Crossing moving obstacles requires different space-time adjustments compared with stationary obstacles. Our aim was to investigate gait spatial and temporal parameters in the approach and crossing phases of a moving obstacle. We hypothesized that obstacle speed affects gait parameters, which allow us to distinguish locomotor strategies. Ten young adults walked and stepped over an obstacle that crossed their way perpendicularly, under three obstacle conditions: control-stationary obstacle, slow (1.07m/s) and fast speed (1.71m/s) moving obstacles. Gait parameters were different between obstacle conditions, especially on the slow speed. In the fast condition, the participants adopted predictive strategies during the approach and crossing phases. In the slow condition, they used an anticipatory strategy in both phases. We conclude that obstacle speed affects the locomotor behavior and strategies were distinct in the obstacle avoidance phases. [ABSTRACT FROM AUTHOR]

This study focuses on the application of coconut fibre insulators, an insulating material rarely utilized in the Mediterranean context. Despite its undoubted thermal performance, some queries are related with of his thermo-hygrometric behavior. More precisely, during the use of coconut for covering building for realizing green roofs, which represent a technological solution often adopted in the case of sustainable buildings or nearly zero energy building. Green roofs represent a valid constructive solution with high thermal performances, adopted in existing and new buildings. This paper investigates the thermo-hygrometric behavior of the concrete and Cross Laminated Timbre slabs, insulated with coconut fibreboards (CF) such as an alternative synthetic insulator, referred to a series of different green roofs scenarios. The results show that coconut fibre insulations are equally comparable to natural and synthetic materials. Therefore, coconut fibre could represent a good chance for realization of green roof having high thermal performance and hygrothermal behavior in the same time. This material could be an alternative solution to the normal synthetic materials actually used, in a perspective of sustainable architecture.

Abstract Introduction Postmenopausal endometriosis is a rare form of a common disease, since the absence of estrogenic hormone production should halt disease progression. Case presentation We present the case of a 54-year-old Italian Caucasian woman in surgical menopause with a history of ovarian endometriosis, who underwent voluntary hormone replacement therapy for seven years. She developed postrenal renal failure due to bilateral compression of the pelvic ureteral tract caused by two large, deeply infiltrating endometriotic nodules with no pelvic pain. She underwent operative laparoscopy with adhesiolysis of enteroenteric adhesions and excision of the endometriotic nodules encompassing the juxtavesical tract of the ureters, without obtaining improvement of renal failure. Conclusion Postmenopausal endometriosis can manifest itself in an unpredictable and potentially very serious manner. It is therefore important to carefully evaluate the risks and benefits of administering hormone replacement therapy to patients with previous endometriosis.

Objective: To determine the effects of aerobic training in randomized controlled clinical trials on functional capacity, motor symptoms, and oxygen consumption in individuals with Parkinson disease (PD) through a systematic literature review and meta-analysis., Data Sources: PUBMED, Web of Science, CINAHL, SciELO, and Medline databases were searched to identify published studies until September 2023., Study Selection: Randomized controlled clinical trials that evaluated the long-term effect of aerobic exercise in individuals with PD were included., Data Extraction: Two independent reviewers extracted the data and assessed the risk of bias and the Grading of Recommendation Assessment, Development, and Evaluation. In case of disagreement, a third reviewer was consulted., Data Synthesis: Thirteen studies were included in the systematic review, and the number of participants was 588 with an average age of 66.2 years (57-73y). The study's exercise intervention lasted between 6 and 70 weeks, with most studies lasting 10-12 weeks, with 3 sessions per week and an average duration of 47 minutes per session. The meta-analysis revealed that aerobic exercise is effective in enhancing maximal oxygen uptake (standardized mean difference, SMD 0.42 [95% CI, 0.18, 0.66; P=.0007]) and functional capacity (SMD 0.48 [95% CI, 0.24-0.71; P<.0001]). In addition, aerobic exercise can reduce the motor-unified Parkinson disease rating scale (mean difference-2.48 [95% CI, -3.16 to -1.81; P<.00001]) score in individuals with PD., Conclusions: Aerobic exercise training conducted 2-3 times a week, with different intensities (low to high), can be an effective intervention for enhancing functional capacity, maximizing oxygen uptake, and reducing the UPDRS scores in individuals with PD., (Copyright © 2024 American Congress of Rehabilitation Medicine. Background and Aim: Sit-stand desks allow individuals to work in either sitting or standing position. While previous studies have reported better performance on the attention network test (ANT) while standing compared to sitting, the relationship between body sway induced by these positions and ANT performance remains unclear. In this study, we aimed to test and expect benefits of body sway (in terms of magnitude and complexity) and improvements in ANT performance when standing (e.g. shorter reaction time) but not when sitting., Methods: Seventeen young adults (mean age = 21) performed reading tasks, questionnaires and ANTs sequentially in both standing and sitting positions. We measured body kinematics from the head, upper back and lower back during the study. We calculated the linear (i.e., velocity) and nonlinear (i.e., fractal dimension) variables of body sway, reaction times and alerting scores from the ANT. Our results showed that when standing, the complexity of sway was significantly negatively correlated with ANT reaction times (shorter reaction time indicating better performance) and significantly positively correlated with the scores of alerting from ANT. Hence, consistent with our expectation, ANT performance was higher when standing potentially because participants adjusted their sway. In contrast, while sitting, there was no significant correlation between body sway and ANT performance. Overall, the complexity of body sway in the standing position may increase alertness levels, potentially leading to better visual task performance. Background : Overweight may present an additional challenge when crossing obstacles. More specifically it may affect adequate foot clearance to reduce the risk of obstacle contact. Thus, the objective of this study was to compare obstacle clearance and spatial-temporal gait parameters during obstacle crossing in young adults with normal body weight and overweight. Methods : Twenty-eight and fifteen individuals were categorized into normal body mass index (18.5-25 kg/m 2 ) and overweight (25-30 kg/m 2 ), respectively. The participants walked along a walkway at their preferred speed and stepped over an obstacle. Spatial-temporal parameters were calculated during the approaching (stride before obstacle) and the crossing (step over the obstacle) phases. Additionally, the leading and trailing foot placements prior to and after the obstacle and toe clearance were calculated. Results : No significant differences were found for the approach, the crossing phases and leading and trailing toe clearance. Analysis of foot placement distance prior to and after the obstacle showed that, compared to the individuals with normal body weight, overweight individuals significantly increased the leading foot placement distance prior to the obstacle (+7 cm, ↑ 6.7%) and increased the trailing foot placement distance after the obstacle (+8.1 cm, ↑ 9%). Conclusions : Our findings indicated that overweight individuals have a different obstacle crossing behavior regarding foot placement distance prior to and after the obstacle compared to normal-weight individuals without differences in spatial-temporal gait parameters or toe clearances. However, the results did not suggest that participants with overweight show a higher risk of tripping.

Objective: The goal of this review was to investigate factors associated with physical activity and sedentary behavior in people with Parkinson disease (PD). The magnitude of these associations was investigated in line with the International Classification of Functioning, Disability and Health components., Methods: A systematic literature review was conducted until February 2023, searching four databases (PubMed, EMBASE, Web of Science, and Scopus) for original articles investigating associations with physical activity or sedentary behavior in people with PD. Two independent researchers performed data extraction, and the risk of bias in the included studies was assessed using the Quality in Prognosis Studies tool. Meta-analyses were conducted to determine the magnitude of the associations, and significant regression models from the included studies were described., Results: Forty-two studies were included. Twenty-one factors associated with overall physical activity were identified. Higher levels of physical activity had a small association with cognition and body mass index and a fair association with 17 factors related to self-efficacy, physical function, mobility, quality of life, age, PD symptoms, and more. Better manual dexterity and functional gait had moderate to good associations with higher levels of physical activity. The regression model with the higher magnitude was composed mostly of contextual factors, except for the body max index. The magnitude of factors associated with physical activity intensity or sedentary behavior could not be identified., Conclusion: Functional gait and manual dexterity were the strongest factors related to physical activity in people with PD. Further investigation is needed to understand the factors associated with physical activity intensity and sedentary behavior., Impact: This study emphasizes the significance of considering contextual factors alongside body function and structure, activity and participation, and the health condition to enhance physical activity improvement during the rehabilitation process. People with Parkinson's disease experience reduced sleep quality compared with their peers. Levodopa may have a direct effect on sleep macrostructure or may improve sleep by enhancing nocturnal motor performance. Therefore, it is important to understand the acute effects of withdrawing levodopa on sleep measures in Parkinson's disease. The purpose of this study was to compare the estimated objective and subjective sleep measures of people with Parkinson's disease sleeping under (ON-night) versus without (OFF-night) the effects of the last daily dopaminergic medication before going to bed. A total of 23 people with Parkinson's disease were instructed to wear an actigraphy device for 4 consecutive nights to objectively measure the sleep behaviour. Subjective sleep measure was assessed each morning using a Likert scale. Participants slept for 3 nights on ON-night and 1 night on OFF-night. They were instructed not to take their last dose of levodopa before going to bed in OFF-night. Sleeping in ON- versus OFF-night increased total sleep time (7.8%, p = 0.032) and sleep efficiency (3.7%, p = 0.019), and decreased duration and number of wakes after sleep onset (22.3%, p = 0.050; and 29.2%, p = 0.013, respectively). However, subjective sleep analysis indicated no significant differences between the two conditions. From a clinical point of view, our results suggest that sleeping on ON-night resulted in an improvement in estimated objective sleep measures compared with sleeping on OFF-night. Mental fatigue can affect cognitive function and interfere with motor performance. We examined if mental fatigue affected gait through age-specific modulation of wavelet-based time-frequency intermuscular beta-band coherence in muscles while walking on a treadmill at 1.2 m·s-1. The Psychomotor Vigilance Task, and the AX-Continuous Performance and the Stroop tests were used to induce mental fatigue in groups of healthy young and older participants. Mental fatigue reduced stance time, stride length, and marginally step width and increased cadence, stride length and stance time variability. In older compared with young participants before the induction of mental fatigue, wavelet-based time-frequency intermuscular beta-band coherence measured during walking was lower in the tibialis-peroneus and tibialis-gastrocnemius muscle pairs in specific phases of the gait cycle. In both age groups, after induction of mental fatigue, selected clusters of wavelet-based time-frequency intermuscular beta-band coherence measured during walking increased in the biceps-semitendinosus, rectus-vastus, tibialis-peroneus, gastrocnemius-soleus, and tibialis-gastrocnemius muscle pairs. Purpose: To verify the effect of resonance tube voice therapy on the vocal aspects of patients with Parkinson's Disease (PD)., Method: Intra-subject comparative controlled clinical trial with a single group assignment. Fourteen individuals with PD (10 men, mean age 66.1 years; four women, mean age 73.75 years) received eight 45-minute sessions of voice therapy, twice a week for 4 weeks. The therapy consisted of semi-occluded vocal tract exercises - phonation method in a resonance tube (glass, 27 cm x 9 mm) in water. Tube depth in water ranged from 2 cm to 9 cm, as the difficulty in carrying out the exercises increased (usual pitch, high pitch, low pitch, ascending/descending glissandos), followed by sentence production. The assessments were made three times: at baseline (Time0), after 30 days without intervention (Time1), and 1 day after eight intervention sessions (Time2). The following aspects were assessed: vocal intensity; acoustic parameters (Smoothed Cepstral Peak Prominence - CPPs, alpha ratio, and L1-L0 difference); auditory-perceptual analysis of the overall degree of vocal quality deviation; voice symptoms (Voice Symptom Scale protocol - VoiSS) and voice-related quality of life (Voice-Related Quality of Life Protocol - V-RQOL). The results were compared at the three times of assessment Time0/Time1/Time2 using one-way repeated measures ANOVA test and Tukey test (5% significance)., Results: intervention significantly increased: vocal intensity, L1-L0 value of vowel /a/ and counting, CPP value in counting, and decreased: the overall degree of vocal quality deviation in 78% of participants, the total score of VoiSS protocol, the limitation, and emotional subscales. Parkinson's disease (PD) is a progressive neurodegenerative disorder, involving motor and non-motor symptoms (NMS). Sleep disturbances (SD) are the second most common NMS in PD and include rapid eye movement (REM) sleep behavior disorder (RBD), excessive daytime sleepiness and insomnia. Freezing of gait (FOG) is a gait impairment frequently reported in people with PD greatly hampering functional independence and quality of life. Presence of FOG has been associated with increased frequency and severity of NMS, including SD. Thus, the aim of this study was to systematically review the literature comparing the number of people with FOG in PD with (PD + SD) and without SD (PD-SD). By systematically searching PubMed and Web of Science databases to identify original peer-reviewed articles, 8 studies including 5251 people with PD (2025 PD + SD and 3226 PD-SD) met eligibility criteria and were included in the review. In 6 studies (4 studies investigating RBD, 2 studies investigating overall sleep quality), the group of PD + SD had higher prevalence of FOG compared with PD-SD. Although a limited number of studies, our findings suggest that PD + SD present more frequently FOG than PD-SD. We aimed to evaluate the intra-session relative and absolute reliability of obstacle-crossing parameters during overground walking in young adults, and to determine the number of trials required to ensure reliable assessment. We analysed data from 43 young male adults who were instructed to walk at a self-selected velocity on a pathway and to step over an obstacle (height = 15 cm; width = 80 cm, thickness = 2 cm) three times. Spatial-temporal gait parameters of the approaching and crossing phases (i.e., before and after the obstacle) and obstacle clearance parameters (i.e., vertical and horizontal distance between the foot and the obstacle during crossing) were computed using a three-dimensional motion analysis system. Intraclass correlation coefficients were used to compute the relative reliability, while standard error of measurement and minimal detectable change were used to assess the absolute reliability for all possible combinations between trials. Results showed that most spatial-temporal gait parameters and obstacle clearance parameters are reliable using the average of three trials. However, the mean of the second and third trials ensures the best relative and absolute reliabilities of most obstacle-crossing parameters. Further works are needed to generalize these results in more realistic conditions and in other populations.

Aerobic exercise with the correct intensity can attenuate motor and non-motor symptoms of Parkinson's disease (PD) and improve the quality of life. However, a specific, validated, non-invasive, and outside the laboratory protocol that assesses physiological variables to prescribe optimal aerobic exercise intensity for people with PD is nonexistent. Therefore, this study aimed to propose a protocol, the Parkinson's critical heart rate test (Parkinson-CHR test), to determine the critical heart rate (CHR) in individuals with PD and verify its validity, reliability, and sensitivity. Fifteen people with idiopathic PD, who were able to practice exercises, were recruited to participate in the study (71.1 ± 6.6 years). The study consisted of two experiments: i) the first one aimed to assess the validity and reliability of the protocol, with participants performing the test twice at a one-week interval; ii) the second experiment aimed to investigate the protocol sensitivity, with individuals being evaluated before and after an 8-week training program according to Parkinson-CHR intensity. In experiment 1, no differences between test and retest were observed in the time to cover the distances (400, 800 and 1200 m), the total heart rate, the critical heart rate, and critical speed (p > 0.05). In experiment 2, there was a reduction in time to cover 400 and 800 m as well as in the total heart rate for all distances after the 8-week training program. Gait impairments negatively affect the quality of life of people with Parkinson's disease (PwPD). Aerobic exercise (AE) is an alternative to alleviate these impairments and its combination with transcranial direct current stimulation (tDCS) has demonstrated synergistic effects. However, the effect of multitarget tDCS application (i.e., motor, and prefrontal cortices simultaneously) combined with physical exercise on gait impairments is still little known. Thus, the proposed randomized clinical trial will verify the acute effects of AE combined with tDCS applied on motor and prefrontal cortices separately and simultaneously on gait (spatial-temporal and cortical activity parameters) in PwPD. Twenty-four PwPD in Hoehn & Yahr stages I-III will be recruited for this crossover study. PwPD will practice AE on treadmill simultaneously with the application of anodal tDCS during four intervention sessions on different days (∼ one week of interval). Active tDCS will be applied to the primary motor cortex, prefrontal cortex, and both areas simultaneously (multitarget), with an intensity of 2 mA for 20 min. For sham, the stimulation will remain at 2 mA for 10 s. The AE will last a total of 30 min, consisting of warm-up, main part (20 min with application of tDCS), and recovery. Exercise intensity will be controlled by heart rate. Spatial-temporal and cortical activity parameters will be acquired before and after each session during overground walking, walking with obstacle avoidance, and walking with a cognitive dual task at self-preferred velocity. An accelerometer will be positioned on the fifth lumbar vertebra to obtain the spatial-temporal parameters (i.e., step length, duration, velocity, and swing phase duration). Prefrontal cortex activity will be recorded from a portable functional near-infrared spectroscopy system and oxygenated and deoxygenated hemoglobin concentrations will be analyzed. Two-way ANOVAs with repeated measures for stimulation and moment will be performed. The findings of the study may contribute to improving gait in PwPD. Background: Due to improvement in movement performance, post-activation performance enhancement (PAPE) may open new possibilities to improve gait performance. However, no study has attempted to translate this phenomenon into walking. Therefore, the study aimed to test whether acute stretching followed by a conditioning exercise can improve subsequent gait performance in healthy adults., Research Question: Can an exercise protocol subsequently improve gait performance?, Methods: Sixteen individuals walked four 10-m trials (in each period) before and after 7 min of an exercise protocol composed of stretching (focusing on the lower limb) and a conditioning exercise (standing calf-raise wearing a vest of 20 kg). Gait spatialtemporal parameters and muscle activity of tibialis anterior and gastrocnemius medialis and lateralis muscles were obtained by a 3D-motion system and wireless electromyography, respectively. Before and after the exercise protocol, kinematic and muscle activity parameters were compared by a one-way ANOVA and the Wilcoxon signed-rank test, respectively., Results: After the exercise protocol, the participants walked with a faster step velocity ( p  < 0.018) and with a lower step duration ( p  < 0.025). Parkinson's disease (PD) is a neurodegenerative disorder characterized by a range of motor and non-motor symptoms. Among the motor complaints, freezing of gait (FOG) is a common and disabling phenomenon that episodically hinders patients' ability to produce efficient steps. Concurrently, sleep disorders are prevalent in PD and significantly impact the quality of life of affected individuals. Numerous studies have suggested a bidirectional relationship between FOG and sleep disorders. Therefore, our objective was to systematically review the literature and compare sleep outcomes in PD patients with FOG (PD + FOG) and those without FOG (PD-FOG). By conducting a comprehensive search of the PubMed and Web of Science databases, we identified 20 eligible studies for inclusion in our analysis. Our review revealed that compared to PD-FOG, PD + FOG patients exhibited more severe symptoms of rapid eye movement sleep behavior disorder in nine studies, increased daytime sleepiness in eight studies, decreased sleep quality in four studies, and more frequent and severe sleep disturbances in four studies. These findings indicate that PD + FOG patients generally experience worse sleep quality, higher levels of daytime sleepiness, and more disruptive sleep disturbances compared to those without FOG (PD-FOG). People with Parkinson's disease (pwPD) have reduced adaptability to postural control during prolonged standing compared to neurologically healthy individuals (control). Objective. The study aimed to characterize postural changes during prolonged standing and their effect on postural control in pwPD compared to control. We recorded the body sway of the second lumbar vertebra of 23 pwPD and 23 control while they performed prolonged standing (15 min). The number and amplitude of the body sway patterns (shifts, fidgets, and drifts), the root mean square, velocity, and frequency of the body sway were analyzed. The number of shifts in the anterior-posterior (AP) and medial-lateral (ML) directions was greater for the pwPD than the control. In addition, the amplitudes of shifts in the AP direction and fidgets in the AP and ML directions were greater for the pwPD than the control. Our results show that: (1) A larger number of shifts of body sway suggest references positions are frequently changing; (2) Fidgets is a pumping mechanism and can be sensory-demand action to restore mechanoreceptors activity on the foot sole; and (3) No drift changes may suggest there is no slow migration of reference position. Hypoxia exposure may promote neuroprotection for people with Parkinson's disease (PwPD). However, to implement hypoxia in practical settings and direct future research, it is necessary to organize the current knowledge about hypoxia responses/effects in PwPD. Thus, the present scoping review elucidates the evidence about hypoxia exposure applied to PwPD. Following the PRISMA Extension for Scoping Reviews, papers were searched in PubMed/NCBI, Web of Science, and Scopus (descriptors: Parkinson and hypoxia, mountain, or altitude). We included original articles published in English until August 12, 2023. Eight studies enrolled participants with early to moderate stages of disease. Acute responses demonstrated that PwPD exposed to normobaric hypoxia presented lower hypoxia ventilatory responses (HVR), perceptions of dyspnea, and sympathetic activations. Cumulative exposure to hypobaric hypoxia (living high; 7 days; altitude not reported) induced positive effects on motor symptoms (hypokinesia) and perceptions of PwPD (quality of life and living with illness). Normobaric hypoxia (isocapnic rebreathe, 14 days, three times/day of 5-7 min at 8-10 % of O2) improved HVR. The included studies reported no harmful effects. Although these results demonstrate the effectiveness and safety of hypoxia exposure applied to PwPD, we also discuss the methodological limitations of the selected experimental design (no randomized controlled trials), the characterization of the hypoxia doses, and the range of symptoms investigated. Background: Diabetic older people tend to present deteriorated performance in balance and locomotion activities, even those without peripheral neuropathy. There is evidence that saccadic eye movements are used to reduce body sway in young and older healthy adults, but it has not been shown that diabetic older people preserve this visuomotor adaptation capacity., Research Question: Are diabetic older women without peripheral neuropathy capable of improving postural stability during a saccadic gaze task?, Methods: Seventeen type 2 diabetic older women (68.2 ± 10.7 years old) and seventeen healthy women, age-matched controls (66.0 ± 8.4 years old) voluntarily participated in the study. All participants were instructed to stand upright, barefoot, as stable as possible, for 30 s. Participants maintained their feet parallel to each other, at standard and narrow bases of support, while either fixating on a stationary target (fixation condition) or performing horizontal saccadic eye movements to follow a target (eccentricity of 11° of visual angle), which continuously disappeared and reappeared immediately on the opposite side (saccade 0.5 Hz and saccade 1.1 Hz conditions)., Results: Results indicated that the diabetic group clearly had deteriorated postural control, as shown by increased values of mean sway amplitude and mean sway velocity. However, diabetic and control groups were similarly capable of using saccadic eye movements to improve their postural stability, reducing their sway velocity compared to a gaze fixation condition., Significance: Diabetes per se (without peripheral neuropathy) amplifies postural sway of older women as compared to their healthy age-matched controls. The imbalance in the concentration of metallic nanoparticles has been demonstrated to play an important role in multiple sclerosis (MS), which may impact cognition. Biomarkers are needed to provide insights into the pathogenesis and diagnosis of MS. They can be used to gain a better understanding of cognitive decline in people with MS (pwMS). In this study, we investigated the relationship between the blood concentration of metallic nanoparticles (blood nanoparticles) and cognitive performance in pwMS. First, four mL blood samples, clinical characteristics, and cognitive performance were obtained from 21 pwMS. All participants had relapse-remitting MS, with a score of ≤4.5 points in the expanded disability status scale. They were relapse-free in the three previous months from the day of collection and had no orthopedic, muscular, cardiac, and cerebellar diseases. We quantified the following metallic nanoparticles: aluminum, chromium, copper, iron, magnesium, nickel, zinc, and total concentration. Cognitive performance was measured by mini-mental state examination (MMSE) and the symbol digit modalities test (SDMT). Pearson's and Spearman's correlation coefficients and stepwise linear regression were calculated to assess the relationship between cognitive performance and blood nanoparticles. We found that better performance in SDMT and MMSE was related to higher total blood nanoparticles (r = 0.40; p < 0.05). Also, better performance in cognitive processing speed and attention (SDMT) and mental state (MMSE) were related to higher blood iron (r = 0.44; p < 0.03) and zinc concentrations (r = 0.41; p < 0.05), respectively. The other metallic nanoparticles (aluminum, chromium, copper, magnesium, and nickel) did not show a significant relationship with the cognitive parameters ( p > 0.05). Linear regression estimated a significant association between blood iron concentration and SDMT performance. In conclusion, blood nanoparticles are related to cognitive performance in pwMS. Our findings suggest that the blood concentration of metallic nanoparticles, particularly the iron concentration, is a promising biomarker for monitoring cognitive impairment in pwMS.

This study aimed to systematically review and summarize the available data regarding the influence of overweight and obesity across the lifespan on obstacle crossing during walking. Four databases were systematically searched with no limitation on publication date following the Cochrane Handbook for Systematic Reviews and PRISMA guidelines. Only full-text English-language articles published in a peer-reviewed journal were eligible. They had to compare obstacle crossing during walking by overweight or obese individuals with individuals of normal body weight. Five studies were considered eligible. All the studies assessed kinematics; only one assessed kinetics, but none investigated muscle activity or obstacle contact. Compared to normal individuals crossing obstacles, overweight or obese individuals exhibited lower velocity, shorter step length, lower cadence, and less time spent in single-limb support. They also exhibited increased step width, more time spent in double support, and greater trailing leg ground force reaction and centre of mass acceleration. Overall, the small number of included studies did not allow us to draw any conclusions. However, being overweight or obese seems to have a potentially negative influence on the kinematics of gait parameters due to a tendency to trip, fall, and suffer severe fall-related injuries when negotiating obstacles on foot in real-life environments.

(1) Background: Stroke is one of the leading causes of disability. To identify the best treatment strategies for people with stroke (PwS), the aim of the current study was to compare the effects of training on a treadmill with functional electrical stimulation (TT-FES) with training on a treadmill (TT), and to analyze the effects of sequence of training on mobility and the parameters of walking ability. (2) Methods: Prospective, longitudinal, randomized and crossover study, in which 28 PwS were distributed into groups, namely the A-B Group (TT-FES followed by TT) and B-A Group (TT followed by TT-FES), using the foot drop stimulator, and were measured with functional tests. (3) Results: We found improved mobility, balance, non-paretic limb coordination, and endurance only in the group that started with TT-FES. However, sensorimotor function improved regardless of the order of training, and paretic limb coordination only improved in the B-A Group, but after TT-FES. These data indicate that the order of the protocols changed the results. (4) Conclusions: Although biomechanical evaluation methods were not used, which can be considered a limitation, our results showed that TT-FES was superior to isolated training on a treadmill with regard to balance, endurance capacity, and coordination of the non-paretic limb.

Background: Gait asymmetry and deficits in gait initiation (GI) are among the most disabling symptoms in people with Parkinson's disease (PwPD). Understanding if PwPD with reduced asymmetry during GI have higher asymmetry in cortical activity may provide support for an adaptive mechanism to improve GI, particularly in the presence of an obstacle., Objective: This study quantified the asymmetry of anticipatory postural adjustments (APAs), stepping parameters and cortical activity during GI, and tested if the presence of an obstacle regulates asymmetry in PwPD., Methods: Sixteen PwPD and 16 control group (CG) performed 20-trials in two conditions: unobstructed and obstructed GI with right and left limbs. We measured, through symmetry index, (i) motor parameters: APAs and stepping, and (ii) cortical activity: the PSD of the frontal, sensorimotor and occipital areas during APA, STEP-I (moment of heel-off of the leading foot in the GI until the heel contact of the same foot); and STEP-II (moment of the heel-off of the trailing foot in the GI until the heel contact of the same foot) phases., Results: Parkinson's disease showed higher asymmetry in cortical activity during APA, STEP-I and STEP-II phases and step velocity (STEP-II phase) during unobstructed GI than CG. However, unexpectedly, PwPD reduced the level of asymmetry of anterior-posterior displacement ( p < 0.01) and medial-lateral velocity ( p < 0.05) of the APAs. Also, when an obstacle was in place, PwPD showed higher APAs asymmetry (medial-lateral velocity: p < 0.002), with reduced and increased asymmetry of the cortical activity during APA and STEP-I phases, respectively., Conclusion: Parkinson's disease were not motor asymmetric during GI, indicating that higher cortical activity asymmetry can be interpreted as an adaptive behavior to reduce motor asymmetry. This study verified whether mechanical variables influence the anaerobic capacity outcome on treadmill running and whether these likely influences were dependent of running experience. Seventeen physical active and 18 amateur runners, males, performed a graded exercise test and constant load exhaustive running efforts at 115% of intensity associated to maximal oxygen consumption. During the constant load were determined the metabolic responses (i.e., gas exchange and blood lactate) to estimate the energetic contribution and anaerobic capacity as well as kinematic responses. The runners showed higher anaerobic capacity (16.6%; p = 0.005), but lesser time to exercise failure (-18.8%; p = 0.03) than active subjects. In addition, the stride length (21.4%; p = 0.00001), contact phase duration (-11.3%; p = 0.005), and vertical work (-29.9%; p = 0.015). For actives, the anaerobic capacity did not correlate significantly with any physiologic, kinematic, and mechanical variables and no regression model was fitted using the stepwise multiple regression, while to runners the anaerobic capacity was significantly correlated with phosphagen energetic contribution (r = 0.47; p = 0.047), external power (r = -0.51; p = 0.031), total work (r = -0.54; p = 0.020), external work (r = -0.62; p = 0.006), vertical work (r = -0.63; p = 0.008), and horizontal work (r = -0.61; p = 0.008), and the vertical work and phosphagen energetic contribution presented a coefficient of determination of 62% (p = 0.001). Based on findings, it is possible to assume that for active subjects, the mechanical variables have no influence over the anaerobic capacity, however, for experienced runners, the vertical work and phosphagen energetic contribution have relevant effect over anaerobic capacity output., (© 2023 The Authors. The effects of a cooling strategy following repeated high-intensity running (RHIR) on soccer kicking performance in a hot environment (>30ºC) were investigated in youth soccer players. Fifteen academy under-17 players participated. In Experiment 1, players completed an all-out RHIR protocol (10×30 m, with 30s intervals). In Experiment 2 (cross-over design), participants performed this running protocol under two conditions: (1) following RHIR 5 minutes of cooling where ice packs were applied to the quadriceps/hamstrings, (2) a control condition involving passive resting. Perceptual measures [ratings of perceived exertion (RPE), pain and recovery], thigh temperature and kick-derived video three-dimensional kinematics (lower limb) and performance (ball speed and two-dimensional placement indices) were collected at baseline, post-exercise and intervention. In Experiment 1, RHIR led to small-to-large impairments ( p  < 0.03;d = -0.42--1.83) across perceptual, kinematic and performance measures. In experiment 2, RPE ( p  < 0.01; Kendall's W = 0.30) and mean radial error ( p  = 0.057; η 2  = 0.234) increased only post-control. Significant small declines in ball speed were also observed post-control ( p  < 0.05; d = 0.35). Post-intervention foot centre-of-mass velocity was moderately faster in the cooling compared to control condition ( p  = 0.04; d = 0.60). In youth soccer players, a short cooling period was beneficial in counteracting declines in kicking performance, in particular ball placement, following intense running activity in the heat.

Background: The aim of this study was to investigate the effects of gradient and speed on running variability (RV) and local dynamic stability (LDS) during uphill running., Hypotheses: (1) Both gradient and speed increase metabolic effort, in terms of heart rate (HR) and perceived exertion (CR10), in line with the contemporary literature, and (2) gradient increases RV and impairs LDS., Study Design: "Crossover" observational design., Level of Evidence: Level 3., Methods: A total of 25 runners completed 10-minute running trials in 3 different conditions and in a randomized order: gradient at 0% (0CON), 2% (2CON), and 2% at isoefficiency speed (2IES). 0CON and 2CON speeds were calculated as the "best 10-km race performance" minus 1 km·h -1 , whereas 2IES speed was adjusted to induce the same metabolic expenditure as 0CON. HR and perceived exertion as well as running kinematic variables were collected across all trials and conditions. Running variability was calculated as the standard deviation of the mean stride-to-stride intervals over 100 strides, while LDS was expressed by the Lyapunov exponent (LyE) determined on running cycle time over different running conditions., Results: Increases in HR and CR10 were observed between 0CON and 2CON ( P < 0.001) and between 2IES and 2CON ( P < 0.01). Higher RV was found in 2CON compared with 0CON and 2IES (both P < 0.001). Finally, the largest LyE was observed in 2IES compared with 0CON and 2CON ( P = 0.02 and P = 0.01, respectively)., Conclusion: Whereas RV seems to be dependent more on metabolic effort, LDS is affected by gradient to a greater extent., Clinical Relevance: Running variability could be used to monitor external training load in marathon runners.

The contribution of cortical activity (e.g. EEG recordings) in various brain regions to motor control during goal-directed manipulative tasks using lower limbs remains unexplored. Therefore, the aim of the current study was to determine the magnitude of associations between EEG-derived brain activity and soccer kicking parameters. Twenty-four under-17 players performed an instep kicking task (18 m from the goal) aiming to hit 1 × 1 m targets allocated in the goalpost upper corners in the presence of a goalkeeper. Using a portable 64-channel EEG system, brain oscillations in delta, theta, alpha, beta and gamma frequency bands were determined at the frontal, motor, parietal and occipital regions separately for three phases of the kicks: preparatory, approach and immediately prior to ball contact. Movement kinematic measures included segmental linear and relative velocities, angular joint displacement and velocities. Mean radial error and ball velocity were assumed as outcome indicators. A significant influence of frontal theta power immediately prior to ball contact was observed in the variance of ball velocity ( R 2  = 35%, P  = 0.01) while the expression of occipital alpha component recorded during the preparatory phase contributed to the mean radial error ( R 2  = 20%, P  = 0.049). Ankle eversion angle at impact moment likely mediated the association between frontal theta power and subsequent ball velocity (β = 0.151, P  = 0.06). The present analysis showed that the brain signalling at cortical level may be determinant in movement control, ball velocity and accuracy when performing kick attempts from the edge of penalty area. Purpose: The current study examined the possible relationships between one-off single night sleep metrics and subsequent kicking performance in a youth soccer context., Methods: Twenty-eight under-17 academy players (15.9 ± 0.8 years-old) completed a kick testing protocol consisting in 20 attempts, 18 m from the goal and against a goalkeeper. Four digital video cameras (240 Hz) allowed to determine 3-D approach run, lower limb and ball velocities. Two additional cameras (60 Hz) were used to calculate 2-D mean radial error, bivariate variable error and accuracy. Over 24 h prior to testing, players were monitored by wrist actigraphy to determine their sleep indices. Self-reported sleep quality, sleepiness and chronotype scale scores (Horne and Östberg morningness-eveningness questionnaire) were also collected immediately before kicking experiment., Results: Multiple linear regressions indicated that wake up time and chronotype contributed to 40% of mean radial error. Self-reported sleep quality influenced respectively on 19% and 24% of accuracy and bivariate variable error variances. Taken together self-reported sleep quality and wake up time explained 33% of accuracy (all p < 0.05). Indicators of kicking velocity were non-significantly correlated with sleep ( r = -0.30-0.29; p > 0.05)., Conclusion: One-off sleep measures showed some sensitivity to acutely detect inter-individual oscillations in kicking performance. Low perceived sleep quality, later wake up time and a chronotype toward evening preference seem either related to immediately subsequent worst ability of ball placement when kicking. Monitoring sleep-wake transition and perceived sleep quality may be important to help prevent acute performance declines in targeting the goal during kick attempts from the edge of penalty area.

Striking an obstacle while walking can be dangerous, reflecting the higher risks of losing one's balance, tripping and falling. Particular situations during which internal resources are limited, such as in a fatigued state, may impair performance when crossing obstacles, enhancing the risks of falls or accidents. Our goal was thus to review the effects of experimentally-induced fatigue (EIF) on gait parameters during obstacle crossing by healthy individuals. We systematically searched PubMed and Web of Science databases using 'fatigue', 'obstacle crossing' and their equivalent terms to extract data from studies investigating this domain. Nine studies were found. First, EIF-related effects on kinetics, EMG and obstacle contacts have been poorly studied. Second, consistent and inconsistent results were found in the kinematic outcomes after EIF. Consistent results included reductions in stride duration and increased step width. Inconsistent results included gait velocity (no-effect vs increased), leading and trailing-foot vertical clearance (reduced vs increased) and horizontal distance from foot to the obstacle before obstacle avoidance (no-effect vs increased). Background: It has been suggested that the protein Brain-derived Neurotrophic Factor (BDNF) plays a neuroprotective role in people with multiple sclerosis (pwMS). Also, BDNF seems to play a role in cognition performance. In the same line, gait in pwMS requires a higher cognitive resource, mainly during complex walking. Thus, maybe BDNF could be related to gait in pwMS., Objective: To investigate the relationship between BDNF and gait spatial-temporal parameters during unobstructed and obstructed conditions and the Timed Up and Go (TUG) in pwMS and healthy controls (HC)., Methods: The study included 20 pwMS (11F/9M, 33.1±7.5 years, Expanded Disability Status Scale- EDSS 2.2±1.2) and 18 HC (13F/5M, 35.5±5.9 years). Both groups performed 20 gait attempts in two conditions: unobstructed walking (10 trials) and avoiding an obstacle. The obstacle was 15 cm in height and made of foam material. The BDNF serum concentration was collected with participants in fasting and completed before the clinical, gait, and mobility assessments. Clinical variables included the Symbol Digit Modality Test (SDMT), the Fatigue Severity Scale (FSS), and the International Physical Activity Questionnaire (IPAQ- short version). Associations between BDNF and spatial-temporal gait parameters, clinical variables, and TUG were determined by Pearson/Spearman correlations with Bonferroni's correction being applied (p<0.0013). Gait was compared by a two-way, repeated-measures ANOVA (group and condition) to characterize our cohort., Results: Reduced BDNF was observed for pwMS (41.66±4.45 ng/ml) in comparison with HC (61.67±7.07, p<0.001). However, although some correlations presented a moderate correlation between BDNF with gait variables, the correlations didn't reach a significant p-value after Bonferroni's correction. Lastly, pwMS presented shorter step length and slower step velocity for both gait conditions, with more evidence for obstacle conditions. Only pwMS changed gait behavior from unobstructed walking to obstacle avoidance conditions (i.e., reduced step length and velocity and increased step duration)., Conclusion: BDNF is not related to either clinical (i.e., EDSS, SDMT, FSS, or IPAQ) or gait parameters in pwMS and HC, even in a condition involving higher cognitive demand. Background: Parkinson's disease (PD) is a chronic and progressive neurodegenerative disease with no cure, presenting a challenging diagnosis and management. However, despite a significant number of criteria and guidelines have been proposed to improve the diagnosis of PD and to determine the PD stage, the gold standard for diagnosis and symptoms monitoring of PD is still mainly based on clinical evaluation, which includes several subjective factors. The use of machine learning (ML) algorithms in spatial-temporal gait parameters is an interesting advance with easy interpretation and objective factors that may assist in PD diagnostic and follow up., Research Question: This article studies ML algorithms for: i) distinguish people with PD vs. matched-healthy individuals; and ii) to discriminate PD stages, based on selected spatial-temporal parameters, including variability and asymmetry., Methods: Gait data acquired from 63 people with PD with different levels of PD motor symptoms severity, and 63 matched-control group individuals, during self-selected walking speed, was study in the experiments., Results: In the PD diagnosis, a classification accuracy of 84.6 %, with a precision of 0.923 and a recall of 0.800, was achieved by the Naïve Bayes algorithm. We found four significant gait features in PD diagnosis: step length, velocity and width, and step width variability. As to the PD stage identification, the Random Forest outperformed the other studied ML algorithms, by reaching an Area Under the ROC curve of 0.786. The Physical Activity Scale for Individuals with Physical Disabilities (PASIPD) is not available to Portuguese-Brazil. This study translates, cross-culturally adapts, and validates the PASIPD for Brazilian individuals with Parkinson's disease. The translation process followed guidelines: initial translation, synthesis, back translation, expert committee, and pretest. The validation and reliability processes were conducted with 40 individuals (15 men and 25 women) with Parkinson's disease. Concurrent validity was evaluated between PASIPD to Brazilian Portuguese, International Physical Activity Questionnaire, and Human Activity Profile. PASIPD to Brazilian Portuguese was found to be moderately correlated with International Physical Activity Questionnaire (r = .474, p < .05); however, there was no correlation with Human Activity Profile (r = .271, p < .05). We used the intrarater reliability with intraclass correlation coefficient and test-retest. Intrarater reliability was high (intraclass correlation coefficient = .80). Internal consistency was considered adequate by Cronbach's alpha (α = .70). PASIPD to Brazilian Portuguese is a valid and reliable instrument for evaluating physical activity levels in Brazilian individuals with Parkinson's disease.

The aim of this study was to investigate the effects of PD and ageing on gaze behaviour and performance of drivers in a simulated task. Ten drivers with PD, ten neurologically healthy older drivers, and ten neurologically healthy younger adult drivers were asked to drive in a car simulator for three minutes, maintaining car speed between 100 and 120 km/h and avoiding collisions. Driver's eye movements were recorded. Drivers with PD had more collisions and spent less time driving within the speed zone than the younger-drivers. Drivers with PD performed an increased number of fixations towards task-irrelevant areas of the visual scene and higher visual entropy, indicating a more random gaze behaviour. Older drivers restricted their visual search to the lane area in order to detect threat-related stimuli. PD led to drops in performance of drivers in the car simulator. Practitioner summary: Parkinson's disease (PD) and ageing process caused a drop in driving performance. Drivers with PD made fewer fixations on task-relevant information and showed higher visual entropy than young adults. Older drivers restricted their visual search to the lane than other areas of interest.

Background: Postural response impairments in postural instability and gait disorders (PIGD) subtype patients may be attributed to Parkinson's disease (PD)-deterioration in central-set (programing/modulating of central outputs during motor responses). Although additional information improves some PD motor impairments, an unanswered question is whether additional information can benefit postural response in PIGD subtype., Objective: To analyze the effect of cumulative additional information on postural responses after perturbation in PIGD and neurologically healthy older adults (CG)., Methods: Perturbations were applied in 16 PIGD and 19 CG by the support-base translation. Participants performed 3 blocks of 5 trials without additional information (B1-B3, Day 1) and 5 trials of each cumulative additional information (C1-C4, Day 2): information about perturbation (C1), visual (C2), verbal (C3), and somatosensory information (C4). Electromyography and center of pressure (CoP) parameters were analyzed by ANOVAs with Group (PIGD × CG) and Block (B1 × B2 × B3) and with Group (PIGD × CG) and Condition (B3 × C1 × C2 × C3 × C4)., Results: PIGD decreased the range of CoP in B3 while CG decreased both range of CoP and the integral of antagonist's muscle activity (iEMG) in B2. Also, PIGD decreased the recovery time in C4 while CG increased the iEMG of agonist's muscle in C2 and antagonist's muscle in all conditions except C2., Conclusion: Additional information provided before postural control assessment influences the postural response in PIGD and CG differently. Background: Postural impairment is one of the most debilitating symptoms in people with Parkinson's disease (PD), which show faster and more variable oscillation during quiet stance than neurologically healthy individuals. Despite the center of pressure parameters can characterize PD's body sway, they are limited to uncover underlying mechanisms of postural stability and instability., Research Question: Do a multiple domain analysis, including postural adaptability and rambling and trembling components, explain underlying postural stability and instability mechanisms in people with PD?, Method: Twenty-four individuals (12 people with PD and 12 neurologically healthy peers) performed three 60-s trials of upright quiet standing on a force platform. Traditional and non-linear parameters (Detrended Fluctuation Analysis- DFA and Multiscale Entropy- MSE) and rambling and trembling trajectories were calculated for anterior-posterior (AP) and medial-lateral (ML) directions., Results: PDG's postural control was worse compared to CG, displaying longer displacement, higher velocity, and RMS. Univariate analyses revealed largely longer displacement and RMS only for the AP direction and largely higher velocity for both AP and ML directions. Also, PD individuals showed lower AP complexity, higher AP and ML DFA, and increased AP and ML displacement, velocity, and RMS of rambling and trembling components compared to neurologically healthy individuals., Significance: Based upon these results, people with PD have a lower capacity to adapt posture and impaired both rambling and trembling components compared to neurologically healthy individuals. Background: Certain aspects of gait may depend on cognition, and the effects of dual-tasking (DT) on gait asymmetry (GA) during obstacle circumvention in people with Parkinson's disease (pwPD) may be walking phase-dependent. While the approaching phase requires allocating significant cognitive resources to perform the task, outweighing the available resources may increase GA. During the avoiding phase, the obstacle could be used as an external cue, enhancing the attention to the obstacle and, consequently, improving gait control and regulating GA., Objective: The study investigated the effects of obstacle circumvention during gait with DT on GA in pwPD, considering the circumvention phase., Methods: Thirty participants, 15 pwPD and 15 neurologically healthy individuals, circumvented an obstacle 20 times (in total) according to side (right and left side) and DT (presence and absence). Spatial-temporal stride parameters, medial-lateral and horizontal trajectory deviations from the original path, strategy to obstacle circumvention (lead limb away from or close to the obstacle during the crossing step), and gaze parameters were calculated during the approaching and avoiding phases. The parameters were grouped considering the side that the obstacle was circumvented during each task, and the symmetry index was calculated., Results: The results showed greater asymmetry of time of fixations, width, duration, and velocity during the approaching phase (p < 0.02), whereas the avoiding phase decreased asymmetry of medial-lateral and horizontal trajectory deviations parameters (p < 0.001) during obstacle circumvention with DT, in both pwPD and control group., Conclusions: The findings offer a basis for considering that goal-directed movement control may regulate GA in pwPD., (Copyright © 2022 Elsevier B.V. Background: Automated mechanical peripheral stimulation (AMPS) is a rehabilitation technique suggested to correct gait abnormalities on Parkinson's disease. Although previous studies have suggested increments in functional performance and gait speed after AMPS intervention, little is known about its effect on gait biomechanics., Objective: To analyze the effect of an AMPS session on functional performance and gait biomechanics in subjects with Parkinson's disease., Methods: Twenty-eight subjects aged 67±3 years old participated in this study. Kinematics and muscle activation were recorded during walking at a preferred gait speed before and after AMPS and sham interventions. Footswitches sensors were used to record the kinematic parameters. Electromyographic (EMG) signals of tibialis anterior (TA) and gastrocnemius lateralis (GL) were recorded. Timed up and go (TUG) test and Short Physical Performance Battery (SPPB) were performed to assess functional performance., Results: GL activation increased after AMPS intervention before and after heel strike (p = 0.04; p < 0.01) and before and after toe-off (p = 0.013; p = 0.038). Also, after AMPS intervention, TA activation increased after heel strike (p = 0.007); and after sham intervention, TA activation reduced before and after heel strike (p = 0.038; and p = 0.007) and before toe-off (p = 0.013). The time of TUG test was shorter after AMPS intervention (p = 0.015)., Conclusion: AMPS intervention changed the EMG activation of ankle muscles during walking and functional performance. Background: Overweight and obesity are significant global health concerns that involve deficits in gait and balance that affect daily activities. Although much is reported about the effect of overweight and obesity on gait during unobstructed walking, not much is known about how overweight and obesity could impact gait under more challenging conditions, such as environments with obstacles., Objective: The aim of this study is to systematically review and synthesize the available data regarding the effects of overweight and obesity on obstacle crossing during walking., Methods: This review will follow the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) guidelines. PubMed, Web of Science, Scopus, and SPORTDiscus will be systematically searched with no limitations on publication date. Only full-text English-language articles published in a peer-reviewed journal will be included. Included articles must have compared obstacle crossing during walking in individuals with overweight or obesity to individuals of normal body weight. A total of 2 independent reviewers will select the articles and extract the following 4 sets of data: (1) study characteristics, (2) sample description, (3) obstacle crossing task protocol, and (4) main results obtained. If a considerable number of homogeneous papers are included, a meta-analysis will be conducted. A preliminary search was conducted in November 2021., Results: The results will include the article selection flowchart as well as tables and figures synthesizing the extracted data on the effects of overweight and obesity on obstacle crossing during walking. The preliminary search identified 73 original records, of which 5 articles met the inclusion criteria., Conclusions: This review will present researchers and clinicians with an overview of published studies that have compared the performance of obstacle crossing for individuals with overweight and obesity to those of normal body weight. Gaining insight into the control strategies adopted by individuals with overweight and obesity is critical for safe and successful obstacle crossing in this population. COVID-19 in Brazil is threatening, and it has forced the government to adopt partial lockdown as a strategy to stop the spread of the virus in the first wave of pandemic (March 2020). These preventive measures during the COVID-19 pandemic may affect the motor and non-motor symptoms in people with Parkinson's disease (PD) and Multiple sclerosis (MS). Thus, the purpose of this study was to investigate the perception during the first wave of COVID-19 pandemic lockdown on motor and non-motor symptoms, and also measure physical activity level, quality of life, and sleep quality in Brazilian people with PD and MS. One hundred and fifty-three participants (PD-97 and MS-56) answered an online survey to identify the perception of motor and non-motor symptoms, and characterize the physical activity level, and quality of life and sleep in these neurological Brazillian population. During the beginning of pandemic lockdown in Brazil, our results indicated that 69% of people with PD and 55% of people with MS reported worse on motor aspects and lower amount of physical activity performed. Also, 75.2% (PD) and 92.9% (MS) of our cohort were considered inactive or sedentary. Based on the perception and behavior of the population studied, people with PD and MS should be encouraged to perform more physical activity in order to reduce the effects of isolation in motor and non-motor aspects of the diseases. Establishing a relationship between gait and posture in patients with Parkinson's disease (PD) is essential for PD treatment and rehabilitation. While previous studies have indicated that gait and posture are independent domains in PD, shared neuromechanisms related to gait and posture control and previous studies investigating the relationship between gait and posture parameters in stroke survivors and neurologically healthy older adults have shown a correlated domain. Thus, this study analyzed the relationship of gait and posture domains, primarily through gait temporal sub-phases (i.e., double support and stance phases) and step width. We analyzed the spatial-temporal gait parameters at the self-selected velocity and center of pressure (CoP) during quiet standing of 22 idiopathic PD participants under and without dopaminergic medication conditions. The association between quiet standing and gait variables was assessed through the Spearman test, controlled by age, disease duration, NFoG-Q, and levodopa dosage. In ON medication, CoP area showed a significant correlation with stance phase and total double support; and RMS ML CoP showed a significant correlation with stance phase, total double support, and step width. In OFF medication, CoP area, RMS AP CoP, RMS ML CoP, and ML CoP velocity significantly correlated with stance phase and total double support. By showing the relationship between gait and posture domains in PD, our study adds novel knowledge about the shared gait-posture control, which could collaborate with new approaches during mobility treatment and assessment., (Copyright © 2022 Elsevier B.V. Background: The aim of this study was to assess hemodynamic and cardiac autonomic response to high-intensity interval exercise (HIIE) versus moderate-intensity continuous exercise (MICE) in individuals with Parkinson's disease (PD)., Methods: Twelve individuals (six men) with PD were randomly assigned to perform HIIE (4 min of warm-up followed by 21 min alternating 1 min at levels 15-17 with 2 min at levels 9-11 of rating of perceived exertion [RPE] in a cycle ergometer), MICE (4 min of warm-up followed by 26 min at levels 11-14 of RPE in a cycle ergometer) and control (CON; 30 min of sitting rest) interventions in separate days. Heart rate (HR), blood pressure (BP), endothelial reactivity and heart rate variability (HRV) were assessed before, immediately after and 45 min after each intervention. HR and exercise workload were measured during each intervention., Results: Despite the within (high- vs. low-intensity intervals of HIIE) and between (HIIE vs. MICE) differences in workload during exercise sessions, HR was not different between high- (average HR=98±18 bpm) and low-intensity (average HR 97±19 bpm) intervals of HIIE, as well as between HIIE (average HR=97±18 bpm) and MICE (average HR=93±19 bpm) throughout the exercise. There were significant, but small, increases (P<0.01) in HR and systolic BP at post HIIE and MICE, which returned to levels similar to preintervention during recovery. There were no within- and between-intervention differences in diastolic BP, endothelial reactivity and HRV., Conclusions: The present results suggest that hemodynamic response to exercise is impaired in individuals with PD.

This study aimed to investigate if the impairing in postural control, induced by ankle fatiguing exercise, remains after 24/48 h in young adults. Center of Pressure (CoP) was assessed in 16 participants (23 ± 3 years old) before, immediately after an ankle fatigability induction protocol (FI) and after 24 or 48 h of recovery using two 60-s trials with eyes open (EO) and closed (EC). The FI consisted of performing the ankle plantar flexion and dorsiflexion movement repeatedly (0.5 Hz). Ankle muscle fatigability increased CoP anterior-posterior (AP - p < 0.02) and medial-lateral (ML - p < 0.009) root mean square (RMS), and AP (p < 0.01) mean velocity immediately after compared to before FI. These effects remained after 24/48 h of recovery: higher CoP AP (p < 0.03) and ML (p < 0.009) RMS. No significant effects for detrend fluctuation analysis and entropy analysis among periods of postural evaluations was found. Fatigue*visual condition interaction revealed an increased AP median frequency (p < 0.001) during EC compared to EO only immediately after FI. Young adults' body sway remains impaired until 48 h, but not the postural control adaptability and complexity. Visual information may not attenuate the late deleterious ankle muscle fatigability effects. Individuals should be cautious during balance tasks and exercise after fatiguing exercise in the next 24/48 h, therefore avoiding unbalances and falls., (Copyright © 2021 Elsevier Ltd. This study examined changes in postural control and gaze performance of faller and non-faller older adults under conditions of visual tasks and optical flow manipulations. Fifteen older non-fallers (69.8 years, ± 3.2) and fifteen older fallers (71.1 years, ± 6.4) stood on a force platform inside a moving room wearing an eye tracker. Four tasks were performed: gaze fixation; predictable saccades; unpredictable saccades; and free-viewing. The stimuli appeared at a frequency of 1.1 Hz during conditions of predictable and unpredictable saccades. Sixteen trials were divided into two blocks. In the first block, the room remained stationary. In the second block, the room oscillated, without the participant's awareness, with a 0.6 cm amplitude and 0.2 Hz frequency. Results showed postural sway attenuation in older fallers during the saccadic tasks compared to gaze fixation and free-viewing tasks, in both stationary and moving room conditions. Both groups showed increased center of pressure (CoP) magnitude during the moving room condition and CoP displacements strongly coupled to the room's movement. The influence of the moving room on the postural sway was reduced during the saccadic tasks for both older groups. Older fallers exhibited higher variability compared to older non-fallers. Gaze behavior differences between groups were dependent on the goals of the visual tasks. Therefore, CoP displacements of older adults are reduced during saccadic tasks regardless of their falling history. However, postural and gaze performance of older fallers suggests increased deterioration of postural and oculomotor control which may be used as a predictor of fall risk., (© 2021. Kicking is a fundamental skill in soccer that often contributes to match outcomes. Lower limb movement features (e.g., joint position and velocity) are determinants of kick performance. However, obtaining kicking kinematics under field conditions generally requires time-consuming manual tracking. The current study aimed to compare a contemporary markerless automatic motion estimation algorithm (OpenPose) with manual digitisation (DVIDEOW software) in obtaining on-field kicking kinematic parameters. An experimental dataset of under-17 players from all outfield positions was used. Kick attempts were performed in an official pitch against a goalkeeper. Four digital video cameras were used to record full-body motion during support and ball contact phases of each kick. Three-dimensional positions of hip, knee, ankle, toe and foot centre-of-mass (CM foot ) generally showed no significant differences when computed by automatic as compared to manual tracking (whole kicking movement cycle), while only z-coordinates of knee and calcaneus markers at specific points differed between methods. The resulting time-series matrices of positions (r 2 = 0.94) and velocity signals (r 2 = 0.68) were largely associated (all p < 0.01). The mean absolute error of OpenPose motion tracking was 3.49 cm for determining positions (ranging from 2.78 cm (CM foot ) to 4.13 cm (dominant hip)) and 1.29 m/s for calculating joint velocity (0.95 m/s (knee) to 1.50 m/s (non-dominant hip)) as compared to reference measures by manual digitisation. Angular range-of-motion showed significant correlations between methods for the ankle (r = 0.59, p < 0.01, large ) and knee joint displacements (r = 0.84, p < 0.001, very large ) but not in the hip (r = 0.04, p = 0.85, unclear ). Markerless motion tracking (OpenPose) can help to successfully obtain some lower limb position, velocity, and joint angular outputs during kicks performed in a naturally occurring environment.

Background: Our understanding of how balance control responds to levodopa over the course of a single day in people with Parkinson's disease (PD) is limited with the majority of studies focused on isolated comparisons of ON vs. OFF levodopa medication., Objective: To evaluate the temporal dynamics of postural control following the first levodopa dose of the day during a challenging standing task in a group of people with PD., Methods: Changes in postural control were evaluated by monitoring cortical activity (covering frontal, motor, parietal and occipital areas), body sway parameters (force platform), and lower limb muscle activity (tibialis anterior and gastrocnemius medialis) in 15 individuals with PD during a semi-tandem standing task. Participants were assessed during two 60 second trials every 30 minutes (ON-30 ON-60 etc.) for 3 hours after the first matinal dose (ON-180)., Results: Compared to when tested OFF-medication, cortical activity was increased across all four regions from ON-60 to ON-120 with early increases in alpha and beta band activity observed at ON-30. Levodopa was associated with increased gastrocnemius medialis activity (ON-30 to ON-120) and ankle co-contraction (ON-60 to ON-120). Changes in body sway outcomes (particularly in the anterior-posterior direction) were evident from ON-60 to ON-120., Conclusions: Our results reveal a 60-minute window within which postural control outcomes may be obtained that are different compared to OFF-state and remain stable (from 60-minutes to 120-minutes after levodopa intake).
