2. The role of psychological flexibility in birth experience for first-time mothers : a mixed methods study

Abstract

Background: Childbirth is a momentous event for a woman where they can feel empowered and transformed. Negative experiences of birth are linked to development of perinatal mental health difficulties including postnatal depression and birth trauma (Ayers et al., 2016; Bell & Anderson, 2016). Birth can be experienced as negative when birth expectations are not met (Goodman et al., 2004; Hauck et al., 2007). Psychological Flexibility (PF) is a concept adopted within the Acceptance and Commitment Therapy (ACT) model (Hayes et al., 1999). PF could illustrate how a woman adapts to situational demands of birth, shifts perspective or expectations and balances competing wishes and values that encompass expectations of birth. This study aimed to 1) explore the relationships between PF, birth satisfaction and birth expectations, 2) explore whether women report PF-related skills as playing a role in sense-making of birth, and 3) explore what aspects women describe as helping and/or hindering sense-making of birth. Methods: A sequential explanatory design following two phase, mixed methods design was used. Phase One involved collection of data pre- and post-birth (N = 68) to explore whether level of PF moderated appraisal of birth. Phase Two involved semi-structured interviews (N = 11) exploring what aspects played a role in sense-making of birth to further explain Phase One findings. Phase One analysis involved nonparametric correlations, independent samples t-test and a Content Analysis. Phase Two involved a deductive-inductive Thematic Analysis (TA). Results: For Phase One, nonparametric correlations indicated no significant relationship between PF and Birth Satisfaction or Birth Expectations. A significant negative relationship was found between Birth Satisfaction and Birth Expectations (p < .001). An independent samples t-test indicated a significant relationship between Birth Satisfaction and Birth Type (p < .001). A Content Analysis indicated the most common reasons for unmet birth expectations: intervention needed, length of labour and medical complications. For Phase Two, the deductive TA considered two themes: Psychological Flexibility and Psychological Rigidity. The theme Psychological Flexibility captured aspects that resembled the six core therapeutic processes of PF (Hayes et al., 2006). The theme Psychological Rigidity captured aspects resembling the six core pathological processes of psychological rigidity (Hayes et al., 2006). The inductive TA constructed five themes: Support & Care, Choice/Control, Personal Processes, Preparedness and Birth Processes. The first theme Support & Care involved four subthemes: communication, influence of others, healthcare and talking. The third theme Personal Processes involved three subthemes: mindset, attributes and internal processes. The fifth theme Birth Processes involved two subthemes: labour and medical challenges. Discussion: Current conceptualisations of birth satisfaction are potentially not capturing important nuances. Discrepancies between Phase One rand Phase Two could be explained by interviews enabling reflective capacity. PF appears to play an important role in sense-making of birth but sense-making also involves other complex processes. Findings could suggest changes to maternity care e.g., antenatal education and providing opportunities for sense-making. Future research could explore the function of acceptance in birth satisfaction and its relationship with perinatal mental health.

Abstract Background There is very little genetically informative research identifying true environmental risks for psychiatric conditions. These may be best explored in regions with diverse environmental exposures. The current study aimed to explore similarities and differences in such risks contributing to depression and fatigue. Methods Home interviews assessed depression (lifetime-ever), fatigue and environmental exposures in 4,024 randomly selected twins from a population-based register in the Colombo district of Sri Lanka. Results Early school leaving and standard of living showed environmentally-mediated effects on depression, in men. In women, life events were associated with depression partly through genetic pathways (however, the temporal order is consistent with life events being an outcome of depression, as well as the other way around). For fatigue, there were environmentally mediated effects (through early school leaving and life events) and strong suggestions of family-environmental influences. Conclusions Compared to previous studies from higher-income countries, novel environmentally-mediated risk factors for depression and fatigue were identified in Sri Lanka. But as seen elsewhere, the association between life events and depression was partially genetically mediated in women. These results have implications for understanding environmental mechanisms around the world.

Abstract

Abstract Background The Sri Lankan twin registry is one of the first to be established in a developing country, and its design has ensured sampling from a wide range of environmental conditions. It thus has great potential to examine environmental and genetic influences on diverse phenotypes, including psychiatric disorders, in the context of a diversity of environmental exposures, which may not have been fully explored in previous twin studies in developed countries. This paper presents the rationale for the study, describes its context, and the methods for twin ascertainment and data collection. Methods A population-based twin register was established in the Colombo district of Sri Lanka using infrastructure designed to periodically update the electoral register. We invited a subsample from this register to participate in the project on common mental disorders, using random ascertainment. A separate non-twin sample was randomly selected from the geographical areas where twins were found. Home interviewers collected diagnostic information on common mental disorders, as well as environmental exposures including life events, socio-economic conditions, and the impact of the civil war and the Tsunami of 2004. Results We identified 19,302 individuals in the creation of the population based twin register. We randomly selected a subsample, of whom 4,387 were eligible to participate and 4,024 agreed to be interviewed (including data on 1,954 complete pairs of twins and 5 sets of triplets). Those who refused consent had a similar mean age and sex ratio to those who were interviewed. We invited 2,485 singletons to participate and 2,019 were interviewed. Conclusion Initial exploration of the data suggests the samples are very representative of the Colombo district of Sri Lanka, so we have created a unique resource for understanding the influences on mental disorders in developing countries, and to compare to the influences found in developed countries.

12. Peripheral neuropathy secondary to a ‘domino’ liver transplant:a case report

Abstract

BackgroundPeripheral neuropathy caused by amyloidosis is one of the well-recognised sequelae of mutations in the transthyretin gene (TTR).Case presentationWe describe a case of peripheral neuropathy in a White British 74 year old man with wild-type TTR, 8 years following receipt of a ‘domino’ liver transplant (from a donor with a TTR mutation). The clinical phenotype and neurophysiology, coupled with presence of ATTR amyloid deposits on fat biopsy, established the diagnosis of ATTR amyloid neuropathy, as a consequence of receipt of a variant-TTR secreting liver. A nerve biopsy was not clinically appropriate for this patient. Such cases are rare since recipients of such livers are typically restricted to people whose natural lifespan is unlikely to stretch into the anticipated symptomatic period of ATTR amyloidosis. However, novel “gene silencing” therapeutics are now available which can dramatically alter the course of this disorder, by reducing the proportion of abnormal proteins.ConclusionsThis represents a rare but predictable iatrogenic side effect, and doctors should be aware of this eventuality occurring in a shorter time span than previously anticipated.

16. Genetic Overlap between Measures of Hyperactivity/Inattention and Mood in Children and Adolescents

Abstract

Objective: Evidence suggests that there is substantial comorbidity between attention-deficit/hyperactivity disorder (ADHD) and major depressive disorder in childhood and adolescence. This study aims to investigate the degree to which etiological factors are shared between the symptoms of these significantly heritable disorders. Method: A twin study design was used to determine to what extent the covariation between the traits of ADHD and depression is genetically or environmentally mediated, based on parental reports. A general community sample of 645 twin pairs aged 5 to 17 years from the Cardiff Study of All Wales and North England Twins project took part in the study. Parent-rated measures of hyperactivity/inattention (Abbreviated Conners Hyperactivity subscale) and depression (Short Mood and Feelings Questionnaire). Results: Phenotypes derived from the scales were significantly correlated in both boys and girls. Bivariate structural equation modeling revealed a large overlap in underlying genetic factors (boys, r[subscript A] = 0.77; girls, r[subscript A] = 0.67) along with a smaller influence of nonshared environment. Conclusions: These findings suggest that there are common genes conferring liability to both hyperactive/inattentive and depressive traits in children and adolescents. This has implications for future molecular genetic research into ADHD and major depressive disorder. Additionally, it indicates that the comorbid clinical presentation of these disorders may reflect a common genetic pathway. (Contains 3 tables and 1 figure.)

18. Genetic and Environmental Influences on Victims, Bullies and Bully-Victims in Childhood

Abstract

Background: Three groups of children are involved in bullying: victims, bullies and bully-victims who are both bullies and victims of bullying. Understanding the origins of these groups is important since they have elevated emotional and behavioural problems, especially the bully-victims. No research has examined the genetic and environmental influences on these social roles. Method: Mother and teacher reports of victimisation and bullying were collected in a nationally representative cohort of 1,116 families with 10-year-old twins. Model-fitting was used to examine the relative influence of genetics and environments on the liability to be a victim, a bully or a bully-victim. Results: Twelve percent of children were severely bullied as victims, 13% were frequent bullies, and 2.5% were heavily involved as bully-victims. Genetic factors accounted for 73% of the variation in victimisation and 61% of the variation in bullying, with the remainder explained by environmental factors not shared between the twins. The covariation between victim and bully roles (r = 0.25), which characterises bully-victims, was accounted for by genetic factors only. Some genetic factors influenced both victimisation and bullying, although there were also genetic factors specific to each social role. Conclusions: Children's genetic endowments, as well as their surrounding environments, influence which children become victims, bullies and bully-victims. Future research identifying mediating characteristics that link the genetic and environmental influences to these social roles could provide targets for intervention.

Abstract

We operate a consultant led ‘post-acute’ model for Acute Neurology service delivery, with Advice and Guidance (an email service for GPs); attendance at twice daily ‘huddle’ meetings on MAU; and inpatient e-referrals. We have two sites, 683 inpatient beds in Gloucester Royal, and 379 in Cheltenham General. 10–15 of these inpatient beds (in Gloucester) are flexibly allocated under neurology.We received 153 inpatient e-referrals over a 1 month audit (October 2017). 78% of these received an inpatient review (the remaining 22% were managed by email); 97% within 24 hours, 7 days per week; 100% by consultants. This averages 5 referrals per day (range 0–9). Patients can be transferred under neurology when beds are available (49 admissions/month).The reasons for referral included focal neurological deficits (27%), headache (21%), epilepsy (14%), movement disorders (10%), unexplained loss of consciousness or possible seizure (9%), altered consciousness (8%), neurological infections (3%), and scan abnormalities (3%).This rapid input is in line with the ABN’s Quality Standard for Unscheduled Care. The referral element of this service requires approximately 1.5–2 PAs on each weekday, and 1 PA on each weekend day. We are also about to start an acute neurology clinic (approx. 20 slots per week).

Abstract

Background: Most studies of post-traumatic stress disorder (PTSD) in low- and middle-income countries (LMICs) have focused on 'high-risk' populations defined by exposure to trauma.Aims:To estimate the prevalence of post-traumatic stress disorder (PTSD) in a LMIC, the conditional probability of PTSD given a traumatic event and the strength of associations between traumatic events and other psychiatric disorders.Method:Our sample contained a mix of 3995 twins and 2019 non-twins. We asked participants about nine different traumatic exposures, including the category ‘other’, but excluding sexual trauma.Results:Traumatic events were reported by 36.3% of participants and lifetime PTSD was present in 2.0%. Prevalence of non-PTSD lifetime diagnosis was 19.1%. Of people who had experienced three or more traumatic events, 13.3% had lifetime PTSD and 40.4% had a non-PTSD psychiatric diagnosis.Conclusions:Despite high rates of exposure to trauma, this population had lower rates of PTSD than high-income populations, although the prevalence might have been slightly affected by the exclusion of sexual trauma. There are high rates of non-PTSD diagnoses associated with trauma exposure that could be considered in interventions for trauma-exposed populations. Our findings suggest that there is no unique relationship between traumatic experiences and the specific symptomatology of PTSD.
