15. Jointly Estimating Subnational Mortality for Multiple Populations

Abstract

Understanding patterns in mortality across subpopulations is essential for local health policy decision making. One of the key challenges of subnational mortality rate estimation is the presence of small populations and zero or near zero death counts. When studying differences between subpopulations, this challenge is compounded as the small populations are further divided along socioeconomic or demographic lines. In this paper, we build on principal component-based Bayesian hierarchical approaches for subnational mortality rate estimation to model correlations across subpopulations. The principal components identify structural differences between subpopulations, and coefficient and error models track the correlations between subpopulations over time. We illustrate the use of the model in a simulation study as well as on county-level sex-specific US mortality data. We find that results from the model are reasonable and that it successfully extracts meaningful patterns in US sex-specific mortality. Additionally, we show that ancillary correlation parameters are a useful tool for studying the convergence and divergence of mortality patterns over time.

Abstract

COVID-19 mortality increases markedly with age and is also substantially higher among Black, Indigenous, and People of Color (BIPOC) populations in the United States. These two facts can have conflicting implications because BIPOC populations are younger than white populations. In analyses of California and Minnesota—demographically divergent states—we show that COVID vaccination schedules based solely on age benefit the older white populations at the expense of younger BIPOC populations with higher risk of death from COVID-19. We find that strategies that prioritize high-risk geographic areas for vaccination at all ages better target mortality risk than age-based strategies alone, although they do not always perform as well as direct prioritization of high-risk racial/ethnic groups. Vaccination schemas directly implicate equitability of access, both domestically and globally.

37. A Flexible Bayesian Model for Estimating Subnational Mortality

Abstract

Reliable mortality estimates at the subnational level are essential in the study of health inequalities within a country. One of the difficulties in producing such estimates is the presence of small populations, where the stochastic variation in death counts is relatively high, and so the underlying mortality levels are unclear. We present a Bayesian hierarchical model to estimate mortality at the subnational level. The model builds on characteristic age patterns in mortality curves, which are constructed using principal components from a set of reference mortality curves. Information on mortality rates are pooled across geographic space and smoothed over time. Testing of the model shows reasonable estimates and uncertainty levels when the model is applied to both simulated data which mimic US counties, and real data for French departments. The estimates produced by the model have direct applications to the study of subregional health patterns and disparities.
