This article provides a discussion on Multimodal Social Semiotics. For this purpose, we gather the main works that represent the thinking of Gunther Kress and collaborators. In this sense, when reviewing his works, we found Michael Halliday’s (1978) theoretical formulations that grounded the sociosemiotic vision such as choice, interest, motivated sign, all functions that guide a specific vision of language and grammar. In the scope of the theory, we find the contributions of Kressian thought on multimodality with an interest in exploring the forms of modern meaning. Thus, based on these principles, we present a set of research that brings reflections of his thinking to the studies of language and communication. [ABSTRACT FROM AUTHOR]

10. A TIPOGRAFIA EM JOGOS DIGITAIS: UMA ANÁLISE MULTIMODAL DO JOGO PEGGLE BLAST.

- Abstract
In this article, we investigate typography as a multimodal resource present in the digital game Peggle Blast (2014), aiming to point out its potential meanings as a semiotic resource and its possible contribution to the understanding of gameplay through the integration of this and the other semiotic resources that make up the images of the mentioned game. We captured two prints and submit them to a descriptive and interpretive analysis based on introductory notions about multimodality (KRESS, 2010; KRESS & VAN LEEUWEN, 2006), social semiotics (KRESS, 2010), digital games (JEWITT, 2005; GEE, 2005) and the typographic characteristics presented by Van Leeuwen (2006) and Serafini and Clausen (2012). The analyzes showed the potential of typography as a communicative semiotic resource present in digital games and its contribution to the construction of meaning beyond the written verbal text. As a pedagogical implication, we point out possibilities that this resource, when explored, may contribute to the teaching of English. [ABSTRACT FROM AUTHOR]

11. Quando os modos verbal e visual se encontram: uma análise das relações de status em atividades de leitura em língua inglesa

- Abstract
The advances in digital technology facilitate the integration of different modes in the same composition. As a consequence, text and image are even more articulated, which makes contemporary communication even more multimodal (KRESS; VAN LEEUWEN, 2006; MARTINEC; SALWAY, 2005). This scenario, therefore, requires changes in education, since it’s necessary to adequate pedagogical practices to the multimodal texts that are read daily by the 21st century reader. Thus, the present study aims to investigate the status relations between the written and visual modes (MARTINEC; SALWAY, 2005) that manifest themselves in texts which compose English reading activities available online and in printed materials in order to identify the role of image in these activities. These relations were observed in 73 reading activities, which were selected according to their multimodal composition and free access. The results showed a predominance of unequal relations in 58 activities whereas the equal relation was identified in 15 activities only. As pedagogical implications, we understand that the role of image still needs to be more discussed with language teachers so they can adequate their practices - not only in the classroom but also in the production of teaching materials - to multimodal texts. We also understand that the system developed by Martinec & Salway (2005) may be an instrument to mediate this discussion. Os avanços tecnológicos digitais facilitam a junção de diferentes modos em um mesmo evento comunicativo. Como consequência, texto e imagem estão cada vez mais articulados, tornando a comunicação contemporânea ainda mais multimodal (KRESS; VAN LEEUWEN, 2006; MARTINEC; SALWAY, 2005). Esse cenário, portanto, requer mudanças na educação, pois torna-se necessário adequar as práticas pedagógicas aos textos multimodais que são lidos no dia-a-dia pelo leitor do século XXI. Assim, o presente estudo tem como objetivo investigar as relações de status entre os modos escrito e visual (MARTINEC; SALWAY, 2005) que se manifestam em textos que compõem atividades de leitura em língua inglesa disponíveis em materiais didáticos em meio impresso e digital, com o intuito de diagnosticar o papel da imagem nestas atividades. Essas relações foram observadas em 73 atividades de leitura, que selecionadas de acordo com sua composição multimodal e o livre acesso aos seus suportes. As análises mostraram a predominância da relação de status desigual, estando a imagem subordinada ao texto verbal em 58 atividades, enquanto que a relação de status igual foi identificada em apenas 15 atividades. Como implicações pedagógicas, entendemos que o papel da imagem ainda precisa ser mais discutido junto aos professores de línguas, para que esses possam adequar suas práticas, tanto em sala de aula, quanto na produção de materiais didáticos, aos textos multimodais. Entendemos ainda que o sistema desenvolvido por Martinec e Salway (2005) pode ser uma ferramenta para mediar essa discussão.

13. O uso da tipografia nas interfaces de jogos: uma leitura multimodal

- Abstract
Technological advances and their consequences in social life have promoted new forms of access to reading, making possible new social practices. These practices are carried out through new means of communication, such as applications, social networks, games, among others. This article analyzes the use of typography present in the interfaces of digital games and promotes an analysis of the multimodal reading, based on Grammar of Visual Design, by Kress and van Leeuwen (2006). We approach multimodality (KRESS; VAN LEEUWEN, 2006), multiliteracies (THE NEW LONDON GROUP, 1996), digital games (GEE, 2003) and typography (VAN LEEUWEN, 2006) as theoretical support for this study. In order to analyze the contribution of typography and multimodality in the reading process of digital games, we investigated game screenshots that contain dialogues during the gameplay. We can observe through typography and imagery analysis the contribution of semiotic resources and their communicative potential in games, providing a multimodal reading, assigning meaning and communicative power to aspects that go beyond the verbal text. Este artigo analisa o uso da tipografia presente nas interfaces de jogos digitais e promove uma análise multimodal com base na Gramática do Design Visual, de Kress e van Leeuwen (2006). Como suporte teórico para este estudo abordamos sobre multimodalidade (KRESS; VAN LEEUWEN, 2006), multiletramentos (THE NEW LONDON GROUP, 1996), jogos digitais (GEE, 2003) e tipografia (VAN LEEUWEN, 2006). Com o objetivo de analisar a contribuição da tipografia e da GDV nas interfaces de jogos digitais, investigamos quatro prints de jogos que contenham diálogos entre os participantes durante a jogabilidade. Os jogos selecionados foram Operate Now: Hospital (Spil Games, 2017), Gardenscapes (Playrix, 2016), Matchington Mansion (Firecraft Studios, 2017) e Hometown Romance (Pretty Teen Games, 2018). Com isso, pudemos observar através da tipografia e análise imagética a contribuição dos recursos semióticos e seu potencial comunicativo em jogos, proporcionando uma leitura multimodal e destaca seu poder comunicativo, atribuindo significados a diferentes modos, não apenas ao verbal.

- Abstract
EJA Interventiva is an interface between Youth and Adult Education and Special Education developed in the Federal District to assist students with intellectual disabilities and autism in an age-grade lag. This article presents a research of qualitative approach in the modality of Case Study that aims to analyze from the perspective of inclusion this project that became Curricular Guideline. The results revealed that EJA Interventiva presents itself as a successful inclusive project by enabling access and permanence in education, the continuation of schooling and the social insertion of students with intellectual disabilities and autism, serving the reparative, equalizing and qualifying purposes placed for Youth and Adult Education. It is evidenced that inclusive educational projects articulated and developed with the school community can guide normative acts. It is pointed out that for the advancement of the ongoing experience and inclusive education, the substitutive possibility of the traditional curriculum weighs with standardized spaces and times, signaling the importance of thinking about curricular proposals built on new bases, open to diversity., A EJA Interventiva é uma interface da Educação de Jovens e Adultos e Educação Especial desenvolvida no Distrito Federal para atendimento de estudantes com deficiência intelectual e autismo em defasagem idade/ano. Este artigo apresenta uma pesquisa de abordagem qualitativa na modalidade de Estudo de Caso, que objetiva analisar na perspectiva da inclusão esse projeto que se tornou Diretriz Curricular. Os resultados revelaram que a EJA Interventiva se apresenta como um projeto inclusivo bem-sucedido ao possibilitar o acesso e a permanência na educação, a continuidade da escolarização e a inserção social de estudantes com deficiência intelectual e autismo, atendendo às finalidades reparadora, equalizadora e qualificadora colocadas para a Educação de Jovens e Adultos. Evidencia-se que projetos educacionais inclusivos articulados e desenvolvidos com a comunidade escolar podem orientar atos normativos. Aponta-se que para o avanço da experiência em andamento e da educação inclusiva pesa a possibilidade substitutiva do currículo tradicional com espaços e tempos padronizados, sinalizando para a importância de se pensar propostas curriculares construídas sob novas bases, abertas à diversidade., La EJA Interventiva es una interfaz entre la Educación para Jóvenes y Adultos y la Educación Especial desarrollada en el Distrito Federal para atender a los estudiantes con discapacidade intelectual y autismo con diferencia de edad / año. Este artículo presenta una investigación de enfoque cualitativo en la modalidad de Estudio de Caso cuyo objetivo fue analizar desde la perspectiva de la inclusión este proyecto que se convirtió en Guìa Curricular. Los resultados há revelado que la EJA Interventiva se presenta como un proyecto inclusivo exitoso al posibilitar el acceso y la permanencia en la educación, la continuación de la escolaridad y la inserción social de los estudiantes con discapacidad intelectual y autismo, al servicio de los propósitos reparadores, igualadores y calificativos establecidos para la Educación de Jóvenes y Adultos. Se hace evidente que los proyectos educativos inclusivos articulados y desarrollados con la comunidad escolar pueden orientar los actos normativos. Se señala que para el avance de la experiencia en proceso y la educación inclusiva pesa la posibilidad sustitutiva del currículo tradicional con espacios y tiempos estandarizados, demostrando la importancia de pensar propuestas curriculares establecidas en nuevas bases, abiertas a la diversidad.

Advances in digital technologies have made us immerse in an increasing world of images, turning us not only consumers, but also producers of multimodal texts. Consequently, communication has become more and more visual, and conceptions on what is to be literate in the 21st Century need to be rethought to prepare its citizens to read, interpret and express themselves through those texts. How to achieve it and what are the gains and losses from this achievement is a concern educators have, especially those in teaching training. This paper is part of a study that aimed at investigating the viability of teaching categories from the Grammar of Visual Design (Kress and van Leeuwen, 2006), as a tool to promote visual literacy when reading and viewing skills are integrated (Barbosa, 2017). In this part of the study, we discuss the meaning eight pre-services teachers made when reading an advertisement, and its implication to their visual literacy according to Callow's model (2005). The results showed the rise of a reader-viewer (Serafini, 2014; Barbosa, 2017), who makes meaning on the verbal and the visual, more conscious, and aware of the importance of integrating those modes, but also more critical and structuralist, and less affective. - Abstract
: Short episodes of ischemia (ischemic preconditioning) protect the heart against ventricular arrhythmias during zero-flow ischemia and reperfusion. However, in clinics, many episodes of ischemia present a residual flow (low-flow ischemia). Here we examined whether ischemic preconditioning protects against ventricular arrhythmias during and after a low-flow ischemia and, if so, by what mechanism(s). Isolated rat hearts were subjected to 60 min of low-flow ischemia (12% residual coronary flow) followed by 60 min of reperfusion. Ischemic preconditioning was induced by two cycles of 5 min of zero-flow ischemia followed by 5 and 15 min of reperfusion, respectively. Arrhythmias were evaluated as numbers of ventricular premature beats (VPBs) as well as incidences of ventricular tachycardia (VT) and ventricular .brillation (VF) during low-flow ischemia and reperfusion. Ischemic preconditioning significantly reduced the number of VPBs and the incidence of VT and of VF during low-flow ischemia. This antiarrhythmic effect of preconditioning was abolished by HOE 140 (100 nM), a bradykinin B2 receptor blocker. Similar to preconditioning, exogenous bradykinin (10 nM) reduced the number of VPBs and the incidence of VT and of VF during low-flow ischemia. Furthermore, the antiarrhythmic effects of both ischemic preconditioning and bradykinin were abolished by glibenclamide (1 µM), a non-specific blocker of ATP-sensitive K+ (KATP) channels. Finally, the antiarrhythmic effects of both ischemic preconditioning and bradykinin were abolished by HMR 1098 (10 µM), a sarcolemmal KATP channel blocker but not by 5-hydroxydecanoate (100 µM), a mitochondrial KATP channel blocker. In conclusion, ischemic preconditioning protects against ventricular arrhythmias induced by low-flow ischemia, and this protection involves activation of bradykinin B2 receptors and subsequent opening of sarcolemmal but not of mitochondrial KATP channels

Objective: In the study of anxiety, the bridge between preclinical research and clinical trials with patients is represented by models of experimental anxiety in humans, which induce anxiety in healthy volunteers in a controlled manner. The present study aimed to investigate an experimental model of human anxiety, based on of the presentation of an anxiogenicscene either isolated (clip) or as part of a full-length movie(plot). Method: Thirty healthy male volunteers were allocated to two groups: Clip (n = 15) and Plot (n = 15). Psychological (through a standardized scale, namely, the State-Trait Inventory Anxiety, and numerical scales /analog)and physiological (through the records of changes in heart rate, temperature change endpoints, skin conductance and muscle tension) parameters were evaluated while subjects watched a white screen (baseline), a neutral scene and an anxiogenic scene. Results: The data a physiological stress response during the anxiogenic scene for both groups. However, the subjective anxiety response was less intense in the Clip group, and it could only be detected by the State-Trait Anxiety Inventory, which is a more elaborate anxiety measuring instrument than the commonly used analog/numeric scales. Conclusion: The data support the employment of movies that involve situations of danger as a method of inducing experimental anxiety. Objetivo: No estudo da ansiedade, a ponte entre a pesquisa pré-clínica e os ensaios clínicos com pacientes é representada pelos modelos de ansiedade experimental em humanos, os quais induzem ansiedade em voluntários saudáveis de maneira controlada. No presente estudo, objetivou-se investigar um modelo de ansiedade experimental humana, baseado na exibição de uma cena ansiogênica isolada (trecho) ou como parte de um filme íntegro (trama). Método: Foram selecionados 30 voluntários saudáveis, alocados para os grupos Trecho (n=15) e Trama (n=15), os quais foram avaliados psicológica (por meio de uma escala padronizada, qual seja, o Inventário de Ansiedade Traço-Estado, além de escalas numéricas/analógicas) e fisiologicamente (através dos registros de alterações da frequência cardíaca, alteração de temperatura de extremidade, condutância da pele e tensão muscular). enquanto assistiam a nenhuma cena (linha de base), a uma cena neutra e a uma cena ansiogênica. Resultados: Os dados mostraram uma resposta fisiológica de estresse durante a apresentação da cena ansiogênica tanto para o grupo Trecho quanto para o grupo Trama. Entretanto, a ansiedade, avaliada subjetivamente, foi menos intensa para o grupo Trecho, sendo detectada somente pelo Inventário de Ansiedade Traço-Estado, um instrumento de mensuração mais elaborado que as usuais escalas analógicas.Conclusão: Os dados apoiam o uso de filmes, que envolvam perigo em seu enredo, como modelo de ansiedade experimental humana, no qual podem ser facilmente avaliados tanto parâmetros psicológicos quanto fisiológicos.

The Children's Literature, which is known nowadays, is a result of a long process of changes, adaptations, creations, innovations, advances and even withdrawal which involved several narratives and which defined for a long time the texts that were selected and dedicated to children. In such processes, the imaginary has always been present, even though the western society would give more importance to reason, which influenced the literary productions for kids. We aim with this work to bring to reflexion, based on historical contextualization of literature for kids, the work that Clarice Lispector dedicated to children, associating her writings to the imaginary of Gaston Bachelard, the idea of day-dreaming and poetic image which is materialized through the literary language. Analyzing the Lispector's books for kids, with emphasis on O mistério do coelho pensante and Quase de verdade, highlighting the aesthetic and imaginary elements which form the claricean narrative and which contribute with a new writing style of the short story for kids, in Brazil after Monteiro Lobato. We have tried to present Lispector's language as an instrument of reflexion and desautomatization of the language, the perceptions of the reader, and as a symbol of infanthood which is constantly amazed with life, finding, this way, a literature for kids which does not work as an ideological disseminating, disciplinarian, moralist or inhibiting of the child, but a literature which expresses, through poetical images, of an infanthood which goes beyond a true fact or a lie A literatura para a infância, que se conhece atualmente, é fruto de um longo processo de mudanças, adaptações, criações, inovações, avanços e até retrocessos que envolveram várias narrativas e que definiram durante muito tempo os textos selecionados e dedicados às crianças. Nesse processo, o imaginário sempre esteve presente, embora a sociedade ocidental valorizasse mais a razão, o que influenciou também na produção literária infantil. Propusemo-nos trazer à reflexão, baseando-nos na contextualização histórica da literatura infantil, a obra que Clarice Lispector dedicou às crianças, associando sua escrita ao imaginário de Gaston Bachelard, à idéia de devaneio e imagem poética que se materializa através da linguagem literária. Analisamos os livros infantis de Lispector, com ênfase em O mistério do coelho pensante e Quase de verdade, ressaltando elementos estéticos e imaginários que formaram a narrativa clariceana e que contribuíram para uma nova escrita do conto para crianças, no Brasil, depois de Monteiro Lobato. Procuramos apresentar a linguagem dessa autora, como instrumento de reflexão e desautomatização da língua, das percepções do leitor, e como símbolo da infância que está em permanente maravilhamento com a vida, encontrando, assim, uma literatura infantil que não traz a função de disseminadora ideológica, disciplinadora, moralista ou inibidora da criança, mas uma literatura que expressa, através de imagens poéticas, sensações de uma infância que vai além de uma verdade ou mentira

We started from the study of the knowledge taught by Math teachers from the State Board of Education – Teaching Managing Department (GERE, Recife-Sul), with the objective of analysing how the curriculum proposed on the Mathematics National Curricular Guidelines (PCN) , Evaluation Matrix (SAEPE) and Pernambuco Curricular Basis (BCC-PE) which takes into account the choices made by teachers. In this analysis, we chose to include questions on the number of classes given and students’ success percentage shown on the State Evaluation Results Report – SAEPE/2002. We also did a short analysis of the textbooks chosen by teachers, more especifically the selection of the contents, so to compare the selection and the sequencing of the knowledges taught by teachers with those shown on the textbooks. In this way, we studied the relation between the time suggested by textbooks and time teachers dedicated to each of the syllabus chosen – which could be leading to the choices of the knowledges taught sequencing. For this, we worked with 43 groups, 22 of 5th graders and 21 of 6th graders and 35 teachers distributed among these groups. In the analysis we could identify a greater emphasis on the relation to the knowledges choices refering to the topics Geometry, Variants and Measures, Algebra and Functions and Information Usage which are explicit on the National Curricular Guidelines as important themes to Mathematics teaching . As for the textbooks, we identified that the choice of knowledges taught are linked to the contents on the textbooks chapters. We also verified that a proof of this stage presented on Chevallard’s Didatical Transposition, the knowledges taught choice, seem to be guided by the relationship the teacher has with the teaching ability. No presente trabalho, buscamos investigar uma das etapas apresentadas na Teoria da Transposição Didática de Chevallard, a etapa das escolhas dos saberes ensinados. Para isso, partimos do levantamento dos registros dos saberes ensinados por professores de matemática de escolas da Gerência Regional de Ensino (GERE) Recife Sul, com o intuito de analisarmos de que maneira os saberes propostos nos Parâmetros Curriculares Nacionais de Matemática, na Matriz de Avaliação (SAEPE) e na Base Curricular Comum do Estado de Pernambuco (BCC-PE) estavam contemplados nas escolhas feitas pelos professores. Nessa análise, optamos por introduzir questões sobre a quantidade de aulas dadas e o percentual de sucesso dos alunos explicitados no Relatório Estadual dos Resultados da Avaliação - SAEPE-2002. Realizamos, também, uma breve análise do livro didático adotado pelos professores, com relação a aspectos referentes à seleção de conteúdos, com o intuito de fazermos a comparação entre a seleção e a seqüenciação dos saberes ensinados por eles com os que são explicitados nos livros didáticos adotados pelos professores, observando, assim, a questão referente à relação entre o tempo proposto nos livros didáticos, e o tempo trabalhado pelos professores para cada um dos saberes elencados (escolhidos), o que poderia ser um direcionador das escolhas e das seqüências dos saberes ensinados. Para tanto tivemos 43 turmas, sendo 22 de 5ª série e 21 de 6ª série, com 35 professores distribuídos nas respectivas séries. Nas análises pudemos identificar uma maior ênfase com relação às escolhas dos saberes referentes ao bloco Números e Operações, apesar dos blocos de Geometria, Grandezas e medidas, Álgebra e Funções e Tratamento da informação serem explicitados nas propostas curriculares como temas importantes de serem incorporados às práticas de ensino. Identificamos, com relação ao livro didático, que as escolhas dos saberes ensinados estão atreladas aos conteúdos explícitos nos capítulos do livro didático. Verificamos,também, indícios de que essa etapa, apresentada na transposição didática de Chevallard, a escolha dos saberes ensinados, parece ser orientada pela proximidade que o professor tem com o saber a ensinar.

27. Echocardio graphic quantification of atherosclerosis leads to cost-effective treatment with statins : data from a prospective study of 336 patients

- Abstract
Objective: Use of statins in prevention of atherosclerosis is effective but expensive. Patient selection gains wider public attention as medication costs in the US and Europe augment by 8 We examined different clinical risk stratification strategies, particularly focusing on echocardiographic atherosclerosis quantification, for their impact on event reduction and cost-effectiveness in statin treatment. Methods and Results: In a prospective, consecutive cohort of 3 3 6 patients referred to non-invasive cardiac examination, risk stratification was done by various combinations of risk factors and noninvasive atherosclerosis quantification. Atherosclerotic burden was determined through measuring ``aortic elastance`` by transthoracic echocardiogram, a validated non-invasive method. Cardiovascular events were recorded at a mean follow-up of one year. Echocardiographically determined atherosclerosis severity and event history, especially in combination, yielded the best selection strategies for statin treatment over a broad range of predetermined funding or required event reductions, surpassing conventional cardiovascular risk factors. From 26.8 statin-preventable events/1000 patients/year (assuming all patients treated), the best selection strategies could avoid: 24 with 66 criteria), 20.1 with >50 or 9.6 with >15 and prior events), while conventional strategies without echo quantification of atherosclerosis were inferior. Conclusion: Non-invasive echocardiographic quantification of atherosclerosis improves efficiency and cost-effectiveness in statin treatment.

This study aimed to analyze the social network Facebook as a potential marketing tool for tourism in northern of Piauí. Using data collected from the observation posts and reposted on Facebook, during the period from January 2010 to June 2011, the material collected were analyzed with quantitative and qualitative approach, which allowed the identification of companies using the Social Network for dissemination of tourism products and services in the State. It is known that the advancement in Information and Communication Technologies - ICTs, has provided various sectors of the economy different ways to establish contacts and social bonds. Among all these paths, we highlight the resources available on the Internet., O objetivo de trabalho foi o de analisar o Facebook enquanto Rede Social como potencial ferramenta de marketing turístico da região Norte do Piauí, através de dados coletados a partir da observação de postagens e respostagens no Facebook, de Janeiro de 2010 a Junho de 2011, analisados com abordagem quanti-qualitativa, que possibilitou a identificação de empresas que utilizam a Rede Social para divulgação de produtos e serviços turísticos no Estado. Sabe-se que o avanço nas Tecnologias de Informação e Comunicação – TICs, tem proporcionado aos diversos setores da economia diferentes caminhos para o estabelecimento de contatos e laços sociais. Entre estes caminhos, destacam-se os recursos disponibilizados na Internet.

This paper discusses multiliteracies - digital, visual, and critical - in multimodal reading activities in English, which are available in websites. This discussion is grounded in theories that discuss multimodality and multiliteracy, such as social semiotics (KRESS; VAN LEEUWEN, 2006), as well as concepts of multiliteracies (STREET, 2012; OLIVEIRA, 2006) and the reading of hypertext (XAVIER, 2005). In an attempt to diagnose a possible congruence between the reading of English language texts and these theories, this study focuses on pedagogical practices identified in instructions, announcements, contents, and types of questions proposed in five online reading activities. Preliminary results show an incongruence between pedagogical practices and the promotion of multiliteracy. [ABSTRACT FROM AUTHOR]

Short episodes of ischemia (ischemic preconditioning) protect the heart against ventricular arrhythmias during zero-flow ischemia and reperfusion. However, in clinics, many episodes of ischemia present a residual flow (low-flow ischemia). Here we examined whether ischemic preconditioning protects against ventricular arrhythmias during and after a low-flow ischemia and, if so, by what mechanism(s). Isolated rat hearts were subjected to 60 min of low-flow ischemia (12% residual coronary flow) followed by 60 min of reperfusion. Ischemic preconditioning was induced by two cycles of 5 min of zero-flow ischemia followed by 5 and 15 min of reperfusion, respectively. Arrhythmias were evaluated as numbers of ventricular premature beats (VPBs) as well as incidences of ventricular tachycardia (VT) and ventricular .brillation (VF) during low-flow ischemia and reperfusion. Ischemic preconditioning significantly reduced the number of VPBs and the incidence of VT and of VF during low-flow ischemia. This antiarrhythmic effect of preconditioning was abolished by HOE 140 (100 nM), a bradykinin B
2 receptor blocker. Similar to preconditioning, exogenous bradykinin (10 nM) reduced the number of VPBs and the incidence of VT and of VF during low-flow ischemia. Furthermore, the antiarrhythmic effects of both ischemic preconditioning and bradykinin were abolished by glibenclamide (1 µM), a non-specific blocker of ATP-sensitive K+ (KATP ) channels. Finally, the antiarrhythmic effects of both ischemic preconditioning and bradykinin were abolished by HMR 1098 (10 µM), a sarcolemmal KATP channel blocker but not by 5-hydroxydecanoate (100 µM), a mitochondrial KATP channel blocker. In conclusion, ischemic preconditioning protects against ventricular arrhythmias induced by low-flow ischemia, and this protection involves activation of bradykinin B2 receptors and subsequent opening of sarcolemmal but not of mitochondrial KATP channels. Introduction: Resuscitation from ventricular fibrillation (VF), particularly from prolonged VF, frequently is complicated by postfibrillatory myocardial dysfunction (postresuscitation stunning). We tested whether this dysfunction can be caused by reduced myofilament Ca
2+ responsiveness after VF-induced myocyte Ca2+ overload. We also tested whether electrical defibrillation shocks contribute to this dysfunction. Methods and Results: Myofilament Ca responsiveness was estimated as ratio of left ventricular developed pressure over myocyte Ca2+ transient amplitudes (assessed as indo-1 fluorescence) in isolated perfused rat hearts before, during, and after VF (1.5 or 10 min) comparing three modes of defibrillation (bipbasic electrical shocks, lidocaine, or spontaneous). We found that, independent of these defibrillation modes, myofilament Ca responsiveness was significantly reduced, particularly after prolonged VF, although hearts were not ischemic or acidotic during and after VF (unchanged coronary flow, myocardial oxygen consumption, and pH of the coronary effluent). This reduction was associated with VF-induced myocyte Ca2+ overload and increasing or decreasing Ca2+ overload during VF (using 1 µM diltiazem or 6 mM extracellular calcium) led to parallel changes of myofilament Ca responsiveness. However, myofilament Ca2+ responsiveness was not associated with the defibrillation shock energy (range 0.1-15.0 J/g wet heart weight). Conclusion: Postfibrillatory myocardial dysfunction can be caused by reduced myofilament Ca2+ responsiveness after VF-induced myocyte Ca2+ overload. Electrical defibrillation shocks (up to 15 J/g wet heart weight). however, do not significantly contribute to this dysfunction. Our findings suggest that early additional therapy targeting intracellular Ca2+ overload may normalize myocyte Ca2+ and partially prevent postresuscitation stunning. [ABSTRACT FROM AUTHOR]- Published

In a similar way to the images which permeate daily in our environment, building stories and recording memories (CALLOW, 1999), constituting absolute truths and presenting ideas as natural and excluding others, the built spaces in which we circulate are commonly permeated by various semiotics resources that are associated and constitute meanings through integrated modes of communication. In turn, research in multimodality has expanded an analytical look at the multiplicity of texts and meanings, awakening a reading that goes beyond the verbal. In this sense, this study focuses on thematic party settings of children's birthday parties, arranged in party venues in João Pessoa-PB, understood as spatial texts that communicate about the social world in which children and other users of these spaces live. The main objective of this study is to discuss how the thematic party settings of birthdays in party venues in the capital of Paraíba communicate meanings and discourses about childhood, signaling interaction relationships with users as spatial texts (STENGLIN, 2004; 2009; RAVELLI; HEBERLE, 2016; RAVELLI; MCMURTRIE, 2016). To meet this objective, we established the following specific objectives: a) Describe the situational and cultural contexts in which the thematic scenarios are embedded, from the perspective of Systemic-Functional Linguistics; b) Analyze, in a descriptive and comparative way, the semiotic resources used in spaces reserved for children's themed party settings, and in party settings that signal comfort, safety, identification and belonging of users to the space, suggesting interaction between these spatial texts and their users, based on specific categories of Spatial Discourse Analysis, derived from Grammar of Visual Design; c) Analyze, comparatively and interpretatively, the dimensions of discursive practice and social practice that involve the party settings, using Critical Discourse Analysis; d) Interpret how the recurrence of thematic party settings contributes to communicating meanings about childhood, shaping socially constructed discourses, based on Social Semiotics and Critical Discourse Analysis. The study goes through notions of childhood, party, thematic party settings, spectacularization and consumerist society. It is supported by the contributions of Systemic-Functional Linguistics (HALLIDAY, 1978), Critical Discourse Analysis (FAIRCLOUGH, 2001; MEURER, 2005), Social Semiotics (HODGE; KRESS, 1988; VAN LEEUWEN, 2005) and Spatial Discourse Analysis (STENGLIN, 2004; 2009; RAVELLI; MCMURTRIE, 2016), in particular the interactive metafunction and its categories, binding and bonding, as well as the organizational metafunction and its categories, information value and salience, arising from the Grammar of Visual Design (KRESS; VAN LEEUWEN, 2006 [1996]). It is a research with a qualitative approach, promoting a descriptive analysis and comparative-interpretative data. These total an inventory with 258 themes, identified in a total of 1,671 party settings set up for children's birthday parties held between 2009 and 2019. The party settings were visualized through photographs posted on social networks of three party venues in Paraíba, in addition to photographs of the space empty, recorded by the researcher. The research corpus is composed of nine of these party settings, selected among the three most recurrent themes of this period, with three party settings representing each party venue. The results point to the spaces destined to the party settings and the party settings as texts that communicate ideas, beliefs and values that imply the conception of a certain Brazilian childhood, from pre-established themes, the chosen space, and the organizational distribution of these party settings, in the halls of the party venues. This communication is established by interaction relationships with users, enhanced by integrated semiotic resources, related to the visual and spatial mode, varying according to theme, space, party setting. RESUMEN De manera similar a las imágenes, que permean cotidianamente nuestro entorno, construyendo historias y registrando recuerdos (CALLOW, 1999), constituyendo verdades absolutas y presentando ideas como naturales y excluyendo otras, los espacios construidos por los que circulamos están comúnmente impregnados por diversos recursos semióticos que se asocian y constituyen significados a través de modos de comunicación integrados. A su vez, la investigación en multimodalidad ha ampliado una mirada analítica sobre la multiplicidad de textos y significados, despertando una lectura que va más allá de lo verbal. En ese sentido, este estudio se centra en los escenarios temáticos de las fiestas de cumpleaños infantiles, organizadas en las casas de fiesta de João Pessoa-PB, entendidas como textos espaciales que comunican sobre el mundo social en el que viven los niños y otros usuarios de estos espacios. El objetivo principal de este estudio es discutir cómo estos escenarios comunican significados y discursos sobre la infancia, señalando relaciones de interacción con los usuarios como textos espaciales (STENGLIN, 2004; 2009; RAVELLI; HEBERLE, 2016; RAVELLI; MCMURTRIE, 2016). Para cumplir con este objetivo, establecimos los siguientes objetivos específicos: a) Describir los contextos de situación y cultura en los que se insertan los escenarios temáticos a través de la Lingüística Sistémico-Funcional; b) Analizar, descriptiva y comparativamente, los recursos semióticos utilizados en los espacios reservados a los escenarios temáticos infantiles, y en los escenarios, que señalan la comodidad, la seguridad, la identificación y la pertenencia de los usuarios al espacio, sugiriendo la interacción entre estos textos espaciales y sus usuarios, a partir de las categorías específicas del Análisis del Discurso Espacial, surgidas del la Gramática del Diseño Visual; c) Analizar, de maneira comparativa e interpretativa, las dimensiones de la práctica discursiva y de la práctica social que involucran los escenarios presentados, utilizando el Análisis Crítico del Discurso; d) Interpretar cómo la recurrencia de los escenarios temáticos contribuye a comunicar significados sobre la infancia, configurando discursos socialmente construidos, desde la Semiótica Social y el Análisis Crítico del Discurso. El estudio pasa por las nociones de infancia, fiesta, escenarios temáticos, espectacularización y sociedad consumista. Se sustenta en los aportes de la Lingüística Sistémico-Funcional (HALLIDAY, 1978), el Análisis Crítico del Discurso (FAIRCLOUGH, 2001; MEURER, 2005), la Semiótica Social (HODGE; KRESS, 1988; VAN LEEUWEN, 2005) y el Análisis del Discurso Espacial (STENGLIN, 2004; 2009; RAVELLI; MCMURTRIE, 2016), en particular la metafunción interactiva y sus categorías, vinculante y unión, así como a la metafunción organizativa y sus categorías, information value y salience, surgidas de la Gramática del Diseño Visual (KRESS; VAN LEEUWEN, 2006 [1996]). Se trata de una investigación con enfoque cualitativo, que promueve un análisis descriptivo y comparativo-interpretativo de los datos. Estos suman un inventario con 258 temas, identificados en el total de 1.671 escenarios montados para fiestas de cumpleaños infantiles celebradas entre 2009 y 2019. Los escenarios se visualizaron a través de fotografías publicadas en las redes sociales de tres casas de fiestas paraibanas, además de fotografías del espacio vacío, registrados por el investigador. El corpus de la investigación está compuesto por nueve de estos escenarios temáticos, seleccionados entre los tres temas más recurrentes de este periodo, con tres escenarios representativos de cada casa de fiesta. Los resultados señalan los espacios destinados a los escenarios y los escenarios como textos que comunican ideas, creencias y valores que implican en la concepción de una determinada infancia brasileña a partir de temas preestablecidos, del espacio elegido y de la distribución organizativa de estos escenarios en los salones de las casas de fiesta. Esta comunicación se establece mediante relaciones de interacción con los usuarios, potenciadas por recursos semióticos integrados, relacionados con el modo visual y espacial, que varían según el tema, el espacio y el escenario. Nesse sentido, este estudo se volta para cenários temáticos de festas infantis de aniversário, dispostos em casas de festas de João Pessoa-PB, compreendidos como textos espaciais que comunicam sobre o mundo social em que vivem as crianças e demais usuários desses espaços. O objetivo principal deste estudo é discutir sobre como esses cenários comunicam significados e discursos sobre infância, sinalizando relações de interação com os usuários enquanto textos espaciais (STENGLIN, 2004; 2009; RAVELLI; HEBERLE, 2016; RAVELLI; MCMURTRIE, 2016). O estudo perpassa noções de infância, festa, cenários temáticos, espetacularização e sociedade consumista. Respalda-se nas contribuições da Linguística Sistêmico-Funcional (HALLIDAY, 1978), Análise Crítica do Discurso (FAIRCLOUGH, 2001; MEURER, 2005), Semiótica Social (HODGE; KRESS, 1988; VAN LEEUWEN, 2005) e Análise do Discurso Espacial (STENGLIN, 2004; 2009; RAVELLI; MCMURTRIE, 2016), em especial na metafunção interativa e suas categorias, ligação e vínculo, bem como na metafunção organizacional e suas categorias, valor de informação e saliência, advindas da Gramática do Design Visual (KRESS; VAN LEEUWEN, 2006 [1996]). É uma pesquisa com abordagem qualitativa, promovendo uma análise descritiva e comparativo-interpretativa dos dados. Estes totalizam um inventário com 258 temáticas, identificadas no total de 1.671 cenários montados para festas infantis de aniversário realizadas entre 2009 e 2019. Os cenários foram visualizados por meio de fotografias postadas em redes sociais de três casas de festas paraibanas, além de fotografias do espaço vazio, registradas pela pesquisadora. O corpus da pesquisa é composto por nove desses cenários temáticos, selecionados entre as três temáticas mais recorrentes desse período, sendo três cenários representativos de cada casa de festa. Os resultados apontam os espaços destinados aos cenários e os cenários como textos que comunicam ideias, crenças e valores que implicam na concepção de determinada infância brasileira, a partir de temáticas pré-estabelecidas, do espaço escolhido, e da distribuição organizacional desses cenários, nos salões das casas de festas. Essa comunicação é estabelecida por relações de interação com os usuários, potencializadas por recursos semióticos integrados, relacionados ao modo visual e espacial, variando conforme a temática, o espaço e o cenário.

43. ATL>Bedside quantification of atherosclerosis severity for cardiovascular risk stratification: a prospective cohort study.

: ObjectivesWe sought to assess the ability of a new noninvasive method to quantify atherosclerosis severity and to examine its power to predict cardiovascular events.: BackgroundDrug prevention of cardiovascular events is effective but costly, leading to a debate about who should receive this treatment. Patient selection is often based on surrogate markers, but quantification of atherosclerosis severity is desirable.: MethodsAtherosclerosis severity was quantified by determination of specific aortic wall elastance in transthoracic echocardiography, applying the biomechanics of pulse wave propagation. After validating the method in 52 patients by measuring aortic plaque burden in transesophageal echo directly, another 336 patients were prospectively studied by monitoring atherosclerotic events at one year and comparing the results with conventional risk stratification.: ResultsSpecific aortic elastance was well correlated with plaque burden (p < 0.0001) and largely independent of confounding variables. Specific aortic elastance predicted the primary end point of “atherosclerotic death, myocardial infarction or stroke” at one year (p < 0.0002). Event rate at one year in the lowest specific elastance tertile was 1.8% (CI 0.0% to 4.3%), in the middle tertile 5.4% (CI 1.1% to 9.7%) and in the highest tertile 12.7% (CI 6.3% to 19%). Secondary end points supported these findings. Stepwise multivariate analysis identified specific aortic elastance, prior atherosclerotic events and left ventricular ejection fraction as independent risk predictors. Specific elastance was of incremental value to clinically identified variables.: ConclusionsBedside measurement of specific aortic elastance allows assessment of atherosclerosis severity. It predicts the risk for future atherosclerotic events beyond conventional risk factors, promising better targeting of pharmacologic prevention and improved cost effectiveness. - Abstract
Objective: Use of statins in prevention of atherosclerosis is effective but expensive. Patient selection gains wider public attention as medication costs in the US and Europe augment by 8% to 10% per year. We examined different clinical risk stratification strategies, particularly focusing on echocardiographic atherosclerosis quantification, for their impact on event reduction and cost-effectiveness in statin treatment., Methods and Results: In a prospective, consecutive cohort of 336 patients referred to non-invasive cardiac examination, risk stratification was done by various combinations of risk factors and noninvasive atherosclerosis quantification. Atherosclerotic burden was determined through measuring "aortic elastance" by transthoracic echocardiogram, a validated non-invasive method. Cardiovascular events were recorded at a mean follow-up of one year. Echocardiographically determined atherosclerosis severity and event history, especially in combination, yielded the best selection strategies for statin treatment over a broad range of predetermined funding or required event reductions, surpassing conventional cardiovascular risk factors. From 26.8 statin-preventable events/1000 patients/year (assuming all patients treated), the best selection strategies could avoid: 24 with 66% of the cost for statin treatment (atherosclerosis and age criteria), 20.1 with <50% of the budget, 12.2 with <30% of the budget or 9.6 with <15% of the budget (using combinations of atherosclerosis and prior events), while conventional strategies without echo quantification of atherosclerosis were inferior., Conclusion: Non-invasive echocardiographic quantification of atherosclerosis improves efficiency and cost-effectiveness in statin treatment.
