Introduction: Heart transplantation (HTx) serves as the gold-standard therapy for end-stage heart failure, yet patients often experience physical deconditioning and cognitive impairments post-surgery. Cardiac rehabilitation (CR) has shown promise in the HTx context. However, uncertainty surrounds the impact of biological sex. Accordingly, the aim of this paper was to investigate the impact of biological sex in a cohort of patients with HTx early admitted to a residential CR program. Methods: This was a retrospective analysis involving patients who underwent HTx at Niguarda Hospital and who subsequently participated in a CR program at IRCCS Fondazione Don Gnocchi, Milan, Italy, between 2010 and 2022. The primary endpoint was time to event (in months), with an event defined as a composite outcome of whichever occurred first of death, allograft rejection, or cardiac allograft vasculopathy up to 30 months follow-up. Results: In a total of 129 patients, 60 % male, and 40 % female, baseline characteristics presented comparably between the sexes. At 6 months, no significant sex differences were observed for the primary composite outcome. However, at 30 months, females exhibited a significantly lower incidence of the primary composite outcome and an increased survival rate. Multivariable analysis confirmed a protective effect of female sex against mortality (F vs. M, HR 0.164, 95 % CI 0.038–0.716, P = 0.0161). Conclusions: Despite limitations, our findings emphasize that sex affects post-HTx long-term follow-up following CR discharge, with more favorable outcomes for female recipients. In an era of tailored management algorithms, it is imperative to take into account the gender gap even in cardiac rehabilitation.

6. Dry grasslands of central-western Po Plain (Italy): implications under Council Directive 92/43/EEC

This paper provides an overall look on the diversity of lowland dry grasslands of the western Po Plain, useful to support their conservation and management. Specific aims were: 1) to identify lowland dry grassland-associated plant communities at alliance level, which is useful for their classification under the Council Directive 92/43/EEC, 2) to assess their synecological and synchorological differences, 3) to define the equivalent Directive habitats and their management implications. Seven subareas were analysed: Lomellina, Ticino River, Sesia River, Po River (North), Orba Stream, Scrivia Stream, and Serio River. Floristic-vegetational relevés were carried out considering vascular plant, moss and lichen species. Cluster analysis were performed to syntaxonomically classify them, while statistical tests were performed to characterize them by means of biological life forms, chorotypes and Ellenberg indicator values. Eight plant communities were classified at alliance level and three plant communities were classified at class level. The equivalence with three Natura 2000 Habitats (H2330, H6110* and H6210) was found. Of the 60 studied sites, the 68% are located inside the Natura 2000 Network, while the remnant 31% are located outside. Possible management actions include: cutting of woody species, mowing, sod-cutting, transplants of typical herbaceous species, and ex novo restoration using harvested seeds from donor grasslands.

Background and purposeClinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation.MethodLogistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number.ResultsFrom March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p = 0.005) and more in smoker patients (OR 1.39; p = 0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p = 0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p = 0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p = 0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p = 0.05). Background: Chronic low-grade inflammation in obesity is linked to white adipose tissue (WAT) dysfunction. Plasma lipopolysaccharide (LPS) activates Toll-like receptor 4 (TLR4), triggering NF-κB and worsening these disturbances. Previously, we showed that histone H3 lysine 27 (H3K27) epigenetic modifications affect WAT gene expression in high-fat-diet mice, identifying key pathways in adipose-derived stem cells (ASCs). This study explores whether NF-κB influences H3K27 modifiers in human ASCs and evaluates fish oil (FO) as a modulator. Methods: Human visceral WAT ASCs were stimulated with LPS and treated with FO enriched with eicosapentaenoic acid (EPA). Flow cytometry, PCR array, RT-PCR, and Western blot assays were used. Results: LPS increased NF-κB activity, elevating KDM6B demethylase levels and H3K27 acetylation. These epigenetic modifications in LPS-stimulated ASCs were associated with persistent changes in the expression of genes involved in adipogenesis, metabolic regulation, and inflammation, even after LPS removal and cell differentiation. FO mitigated these effects, reducing H3K27 acetylation and promoting methylation. Conclusions: FO demonstrates potential in modulating inflammation-induced epigenetic changes and preserving adipocyte function.

At the end of October 2023, CoSMEU (Italian Society of Emergency Medicine Residents) conducted an electronic survey among the Italian Emergency Medicine (EM) residents. Technical skills, lecture programs, research, and types of employment have been investigated. 421 (21.5.%) residents from all the 34 Italian EM schools responded. Great heterogeneity of training and the lack of opportunities in research about EM topics are reported. The importance of research in EM was recognized by half of those surveyed, but only one-third are actively engaged in the EM scientific community. While one-third of EM residents have accepted a job contract, nearly all of them are against contracts with Calabria Decree from the 2nd year of residency. The ideal workplace is a combination of in-hospital and pre-hospital settings, and almost all of the EM residents believe that the most frightening part of being an EM doctor is the hard-working condition.

Background and purposeSome studies have shown that air pollution, often assessed by thin particulate matter with diameter below 2.5 µg/m3 (PM2.5), may contribute to severe COVID-19 courses, as well as play a role in the onset and evolution of multiple sclerosis (MS). However, the impact of air pollution on COVID-19 has never been explored specifically amongst patients with MS (PwMS). This retrospective observational study aims to explore associations between PM2.5 and COVID-19 severity amongst PwMS.MethodsData were retrieved from an Italian web-based platform (MuSC-19) which includes PwMS with COVID-19. PM2.5 2016-2018 average concentrations were provided by the Copernicus Atmospheric Monitoring Service. Italian patients inserted in the platform from 15 January 2020 to 9 April 2021 with a COVID-19 positive test were included. Ordered logistic regression models were used to study associations between PM2.5 and COVID-19 severity.ResultsIn all, 1087 patients, of whom 13% required hospitalization and 2% were admitted to an intensive care unit or died, were included. Based on the multivariate analysis, higher concentrations of PM2.5 increased the risk of worse COVID-19 course (odds ratio 1.90; p = 0.009).ConclusionsEven if several other factors explain the unfavourable course of COVID-19 in PwMS, the role of air pollutants must be considered and further investigated.

Habitats Directive (92/43/EEC) is the cornerstone of nature conservation in Europe and is at the core of the EU Biodiversity Strategy for 2030. There is room, however, for its improvement, at least for northern Italy, where ambiguities in the definition of habitat types of Annex I of the Habitats Directive are not novel and interpretation difficulties have been highlighted. Sharpening the characterization of habitat types represents an opportunity for lowering classification uncertainties and improving conservation success. With the aim to refine the definitions of habitat types and associated typical species of the Habitats Directive, a group of vegetation scientists of the Italian Society of Vegetation Science based in northern Italy made the exercise of finding viable proposals for those habitat types having a problematic interpretation in the Alpine biogeographical region of Italy. Such proposals arise from group discussions among scientists, and professionals, thus offering a shared view. We prepared 9 habitat proposals important for this geographic area. They include new habitat types at the European level, new subtypes within pre-existing habitat types, including some adjustments of the recently proposed subtypes with respect to northern Italy, and recognition of priority criteria for a pre-existing habitat type. With a vision of tailored conservation, our proposals represent a starting point in view of a future update of Annex I. Furthermore, the list of typical species could be useful for preparing expert systems for automatic classification. Irrespective of legally binding solutions in place, we caution these proposals represent relevant baseline conservation indications that local and regional administrations of the Alpine Arch should consider.

INTRODUZIONE: Recenti studi hanno evidenziato come l’Health Literacy (HL) rappresenti uno dei determinanti della salute fondamentali per il benessere delle persone. Nonostante questo concetto sia ampiamente approvato dalla comunità scientifica, il trend di ricerca verte su outcomes quali disinformazione e carenza di HL nel soggetto con problematiche legate alla dimensione della salute mentale, delle malattie croniche, utilizzo e abuso di droghe, piuttosto che valutarne l’effettivo livello nella popolazione. È quindi necessaria un’analisi più ampia e generalizzata, che appoggi la comunità Europea nella valutazione dei livelli di HL nella popolazione, così da dimostrare quanto un suo incremento, permetta un miglioramento sia della salute degli individui, che del Sistema Sanitario. OBIETTIVO: Misurare le competenze di HL in un campione di studenti delle classi quinte frequentanti istituti superiori, con un’età compresa tra i 17 ed i 19 anni. MATERIALI E METODI: Questo studio monocentrico, descrittivo, si è svolto nel periodo tra il 15 ed il 30 settembre 2021, ed ha utilizzato il questionario HLS Q-16 (European Health Literacy Survey Questionnaire) validato da Lorini et al. (2019), per valutare il livello di HL. RISULTATI: i risultati sono in accordo con quanto ottenuto da Lorini et al. (2019) e consentono di affermare che più della metà del campione, risulta avere un livello di HL problematico o inadeguato. CONCLUSIONI: risulta fondamentale continuare la ricerca in questo ambito fortemente inesplorato sul territorio italiano ed analizzare in modo oggettivo e dimostrabile, dei potenziali correttivi che possano rimediare alla carenza di HL nel cittadino.

Background: Severe mental illness (SMI) is associated with increased cardiovascular risk. Dyslipidaemia is a potentially modifiable risk factor, which may be inadequately managed in patients with SMI. Objectives: To assess management of dyslipidaemia in patients with SMI versus healthy controls (HCs) in 2005 and 2015. Design and methods: Using Danish registers, we identified adult patients with SMI in the Greater Copenhagen Area (schizophrenia spectrum disorders or bipolar disorder) with ⩾1 general practitioner contact in the year before 2005 and 2015, respectively, and HCs without SMI matched on age and gender (1:5). Outcomes were lipid-profile measurements, presence of dyslipidaemia and redemption of lipid-lowering pharmacotherapy. Differences in outcomes between patients with SMI and controls were measured with multivariable logistic regression. Results: We identified 7217 patients with SMI in 2005 and 9939 in 2015. After 10 years, patients went from having lower odds of lipid measurements to having higher odds of lipid measurements compared with HCs [odds ratio (OR) 2005 0.70 (99% confidence interval (CI) 0.63–0.78) versus OR 2015 1.34 (99% CI 1.24–1.44); p 2005 versus 2015

BackgroundSeptic shock (SS) and cardiogenic shock (CS) are two types of circulatory shock with a different etiology. Several studies have described the molecular alterations in SS patients, whereas the molecular factors involved in CS have been poorly investigated. We aimed to assess in the whole blood of CS and SS patients, using septic patients without shock (SC) as controls, transcriptomic modifications that occur over 1 week after ICU admission and are common to the two types of shock.MethodsWe performed whole blood RNA sequencing in 21 SS, 11 CS, and 5 SC. A syndrome occurring after adenoviral vector anti-SARS-CoV-2 vaccination, characterized by thrombocytopenia, venous thrombosis, and circulating anti-PF4 antibodies, known as vaccine-induced immune thrombotic thrombocytopenia (VITT), is well described. Data on the long-term course of this syndrome are lacking. Our aim is to report the clinical and laboratory features of a patient with VITT from diagnosis and during 21 months of follow-up. Cerebral venous thrombosis associated with elevated D-dimer, low fibrinogen, thrombocytopenia, and anti-PF4 antibodies positivity occurred in this patient after ChAdOx1 nCoV-19 vaccination. Cerebral thrombosis required a revascularization procedure and decompressive craniectomy. Upon dexamethasone and anticoagulant treatment initiation, the platelet count recovered. However, a persistently high anti-PF4 antibody titer, without thrombosis recurrence, was observed. Little is known about the long-term persistence of anti-PF4 antibodies, their clinical significance, and their possible role in guiding therapeutic decisions. In our patient, we decided to continue anticoagulant treatment beyond 21 months with parallel anti-PF4 antibody monitoring.

- Abstract
Recombination activating genes (RAGs) are tightly regulated during lymphoid differentiation, and their mutations cause a spectrum of severe immunological disorders. Hematopoietic stem and progenitor cell (HSPC) transplantation is the treatment of choice but is limited by donor availability and toxicity. To overcome these issues, we developed gene editing strategies targeting a corrective sequence into the human RAG1 gene by homology-directed repair (HDR) and validated them by tailored two-dimensional, three-dimensional, and in vivo xenotransplant platforms to assess rescue of expression and function. Whereas integration into intron 1 of RAG1 achieved suboptimal correction, in-frame insertion into exon 2 drove physiologic human RAG1 expression and activity, allowing disruption of the dominant-negative effects of unrepaired hypomorphic alleles. Enhanced HDR-mediated gene editing enabled the correction of human RAG1 in HSPCs from patients with hypomorphic RAG1 mutations to overcome T and B cell differentiation blocks. Gene correction efficiency exceeded the minimal proportion of functional HSPCs required to rescue immunodeficiency in Rag1(-/- )mice, supporting the clinical translation of HSPC gene editing for the treatment of RAG1 deficiency.

Abstract Background Thrombotic thrombocytopenic purpura (TTP) is a rare, life-threatening thrombotic microangiopathy (TMA) requiring urgent treatment. Standardization of its diagnosis and optimal management is challenging. This study aimed to evaluate the role of centralized, rapid testing of ADAMTS13 in patients experiencing acute TMAs requiring plasma-exchange (PEX) and to estimate the incidence of TTP in a large Italian Region. Methods We perfomed a cohort study in the frame of the project “Set-up of a Lombardy network for the study and treatment of patients undergoing apheresis”, including 11 transfusion centers in the Region. Consecutive patients referred from 2014 to 2016 with acute TMAs requiring PEX were enrolled. Centralized ADAMTS13 activity testing was performed at the Milan Hemophilia and Thrombosis Center within 24 h. Results Forty-three TMA patients (44 events) were enrolled, of whom 35 (81%) had severe ADAMTS13 deficiency. Patients with severe ADAMTS13 deficiency were younger, mainly women, with a higher prevalence of autoimmune disorders and a lower prevalence of cancer. Background: Chronic low-grade inflammation in obesity is linked to white adipose tissue (WAT) dysfunction. Plasma lipopolysaccharide (LPS) activates Toll-like receptor 4 (TLR4), triggering NF-κB and worsening these disturbances. Previously, we showed that histone H3 lysine 27 (H3K27) epigenetic modifications affect WAT gene expression in high-fat-diet mice, identifying key pathways in adipose-derived stem cells (ASCs). This study explores whether NF-κB influences H3K27 modifiers in human ASCs and evaluates fish oil (FO) as a modulator. Methods: Human visceral WAT ASCs were stimulated with LPS and treated with FO enriched with eicosapentaenoic acid (EPA). Flow cytometry, PCR array, RT-PCR, and Western blot assays were used. Results: LPS increased NF-κB activity, elevating KDM6B demethylase levels and H3K27 acetylation. These epigenetic modifications in LPS-stimulated ASCs were associated with persistent changes in the expression of genes involved in adipogenesis, metabolic regulation, and inflammation, even after LPS removal and cell differentiation. FO mitigated these effects, reducing H3K27 acetylation and promoting methylation. Conclusions: FO demonstrates potential in modulating inflammation-induced epigenetic changes and preserving adipocyte function. [ABSTRACT FROM AUTHOR]
