1. 'I Wouldn’t Even Know What to Ask for': Patients’ and Caregivers’ Experiences of Psychological Support for Huntington’s Disease in Italy

Abstract

People with Huntington’s disease (HD) often experience psychological difficulties linked with disease progression and the adjustment to living with a chronic condition, which are also frequently shared by their informal caregivers (e.g., partners). Although limited, the current literature on psychological care for people with HD shows that interventions have the potential to drive improvements in mental health and quality of life. However, the experience of accessing and receiving psychological support for HD remains unclear across several countries. This study adopted a qualitative design to explore the experiences of psychological support for HD from the perspectives of patients and caregivers living in Italy. Semi-structured interviews were carried out with 14 participants—7 patients with early-manifest HD and 7 partners acting as their caregivers. The resulting data were analysed through thematic analysis. Four overarching themes were identified: (1) the availability of psychological support for HD, (2) barriers to accessing psychological support, (3) enablers to accessing psychological support, and (4) the future development of public psychological provision for HD. In Italy, patients and caregivers perceive public psychological support for HD as unavailable or inadequate, and private therapy is often seen as unaffordable. Barriers such as distrust in public healthcare and preconceptions about therapy may limit access, while advice from HD organisations and seeking therapy for other reasons may act as enablers. A strong emphasis is put on the need for accessible public psychological support throughout all the stages of the condition.

Abstract

Abstract Background Cancer cells are characterized by chromosomal instability (CIN) and it is thought that errors in pathways involved in faithful chromosome segregation play a pivotal role in the genesis of CIN. Cohesin forms a large protein ring that binds DNA strands by encircling them. In addition to this central role in chromosome segregation, cohesin is also needed for DNA repair, gene transcription regulation and chromatin architecture. Though mutations in both cohesin and cohesin-regulator genes have been identified in many human cancers, the contribution of cohesin to cancer development is still under debate. Methods Normal mucosa, early adenoma, and carcinoma samples deriving from 16 subjects affected by colorectal cancer (CRC) were analyzed by OncoScan for scoring both chromosome gains and losses (CNVs) and loss of heterozygosity (LOH). Then the expression of SMC1A was analyzed by immunochemistry in 66 subjects affected by CRC. The effects of SMC1A overexpression and mutated SMC1A were analyzed in vivo using immunocompromised mouse models. Finally, we measured global gene expression profiles in induced-tumors by RNA-seq. Results Here we showed that SMC1A cohesin core gene was present as extra-copies, mutated, and overexpressed in human colorectal carcinomas. We then demonstrated that cohesin overexpression led to the development of aggressive cancers in immunocompromised mice through gene expression dysregulation. Conclusion Collectively, these results support a role of defective cohesin in the development of human colorectal cancer.

Abstract

The prevalence of Huntington disease (HD) has increased over time; however, there is a lack of up-to-date evidence documenting the economic burden of HD by disease stage. This study provides an estimate of the annual direct medical, nonmedical, and indirect costs associated with HD from participants in the Huntington's Disease Burden of Illness (HDBOI) study in five European countries and the USA.The HDBOI is a retrospective, cross-sectional study. Data collection was conducted between September 2020 and May 2021. Participants were recruited by their HD-treating physicians and categorized as early stage (ES), mid stage (MS), or advanced stage (AS) HD. Data were collected via three questionnaires: a case report form, completed by physicians who collected health care resource use associated with HD to compute direct medical cost, and optional patient and caregiver questionnaires, which included information used to compute nondirect medical and indirect costs. Country-specific unit cost sources were used.HDBOI cost estimates were €12,663 (n = 2094) for direct medical costs, €2984 (n = 359) for nondirect medical costs, and €47,576 (n = 436) for indirect costs. Costs are higher in patients who are at later stages of disease; for example, direct medical costs estimates were €9220 (n = 846), €11,885 (n = 701), and €18,985 (n = 547) for ES, MS, and AS, respectively. Similar trends were observed for nondirect and indirect costs. Costs show large variations between patients and countries.Cost estimates from the HDBOI study show that people with HD and their caregivers bear a large economic burden that increases as disease progresses.

Abstract

The “Spazio Huntington—A Place for Children” program was launched in 2019. The aim was to contact at risk kids within Huntington disease (HD) families, to provide counseling to their parents and to start a prospective follow-up of kids suspicious to manifest pediatric HD (PHD). We met 25 at risk kids in two years, four of whom with PHD and highly expanded (HE) mutations beyond 80 CAG repeats. We rated motor, neuropsychological and behavioral changes in all PHD kids by the Unified HD Rating Scale (UHDRS)-total motor score (TMS) and additional measures of (1) cognitive level (Leiter International Performance Scale), (2) adaptive functioning (Adaptive Behavior Assessment Systems), (3) receptive language (Peabody Picture Vocabulary Test) and (4) behavioral abnormalities (Child Behavior Check List and Children’s Yale–Brown Obsessive Compulsive Scale). All PHD kids showed a severe progression of neurological and psychiatric manifestations including motor, cognitive and behavioral changes. The magnetic resonance imaging contributed to confirm the suspicious clinical observation by highlighting very initial striatum abnormalities in PHD. Spazio Huntington is a program to prospectively study PHD, the most atypical face of HD, and may represent the basis to recruit PHD patients in future clinical trials.

Abstract

Background The development of effective therapies for Huntington’s disease (HD) requires an active, informed, and lasting commitment from HD families to research. Specifically, those traditionally less involved in studies should be heard and engaged in this process: persons at risk for HD (HDRisk) and persons with premanifest HD (PreHD). Aim To determine which factors affect the willingness of persons with HDrisk and PreHD to participate in research and check for differences related to HD status. Methods The European Huntington Association (EHA) created an online survey to assess the perceptions and experiences about research participation among persons with HDRisk and PreHD. The two groups were compared on their answers to questions about research experience and knowledge, sources of information about research, the importance of reasons for getting involved and not getting involved in studies, and factors preventing and facilitating study participation. Results/Outcome The survey was filled by 263 persons with HDRisk and 262 persons with PreHD. While the overall motivation of both groups to engage in research was high, respondents with PreHD reported significantly more research experience and better research knowledge than respondents with HDRisk. Respondents with HDRisk identified more barriers to research participation and the need for more support to engage in studies compared to respondents with PreHD. Conclusions Motivation to take part in studies is high, despite limited research experience and literacy. This motivation is strongly influenced by HD status. Our findings highlight the relevance of planning tailored interventions to better support an informed participation of specific HD groups in research.

Abstract

There has been great progress in Huntington’s disease (HD) research. Yet, effective treatments to halt disease before the onset of disabling symptoms are still unavailable. Scientific breakthroughs require an active and lasting commitment from families. However, they are traditionally less involved and heard in studies. Accordingly, the European Huntington Association (EHA) surveyed individuals at risk (HDRisk) and with premanifest HD (PreHD) to determine which factors affect their willingness to participate in research. Questions assessed research experience and knowledge, information sources, reasons for involvement and noninvolvement, and factors preventing and facilitating participation. The survey included 525 individuals, of which 68.8% never participated in studies and 38.6% reported limited research knowledge. Furthermore, 52% trusted patient organizations to get research information. Reasons for involvement were altruistic and more important than reasons for noninvolvement, which were related to negative emotions. Obstacles included time/financial constraints and invasive procedures, while professional support was seen as a facilitator. PreHD individuals reported less obstacles to research participation than HDRisk individuals. Overall, a high motivation to participate in research was noted, despite limited experience and literacy. This motivation is influenced by subjective and objective factors and, importantly, by HD status. Patient organizations have a key role in fostering motivation through education and support.

Il comparto suolo e sottosuolo è uno dei comparti che maggiormente viene approfondito per la redazione degli studi di impatto ambientale. La sua caratterizzazione gioca un ruolo fondamentale nell’ambito della disciplina ambientale, sia per definire lo stato preesistente dell’area oggetto di studio, sia per valutare come l’intervento proposto potrebbe alterare tale condizione. Nel seguente lavoro si descrivono, dunque, gli strumenti utili per rilevare e studiare le principali caratteristiche delle componenti e dei fattori ambientali e si riporta l’analisi di un caso studio relativo ad una vasta area soggetta al preoccupante fenomeno della combustione dei rifiuti illecitamente abbandonati. L’approfondimento del caso studio si pone l’obiettivo di illustrare le analisi effettuate e i risultati ottenuti, in termini di classificazione dei terreni in classi di rischio ai fini dell’uso agricolo, per garantire la qualità delle produzioni agroalimentari a tutela della salute umana.

Abstract

Background Huntington's disease is caused by a CAG repeat expansion in the huntingtin gene, HTT. Age at onset has been used as a quantitative phenotype in genetic analysis looking for Huntington's disease modifiers, but is hard to define and not always available. Therefore, we aimed to generate a novel measure of disease progression and to identify genetic markers associated with this progression measure. Methods We generated a progression score on the basis of principal component analysis of prospectively acquired longitudinal changes in motor, cognitive, and imaging measures in the 218 indivduals in the TRACK-HD cohort of Huntington's disease gene mutation carriers (data collected 2008–11). We generated a parallel progression score using data from 1773 previously genotyped participants from the European Huntington's Disease Network REGISTRY study of Huntington's disease mutation carriers (data collected 2003–13). We did a genome-wide association analyses in terms of progression for 216 TRACK-HD participants and 1773 REGISTRY participants, then a meta-analysis of these results was undertaken. Findings Longitudinal motor, cognitive, and imaging scores were correlated with each other in TRACK-HD participants, justifying use of a single, cross-domain measure of disease progression in both studies. The TRACK-HD and REGISTRY progression measures were correlated with each other (r=0·674), and with age at onset (TRACK-HD, r=0·315; REGISTRY, r=0·234). The meta-analysis of progression in TRACK-HD and REGISTRY gave a genome-wide significant signal (p=1·12 × 10−10) on chromosome 5 spanning three genes: MSH3, DHFR, and MTRNR2L2. The genes in this locus were associated with progression in TRACK-HD (MSH3 p=2·94 × 10−8 DHFR p=8·37 × 10−7 MTRNR2L2 p=2·15 × 10−9) and to a lesser extent in REGISTRY (MSH3 p=9·36 × 10−4 DHFR p=8·45 × 10−4 MTRNR2L2 p=1·20 × 10−3). The lead single nucleotide polymorphism (SNP) in TRACK-HD (rs557874766) was genome-wide significant in the meta-analysis (p=1·58 × 10−8), and encodes an aminoacid change (Pro67Ala) in MSH3. In TRACK-HD, each copy of the minor allele at this SNP was associated with a 0·4 units per year (95% CI 0·16–0·66) reduction in the rate of change of the Unified Huntington's Disease Rating Scale (UHDRS) Total Motor Score, and a reduction of 0·12 units per year (95% CI 0·06–0·18) in the rate of change of UHDRS Total Functional Capacity score. These associations remained significant after adjusting for age of onset. Interpretation The multidomain progression measure in TRACK-HD was associated with a functional variant that was genome-wide significant in our meta-analysis. The association in only 216 participants implies that the progression measure is a sensitive reflection of disease burden, that the effect size at this locus is large, or both. Knockout of Msh3 reduces somatic expansion in Huntington's disease mouse models, suggesting this mechanism as an area for future therapeutic investigation. Abstract

Huntington disease (HD) is a neurodegenerative, dominantly inherited disorder, which may result in a debilitating movements disorder and cognitive decline with progressive loss of independence. HD frequency in the Middle East has never been defined after the HTT gene discovery and cases of HD have been reported only sporadically.1 In July 2013, Italian League for Research on Huntington Disease Foundation was contacted by a family from Oman, who reported to be affected by HD since many generations. Indeed, that family may represent the largest HD cluster ever described in the Middle East, owing to a high number of intermarriages and subsequent increase in mutation heritability risk. Given the gaps in our knowledge about HD in the Middle East, we took the opportunity to study and determine the incidence and prevalence of HD in this region. We performed an observational, service based, study from August 2013 to March 2019 in the Muscat region, Sultanate of Oman. Prevalence was estimated on the number of affected and at-risk individuals, who were referred to the National Genetic Centre (NGC) of the Royal Hospital in Muscat, the main Ministry of Health Country health organisation. We are, therefore, confident to have an accurate representation of most HD cases and families who reside in this region. A priori risk, that is, the risk of disease associated with an individual at birth, was separated into groups: 50% risk (a first-degree relative is affected with or mutation-positive for HD), and >50% risk (an individual with both at-risk parents or one affected and one at-risk parent). Abstract

Background Cancer cells are characterized by chromosomal instability (CIN) and it is thought that errors in pathways involved in faithful chromosome segregation play a pivotal role in the genesis of CIN. Cohesin forms a large protein ring that binds DNA strands by encircling them. In addition to this central role in chromosome segregation, cohesin is also needed for DNA repair, gene transcription regulation and chromatin architecture. Though mutations in both cohesin and cohesin-regulator genes have been identified in many human cancers, the contribution of cohesin to cancer development is still under debate. Methods Normal mucosa, early adenoma, and carcinoma samples deriving from 16 subjects affected by colorectal cancer (CRC) were analyzed by OncoScan for scoring both chromosome gains and losses (CNVs) and loss of heterozygosity (LOH). Then the expression of SMC1A was analyzed by immunochemistry in 66 subjects affected by CRC. The effects of SMC1A overexpression and mutated SMC1A were analyzed in vivo using immunocompromised mouse models. Finally, we measured global gene expression profiles in induced-tumors by RNA-seq. Results Here we showed that SMC1A cohesin core gene was present as extra-copies, mutated, and overexpressed in human colorectal carcinomas. We then demonstrated that cohesin overexpression led to the development of aggressive cancers in immunocompromised mice through gene expression dysregulation. Conclusion Collectively, these results support a role of defective cohesin in the development of human colorectal cancer. Abstract

Background LIRH Foundation started to collect data from HD subjects and families since 2001. The collection includes data from LIRH archives, REGISTRY (since 2004) and ENROLL-HD (since 2014) data. Aims To develop a large HD database starting from ENROLL-HD data. Methods The database was constructed through shiny, plyr and DT R packages (R 3.4.4 version) for automatically merging different clinical items. Results Since July 2014, we recruited 626 people into ENROLL-HD, until May 2018. Follow-ups are available for 432 subjects (one-year), 268 (two-years) and 152 (three years). Our cohort includes 404 gene positive (346 patients and 58 pre-manifest) individuals, 153 genetically ‘unknown’ (50% at risk), 36 gene negative and 43 family member subjects. Mean age at onset is 47,6±14,7 (range 7–86), thus including adolescent ( 60 years; N=35 subjects) cohorts. Onset symptoms includes motors signs in 47,9%, mixed in 34,9%, psychiatric in 15.6% and cognitive in 1.5% cases. Suicide ideation occurred in 17,4%, calculated according to the C-SSRS scale activation. Mean, fully penetrant expanded repeat number is 44,4±5,06 (range 40–84); two 2 subjects have intermediate alleles (29,5±0,7; range 29–30), and 23 reduced penetrance (38,2±0,8; range 36–39). Disease stage, according to the TFC scale score (9,7±3,9; range 1–13), ranges between 1 to 5. Conclusions A proper database and stratification of HD subjects according to genetic condition, symptom presentation, mutation length and development of further symptoms is crucial for both improving the disease knowledge and to driving people to research plans.

Abstract

Background Many young people from the 700+ families served by LIRH reported a profound sense of isolation due to the fact that they do not feel like talking about their condition with friends, neither with teachers or with the boyfriend/girlfriend. Objective Setting up a national youth network to offer to children and young adults (no matter if they are affected or at-risk) the opportunity to know each other, to meet and talk with no shame and no fear and to develop HD educational programs. Methods Identification of the target (13 to 35 years old people); identification of a ‘funding group’– choice of a ‘group leader’; communication and networking. Results First Sept-2017 funding group was made up by 8 people. The second meeting in March 2018 was attended by 15 people. The network counts at present >30 people from 8 out of 20 Italian regions. The presentation video had more than 14.000 views in less than 10 days. A FB page, an Instagram Page and a Youtube Channel were launched. Conclusions Noi Huntington is the name that the funding group chosed to underline the need and the sense of belonging to the same community: NOI means WE. It is about to launch its own website and is planning several educational initiatives. This is a positive example of empowerment in the HD youth community.

Proper chromosome segregation is crucial for preserving genomic integrity, and errors in this process cause chromosome mis-segregation, which may contribute to cancer development. Sister chromatid separation is triggered by Separase, an evolutionary conserved protease that cleaves the cohesin complex, allowing the dissolution of sister chromatid cohesion. Here we provide evidence that Separase participates in genomic stability maintenance by controlling replication fork speed. We found that Separase interacted with the replication licensing factors MCM2–7, and genome-wide data showed that Separase co-localized with MCM complex and cohesin. Unexpectedly, the depletion of Separase increased the fork velocity about 1.5-fold and caused a strong acetylation of cohesin's SMC3 subunit and altered checkpoint response. Notably, Separase silencing triggered genomic instability in both HeLa and human primary fibroblast cells. Our results show a novel mechanism for fork progression mediated by Separase and thus the basis for genomic instability associated with tumorigenesis.

Abstract

Huntington disease (HD) prevalence shows geographic variability and has been recently updated by taking into account the mutation diagnostic test. In Italy, the last epidemiological estimation was reported well before the HTT gene discovery and the availability of the corresponding genetic test. It reported a prevalence of affected subjects ranging between 2.3 and 4.8/100,000 in some restricted areas of Northern Italy. We have performed a service-based epidemiological analysis in a very restricted geographic area named Molise, where our institutions currently operate and represent the only point of reference for rare neuropsychiatric diseases. The estimated prevalence rate found was 10.85/100,000 (95% confidence interval (CI): 7.20-14.50), remarkably higher than that previously described before the gene test analysis was available, and expected to an increase of an additional 17% by 2030, because of Italian population aging. According to our analysis, we estimate that about 6500 subjects are currently affected by HD in Italy, and that this number will further increase in the next decades because of population aging, variable phenotype penetrance and improved life expectancy.

Abstract

International audience; OBJECTIVE: There is controversy about the clinical consequences of intermediate alleles (IAs) in Huntington disease (HD). The main objective of this study was to establish the clinical manifestations of IA carriers for a prospective, international, European HD registry.METHODS: We assessed a cohort of participants at risk with RESULTS: Of 12,190 participants, 657 (5.38%) with CONCLUSIONS: Although aging worsened the UHDRS scores independently of the genetic status, IAs might confer a late-onset abnormal motor and cognitive phenotype. Abstract

Background The Italian population is genetically heterogeneous with a variable HD frequency ranging around 11/100,000 individuals (Squitieri et al., 2016) and unusual haplotypes distribution. Enroll-HD represents an opportunity to monitor HD and to collect data and samples for research studies. Aims To assist HD people following a model validated by research protocols. Methods Data were collected according to the ethically approved Enroll-HD protocol at LIRH sites in Northern, Middle and Southern Italy. Results Patients’ recruitment in Enroll-HD started in July 2014. At present, we have enrolled 329 subjects until May 2016, whose one-year follow-up was available for 177 of them. Our cohort included 278 gene positive (241 patients and 37 pre-manifest) individuals (84.5% of the whole samples), 37 genetically “unknown” subjects (mainly 50% at risk), five gene negative and 9 family member subjects (i.e. partners). Mean age at onset was 42.8 ± 12.2 years (range 10–83), thus including juvenile ( 60 years) cohorts. Onset symptoms included motors signs in 48.8%, mixed in 32.9%, psychiatric in 15.9% and cognitive in 1.9% cases. Suicide ideation occurred in 26.7%, calculated according to the C-SSRS scale activation. Mean repeat expansion number was 43.8 ± 4.9 CAG, spanning between low and high mutation size penetrance. Disease stage, according to the TFC scale score, ranged between 1 to 5. Conclusions LIRH initiative aims to connect patients’ community with hospitals and research centres focused on HD. HD staging is critical to properly assist and address people to clinical trials. Background LIRH Foundation is a not for profit organisation whose mission is to conduct and support research on Huntington’s disease (HD), providing patients with free of charge assistance, promoting awareness raising initiatives, training professionals and spreading research results. LIRH Foundation has outpatient services for HD in Northern, Middle and Southern Italy. Specific aim To encourage patients not to stay hidden but to seek for proper assistance by, at the same time, contributing to research progress. Methods A systematic, clear, easily accessible and scientifically validated communication and information activity conducted by press release, website, webmarketing campaigns, conferences, video, interviews, social networks, brochure, newsletter, networking. Results Incoming calls to our Toll-Free Number 800.388.330: 3.783 incoming calls in 2015 (20% increase vs 2014). 1.400 incoming calls in the first quarter of 2016. Website: 11.577 users (average) in the first quarter of 2016 vs 3.456 in the same period of 2015; 27.295 page views (average) in the first quarter of 2016 vs 13.135 same period 2015. Assisted HD subjects: Jul 2015-Jun 2016: 590 visits, including 72 first visits vs Jul 2014 – Jun 2016: 250 visits, including 25 first visits Conclusions Quality and consistency of disclosed information are recognised by patients, who feel encouraged to approach our organisation and to participate in experimental trials (i.e Enroll-HD, Legato-HD and Open PRIDE-HD).

Abstract

Background Huntington disease (HD) frequency is uncertain and epidemiological analyses are requested to determine whether HD prevalence is underestimated. Last epidemiological esteem in Italy was described before HTT gene discovery, reporting a frequency of affected subjects ranging between 2.3 to 4.8/100.000 in Northern Italy. Aims To determine HD prevalence in Italy. Methods Since 1997, we started to collect data and DNA samples for diagnosis and research purposes, from HD subjects originating from every part of Italy. To date, we have data and samples from 730 patients from a total of 557 unrelated families. To decrease the bias due to methodological and regional variations, we focused on a well-selected Italian region, Molise, Mid-Southern Italy, showing a population of 313.341 inhabitants at 2013, where Neuromed and LIRH are the only referent Institutions for HD and related diseases. Results Thirty-four patients had clinical diagnosis of HD, from 31 unrelated families. Twenty-six patients received genetic confirmation, 8 of them had strictly clinical diagnosis. The estimated prevalence was 10.85/100,000 (95% CI: 7.20–14.50). A total of 464 asymptomatic and untested individuals (124 with 50% risk and 156 with 25% risk) were identified on family pedigrees. When including premanifest mutation carriers who had performed a predictive test, we found a total of 42 individuals with an estimated prevalence of 13.40/100.000 (95% CI: 9.35–17.45). Conclusions Our data suggest, for the first time, that there may be up to 6500 HD patients in Italy and that previous analyses underestimated.

Abstract

Background Huntington disease (HD) was sporadically described in the Arabic population, so far. Since Arabic people frequently marry within the same family, the mutation risk is reiterating in offspring. In Oman, only few and anecdotal reports, are known. Aims To set a HD Centre in the Middle East. Methods A Neurogenetic Centre with outpatient and molecular diagnostic services focused on HD, was inaugurated by Ministry of Health at Royal Hospital, Oman, in November 2013. We analysed clinical, genetic and demographic features of affected and at risk subjects from a large and inbred family from Muscat and surroundings. Results In this six generations family we have visited 23 alive symptomatic subjects of 42 reported by genealogical retrospective analysis. We counted about 84, 50% and 86, 25%, at risk individuals. We found 18 people with a theoretical risk >50%, due to intermarriages and four with a risk of homozygousity for CAG mutation. DNA testing was provided, after informed consent, with the support of genetic counselling, and clinical data were collected by the Unified HD Rating Scale. Age at onset ranged between 2 and 79 years, (CAG)n between 39 (oldest onset) and over 80 (earliest onset) repeats. By careful interview of family members, we deduced that the ancestor carrying the HD mutation to Oman immigrated from Tanzania, Africa. Conclusions Our collaborative effort aims to improve medical assistance and social awareness on HD and related diseases in Middle East. Accordingly, we have now contributed to set the first HD family association in Middle East (www.hdom.org), in strict collaboration with Italian LIRH (www.lirh.it).
