Abstract

The linearised Laplace method for estimating model uncertainty has received renewed attention in the Bayesian deep learning community. The method provides reliable error bars and admits a closed-form expression for the model evidence, allowing for scalable selection of model hyperparameters. In this work, we examine the assumptions behind this method, particularly in conjunction with model selection. We show that these interact poorly with some now-standard tools of deep learning--stochastic approximation methods and normalisation layers--and make recommendations for how to better adapt this classic method to the modern setting. We provide theoretical support for our recommendations and validate them empirically on MLPs, classic CNNs, residual networks with and without normalisation layers, generative autoencoders and transformers., Comment: Paper appearing at ICML 2022

2. An educated warm start for deep image prior-based micro CT reconstruction

Abstract

Deep image prior (DIP) was recently introduced as an effective unsupervised approach for image restoration tasks. DIP represents the image to be recovered as the output of a deep convolutional neural network, and learns the network’s parameters such that the model output matches the corrupted observation. Despite its impressive reconstructive properties, the approach is slow when compared to supervisedly learned, or traditional reconstruction techniques. To address the computational challenge, we bestow DIP with a two-stage learning paradigm: (i) perform a supervised pretraining of the network on a simulated dataset; (ii) fine-tune the network’s parameters to adapt to the target reconstruction task. We provide a thorough empirical analysis to shed insights into the impacts of pretraining in the context of image reconstruction. We showcase that pretraining considerably speeds up and stabilizes the subsequent reconstruction task from real-measured 2D and 3D micro computed tomography data of biological specimens.

3. Unsupervised knowledge-transfer for learned image reconstruction

Abstract

Deep learning-based image reconstruction approaches have demonstrated impressive empirical performance in many imaging modalities. These approaches usually require a large amount of high-quality paired training data, which is often not available in medical imaging. To circumvent this issue we develop a novel unsupervised knowledge-transfer paradigm for learned reconstruction within a Bayesian framework. The proposed approach learns a reconstruction network in two phases. The first phase trains a reconstruction network with a set of ordered pairs comprising of ground truth images of ellipses and the corresponding simulated measurement data. The second phase fine-tunes the pretrained network to more realistic measurement data without supervision. By construction, the framework is capable of delivering predictive uncertainty information over the reconstructed image. We present extensive experimental results on low-dose and sparse-view computed tomography showing that the approach is competitive with several state-of-the-art supervised and unsupervised reconstruction techniques. Moreover, for test data distributed differently from the training data, the proposed framework can significantly improve reconstruction quality not only visually, but also quantitatively in terms of PSNR and SSIM, when compared with learned methods trained on the synthetic dataset only.

Abstract

Image reconstruction methods based on deep neural networks have shown outstanding performance, equalling or exceeding the state-of-the-art results of conventional approaches, but often do not provide uncertainty information about the reconstruction. In this work we propose a scalable and efficient framework to simultaneously quantify aleatoric and epistemic uncertainties in learned iterative image reconstruction. We build on a Bayesian deep gradient descent method for quantifying epistemic uncertainty, and incorporate the heteroscedastic variance of the noise to account for the aleatoric uncertainty. We show that our method exhibits competitive performance against conventional benchmarks for computed tomography with both sparse view and limited angle data. The estimated uncertainty captures the variability in the reconstructions, caused by the restricted measurement model, and by missing information, due to the limited angle geometry.

Abstract

MicroRNA-210-3p is the most prominent hypoxia regulated microRNA, and it has been found significantly overexpressed in different human cancers. We performed the expression analysis of miR-210-3p in a retrospective cohort of breast cancer patients with a median follow-up of 76 months (n = 283). An association between higher levels of miR-210-3p and risk of disease progression (HR: 2.13, 95%CI: 1.33-3.39, P = 0.002) was found in the subgroup of patients treated with Epirubicin and Cyclophosphamide followed by Docetaxel. Moreover, a cut off value of 20.966 established by ROC curve analyses allowed to discriminate patients who developed distant metastases with an accuracy of 85% at 3- (AUC: 0.870, 95%CI: 0.690-1.000) and 83% at 5-years follow up (AUC: 0.832, 95%CI: 0.656–1.000). Whereas the accuracy in discriminating patients who died for the disease was of 79.6% at both 5- (AUC: 0.804, 95%CI: 0.517–1.000) and 10-years (AUC: 0.804. 95%CI: 0.517–1.000) follow-up. In silico analysis of miR-210-3p and Docetaxel targets provided evidence for a putative molecular cross-talk involving microtubule regulation, drug efflux metabolism and oxidative stress response. Overall, our data point to the miR-210-3p involvement in the response to therapeutic regimens including Docetaxel in sequential therapy with anthracyclines, suggesting it may represent a predictive biomarker in breast cancer patients.

Abstract

Aberrant promoter methylation of several known or pu-tative tumor suppressor genes occurs frequently during carcinogenesis, and this epigenetic change has been con-si der ed as a po ten ti al mol ec u la r ma rke r fo r ca nc er. We examined t he methylation status of n ine g enes ( APC, CDH1, CTNNB1, TIMP3, ESR1, GSTP1, MGMT, THBS1 , and TM S1 ), by quantitative methylation specific P CR . Synchronous preinvasive lesions (atypical ductal hyper-plasia and/or ductal carcinoma in situ ) and invasive duc-tal breast carcinoma from 52 patients, together with pure lesions from 24 patients and 12 normal tissues paired to tumor a nd 20 normal brea st distant from t umor were analyzed. Aberrant promoter methylation was detected in both preinvasive and invasive lesions for genes APC, CDH1, CTNNB1, TIMP3, ESR1 ,and GSTP1 . H ow ever, hierar chic al m ix ed model and Gener alized Estimating Equations model a nalyses show ed that only APC, CDH1, and CTNNB1 promoter regions showed a higher frequency and methylation levels in pathologic samples when compared with normal breast. Whereas APC and CTNNB1 did not show differences in methylation levels or frequencies, CDH1 showed higher methylation levels in invasive tumors as compared with preinvasive lesions (P < 0.0 4, Mann -Wh itn ey test wi th permu tat io n corr ec -tion). The analysis of APC, CDH1 , and CTNNB1 methyl-ation status was able to distinguish between normal and pathologic samples with a sensitivity of 67% (95% confi-dence i nterval, 60-71%) a nd a s pecificity of 75% (95% confidence interval, 69-81%). Our data point to the direct involveme nt of APC, CDH1 ,and CTNNB1 promoter methylation in the early stages of breast cancer progres-si on and su gge st th at th ey ma y re p re se n t a use ful to o l for t he detection of t umor cells in clinical specimens.

Abstract

Context: Nerve growth factor is a neurotrophic factor that promotes the survival of small fiber sensory neurons and sympathetic neurons in the peripheral nervous system. Recombinant human nerve growth factor (rhNGF) has demonstrated efficacy as treatment for peripheral neuropathy in experimental models and phase 2 clinical trials.Objective: To evaluate the efficacy and safety of a 12-month regimen of rhNGF in patients with diabetic polyneuropathy.Design: Randomized, double-blind, placebo-controlled phase 3 trial conducted from July 1997 through May 1999.Setting: Eighty-four outpatient centers throughout the United States.Patients: A total of 1019 men and women aged 18 to 74 years with either type 1 or type 2 diabetes and a sensory polyneuropathy attributable to diabetes.Interventions: Patients were randomly assigned to receive either rhNGF, 0.1 microg/kg (n = 504), or placebo (n = 515) by subcutaneous injection 3 times per week for 48 weeks. Patients were assessed at baseline, 12 weeks, 24 weeks, and 48 weeks.Main Outcome Measures: The primary outcome measure was a change in neuropathy between baseline and week 48, demonstrated by the Neuropathy Impairment Score for the Lower Limbs, compared between the 2 groups. Secondary outcome measures included quantitative sensory tests using the CASE IV System, the Neuropathy Symptom and Change questionnaire, the Patient Benefit Questionnaire (PBQ), and a global symptom assessment, as well as nerve conduction studies and occurrence of new plantar foot ulcers. Patients also were evaluated for presence of adverse events.Results: Among patients who received rhNGF, 418 (83%) completed the regimen compared with 461 (90%) who received placebo. Administration of rhNGF was safe, with few adverse events attributed to treatment apart from injection site pain/hyperalgesia and other pain syndromes. However, neither the primary end point (P =.25) nor most of the secondary end points demonstrated a significant benefit of rhNGF. Exceptions were the global symptom assessment (P =.03) and 2 of 32 comparisons within the PBQ, which showed a modest but significant benefit of rhNGF (P =.05 for severity of pain in the legs and P =.003 for 6-month symptoms in the feet and legs).Conclusion: Unlike previous phase 2 trials, this phase 3 clinical trial failed to demonstrate a significant beneficial effect of rhNGF on diabetic polyneuropathy. Abstract

Inoculation of the cell-adapted WW strain of Theiler's virus into mice produces a chronic demyelinating infection of the central nervous system (CNS) characterized by a remitting relapsing course. During remission, extensive remyelination of spinal cord white matter is observed. Remyelination is carried out by both Schwann cells and oligodendrocytes. This paper examines the possible mechanisms of entry of Schwann cells into the CNS, their possible source in different regions of the white matter, their relations with various CNS elements, and the relative activity of these cells versus that of oligodendrocytes. Observations suggest that Schwann cells, originating from peripheral roots and from perivascular areas, migrate into white matter through gaps in the glial limiting membrane ( GLM ), probably caused by active mononuclear inflammatory cells. Schwann cell invasion and axonal contact appear to be facilitated by the presence of collagen matrix along their pathway of migration. No alterations of astrocytes in the immediate vicinity of Schwann cells were observed, and free contact between Schwann cells and different neuroglial elements was present in the initial stages of Schwann cell migration. While Schwann cells were the predominant myelinating cells in the outer white matter, oligodendrocytes were numerous and very active in the inner portions of the spinal cord column. Although oligodendrocytes produced thinner myelin than normal, in most areas essentially complete remyelination by these cells was observed. These results contrast with those of previous studies of DA infected mice in which remyelination is sporadic in the presence of unabated inflammation which continues without remission for many months after infection. It is suggested that oligodendroglial cells are quite capable of extensive remyelinating activity in this infection, provided the noxa responsible for myelin injury subsides. The host inflammatory response appears to be the most likely noxa impeding remyelination in this model.

39. High specificity of quantitative methylation-specific PCR analysis for MGMT promoter hypermethylation detection in gliomas.

Abstract

Normal brain tissue from 28 individuals and 50 glioma samples were analyzed by real-time Quantitative Methylation-Specific PCR (QMSP). Data from this analysis were compared with results obtained on the same samples by MSP. QMSP analysis demonstrated a statistically significant difference in both methylation level (P = .000009 Mann Whitney Test) and frequencies (P = .0000007, Z-test) in tumour samples as compared with normal brain tissues. Although QMSP and MSP showed similar sensitivity, the specificity of QMSP analysis was significantly higher (93%; CI95%: 84%-100%) as compared with MSP (64%; 95%CI: 46%-82%). Our results suggest that QMSP analysis may represent a powerful tool to identify glioma patients that will benefit from alkylating agents chemotherapy. [ABSTRACT FROM AUTHOR]

Abstract

International audience; Members of the carbonic anhydrase family are functionally involved in the regulation of intracellular and extracellular pH in physiological and pathological conditions. Their expression is finely regulated to maintain a strict control on cellular homeostasis, and it is dependent on the activation of extracellular and intracellular signaling pathways. Combining RNA sequencing (RNA-seq), NanoString, and bioinformatics data, we demonstrated that the expression of carbonic anhydrase 12 (CAXII) is significantly different in luminal and triple negative breast cancer (BC) models and patients, and is associated with the activation of an epithelial mesenchymal transition (EMT) program. In BC models, the phorbol ester 12-myristate 13-acetate (PMA)-mediated activation of protein kinase C (PKC) induced a down-regulation of CAXII with a concomitant modulation of other members of the transport metabolon, including CAIX and the sodium bicarbonate cotransporter 3 (NBCn1). This is associated with a remodeling of tumor glycolytic metabolism induced after PKC activation. Overall, this analysis highlights the dynamic nature of transport metabolom and identifies signaling pathways finely regulating this plasticity.

Abstract

Background: Environmental exposure to trichloroethylene (TCE), a carcinogenic dry-cleaning chemical, may be linked to Parkinson's disease (PD)., Objective: The objective of this study was to determine whether PD and cancer were elevated among attorneys who worked near a contaminated site., Methods: We surveyed and evaluated attorneys with possible exposure and assessed a comparison group., Results: Seventy-nine of 82 attorneys (96.3%; mean [SD] age: 69.5 [11.4] years; 89.9% men) completed at least one phase of the study. For comparison, 75 lawyers (64.9 [10.2] years; 65.3% men) underwent clinical evaluations. Four (5.1%) of them who worked near the polluted site reported PD, more than expected based on age and sex (1.7%; P = 0.01) but not significantly higher than the comparison group (n = 1 [1.3%]; P = 0.37). Fifteen (19.0%), compared to four in the comparison group (5.3%; P = 0.049), had a TCE-related cancer., Conclusions: In a retrospective study, diagnoses of PD and TCE-related cancers appeared to be elevated among attorneys who worked next to a contaminated dry-cleaning site. © 2024 The Authors. 43. Unsupervised knowledge-transfer for learned image reconstruction.

Abstract

Deep learning-based image reconstruction approaches have demonstrated impressive empirical performance in many imaging modalities. These approaches usually require a large amount of high-quality paired training data, which is often not available in medical imaging. To circumvent this issue we develop a novel unsupervised knowledge-transfer paradigm for learned reconstruction within a Bayesian framework. The proposed approach learns a reconstruction network in two phases. The first phase trains a reconstruction network with a set of ordered pairs comprising of ground truth images of ellipses and the corresponding simulated measurement data. The second phase fine-tunes the pretrained network to more realistic measurement data without supervision. By construction, the framework is capable of delivering predictive uncertainty information over the reconstructed image. We present extensive experimental results on low-dose and sparse-view computed tomography showing that the approach is competitive with several state-of-the-art supervised and unsupervised reconstruction techniques. Moreover, for test data distributed differently from the training data, the proposed framework can significantly improve reconstruction quality not only visually, but also quantitatively in terms of PSNR and SSIM, when compared with learned methods trained on the synthetic dataset only., (© 2022 The Author(s). Abstract

The RNA-binding protein ALYREF (THOC4) is involved in transcriptional regulation and nuclear mRNA export, though its role and molecular mode of action in breast carcinogenesis are completely unknown. Here, we identified high ALYREF expression as a factor for poor survival in breast cancer patients. ALYREF significantly influenced cellular growth, apoptosis and mitochondrial energy metabolism in breast cancer cells as well as breast tumorigenesis in orthotopic mouse models. Transcriptional profiling, phenocopy and rescue experiments identified the short isoform of the lncRNA NEAT1 as a molecular trigger for ALYREF effects in breast cancer. Mechanistically, we found that ALYREF binds to the NEAT1 promoter region to enhance the global NEAT1 transcriptional activity. Importantly, by stabilizing CPSF6, a protein that selectively activates the post-transcriptional generation of the short isoform of NEAT1, as well as by direct binding and stabilization of the short isoform of NEAT1, ALYREF selectively fine-tunes the expression of the short NEAT1 isoform. Overall, our study describes ALYREF as a novel factor contributing to breast carcinogenesis and identifies novel molecular mechanisms of regulation the two isoforms of NEAT1., (© 2022. 45. OnabotulinumtoxinA Dosing, Disease Severity, and Treatment Benefit in Patients With Cervical Dystonia: A Cohort Analysis From CD PROBE.

Abstract

Introduction: The Cervical Dystonia Patient Registry for Observation of OnabotulinumtoxinA Efficacy (CD PROBE) study (ClinicalTrials.gov identifier: NCT00836017), a multicenter, prospective, observational registry, was designed to identify real-world practices and outcomes for patients with cervical dystonia (CD) treated with onabotulinumtoxinA (onabotA). This secondary analysis from CD PROBE aims to determine the impact of presentation subtype on onabotA utilization and CD severity., Materials and Methods: The study cohort includes those who completed all 3 treatments, 4 office visits, and had data recorded for all assessments. Patient outcomes were assessed with the Cervical Dystonia Impact Profile (CDIP-58), Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS), and determination of CD severity. Treatment interval, dose, and adverse events (AEs) were also recorded. Data were stratified according to prior exposure to botulinum toxins (BoNTs) and analyzed with descriptive statistics., Results: Torticollis was the most common presentation subtype in the study cohort ( N = 350); the proportion of patients with torticollis was highest in those with severe disease. At each treatment, between 40.7 and 65.2% of those categorized as severe shifted to moderate or mild severity after treatment. Sustained improvements in CDIP-58 and TWSTRS were observed regardless of prior exposure to BoNTs. Dosing of onabotA generally increased from injection 1 to injection 3 and tended to be lower for patients naïve to BoNT. Median time interval between injections for the study cohort was 94.0 to 97.5 days. The most common AEs (dysphagia, muscular weakness) and injection intervals were similar between naïve vs. non-naïve patients; there were no serious treatment-related AEs., Conclusions: This secondary cohort analysis from CD PROBE demonstrates that three repeat treatments with onabotA at intervals consistent with labeling attenuated disease severity and neck pain, resulting in sustained improvements in physician- and patient-reported outcomes. Abstract

Background: Several monogenic causes for isolated dystonia have been identified, but they collectively account for only a small proportion of cases. Two genome-wide association studies have reported a few potential dystonia risk loci; but conclusions have been limited by small sample sizes, partial coverage of genetic variants, or poor reproducibility., Objective: To identify robust genetic variants and loci in a large multicenter cervical dystonia cohort using a genome-wide approach., Methods: We performed a genome-wide association study using cervical dystonia samples from the Dystonia Coalition. Logistic and linear regressions, including age, sex, and population structure as covariates, were employed to assess variant- and gene-based genetic associations with disease status and age at onset. We also performed a replication study for an identified genome-wide significant signal., Results: After quality control, 919 cervical dystonia patients compared with 1491 controls of European ancestry were included in the analyses. We identified one genome-wide significant variant (rs2219975, chromosome 3, upstream of COL8A1, P-value 3.04 × 10 -8 ). The association was not replicated in a newly genotyped sample of 473 cervical dystonia cases and 481 controls. Gene-based analysis identified DENND1A to be significantly associated with cervical dystonia (P-value 1.23 × 10 -6 ). One low-frequency variant was associated with lower age-at-onset (16.4 ± 2.9 years, P-value = 3.07 × 10 -8 , minor allele frequency = 0.01), located within the GABBR2 gene on chromosome 9 (rs147331823)., Conclusion: The genetic underpinnings of cervical dystonia are complex and likely consist of multiple distinct variants of small effect sizes. 47. Oromandibular Dystonia: A Clinical Examination of 2,020 Cases.

Abstract

Objective: The goal of this study is to better characterize the phenotypic heterogeneity of oromandibular dystonia (OMD) for the purpose of facilitating early diagnosis. Methods: First, we provide a comprehensive summary of the literature encompassing 1,121 cases. Next, we describe the clinical features of 727 OMD subjects enrolled by the Dystonia Coalition (DC), an international multicenter cohort. Finally, we summarize clinical features and treatment outcomes from cross-sectional analysis of 172 OMD subjects from two expert centers. Results: In all cohorts, typical age at onset was in the 50s and 70% of cases were female. The Dystonia Coalition cohort revealed perioral musculature was involved most commonly (85%), followed by jaw (61%) and tongue (17%). OMD more commonly appeared as part of a segmental dystonia (43%), and less commonly focal (39%) or generalized (10%). OMD was found to be associated with impaired quality of life, independent of disease severity. On average, social anxiety (LSA score: 33 ± 28) was more common than depression (BDI II score: 9.7 ± 7.8). In the expert center cohorts, botulinum toxin injections improved symptom severity by more than 50% in ~80% of subjects, regardless of etiology. Conclusions: This comprehensive description of OMD cases has revealed novel insights into the most common OMD phenotypes, pattern of dystonia distribution, associated psychiatric disturbances, and effect on QoL. 48. Video-based Parkinson's disease assessments in a nationwide cohort of Fox Insight participants.

Abstract

Introduction: Parkinson's disease (PD) research is hampered by slow, inefficient recruitment and burdensome in-person assessments that may be challenging to conduct in a world affected by COVID-19. Fox Insight is an ongoing prospective clinical research study that enables individuals to participate in clinical research from their own homes by completing online questionnaires. To date, over 45,000 participants with and without PD have enrolled. We sought to validate self-reported PD diagnosis in the Fox Insight cohort, assess the validity of other self-reported health information, and evaluate the willingness of participants to participate in video-based research studies., Methods: Individuals with and without self-reported PD enrolled in Fox Insight were invited to participate in this virtual research study. Participants completed online questionnaires and two virtual visits, during which we conducted standard cognitive and motor assessments. A movement disorder expert determined the most likely diagnosis, which was compared to self-reported diagnosis., Results: A total of 203 participants from 40 U.S. states, 159 with remote clinician-determined PD and 44 without, completed the study (59% male, mean (SD) age 65.7 (9.8)). Level of agreement between self-reported PD diagnosis in Fox Insight and clinician-determined diagnosis was very good ((kappa = 0.85, 95% CI 0.76-0.94). Overall, 97.9% of participants were satisfied with the study, 98.5% were willing to participate in a future observational study with virtual visits, and 76.1% were willing to participate in an interventional trial with virtual visits., Conclusion: Among the Fox Insight cohort, self-reported diagnosis is accurate and interest in virtual research studies is high., (© 2021 Published by Elsevier Ltd.)

Abstract

Aim: To explore the functional profile of circulating monocytes and decidual macrophages at term human pregnancy and their contribution to tissue repair upon stimulation ex vivo with decidual factors and the vasoactive intestinal peptide (VIP)., Methods: Peripheral blood monocytes were isolated from pregnant and non-pregnant volunteers and tested in vitro with decidual explants from term placenta and VIP. The effect of VIP on decidual explants and the effect of its conditioned media on monocytes or decidual macrophages isolated by magnetic beads was carried out by RT-qPCR and ELISA for cytokines expression and release. Migration assays were performed in transwell systems. Efferocytosis was assessed in monocytes or decidual macrophages with CFSE-labelled autologous apoptotic neutrophils and quantified by flow cytometry. Monocyte and decidual macrophages wound healing capacity was evaluated using human endometrial stromal cell monolayers. Immunohistochemistry was performed in serial tissue sections of different placentas., Results: VIP is expressed in the villi as well as in trophoblast giant cells distributed within the decidua of term placenta. VIP induced the expression of antiinflmammatory markers and monocyte chemoattractant CCL2 and CCL3 in decidual tissues. Monocytes presented higher migration towards decidual explants than CD4 and CD8 cells. VIP-conditioned monocytes displayed an enhanced efferocytosis and wound healing capacity comparable to that of decidual macrophages. Moreover limited efferocytosis of pregnant women monocytes was restored by VIP-induced decidual factors., Conclusion: Results show the conditioning of monocytes by decidual factors and VIP to sustain processes required for tissue repair and homeostasis maintenance in term placenta., (© 2020 Scandinavian Physiological Society. Abstract

While the molecular functions of miR-200 family have been deeply investigated, a role for these miRNAs as breast cancer biomarkers remains largely unexplored. In the attempt to clarify this, we profiled the miR-200 family members expression in a large cohort of breast cancer cases with a long follow-up (H-CSS cohort) and in TCGA-BRCA cohort. Overall, miR-200 family was found upregulated in breast tumors with respect to normal breast tissues while downregulated in more aggressive breast cancer molecular subtypes (i.e. Luminal B, HER2 and triple negative), consistently with their function as repressors of the epithelial-to-mesenchymal transition (EMT). In particular miR-141-3p was found differentially expressed in breast cancer molecular subtypes in both H-CSS and TCGA-BRCA cohorts, and the combined analysis of all miR-200 family members demonstrated a slight predictive accuracy on H-CSS cancer specific survival at 12 years (survival c-statistic: 0.646; 95%CI 0.538-0.754).
