Background: The main objective of the present study is to evaluate the effects and possible benefits with regard to the postoperative period of lower third molar extractions, comparing the intraalveolar application of a bioadhesive gel of 0.2% chlorhexidine (CHX) to the use of a mouthwash with a super-oxidized solution, (SOS) Dermacyn® Wound Care (Oculus Innovative Sciences lnc., California, USA). Material and Methods: A randomized double-blind study was carried out in 20 patients with a split-mouth design, with a total of 40 extractions of symmetrically impacted bilateral lower third molars. Patients were divided into two groups, a control group (C = 20) and an experimental group (D = 20). Any infectious complications, wound healing, plaque accumulation in the stitches, and presence of trismus and inflammation were evaluated using the distance between different facial points, at three, eight, and fifteen days after extraction. Pain, swelling, and amount of analgesics taken were evaluated using the VAS scale throughout the 15 days following extraction. Tol erance to treatment was evaluated using a verbal scale. Results were statistically compared using the Student’s t- and chi-squared tests. Results: No statistically significant differences were found between the two groups with regard to infectious com plications, swelling, or wound healing. Use of analgesics and self-reported pain levels were slightly lower in the experimental group than in the control group during days 6 and 7 of the study (p < 0.05). The global treatment tolerance was satisfactory and similar in both groups. Conclusions: Both CHX and SOS are effective at improving the postoperative period after extraction of lower third molars.

Introducción: El objetivo de este trabajo fue el realizar exéresis de lesiones hiperplásicas en la mucosa yugal comparando el láser de 532nm (KTP), láser muy usado en Dermatología con excelentes resultados pero en la actualidad presenta un corto recorrido en Cirugía Bucal, versus el Láser de Diodo 980nm, el cual lleva tiempo siendo utilizado con éxito en el campo de la Cirugía Bucal, teniendo en cuenta el dolor, la cicatrización, inflamación y el consumo de fármacos que se produjo en el postoperatorio. Material y método: Se trata de un estudio prospectivo en una serie de 20 pacientes con lesiones hiperplásicas en la mucosa yugal en el que se establecieron dos grupos. La elección del Láser de 532nm ó el Láser de Diodo 980nm se hizo de forma aleatoria. La potencia utilizada fue de 1,5W en ambos grupos a modo continuo y con una fibra óptica de 320um. Tras esto se realizó una medición con escala EVA del dolor, cicatrización, inflamación y del consumo de fármacos en el postoperatorio tanto en el día de la intervención, a las 24 horas 14 y 28 días tras la extirpación. Resultado y Discusión: En cuanto a las variables estudiadas de dolor e inflamación se observa que a las 24 horas hubo una mayor proporción de sujetos con dolor e inflamación ligero-moderado y a su vez una ligera toma superior de Paracetamol con el láser de 980nm frente al láser de 532nm o KTP aunque los datos no fueron estadísticamente significativos; si encontramos diferencias significativas a los 28 días en el dolor (p=0,023) e inflamación(p=0,023), pero siempre en la parámetro de ausencia por lo que en ambos láseres encontramos ausencia de dolor. Está diferencia de percepción del dolor, puede estar relacionado con la mayor penetración en profundidad en el tejido debido a la falta de absorción en superficie del láser de diodo de 980nm. Respecto a la cicatrización en los dos tipos de láser a lo largo de las visitas, no se detectan diferencias entre los dos grupos y en ambos hay ausencia de cicatriz retrácti, Introduction: The aim of this study is to evaluate the resection of hyperplastic lesions on the buccal mucosa comparing the 532nm laser (KTP), versus diode 980nm laser, considering the pain, scarring, inflammation and drug consumption that occurred postoperatively with each lasers. Material and Methods: We performed a prospective study of a consecutive series of 20 patients in two groups that presents hyperplastic lesions on the buccal mucosa. The choice of the laser 532nm or diode 980nm laser for the surgery was made randomly. The power used was 1.5W in both groups in a continuous mode with a 320 μm optical fiber. Evaluating postoperatively parameters of pain, scarring, inflammation and consumption of drugs were record by a Numerical Rating Scale, the day of the surgery, 24 hours after, 14 and 28 days after. Results and Discussion: Pain and inflammation was light-moderate and the consumption of Paracetamol was somewhat higher in the diode 980nm laser against the 532nm or KTP laser at the 24 hours, although data were not statistically significant; significant differences were found at 28 days in pain (p = 0.023) and inflammation (p = 0.023), but always in the absence parameter so we find no pain in both lasers. The difference in pain and inflammation at 24 hours time point may be due to the greater ability to penetrate the tissue by the diode 980 nm laser. Scarring in the two types of laser were no differences along the visits, with not detected scar retractable. Conclusion: Both lasers are effective for the oral soft tissue surgery providing numerous benefits.

En este artículo presentamos un caso clínico de un paciente varón de 19 años de edad que acudió a las consultas externas del Servicio de Cirugía Oral y Maxilofacial de los Hospitales Universitarios Virgen del Rocío de Sevilla debido al hallazgo radiológico casual de un premolar incluido por parte de su odontólogo.
