speaking children in South Africa. Despite the differences in wealth between these communities, ASD is underdiagnosed in both settings, and generally not reported in clinical or educational records. Moreover, in both countries, there is low availability of services. In both cases, local knowledge helped researchers to address both ethnographic as well as practical problems. Researchers identified the ways in which these communities generate and negotiate the cultural meanings of developmental disorders. Researchers incorporated that knowledge, as they engaged communities in a research protocol, adapted and translated screening and diagnostic tools, and developed methods for screening, evaluating, and diagnosing children with ASD.
