Background: The COVID-19 pandemic relies on real-time polymerase chain reaction (qRT-PCR) for the detection of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), to facilitate roll-out of patient care and infection control measures. There are several qRT-PCR assays with little evidence on their comparability. We report alterations to the developers’ recommendations to sustain the testing capability in a resource-limited setting. Methods: We used a SARS-CoV-2 positive control RNA sample to generate several 10-fold dilution series that were used for optimization and comparison of the performance of the four qRT-PCR assays: i) Charité Berlin primer-probe set, ii) European Virus Archive – GLOBAL (EVAg) primer-probe set, iii) DAAN premixed commercial kit and iv) Beijing Genomics Institute (BGI) premixed commercial kit. We adjusted the manufacturer- and protocol-recommended reaction component volumes for these assays and assessed the impact on cycle threshold (Ct) values. Results: The Berlin and EVAg E gene and RdRp assays reported mean Ct values within range of each other across the different titrations and with less than 5% difference. The DAAN premixed kit produced comparable Ct values across the titrations, while the BGI kit improved in performance following a reduction of the reaction components. Conclusion: We achieved a 2.6-fold and 4-fold increase in the number of tests per kit for the commercial kits and the primer-probe sets, respectively. All the assays had optimal performance when the primers and probes were used at 0.375X, except for the Berlin N gene assay. The DAAN kit was a reliable assay for primary screening of SARS-CoV-2 whereas the BGI kit’s performance was dependent on the volumes and concentrations of both the reaction buffer and enzyme mix. Our recommendation for SARS-CoV-2 diagnostic testing in resource-limited settings is to optimize the assays available to establish the lowest volume and suitable concentration of reagents required to produce valid results.

This in-vitro study aimed to analyse the effect of brushing and different brushing parameters (kind of toothpaste, kind of toothbrush, brushing force) on erosive tooth wear of primary bovine enamel and dentin. Specimens were prepared from primary bovine enamel or dentin (each group n = 12) and cyclically eroded (6 × 60 s/d, citric acid, pH 2.4) and brushed with children's toothbrushes (2 × 15 s/d) over 5 days. The brushing parameters under investigation were: toothpaste (fluoridated, fluoride-free), toothbrush (manual; rotating-oscillating and sonic, each at two different activation modes) and brushing force (1 N, 2 N). Specimens that were only eroded and not brushed served as controls. Enamel and dentin wear was quantified using widefield confocal microscopy. Statistical analysis was performed using three- and one-way ANOVAs followed by Scheffe's (enamel) or Tamhane's (dentin) post-hoc tests (p < 0.05). Brushing with the fluoridated toothpaste was able to significantly reduce erosive wear in enamel (by 15 to 37%, 6 of 10 groups) and in dentin (by 58 to 72%, all groups), while brushing with the fluoride-free toothpaste was not different from the controls. Considering the kind of toothpaste and brushing force, slight differences between the toothbrushes were observed in enamel, but not in dentin. Within the same toothbrush and activation mode, almost no differences between 1 and 2 N brushing force were detected. This in vitro study aimed to analyse the effect of various children's toothpastes on erosive tooth wear of primary bovine enamel and dentin. Enamel and dentin specimens (n = 12) were cyclically eroded (6 × 60 s/d, citric acid, pH 2.4) and brushed (2 × 15 s/d, 2 N) over 5 days. Nine fluoride toothpastes (500 to 1450 ppm) and eight toothpastes containing no fluoride or other active ingredients (hydroxyapatite and/or xylitol) were tested. Unbrushed specimens served as control. Tissue loss was quantified using widefield confocal microscopy and statistically analysed using two-way and one-way ANOVAs followed by Scheffe's (enamel) or Tamhane's (dentin) post-hoc tests (p < 0.05). Only one fluoride toothpaste (1400 ppm) was able to reduce erosive wear of enamel significantly by 15% compared to the control (p adj.  = 0.002). All fluoridated toothpastes reduced dentin surface loss significantly by 32 to 69% compared to the control (p adj.  ≤ 0.001), while fluoride-free toothpastes were unable to reduce dentin loss significantly (p adj.  ≥ 0.971). Most fluoridated toothpastes caused less erosive wear compared to fluoride-free toothpastes. In this paper, we present the design and performance of the upgraded University of Florida torsion pendulum facility for testing inertial sensor technology related to space-based gravitational wave observatories and geodesy missions. In particular, much work has been conducted on inertial sensor technology related to the Laser Interferometer Space Antenna (LISA) space gravitational wave observatory mission. A significant upgrade to the facility was the incorporation of a newly designed and fabricated LISA-like gravitational reference sensor (GRS) based on the LISA Pathfinder GRS. Its LISA-like geometry has allowed us to make noise measurements that are more representative of those in LISA and has allowed for the characterization of the mechanisms of noise induced on a LISA GRS and their underlying physics. Noise performance results and experiments exploring the effect of temperature gradients across the sensor will also be discussed. The LISA-like sensor also includes unique UV light injection geometries for UV LED based charge management. Pulsed and DC charge management experiments have been conducted using the University of Florida charge management group's technology readiness level 4 charge management device. These experiments have allowed for the testing of charge management system hardware and techniques as well as characterizations of the dynamics of GRS test mass charging. Background: Face masking is associated with self-perceived dry mouth and halitosis. Aim of the study was to measure the effect of different face masks on salivary parameters and halitosis., Methods: The randomized controlled crossover clinical trial with four periods included 40 oral healthy participants using different face masks (cloth mask, surgical mask, filtering facepiece 2 [FFP2] mask) or no mask (control) for 4 h in random order. Unstimulated salivary flow rate (primary outcome) and stimulated salivary flow rate, salivary pH and buffer capacity of stimulated and unstimulated saliva (secondary outcomes, blinded), and volatile sulfur compounds (secondary outcome) were measured before and after the 4-h periods. Statistical analysis was performed by repeated measures ANOVA (p < 0.05)., Results: Of 40 randomized participants, 39 completed the study. Unstimulated salivary flow rate prior to face masking amounted to 0.6 ± 0.3 ml/min. Face masking had no significant effect on unstimulated salivary flow (p = 0.550). Face masking had also no significant effect on the other salivary parameters (p ≥ 0.518). The concentration of volatile sulfur compounds (VSC) prior to face masking amounted to 157.3 ± 59.7 ppb. After face masking, the concentration of VSC increased slightly, but not significantly (p = 0.055): 168.1 ± 76.3 ppb (control), 199.3 ± 132.7 ppb (cloth masks), 188.5 ± 101.1 ppb (surgical masks), and 189.7 ± 90.1 ppb (FFP2 masks)., Conclusion: Four hours of face masking did not change the salivary flow rate, pH, and buffer capacity, and had no significant effect on VSC's levels. There is increasing interest in deep UV Light-Emitting Diodes (LEDs) for applications in water purification, virus inactivation, sterilization, bioagent detection, and UV curing, as well as charge management control in the Laser Interferometer Space Antenna (LISA), which will be the first gravitational wave detector in space. To fully understand the current state of commercial UV LEDs and assess their performance for use on LISA, large numbers of UV LEDs need to be tested across a range of temperatures while operating in air or in a vacuum. We describe a new hardware system designed to accommodate a high volume of UV LED performance tests and present the performance testing results from over 200 UV LEDs with wavelengths in the 250 nm range.

This paper describes a novel, to the best of our knowledge, approach to build ultrastable interferometers using commercial mirror mounts anchored in an ultralow expansion (ULE) base. These components will play a critical role in any light particle search (ALPS) and will also be included in ground testing equipment for the upcoming laser interferometer space antenna (LISA) mission. Contrary to the standard ultrastable designs where mirrors are bonded to the spacers, ruling out any later modifications and alignments, our design remains flexible and allows the alignment of optical components at all stages to be optimized and changed. Here we present the dimensional stability and angular stability of two commercial mirror mounts characterized in a cavity setup. The long-term length change in the cavity did not exceed 30 nm and the relative angular stability was within 2 µrad, which meet the requirements for ALPS. We were also able to demonstrate 1 p m / H z length noise stability, which is a critical requirement for various subsystems in LISA. These results have led us to design similar opto-mechanical structures, which will be used in ground verification to test the LISA telescope.

This study aimed to analyse if the erosion-protective potential of the salivary pellicle is different between female and male subjects. Bovine enamel and dentin specimens (each n = 3) were exposed to the oral cavity of healthy female or male volunteers (each n = 25, females: 25.8 ± 3.5 years, males: 26.7 ± 4.0 years) for 120 min to form a salivary pellicle. Subsequently, each 2 enamel and 2 dentin specimens were eroded with hydrochloric acid (pH 2.6, 60 s). Specimens of the control group (each n = 30) were eroded without presenting a salivary pellicle. Calcium release into the acid was determined photometrically. Additionally, total protein content in the pellicle (each n = 1 enamel and dentin specimen/volunteer) and different salivary parameters (flow rate, pH, buffer capacity, protein, albumin, calcium, phosphate, fluoride) were assessed. Statistical analyses were performed by one-way ANOVA, t tests, multiple linear regressions and Pearson correlations (p < 0.05). The erosion-protective capacity was not significantly different among female (calcium release [% of control]: enamel: 82.6 ± 28.1, dentin: 80.7 ± 24.0) and male (enamel: 76.0 ± 27.5, dentin: 87.1 ± 34.9) subjects. The protein content of the pellicle was not different between female and male subjects. The protein content and pH of unstimulated saliva were significantly reduced in female compared to male volunteers. Calcium release was neither correlated with the protein content of the salivary pellicle nor with salivary parameters. We have investigated whether patient reported outcomes provided by patients with Birmingham Hip Resurfacing (BHR) changed after negative media coverage of metal-on-metal (MOM) hip replacement. We also investigated whether patients whose procedures were performed by a designer surgeon behaved differently to those performed elsewhere. 1178 consecutive BHR procedures performed between January 2002 and December 2006, by one of the designer surgeons in his private practice, were reviewed. We also reviewed 402 BHRs undertaken by two non-designer surgeons in both their NHS and private practice. 150 of the latter cohort were undertaken at an NHS hospital and 252 at an independent private hospital. All patients had annual Oxford Hip Scores (OHS) collected. We chose 2007 as pre-"media attention" and compared scores from this year against subsequent years. We found no clinically significant change in OHS between 2007 and subsequent years, at all centres. We conclude that negative media reporting does not appear to have had an impact on patients' perceived outcome after BHR. In consequence, patients who have undergone this type of hip resurfacing and show deterioration should be investigated.

Precision phase readout of optical beat note signals is one of the core techniques required for intersatellite laser interferometry. Future space based gravitational wave detectors like eLISA require such a readout over a wide range of MHz frequencies, due to orbit induced Doppler shifts, with a precision in the order of μrad/√Hz at frequencies between 0.1 mHz and 1 Hz. In this paper, we present phase readout systems, so-called phasemeters, that are able to achieve such precisions and we discuss various means that have been employed to reduce noise in the analogue circuit domain and during digitisation. We also discuss the influence of some non-linear noise sources in the analogue domain of such phasemeters. And finally, we present the performance that was achieved during testing of the elegant breadboard model of the LISA phasemeter, which was developed in the scope of a European Space Agency technology development activity.

Purpose: This study aimed at describing the anatomy of the zona orbicularis (ZO), based on magnetic resonance arthrography (MRA) and magnetic resonance imaging (MRI) scans and to assess the presence of synovial folds in relation to the ZO., Methods: A retrospective review was performed using consecutive hip and pelvic MRA and MRI examinations from our institution. We identified 25 normal scans of each variety. Patients were scanned in a neutral hip position and 3D FIESTA sequence images were included in a number of cases. Using electronic callipers, measurements were obtained of the ZO thickness and of the location of the ZO with respect of the femoral head and neck., Results: On MRA, the ZO appeared as a horseshoe in 18/25 patients, being absent anteriorly. On MRI the ZO was less consistent and absent in 12/25 posteriorly, in 8/25 inferiorly and in 2/25 anteriorly. Where present, the ZO usually coincided with the boundary of femoral head sphericity and the narrowest point of the isthmus of the femoral neck. The medial synovial fold was identified in all MRA studies (25/25)., Conclusions: The ZO of the hip is most consistently identified when the joint is distended and in approximately 75% of cases appears as a horseshoe-shaped structure. Superiorly, the ZO is aligned perpendicular to the long axis of the femoral neck. The ZO twists from postero-lateral to antero-medial as it moves inferiorly. Our findings are consistent with the hypothesis that the ZO functions as a ring that resists femoral head distraction and contributes to dynamic circulation of synovial fluid.

Patient reported outcome measures (PROMs) are used to gauge clinical performance. The PROMs outcome programme at our centre achieves a preoperative data capture rate of 99%. This falls to 90.6%, 89%, 83% and 79% at the six-week, six-month, one-year and two-year time points, respectively. The study aims were to determine factors associated with patients who did not respond to outcome questionnaires following total hip replacement (THR), and the potential implications this may have when assessing patients following THRs. During the first year of the PROMs programme, 1,322 patients underwent unilateral primary THR at our institution. Of these, 1,311 completed preoperative questionnaires. Thirty-eight patients (2.9%) died within two years of surgery and have been excluded. For the remaining 1,273 patients, we identified those who did not return postoperative questionnaires at each of our review time points. Younger age, lower baseline EQ5D and Oxford Hip scores (OHS) were significantly associated with non-response (p<0.001). Patients with lower satisfaction scores, OHS and EQ5D scores, were less likely to respond to subsequent questionnaires. A significant association between non-response and deprivation (p<0.001) was demonstrated. Our findings suggest that the more satisfied patients are over-represented and our reported outcome results are better than they would have been if all patients had responded. This phenomenon may apply to studies where those categorised as "lost to follow-up" represent a subset of patients who have disengaged due to poor outcome or satisfaction.

This study aimed to assess if patient-matched cutting blocks reduce operating time, blood loss and length of stay on top of improving implant alignment and offer operational or economic benefits, as claimed by manufacturers. We retrospectively reviewed patients undergoing TKA using patient matched technology and compared them with patients undergoing TKA using standard instrumentation; all were operated on between September 2010 and June 2012. All procedures were performed by a single surgeon at a single centre using the same implants. We collected data on operating time, length of stay and blood loss and also measured component alignment. Thirty-nine patients underwent TKA using patient-matched technology during the study period. Data was compared with that from 50 patients undergoing TKA using standard instrumentation during the same period. We found no significant difference in operating time, length of stay or blood loss between the two groups. There was also no difference in femoral or tibial component alignment although we did observe that the femorotibial angle of TKAs using patient-matched technology was 0.9 degrees more valgus (183.5 degrees versus 182.6 degrees, p = 0.035). In this study, patient-matched technology did not appear to give any clinical advantages over standard techniques although, equally, it did not appear to show any disadvantages. Further studies are needed to evaluate whether operational or economic benefits may be achieved by adoption of patient matched instrumentation.

Interferometric gravitational wave detectors with an unequal and time-varying arm length configuration like the Laser Interferometer Space Antenna rely on time-delay interferometry (TDI) for laser frequency noise subtraction. However, the TDI algorithm requires a laser ranging scheme with meter accuracy over a five million kilometer arm length. At the end of each arm only about 100 pW of light power will be detected for gravitational wave measurements and only 1% of this power can be used for laser ranging in order to avoid degradation in the phase stability of the science measurements. Here, we present the first experimental demonstration of such a ranging scheme at 1 pW power levels using a Direct Sequence Spread Spectrum (DS/SS) modulation. This type of modulation also enables optical communication by encoding data with ranging signals and provides significant noise reduction against spurious interfering signals for bidirectional ranging. Experimental results show ranging measurements of 42 cm at 3 Hz and the viability of highly reliable data transfer at several kilobits per second., (© 2011 Optical Society of America)
