Purpose: The purpose of this study is to compare the thickness and depth measurements of the lamina cribrosa (LC) obtained using a swept-source optical coherence tomography (SS-OCT) device in idiopathic intracranial hypertension (IIH) patients and healthy subjects. Methods: This retrospective, cross-sectional observational study included 16 eyes with IIH and 20 control eyes. The LC measurements with serial horizontal B scans of the optic nerve head were obtained using SS-OCT (Topcon 3D DRI OCT Triton). The anterior lamina surface (ALS) depth, posterior lamina surface (PLS) depth, and LC thickness measurements were evaluated. Results: In patients with IIH, the mean ALS depth was 225.00 ± 58.57 μm and the mean PLS depth was 449.75 ± 63.50 μm. In the IIH control group, the corresponding values were 359.40 ± 105.38 and 570.10 ± 99.41 μm (P < 0.05). The difference in LC thickness between the IIH and control subjects was not statistically significant. Conclusion: LC can be evaluated using an SS-OCT device. LC was displaced anteriorly in patients with IIH compared with normal controls. The assessment of LC level with SS-OCT in IIH cases is a valuable and reproducible adjunctive imaging method in terms of diagnosis and follow-up.

INTRODUCTION[|]The aim of the present study was to compare intraocular pressure (IOP) level as measured with Goldmann applanation tonometry (GAT) and Icare rebound tonometry (RBT; Icare Finland Oy, Vantaa, Finland) in patients with varied central corneal thickness (CCT). [¤]METHODS[|]Total of 138 eyes of 138 consecutive patients from glaucoma department were studied. Ultrasound pachymeter was used to measure CCT in all cases and patients were divided into 3 groups according to CCT. Group 1 comprised patients with thin CCT (570 μ) composed Group 3. IOP was measured with RBT, which uses average of 6 readings, followed by GAT. Measurements of groups were compared.[¤]RESULTS[|]Mean age of patients was 59.4+-10.9 years. Mean CCT value was 494.9+-15.9 μ in Group 1 (n=40), 549.1+-16.3 μ in Group 2 (n=52), and 604.0+-22.2 μ in Group 3 (n=46). Mean IOP measurement obtained by GAT and RBT was 13.2+-4.0 and 13.8+-3.9 mmHg, respectively, in Group 1 (p=0.013), 16.9+-3.2 and 16.8+-3.1 mmHg in Group 2 (p=0.745), and 18.1+-3.4 and 18.4+-3.8 mmHg in Group 3 (p=0.359). Mean difference in IOP for entire study group between GAT and RBT was -0.21+-1.7 mmHg (p=0.15). This difference was statistically significant only in Group 1 (p=0.013). INTRODUCTION[|]This study was designed to compare optic nerve head (ONH) parameters and corneal biomechanical properties of healthy and glaucomatous macrodiscs.[¤]METHODS[|]This cross-sectional study included total of 234 eyes, of which 92 served as normal controls, 92 had healthy macrodiscs, and 50 had glaucomatous macrodiscs. Measurement of ONH parameters in all p¬atients was performed using optic coherence tomography (OCT). Corneal hysteresis (CH) was measured in each patient using Ocular Response Analyzer (ORA; Reichert, Inc., Depew, NY, USA). Central corneal thickness (CCT) was determined by ultrasonic pachymetry. All OCT and ORA parameters of healthy and glaucomatous macrodiscs were compared.[¤]RESULTS[|]Optic disc area of healthy macrodiscs and glaucomatous macrodiscs was similar, and larger than observed in control group (p=0.70, p=0.0001, respectively). Rim area was significantly thinner in glaucoma group than healthy macrodisc group or control group. (p=0.0001, p=0.022, respectively) Mean cup area of glaucomatous discs was larger than healthy macrodiscs (p=0.001). Cup/disc (C/D) area ratio, and horizontal and vertical C/D ratios were higher in glaucoma group than healthy macrodisc group (p=0.002, p=0.002, p=0.018, respectively) ORA analyses revealed that CH of glaucoma patients was lower than that of healthy macrodisc or control group (p=0.048, p=0.035, respectively), whereas, mean CH of macrodisc group was similar to that of control group (p=0.988). Mean CCT of macrodisc group was higher than that observed in control or glaucoma patients (p=0.015, p=0.045).[¤]DISCUSSION AND CONCLUSION[|] It was concluded that OCT analyses can help differentiate healthy macrodiscs from glaucomatous discs. CH and corneal thickness measurements of healthy macrodiscs are greater than those of glaucomatous optic discs.[¤]

Purpose. To describe our surgical technique in the management of pseudophakic malignant glaucoma refractory to conventional treatment. Methods. Two pseudophakic eyes with malignant glaucoma underwent peripheral iridectomy, lens capsulectomy, hyaloidectomy, and anterior vitrectomy through a clear corneal incision by using a vitreous cutter. Results. Prompt resolution of malignant glaucoma was achieved in both cases and no recurrence was observed during postoperative followup of five months. Conclusions. An anterior segment surgeon can treat pseudophakic malignant glaucoma successfully by using a vitreous cutter inserted through a corneal incision and performing peripheral iridectomy, capsulo-hyaloidectomy, and anterior vitrectomy.

In this report, we present a case with presumed unilateral tuberculosis uveitis from an endemic region. A 23-year-old male presented with decreased vision in his left eye for 15 days. Visual acuities were 1.0 in his right eye and 0.3 in his left eye. Ophthalmologic examination was normal for the right eye. Slit-lamp examination revealed 2+ cells in the vitreous without anterior chamber reaction in his left eye. Fundus examination revealed occlusive vasculitis and granuloma. His history revealed that he had a respiratory infection with fever 3 months ago while visiting his native country, Rwanda, and was treated with non-specific antibiotic therapy. His visual symptom started 2 weeks after his systemic symptoms resolved. Laboratory findings included 15 mm induration in purified protein derivative tuberculin skin test, HIV negativity, and parenchymal lesions in chest X-ray. Bronchoalveolar lavage was negative for acid-fast bacillus. A pulmonary disease consultant reported presumed tuberculosis because of the patient’s history. Anti-tuberculosis treatment was initiated. The patient’s visual acuity improved rapidly and his signs regressed. A careful history should be taken from patients with uveitis. Travel to tuberculosis-endemic areas may be important for diagnosis and should be asked about directly.

Objectives: To evaluate the effect of adjuvant intracameral triamcinolone acetonide (TA) on the surgical results of trabeculectomy with mitomycin C. Materials and Methods: All consecutive trabeculectomy cases performed in the glaucoma clinic between July 2012 and December 2013 were retrospectively reviewed from the patient charts. Only those with follow-up of 12 months or longer were included. Patients with intraoperative intracameral TA (study group, n=19) were compared to those without TA (control group, n=21) in terms of surgical success, intraocular pressure (IOP) change, medication use and complications. Results: Forty eyes of 31 patients (21 male/10 female, mean age 64.2±13.8 years) were included in the study. The mean follow-up period was 20.9±5.1 months and 20.7±6.7 months in the study and control groups, respectively (p=0.830). Baseline IOP was 26.4±9.9 and 25.2±7.6 mmHg (p=0.979), and final IOP was 12.7±2.6 and 13.6±3 mmHg in both groups respectively (p=0.226). At the final follow-up, complete success was observed in 68.4% and 52.4% of the study and control groups (p=0.349) and anti-glaucoma medication was used by 31.6% (mean number of medications: 0.79±1.2) and 47.6% (mean number of medications: 1.33±1.7), respectively (p>0.05). Bleb encapsulation, leakage, suture-lysis and hypotony rates were similar in both groups (for all, p>0.05). Cataract progression was noted in six (35.3%) and in five (26.3%) of the phakic eyes in the study and control groups, respectively (p=0.720). Conclusion: When used intracamerally, TA did not increase the complication rate in trabeculectomy surgery. Although the group that received TA showed lower IOP levels, use of fewer medications and fewer eyes requiring medication, the differences did not reach significance.

Pur po se: To evaluate the results of bleb needling of encapsulated blebs with 1% mitomycine-C (Mtm-C) after trabeculectomy surgery. Ma te ri al and Met hod: 98 eyes of 90 glaucoma patients who had undergone trabeculectomy between July 2009 and September 2011 were evaluated retrospectively. Twenty-five eyes of 24 patients who developed encapsulated bleb with at least 3-month follow-up were enrolled in the study. These patients had undergone bleb needling with 1% Mtm-C (16 eyes) since medications were not efficient for IOP control. The preand post-needling intraocular pressures (IOP) were compared with IOPs being controlled with anti-glaucoma medications (9 eyes). Re sults: Encapsulated bleb was seen in 25 (25.6%) eyes of 24 patients out of 98 eyes of 90 patients who had trabeculectomy. The mean age was 62.6±13.6 (23-83) years and the mean follow-up time was 5.7±4.4 (3-20) months. The mean time that encapsulated blebs were diagnosed was 4.1±1.5 (2.5-8) weeks after the trabeculectomy. The number of eyes with IOP controlled with medication only was 9 (36%) with 5.7±4.0 (3- 14) months mean follow-up time. Mtm-C 1% bleb needling was performed at mean 9.1±3.2 (3-15) weeks after trabeculectomy in 16 eyes (64%) which had uncontrolled IOP with medical treatment. Pur po se: To determine demographic and clinical factors that affect the outcomes of selective laser trabeculoplasty (SLT) in patients with primary open-angle glaucoma (POAG). Ma te ri al and Met hod: In this retrospective study, we analyzed the medical records of 54 eyes of 39 patients who underwent SLT treatment for POAG. Effects of demographic characteristics including age, sex, treated eyes, and family history of glaucoma on SLT success were investigated. Effects of clinical features including baseline intraocular pressure (IOP), baseline number of antiglaucomatous medications, baseline cup to disc ratio (c/d), trabecular meshwork (TM) pigmentation, central corneal thickness, and number of laser spots on SLT success were also evaluated. Success was defined as a ≥ 20% reduction in IOP without additional medications, laser, or glaucoma surgery. Pur po se: To evaluate the effectiveness of laser suture lysis in patients with high intraocular pressure (IOP) after trabeculectomy. Ma te ri al and Met hod: We evaluated retrospectively 22 eyes of 22 patients who had undergone trabeculectomy followed by laser suture lysis due to high IOP between July 2009 and February 2011. The process was performed in patients who had high IOP with bleb formation after ocular massage in early postoperative period. Re sults: The mean age was 62.7±10.8 (38-77) years and the male/female ratio was 11/11 in the study group. A 21-year-old male patient presented to our hospital with a blunt paintball trauma to his left eye. The patient had corneal edema, traumatic cataract, traumatic mydriasis, and secondary glaucoma in his left eye. After corneal edema resolved, cataract surgery was performed. Because of uncontrolled intraocular pressure despite antiglaucoma medications, an Ahmed glaucoma valve was implanted. In the postoperative period, the patient’s visual acuity was 1/10.
