Abstract

Spontaneous ventricular tachycardia (VT) rarely occurs in pediatric patients, therefore; management recommendations are very limited [1]. We present an adolescent with palpitations found to have a monomorphic wide complex rapid tachyarrhythmia. Despite a very rapid ventricular rate of 235 beats per minute (bpm) she remained hemodynamically stable and was successfully pharmacologically cardioverted. We highlight the considerations in managing a hemodynamically stable pediatric patient with VT with emphasis on the utilization of pharmacologic therapies as opposed to electrical cardioversion.

29. Laparoscopic cyst fenestration in the treatment of polycystic liver disease

Abstract

The PLD, included in the group of malformative lesions of ductal plate, is characterized by progressive development of multiple parenchymal cysts. Different surgical treatments have been proposed for symptomatic patients with PLD: percutaneus aspiration, fenestration, hepatic resection and liver transplantation. The aim of this report is to outline the treatment of polycystic liver disease with laparoscopic fenestration.Of thirteen patients with PLD, in all cases associated with polycystic kidney disease, 7 patients underwent to laparoscopic treatment of cysts fenestration, and 6, asymptomatic and not amenable to surgical treatment, underwent to clinical follow-up. Of the 7 patients, 5 have symptoms related to "effect mass" 1-2-3 of hepatic cysts while in the 2 asymptomatics the fenestration of the cysts was performed during the procedure of laparoscopic cholecystectomy for cholelithiasis.The best indication for laparoscopic fenestration are those cases of PLD characterized by a relatively limited number of large cysts, preferably situated in the anterior segments of the liver and in the left lobe; in this patients, laparoscopic fenestration reduces significantly the volume of the liver and relieves symptoms. The laparoscopic fenestration of hepatic cyst, in carefully selected patients, is an effective technique in terms of morbidity, mortality, conversion rate and recurrence rates; while in patients with cyst diffuse in liver parenchyma is indicated the hepatic resection or liver transplantation. The optimal surgical approach is still evolving, the type of approach is related to extent and distribution of the cysts, and vascular anatomy of normal segment of the liver.Laparoscopic fenestration, Policystic liver.

Abstract

Ground-based gamma-ray astronomy has had a major breakthrough with the impressive results obtained using systems of imaging atmospheric Cherenkov telescopes. Ground-based gamma-ray astronomy has a huge potential in astrophysics, particle physics and cosmology. CTA is an international initiative to build the next generation instrument, with a factor of 5-10 improvement in sensitivity in the 100 GeV-10 TeV range and the extension to energies well below 100 GeV and above 100 TeV. CTA will consist of two arrays (one in the north, one in the south) for full sky coverage and will be operated as open observatory. The design of CTA is based on currently available technology. This document reports on the status and presents the major design concepts of CTA. © 2011 The Author(s).

33. Hacia el encuentro dialógico como estrategia metodológica: una lógica de acción cualitativa.

Abstract

Generar el encuentro dialógico como estrategia de recolección de información de estudios cualitativos parte de la exploración de los fundamentos teóricos de la hermenéutica de Gadamer (2005) y de la complejidad de Prigogine (1997), constituye el propósito esencial de este ensayo. La labor investigativa consistió en la revisión documental de las principales obras de estos autores, con la intención de definir la orientación ontológica y protológica de esta lógica de acción con la intención de comprender los fenómenos sociales y humanos. Los hallazgos develan que el encuentro dialógico es un espacio interdisciplinario que invita a un juego de palabras, en el que los interlocutores deben estar conscientes de sus límites relacionales durante el desarrollo conversacional, el cual se estructura en tres momentos: i) fenomenológico, ii) hermenéutico y iii) emergente. Se define la temática a dialogar, acordar y consensuar; por lo tanto, su naturaleza es lingüística, dialéctica, emergente, relacional, consensuada, lo que posibilita la comprensión - generación de conocimiento mediante la dialéctica de la pregunta. La tarea del encuentro está, precisamente, en la toma de conciencia de cada interlocutor de sus propios límites relacionales, en discutir, conversar, acordar, generar consensos y trascender el contenido dialógico o mundo de sentido colectivo.

Abstract

First-degree relatives of type 2 diabetic patients with or without a family history of hypertension are at increased risk for cardiovascular diseases. The aim of this study was to verify some possible hemostatic alterations in first-degree relatives of type 2 diabetic, normotensive and hypertensive patients. In 78 non-diabetic, normotensive first-degree relatives of type 2 diabetic patients (47 without a family history of hypertension and 31 with a family history of hypertension) and in 36 normoglycemic, normotensive subjects with no family history of hypertension, we evaluated plasma levels of fasting glucose and insulin, tissue-type plasminogen activator (t-PA), plasminogen activator-inhibitor (PAI-1), D-dimer (DD) and prothrombin fragment 1 + 2 (F1+2). Insulin resistance, calculated by the HOMA model, and plasma levels of t-PA and PAI-1 were significantly higher in relatives of diabetics compared to controls. As far as the thrombin activation indexes are concerned, we detected a significant increase in DD and F1+2 in relatives of diabetics with hypertension compared to other study subjects. In conclusion, our data indicate that familial predisposition may influence the hemostatic system in first-degree relatives of diabetic and/or hypertensive patients.

Abstract

This study assessed the effects of the angiotensin-converting enzyme (ACE) inhibitor cilazapril on the main haemostatic variables in 22 patients, of either sex, with newly diagnosed uncomplicated essential hypertension. In the patients and in 10 control subjects, plasma levels of thrombomodulin, β-thromboglobulin, D-dimer, tissue-type plasminogen activator (t-PA) and plasminogen activator inhibitor (PAI-1) had previously been measured. Only the levels of t-PA and PAI-1 were found to be higher than in controls. All these haemostatic evaluations were carried out after 6 and 12 months of treatment with an ACE inhibitor, cilazapril, 5 mg/day. This treatment significantly lowered the mean arterial pressure in the whole group from 133 to 106 mm Hg (after 6 months) and to 105 mm Hg (after 12 months), p < 0.05. No significant difference in any haemostatic parameters was observed after 6 and 12 months of treatment. The present study confirmed that treatment with cilazapril for 12 months lowers daytime ambulatory mean arterial pressure in patients with essential hypertension, without any significant increase in the tendency of blood to clot.
