Abstract

Generative neural networks trained on SMILES can design innovative bioactive molecules de novo. These so-called chemical language models (CLMs) have typically been trained on tens of template molecules for fine-tuning. However, it is challenging to apply CLM to orphan targets with few known ligands. We have fine-tuned a CLM with a single potent Nurr1 agonist as template in a fragment-augmented fashion and obtained novel Nurr1 agonists using sampling frequency for design prioritization. Nanomolar potency and binding affinity of the top-ranking design and its structural novelty compared to available Nurr1 ligands highlight its value as an early chemical tool and as a lead for Nurr1 agonist development, as well as the applicability of CLM in very low-data scenarios.

Abstract

Herein, we present a novel and metal‐free approach to the stereospecific synthesis of E‐stilbenes. Starting from substituted 6‐arylhexa‐3,5‐dien‐2‐ones, a multicomponent enolacetylation/intermolecular Diels–Alder reaction was performed using ethyl acetate as a green solvent. The obtained cycloadducts were then oxidized without purification to produce the stilbenic products in good yields up to 67 % and with complete (E)‐stereospecificity and regioselectivity. Moreover, heterostilbenes were synthetized using this approach, displaying the potential applications of this protocol in pharmaceutical and material sciences. The proposed methodology was further extended to the synthesis of diarylacetylenes, furnishing a new metal‐free synthetic access to this important class of compounds. Furthermore, DFT calculations were performed confirming a concerted [4+2] reaction mechanism of the key ring‐forming step. At last, the energetic analysis of the possible transition states helps to shed some light on the experimentally observed total regio‐ and chemoselectivity. [ABSTRACT FROM AUTHOR]

Abstract

Background: Atrial fibrillation (AF) is a frequent complication of acute myocardial infarction (AMI) and is associated with a worse prognosis. Patients with chronic kidney disease are more likely to develop AF. Whether the association between AF and glomerular filtration rate (GFR) is also true in AMI has never been investigated. Methods: We prospectively enrolled 2445 AMI patients. New-onset AF was recorded during hospitalization. Estimated GFR was estimated at admission, and patients were grouped according to their GFR (group 1 (n = 1887): GFR >60; group 2 (n = 492): GFR 60–30; group 3 (n = 66): GFR <30 mL/min/1.73 m2). The primary endpoint was AF incidence. In-hospital and long-term (median 5 years) mortality were secondary endpoints. Results: The AF incidence in the population was 10%, and it was 8%, 16%, 24% in groups 1, 2, 3, respectively (p < 0.0001). In the overall population, AF was associated with a higher in-hospital (5% vs. 1%; p < 0.0001) and long-term (34% vs. 13%; p < 0.0001) mortality. In each study group, in-hospital mortality was higher in AF patients (3.5% vs. 0.5%, 6.5% vs. 3.0%, 19% vs. 8%, respectively; p < 0.0001). A similar trend was observed for long-term mortality in three groups (20% vs. 9%, 51% vs. 24%, 81% vs. 50%; p < 0.0001). The higher risk of in-hospital and long-term mortality associated with AF in each group was confirmed after adjustment for major confounders. Conclusions: This study demonstrates that new-onset AF incidence during AMI, as well as the associated in-hospital and long-term mortality, increases in parallel with GFR reduction assessed at admission.

26. Abstract 12282: Impact of New Onset Atrial Fibrillation on Long-term Mortality According to Renal Function in Acute Myocardial Infarction

Abstract

Introduction:Atrial fibrillation (AF) is a frequent complication of acute myocardial infarction (AMI), occurring in 6% to 21% of patients, and is associated with an increased risk of in-hospital and long-term mortality. Moreover, patients with chronic kidney disease are more likely to develop AF than the general population. Whether the association between AF and renal function, in terms of incidence and prognostic implications, is also true in the acute setting of AMI has not been fully investigated, thus far.Hypothesis:The aim of this study was to assess the incidence of new onset AF according to renal function and its relationship with long-term mortality in a real-world cohort of AMI patients.Methods:We included in the study 2,136 AMI patients (972 STEMI and 1164 NSTEMI). New onset AF was recorded during Coronary Care Unit stay. Patients with a history of prior AF and those discharged from the index event still in AF were not included (n=68). Glomerular filtration rate (eGFR; MDRD equation) was estimated at hospital admission and patients were grouped according to their eGFR value (Group 1 [n=1676; 78%] with eGFR>60 ml/min/1.73m2; Group 2 [n=407; 19%] with eGFR between 60 and 30 ml/min/1.73m2; Group 3 [n=53; 3%] with eGFR <30 ml/min/1.73m2). The primary endpoint was long-term (median 5 years) all-cause mortality.Results:The incidence of new onset AF in the whole population was 10%, and it was associated with a higher long-term mortality (26% vs. 17%; HR 3.19 [95% CI 2.46-4.16] P<0.001). The AF incidence was 8%, 17%, and 26% in Group 1, 2, and 3, respectively (P<0.001). Long-term mortality increased going from Group 1 to Group 2 and to Group 3 (11%, 33%, 64%, respectively; P<0.001). In each group, it was significantly higher in patients with AF as compared to those without (Group 1: 22% vs. 10%, HR 2.57 [1.69-3.91], P<0.001); Group 2: 52% vs. 29%, HR 2.18 [1.45-3.27], P<0.001; Group 3: 86% vs. 56%, HR 1.95 [0.93-4.08]; P=0.07).Conclusions:The study indicates that the incidence of new onset AF during the acute phase of AMI increases in parallel with the severity of renal dysfunction. The high long-term mortality risk associated with reduced renal function is further increased when AF occurs during the index event.

27. A straightforward synthesis of functionalized 6 H -benzo[ c ]chromenes from 3-alkenyl chromenes by intermolecular Diels-Alder/aromatization sequence.

Abstract

A new and metal-free approach to the synthesis of substituted 6 H -benzo[ c ]chromenes has been developed. This three-step synthetic sequence starts from variously substituted salicylaldehydes and α,β-unsaturated carbonyl compounds to form the chromene core. The de novo ring-forming key step is based on a highly regioselective intermolecular Diels-Alder cycloaddition between 3-vinyl-2 H -chromenes and methyl propiolate, followed by oxidative aromatization of the cyclohexadiene cycloadduct intermediate to obtain the final products in good yields (up to 94% over two steps). A modular and divergent design was followed, including a multicomponent reaction, to maximize the scaffold diversity obtained from our approach. The mechanism, investigated by DFT calculations, was confirmed to be concerted through a slightly asynchronous transition state. Energetic analysis of the transition states which have been found confirmed the experimental results in terms of regioselectivity and reactivity tendencies.
