Abstract

BackgroundThere is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis.MethodsWe identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, had CD4 cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4 cell count and HIV-RNA via inverse probability weighting.ResultsOut of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis.ConclusionWith the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries.

Abstract

The introduction of highly active antiretroviral therapy (HAART) has led to significant improvements in survival and morbidity for HIV-positive patients. Though HIV can now be well managed with treatment, interlinked barriers to successful treatment are still prevalent. In particular, low adherence to therapy, resistance to drugs and drug toxicity can have a considerable impact on the success of HAART. The potential of HAART is limited from the outset if patients are infected with a drug-resistant virus. Evidence suggests that a small minority of patients are starting HAART with drugs that the virus is resistant to, and consequently, are less likely to achieve virological suppression. A large proportion of resistance tests performed after patients‟ start HAART are not followed by a switch within 4 months of the result being received. This raises questions as to why the test was performed and whether limited future drug options are of concern. A single abnormal laboratory value may be the result of random fluctuations and may not necessarily be a reason for concern over drug toxicity. I derive a more stringent definition of an ALT flare and compare this definition with that commonly found in the literature. Treatment interruptions, perhaps due to drug toxicity, should not take place when the viral load is detectable. Patients who have achieved viral suppression after interrupting treatment and who have failed a higher number of HAART regimens are at a greater risk of viral rebound, though this risk is reduced substantially as duration of suppression increases. A score to characterise laboratory abnormalities is derived and used to predict mortality. Several methods were used; I felt the most appropriate was that based on the estimates from a regression model in which the current laboratory measurements were fitted; only three routinely measured laboratory measures were needed to calculate the score.

Abstract

Six isothiocyanato complexes of dioxotungsten(VI) with Mannich base ligands having the formula [WO2(NCS)2L-L] (where L-L = N-[1-morpholinobenzyl] acetamide (MBA), N-[1-piperidinobenzyl] acetamide (PBA), N-[1-morpholino(-4-nitrobenzyl)] benzamide (MPNBB), N-[1-piperidino(-3-nitrobenzyl)] benzamide (PMNBB), N-[1-morpholino(-2-nitrobenzyl)] acetamide (MONBA) and N-[1-morpholino(-3-nitrobenzyl)] acetamide (MMNBA)) have been synthesized by the reaction of tetraisothiocyanatodioxotungsten(VI) anion with the ethanolic solution of the corresponding ligand in aqueous medium in the presence of HCl. The complexes have been characterized by elemental analyses, FTIR, 1H NMR, 13C NMR, DEPT, ESI mass spectra, TGA/DTA, molar conductivity measurements, electronic spectra, magnetic moment measurements, SEM and XRD. Finally, the ligands and the corresponding complexes have been tested against Agrobacterium sp BN-2A and it has been observed that complexes show enhanced activity as compared to the ligands. The complexes on calcination give rod-like WO3 nanoparticles.

7. On the nature of supramolecular colour phenomenon

Abstract

Colour is of special interest in chemistry, physics, biology, medicine and of course for culture and life style. The relation between colour and constitution of organic materials at molecular level, to be called mono molecular colour phenomenon (MMCP), is well understood based on PPP-MO calculations of energy levels of excited states upon light absorption. However, such a relation cannot always be extrapolated to supramolecular state in which colorants normally exist both in nature and for their actual functionality. The present study provides examples of both mono molecular colour phenomenon (MMCP) and supramolecular colour phenomenon (SMCP), and a rationale of their differentiation.

Abstract

This paper is not just about the benefits of the smart plastics, but also about the potential remedies of the environmental concerns of their use. It is also a discussion about a potential pathway to an efficient use of fossil (combustion) fuels and the reduction of the emissions caused due to their use, in general. Morpholino-poly (piperazinyl-morpholinyl-triazin) imparts sustainable fire-safety and subdued incineration behaviour to plastics combined with weight reduction, and facilitates their melt processing. Additionally, it enables their ultimate environmentally friendly disposal as waste, after the ultimate use, coupled with simultaneous generation of usable energy. The knowledge & experience thus gained from energy extraction of waste plastics provide guidelines for future research to increase the efficiency of energy extraction and the reduction of emissions from combustion fuels in general. The technology is also intended to safeguard against open burning of plastics that leads to air-pollution flashover and forest fires. The various aspects of the experimental data generated are presented and plausible mechanisms of the technology are discussed.

Abstract

Introduction: Using 125Iα-Bgtx as a ligand, postsynaptic cholinergic nicotinic receptors has been identified in species. Presynaptic receptors can be defined as receptors at or near the nerve terminal that can positively or negatively modulate transmitter release, that directly or indirectly influence the probability of an action potential resulting in exocytosis. These receptors are identified by utilizing b-Bgtx. Our study involves the identification of these receptors in different regions of brain. Material and methods: The activity of postsynaptic AChR and presynaptic membrane receptor in membrane preparation and triton extract of muscle and brain tissue was determined by virtue of its high affinity for a and b-Bgtx by rapid filtration method in case of membrane preparations and ammonium sulphate precipitation method in triton extracts i.e; solubilized receptor. Results: Specific α-Bgtx binding as fmoles/mg tissue was observed in occipital lobe (6.1±0.1) parietal lobe (6±0.6), temporal lobe (5.2±1.4), cerebral cortex (6.2±0.3), hippocampus(5.6±0.2), hypothalamus (4.3±0.4) and brain stem (4.8±0.2). No specific binding of α-Bgtx was found in cerebellum and frontal lobe. β-Bgtx binding activity was present in all the above regions of brain and was in range of 4.2-4.9 fmoles/mg tissue. Presynaptic receptors also shows immunoreactivity with myasthenic sera. Conclusions: In addition to muscle, β-Bgtx binding proteins are present in each regions of the brain, while as a-Bgtx binding proteins are absent in some regions. β-Bgtx binding sites could play an important role in the pathogenesis of myasthenia gravis.

28. Thermodynamic studies of some electrolytes in formamide

Abstract

The work presented in this thesis constitutes a thermodynamic study of the behaviour of the electrolytes CdCl2, HCl, HBr and CdBr2 in formamide by means of e.m.f., solubility and calorimetric measurements. The e.m.f. of cells, (1) Cd/CdCl2(s)/Cl-/AgCl-Ag, and (2) Cd-Hg/CdCl2(s)/Cl-/AgCl-Ag where CdCl2(s) is a solid solvate with formamide, have been measured using HCl and KCl solutions in formamide at various chloride concentrations, at 5, 10,15, 20, 25 and 30°C. The standard potential of the Cd/CdCl2(s) electrode in formamide has thus been evaluated at these temperatures. The thermodynamic quantities, ? G° and ? H° for the interaction of anhydrous cadmium chloride with formamide to form the solid solvate have been calculated. The heat of this interaction has been measured calorimetrically and is in excellent agreement with the calculated from the temperature dependence of e.m.f. data. The composition of the complex has been established by thermo-gravimetric analyses to be CdCl2.21/2 HCONH2. The solubility of CdCl2 in NaCl solutions in formamide has been measured, using a radio-chemical technique at various chloride concentrations and temperatures. At 25°C, the solubility of CdCl2 in pure formamide is 0.022 g moles Kg-1. Measurements of the e.m.f. of the cell, Cd-Hg/CdCl2(m)/AgCl-Ag, where m is the molality of the CdCl2 in unsaturated solutions in formamide, have also been made at 25°C, with the object of determining the standard potential of the cadmium electrode in formamide. The cells, (1) Pt, H2 / HCl (m) in formamide/ AgCl-Ag, and (2) Pt, H2 / HBr (m) in formamide/ AgBr-Ag, have been examined at 5, 15 and 25°C. The standard potential of AgCl-Ag and AgBr-Ag electrodes have been evaluated at these temperatures. The results for the cell Pt, H2/ HCl (m) in formamide / AgCl-Ag at 25°C are in good agreement with a recent literature value. The activity coefficients of HCl and HBr in this solvent have been calculated. Measurements on the cell Pt, H2/HCl (m) in formamide/AgCl-Ag were combined with the solubility products of AgCl in formamide, measured at 15 and 25°C by Dhabanandana1 and the standard potential of the silver electrode E°Ag was calculated at these temperatures. Measurements on the cell Pt, H2 / HBr (m) in formamide/AgBr-Ag together with the above E°Ag values have then been used to calculate the solubility products for AgBr in formamide at 15 and 25°C, The heat of precipitation of AgBr in formamide has been measured calorimetrically and is in good agreement with the value calculated from the temperature dependence of the solubility product. The cells, (1) Cd/CdBr (s)/Br-/AgBr-Ag and (2) Cd-Hg/CdBr(s) /Br-/AgBr-Ag have also been studied at 5,15 and 25°C. The e.m.f.'s of these cells were combined with the standard potential of AgBr-Ag electrode at these temperatures to evaluate the standard potential of the Cd-CdBr2 electrode.

Abstract

HIV-1 drug resistance represents a major obstacle to infection and disease control. This retrospective study analyzes trends and determinants of resistance in antiretroviral treatment (ART)-exposed individuals across 7 countries in Europe. Of 20 323 cases, 80% carried at least one resistance mutation: these declined from 81% in 1997 to 71% in 2008. Predicted extensive 3-class resistance was rare (3.2% considering the cumulative genotype) and peaked at 4.5% in 2005, decreasing thereafter. The proportion of cases exhausting available drug options dropped from 32% in 2000 to 1% in 2008. Reduced risk of resistance over calendar years was confirmed by multivariable analysis. © 2013 The Author.

Abstract

A shock-tube and static reactor were used to investigate the kinetic parameters for the unimolecular thermal isomerizations of methylcyclopropane to four isomeric butenes at temperatures ranging from 695 to 1154 K. The kinetic parameters of cis- and trans-2-butene were found to be nearly similar. The overall reaction for 2-methylpropane formation was found to be higher than that for the production of 1-butene and of cis- and trans-2-butene because of the higher energy needed to break the C(2)-C(3) bond.

Abstract

Background: There is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis. Methods: We identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, hadCD4+ cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4+ cell count and HIV-RNA via inverse probability weighting. Results: Out of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis. Conclusion: With the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries. © 2014 Wolters Kluwer Health | Lippincott Williams & Wilkins.

Abstract

Background & objectives: Dried blood spotted on to filter paper has been found suitable for a large number of studies. In tropical countries with varying temperature conditions the use of dried blood needs to be validated. We carried out this study to assess the use of blood spotted filter paper as a transport system to study genotyping of Apo E gene. Methods: Fifty five patients visiting Cardiothoracic Neuroscience Centre (CNC) OPD at the All India Institute of Medical Sciences (AIIMS), New Delhi, and referred for lipid investigations to Cardiac Biochemistry Laboratory were selected at random. Blood was spotted on to Whatman 3 MM filter paper, dried and stored at room temperature. Genomic DNA was extracted and genotyping was carried out at the end of 0, 3 and 12 months. The study was further validated using samples collected on to filter paper from four centres and stored for eight years at room temperature. The temperature and humidity conditions of the centre varied widely. Results: Fifty five samples collected on to filter paper showed exact match of the genotyping when compared to fresh blood. In dried blood samples collected and stored for 1 yr at room temperature DNA extraction and apo E genotyping was done successfully. Interpretation & conclusions: The present results showed the feasibility of using dried blood samples on filter paper for apo E genotyping in tropical temperature. The findings need to be validated on a large sample before being recommended for use.

Abstract

Synthesis of the four possible dithiophene analogs of biphenylene (1), viz., cyclobuta[1,2-b:3,4-d′]dithiophene (3), cyclobuta[1,2,-b:3,4-b′]-dithiophene (4), cyclobuta[1,2-b:3,4-c′]dithiophene (5), and cyclobuta-[1,2-c:3,4-c′]dithiophene (6) has been explored using several different approaches. All these attempts have been unsuccessful similarly as the former efforts of Mc Omie and Wynberg. Equally unsuccessful were the attempts to obtain the iron tricarbonyl complexes of these compounds which were thought to be more stable. The HMO calculations carried out for cyclobutadithiophenes indicate that these four compounds will be highly unstable, also in view of the considerable ring strain.

The decomposition of tert-butyl alcohol (TBA) is examined through determinations of deuterium kinetic isotope effects and DFT calculations to define the transition structure for the reaction. Calculations with B3LYP/6-31G theory defined the transition structure for 1,2-elimination reaction, the internal reaction coordinate path, and kH/KD predictions.

Abstract

Introduction: Using 125Iα-Bgtx as a ligand, postsynaptic cholinergic nicotinic receptors has been identified in species. Presynaptic receptors can be defined as receptors at or near the nerve terminal that can positively or negatively modulate transmitter release, that directly or indirectly influence the probability of an action potential resulting in exocytosis. These receptors are identified by utilizing b-Bgtx. Our study involves the identification of these receptors in different regions of brain. Material and methods: The activity of postsynaptic AChR and presynaptic membrane receptor in membrane preparation and triton extract of muscle and brain tissue was determined by virtue of its high affinity for a and b-Bgtx by rapid filtration method in case of membrane preparations and ammonium sulphate precipitation method in triton extracts i.e; solubilized receptor. Results: Specific α-Bgtx binding as fmoles/mg tissue was observed in occipital lobe (6.1±0.1) parietal lobe (6±0.6), temporal lobe (5.2±1.4), cerebral cortex (6.2±0.3), hippocampus(5.6±0.2), hypothalamus (4.3±0.4) and brain stem (4.8±0.2). No specific binding of α-Bgtx was found in cerebellum and frontal lobe. β-Bgtx binding activity was present in all the above regions of brain and was in range of 4.2-4.9 fmoles/mg tissue. Presynaptic receptors also shows immunoreactivity with myasthenic sera. Conclusions: In addition to muscle, β-Bgtx binding proteins are present in each regions of the brain, while as a-Bgtx binding proteins are absent in some regions. β-Bgtx binding sites could play an important role in the pathogenesis of myasthenia gravis.

Abstract

The Arrhenius parameters for the gas phase, unimolecular structural isomerizations of 1,1,2-trimethylcyclopropane to three isomeric methylpentenes and two dimethylbutenes have been determined over a wide range of temperatures, 688–1124 K, using both static and shock tube reactors. For the overall loss of reactant, Ea = 63.7 (± 0.5) kcal/mol and log10A = 15.28 (± 0.12). These values are higher by 2.6 kcal/mol and 0.7–0.8 than previously reported from experimental work or predicted from thermochemical calculations. Ea for the formation of trans-4-methyl-2-pentene is 1.5 kcal/mol higher than Ea for the formation of the cis isomer, which is identical to the Ea difference previously reported for the formation of trans- and cis-2-butene from methylcyclopropane. Substitution of methyl groups for hydrogen atoms on the cyclopropane ring is expected to weaken the CC ring bonds, and it has been reported previously that activation energies for structural isomerizations of methylcyclopropanes do decrease substantially over the series cyclopropane > methylcyclopropane > 1,1- or 1,2-dimethylcyclopropane. However, the present study shows that the trend does not continue beyond dimethylcyclopropane isomerization. Besides reductions in CC bond energy, steric interactions may be increasingly important in determining the energy surface and conformational restrictions near the transition state in isomerizations of the more highly substituted methylcyclopropanes. © 2006 Wiley Periodicals, Inc. Abstract

Background: There is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis.Methods: We identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, hadCD4+cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4+cell count and HIV-RNA via inverse probability weighting.Results: Out of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis.Conclusion: With the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries.

Abstract

Thermal decomposition of (CH 3 ) 2 C(OH)CD 3 in a static reactor and in a shock tube reactor led to intramolecular symmetry-corrected k H /k D kinetic isotope effects for eliminations of HOH and HOD of 1.80 ′ 0.08 at 436 to 481 °C and 1.54 ′ 0.12 at 813 to 883 °C. Calculations with B3LYP/6-31G* theory defined the transition structure for the 1,2-elimination reaction, the internal reaction coordinate path, and k H /k D predictions. The reaction takes place through a four-centered transition structure approached through very different progressions of bond length changes along the reaction coordinate.

METHODS: We analyzed data from 31,620 patients with no prior ADEs who started combination antiretroviral therapy. Cox proportional hazards models were used to estimate mortality hazard ratios for each ADE that occurred in >50 patients, after stratification by cohort and adjustment for sex, HIV transmission group, number of antiretroviral drugs initiated, regimen, age, date of starting combination antiretroviral therapy, and CD4+ cell count and HIV RNA load at initiation of combination antiretroviral therapy. Abstract

Rate constants for the thermal isomerizations of vinylcyclopropane to cyclopentene have been measured over a wide temperature range, 577−1054 K, in a static reactor and a single-pulse shock tube; they are well represented by log(A, s-1) = 14.3 and Ea = 51.7 kcal/mol. This activation energy is higher than two previously reported values by some 2 kcal/mol; rate constants at high temperatures are about a factor of 2 larger than calculated from the Arrhenius parameters derived from the earlier low-temperature data. The thermal decomposition and isomerization reactions of vinylcyclobutane to give ethene plus 1,3-butadiene and cyclohexene have also been followed in shock-tube kinetic studies at 839−965 K. Abstract

We evaluated the emergence of drug resistance in patients failing first-line regimens containing one nonnucleoside reverse transcriptase inhibitor (NNRTI) administered with zidovudine (ZDV) + lamivudine (the ZDV group) or non-thymidine analogues (non-TAs) (tenofovir or abacavir, + lamivudine or emtricitabine; the non-TA group).Three hundred HIV-1-infected patients failing a first-line NNRTI-containing regimen (nevirapine, n = 148; efavirenz, n = 152) were included in the analysis. Virological failure was defined as viraemia ≥ 400 HIV-1 RNA copies/mL for the first time at least 6 months after starting the NNRTI-based regimen. For each patient, a genotypic resistance test at failure was available. The presence of drug-resistance mutations in HIV-1 reverse transcriptase was evaluated by comparing patients treated with NNRTI + zidovudine + lamivudine vs. those treated with NNRTI + non-TA.A total of 208 patients were failing with NNRTI + zidovudine + lamivudine and 92 with NNRTI + non-TA. No significant differences were observed between the non-TA group and the ZDV group regarding the time of virological failure [median (interquartile range): 12 (8-25) vs. 13 (9-32) months, respectively; P = 0.119] and viraemia [median (interquartile range): 4.0 (3.2-4.9) vs. 4.0 (3.3-4.7) log₁₀ copies/mL, respectively; P = 0.894]. Resistance to reverse transcriptase inhibitors (RTIs) occurred at a significant lower frequency in the non-TA group than in the ZDV group (54.3 vs. 75.5%, respectively; P = 0.001). This difference was mainly attributable to a significantly lower prevalence of NNRTI resistance (54.3 vs. 74.0%, respectively; P = 0.002) and of the nucleoside reverse transcriptase inhibitor (NRTI) mutation M184V (23.9 vs. 63.5%, respectively; P 0.001) in the non-TA group compared with the ZDV group. As expected, the mutation K65R was found only in the non-TA group (18.5%; P 0.001).At first-line regimen failure, a lower prevalence of RTI resistance was found in patients treated with NNRTI + non-TA compared with those treated with NNRTI + zidovudine + lamivudine. These results confirm that the choice of backbone may influence the prevalence of drug resistance at virological failure.
