Bronchopulmonary sequestration is an uncommon abnormality of the respiratory system that often manifests as recurrent pneumonia in childhood or as an incidental discovery on thoracic imaging in adults. The sequestration receives its blood supply from anomalous systemic circulation in contrast to pulmonary circulation and can manifest in either intrapulmonary or extrapulmonary forms. We discuss the instance of a young asymptomatic man who visited us seeking for medical clearance to travel abroad. An opacity was discovered on the chest X-ray, which was followed by a contrast-enhanced CT scan that revealed a well-defined, lobulated soft tissue attenuating lesion in the left lower hemithorax suggesting pulmonary sequestration., (© 2024 The Authors. Gastroepiploic artery aneurysms (GEAAs) are rare subtype of Splanchnic artery aneurysms (SAAs) with estimated prevalence of 0.4% out of total SAAs. Most common causes include atherosclerosis, trauma, vasculitis, infections, pancreatitis, biliary diseases etc. GEAAs are more common in male and the rupture occurs usually after 50 years. Spontaneous rupture of GEAAs in females are rare and very less cases have been reported till now. A 35 years old female with no significant past medical history and no history of trauma presented to ED with features suggestive of shock. CECT (axial and coronal plane) revealed a focal outpouching in right gastroepiploic artery with evidence of active contrast extravasation suggestive of spontaneous rupture of Right Gastroepiploic artery aneurysm. Patient was resuscitated and Exploratory Laparotomy along with ligation and aneurysmectomy. Spontaneous rupture of GEAAs in young female is very rare. Urgent treatment is needed in case of unstable patients because of high risk of rupture and higher mortality rates of GEAAs., (© 2024 The Authors. Acute gastric volvulus, a rare and life-threatening condition, often presents diagnostic challenges due to atypical symptoms. We report a unique case of nonincarcerated organo-axial gastric volvulus within a para-esophageal hiatus hernia in a 38-year-old male. Diagnosis relied on imaging, emphasizing the crucial role of CT scans. Successful open surgical intervention addressed the complexity, highlighting the evolving understanding and tailored approaches in managing this uncommon condition, essential for improved outcomes and reduced morbidity and mortality., (© 2024 The Authors. Hepatolithiasis is a rare condition requiring multidisciplinary treatment approach. In this case report we present a case of multiple hepatolithiasis successfully treated with right hepatectomy. A 54 years-old asymptomatic female with previous history of hepaticojejunostomy for recurrent CBD stone was diagnosed with hepatolithiasis during routine follow-up. Hepatolithiasis has multifactorial causation one of which is thought to be previous biliary surgery. This case report highlights the importance of routine ultrasound imaging during follow-up of the patient with history of previous biliary surgery for early detection of hepatolithiasis thus, mitigating further complications., (© 2024 The Authors. Mad honey contains grayanotoxin, which is commonly derived from the nectar of a few Rhododendron species. It is commonly used by natives of the Himalayas in the belief of its medicinal use., Case Presentation: The authors report a case of 62 years old male with mad honey poisoning who was presented to the emergency department with loss of consciousness and had bradycardia and hypotension on arrival. The patient received intravenous fluids, atropine, and vasopressor support and was closely monitored in the coronary care unit for 48 h., Discussion: Grayanotoxin I and II are believed to be primarily responsible for mad honey intoxication and act by persistent activation of voltage-gated sodium channels. Hypotension, dizziness, nausea, vomiting, and impaired consciousness are the common presentation of mad honey intoxication. Toxic effects are usually mild and close monitoring for 24-48 h is sufficient but life-threatening complications like cardiac asystole, convulsions, and myocardial infarction have also been reported., Conclusion: Most cases of mad honey intoxication only need symptomatic treatment and close observation but the potential for deterioration and life-threatening complications must also be considered., Competing Interests: The authors declare that they have no conflicts of interest., (Copyright © 2023 The Author(s).
