14. Surviving Fatal Methemoglobinemia: A Case Report.

Abstract

Background: Methemoglobinemia is one of the rare causes of cyanosis, encountered in the Emergency Department. It can be congenital or acquired, affecting the oxygen binding capacity of hemoglobin, thus shifting the oxyhemoglobin dissociation curve to the left. It is potentially life-threatening, but it responds well to specific therapies, if recognized and intervened early. MetHb levels >70% are often described as incompatible with life. Case: Here, we describe two cases of near-fatal MH (MetHb levels >70%), one with intentional and the other with accidental ingestion of substance-producing MH, and its management which highlights how timely diagnosis and prompt appropriate treatment in the Emergency Department can be lifesaving. Both cases were at physiological extremes on arrival to the ED. However, the early clinical suspicion and prompt diagnosis of methemoglobinemia, which was confirmed by blood co-oximetry, helped expedite the delivery of specific anti-dote for methemoglobin, i.e., intravenous methylene blue. Both patients responded well to the treatment and were hemodynamically stable within 6 hours. Conclusion: Cyanosis, which does not respond to oxygen, and low saturation levels not responding to oxygen therapy should raise the suspicion of Methemoglobinemia (MH). Early diagnosis and prompt administration of methylene blue are the key factors for survival. [ABSTRACT FROM AUTHOR]

Abstract

The growing emphasis of consumers on health, sustainability, and ethical considerations has resulted in a surge in the popularity of organic food products this study's primary objective is to investigate and gain a comprehensive understanding of the key factors influencing consumer preferences, market dynamics, and supply chain management within the organic food industry. Using the smart PLS-SEM analytical technique, the research explores the relationships and impacts of various variables such as health concern and lifestyle, personal attitude, environmental benefit, communication, social influence, price, and organic food knowledge on consumers' purchase intentions (dependent variable) in the Delhi-NCR region. Through data analysis and expert opinion surveys, the study provides valuable insights into regional consumer trends, business opportunities, and regulatory implications specific to organic food products. It also offers recommendations for businesses to optimise supply chain management, refine marketing strategies, and develop innovative products. The research takes into account context-specific factors unique to Delhi-NCR, including cultural, socio-economic, and demographic influences, which significantly shape consumer preferences and attitudes toward organic food items. Additionally, the study's findings can be a valuable resource for programs supporting organic farmers and producers in the Delhi-NCR region, aiding their growth and success in the expanding organic food market.

Abstract

The urgent need for a cure for early phase COVID-19 infected patients critically underlines drug repositioning strategies able to efficiently identify new and reliable treatments by merging computational, experimental, and pharmacokinetic expertise. Here we report new potential therapeutics for COVID-19 identified with a combined virtual and experimental screening strategy and selected among already approved drugs. We used hydroxychloroquine (HCQ), one of the most studied drugs in current clinical trials, as a reference template to screen for structural similarity against a library of almost 4000 approved drugs. The top-ranked drugs, based on structural similarity to HCQ, were selected for in vitro antiviral assessment. Among the selected drugs, both zuclopenthixol and nebivolol efficiently block SARS-CoV-2 infection with EC50 values in the low micromolar range, as confirmed by independent experiments. The anti-SARS-CoV-2 potential of ambroxol, amodiaquine, and its active metabolite (N-monodesethyl amodiaquine) is also discussed. In trying to understand the "hydroxychloroquine" mechanism of action, both pKa and the HCQ aromatic core may play a role. Further, we show that the amodiaquine metabolite and, to a lesser extent, zuclopenthixol and nebivolol are active in a SARS-CoV-2 titer reduction assay. Given the need for improved efficacy and safety, we propose zuclopenthixol, nebivolol, and amodiaquine as potential candidates for clinical trials against the early phase of the SARS-CoV-2 infection and discuss their potential use as adjuvant to the current (i.e., remdesivir and favipiravir) COVID-19 therapeutics.
