Abstract

Objective: Older patients with cancer value functional outcomes as much as survival, but surgical studies lack functional recovery (FR) data. The value of a standardized frailty assessment has been confirmed, yet it's infrequently utilized due to time restrictions into everyday practice. The multicenter GOSAFE study was designed to (1) evaluate the trajectory of patients' quality of life (QoL) after cancer surgery (2) assess baseline frailty indicators in unselected patients (3) clarify the most relevant tools in predicting FR and clinical outcomes. This is a report of the study design and baseline patient evaluations. Materials & Methods: GOSAFE prospectively collected a baseline multidimensional evaluation before major elective surgery in patients (70 years) from 26 international units. Short-/mid-/Iong-term surgical outcomes were recorded with QoL and FR data. Results: 1003 patients were enrolled in a 26-month span. Complete baseline data were available for 977(97.4%). Median age was 78 years (range 70-94); 52.8% males. 968(99%) lived at home, 51.6% without caregiver. 54.4% had 3 medications, 5.9% none. Patients were dependent (ADL < 5) in 7.9% of the cases. Frailty was either detected by G8 = 2(37.4%), TUG > 20 s (5.2%) or ASAIII-IV (48.8%). Major comorbidities (CACI > 6) were detected in 36%; 20.9% of patients had cognitive impairment according to Mini-Cog. Conclusion: The GOSAFE showed that frailty is frequent in older patients undergoing cancer surgery. QoL and FR, for the first time, are going to be primary outcomes of a real-life observational study. The crucial role of frailty assessment is going to be addressed in the ability to predict postoperative outcomes and to correlate with QoL and FR. (C) 2019 Elsevier Ltd. All rights reserved.

37. Full-Analog Parasitic Capacitance Compensation for AC-Excited Differential Sensors

Abstract

Capacitive sensors are popular, especially due to the possibility to be integrated together with the readout circuit. In particular, differential implementations are intrinsically insensitive to common-mode noise. Unfortunately, stray capacitances can significantly alter the sensor output value, worsening the performance. This article proposes a novel solution based on an autotuning feedback loop to compensate for parasitic effects, thus preserving measurement performance. In particular, a voltage-controlled negative impedance converter is driven by a feedback loop, in order to control the current flowing in the sensor. The proposed solution can work with the both linear and hyperbolic type of sensors and with any voltage-mode sensor interfaces. An experimental setup, comprising the proposed compensating circuit, a De-Sauty bridge-based sensor front end and an emulated sensor, has been arranged to evaluate obtainable performance. Results show that the linearity error is decreased from more than 10%, without compensation, to less than 1%, when the proposed system is applied. © 1963-2012 IEEE.

Abstract

Capacitive sensors are low-cost and robust devices that can be easily scaled to very small sizes, thus making them suitable for implementation as a micro electro-mechanical system (MEMS). Differential arrangements of the sensors are also available, providing improved rejection of common mode interference. Due to their nature, an AC (sinusoidal) excitation signal is usually adopted. Various types of front-end circuits have been proposed in the past, exploiting different techniques such as conversion of capacitance to current or frequency or analog-to-digital conversion and adopting different approaches, such as full analog or full digital architectures. This paper proposes a digital and microcontroller-based system for AC-excited differential sensors aiming at minimizing cost and power needs, in sight of future large volume applications as for Internet of Things (IoT) paradigm. A proof of concept implementation has been realized and experimentally validated, obtaining a relative error in the measurand estimation on the order of 1%, when the parasitic effects can be neglected. © 2020 IEEE.

Abstract

Introduction. Uveal melanoma is the most common intraocular tumor in the adult population. It can affect any part of the uveal tract: the iris, ciliary body, and choroid. Historically, enucleation has been the mainstay of treatment for primary melanoma. In the last decade, however, radiotherapy has acquired an increasingly important role and has now become our first-line modality. However, it is still widely debated what is the most effective radiotherapy technique for this tumor. Purpose to perform a literature review on the utility of radiotherapy for primary ocular melanoma and determine the most effective radiotherapy technique. Materials and Methods. We included all systematic and narrative reviews on the topic, published between September 2007 and November 2017 on PubMed and SCOPUS. Two independent reviewers assessed the eligibility criteria for each article using the PRISMA checklist. The methodological quality of narrative and systematic reviews was evaluated with the INSA and AMSTAR checklists, respectively. Results. Our study analyzed a total of 23 studies, including 18 narrative reviews and 5 systematic reviews. Radiotherapy with Brachytherapy, Proton Therapy, SRS/SRT with gamma knife and cyber knife, are the most common choices for the treatment of primary ocular melanoma. These techniques allow for excellent lesion spread control, eye, and vision conservation, and improve overall patients' quality of life. Among the narrative reviews, the highest INSA score was 5/7, the lowest 2/7, the mean was 3.83/7 and median was 4/7. Among the systematic reviews, the highest AMSTAR score was 9/12, the lowest 4/12, the mean 5.6/7 and median 4/7. Conclusion. The number of studies available on this topic is scarce. Among those published, the methodological quality is modest, as assessed with the INSA and AMSTAR checklists. Abstract

This work deals with the design of a new readout electronics for silicon photomultipliers sensors. The so-called SiPMs sensors are an emerging technology currently diffusing in many applications and, among them, in the definition of a new generation of LIDAR systems. The latter, nowadays have a primary role in the evolution process that is involving Smart Cities, being an enabling technology in different fields. The solution here proposed is realized at electronic level with a 150 nm technology process from LFoundry and results provide a feasible demonstration of the capability of the proposed design approach to be employed in practical applications.
