Abstract

A large yet heterogeneous body of literature exists suggesting that endothelial dysfunction appears early in type 1 diabetes, due to hyperglycemia-induced oxidative stress. The latter may also affect vascular smooth muscles (VSM) function, a layer albeit less frequently considered in that pathology. This meta-analysis aims at evaluating the extent, and the contributing risk factors, of early endothelial dysfunction, and of the possible concomitant VSM dysfunction, in type 1 diabetes. PubMed, Web of Sciences, Cochrane Library databases were screened from their respective inceptions until October 2019. We included studies comparing vasodilatory capacity depending or not on endothelium (i.e., endothelial function or VSM function, respectively) in patients with uncomplicated type 1 diabetes and healthy controls. Fifty-eight articles studying endothelium-dependent function, among which 21 studies also assessed VSM, were included. Global analyses revealed an impairment of standardized mean difference (SMD) (Cohen's d) of endothelial function: -0.61 (95% CI: -0.79, -0.44) but also of VSM SMD: -0.32 (95% CI: -0.57, -0.07). The type of stimuli used (i.e., exercise, occlusion-reperfusion, pharmacological substances, heat) did not influence the impairment of the vasodilatory capacity. Endothelial dysfunction appeared more pronounced within macrovascular than microvascular beds. The latter was particularly altered in cases of poor glycemic control [HbA > 67 mmol/mol (8.3%)]. This meta-analysis not only corroborates the presence of an early impairment of endothelial function, even in response to physiological stimuli like exercise, but also highlights a VSM dysfunction in children and adults with type 1 diabetes. Endothelial dysfunction seems to be more pronounced in large than small vessels, fostering the debate on their relative temporal appearance. 11

34. Effect of corrosion degree on mechanical properties of reinforcements buried for 60 years

Abstract

This paper studies the influence of the corrosion degree calculated by the mass loss and by the smaller diameters on the yield strength, ultimate strength and final elongation. Reinforcements buried and naturally corroded for 60 years were studied. The mechanical properties of the protruding reinforcing steel were compared to reference bars, which also remained buried for 60 years, but without going through the corrosion process. Micrographs, besides the chemical composition and the characterization of the aggressiveness of the soil were realized. The micrographs and the chemical composition show the presence of pits in the reinforcements and sulfur contents for steel above the prescriptions of the time when the foundations were initially implement, respectively. The results also show that the effects of pitting corrosion on the mechanical properties of the naturally corroded bars may not be adequately expressed by the mass loss. This type of corrosion (pits) produces geometric variations in the cross sections along the length of the test specimens, generating stress gradients between successive sections. This has a noticeable impact on the mechanical properties of the reinforcements. In general, the effects of corrosion are more pronounced on the ductility of the reinforcement. Regarding the aggressiveness of the soil, high corrosion rates were identified in the reinforcement, even with the soil being classified as essentially non-corrosive. Resumo Este artigo estuda a influência do grau de corrosão, calculado pela perda de massa e pelos menores diâmetros, sobre a resistência ao escoamento, limite de resistência e alongamento final em armaduras corroídas naturalmente. Essas armaduras permaneceram enterradas por 60 anos. As propriedades mecânicas das armaduras de espera foram comparadas às de barras de referência, que também permaneceram enterradas por 60 anos, mas sem sofrer o processo de corrosão. Foram realizadas micrografias, além da determinação da composição química e da caracterização da agressividade do solo onde as armaduras estavam enterradas. As micrografias e a composição química mostram a presença de pites nas armaduras e teores de enxofre acima das prescrições recomendadas para o aço na época de execução das fundações, respectivamente. Os resultados também mostram que os efeitos da corrosão por pites sobre as propriedades mecânicas das barras corroídas naturalmente podem não ser adequadamente expressos pela perda de massa. Esse tipo de corrosão (pites) produz variações geométricas nas seções transversais ao longo do comprimento dos corpos de prova, gerando gradientes de esforços entre seções sucessivas, que impactam de forma notória sobre suas propriedades mecânicas. De uma maneira geral, os efeitos da corrosão são mais pronunciáveis sobre a ductilidade das barras. Com relação à agressividade do solo, altas velocidades de corrosão foram identificadas nas armaduras, mesmo com o solo sendo classificado como essencialmente não corrosivo.

37. From Breath-hold diving to patient's bed : The normobaric oxygen paradox

Abstract

There is a great concern regarding the increased need for blood products in an aging population and the cost of safe blood transfusion in developing countries. The use of blood red cell precursor enhancers like exogenous erythropoietin (EPO) is one of the alternative techniques to blood transfusion as it stimulates the hemoglobin and reticulocytes production in anaemic patients. In this presentation, Dr. Balestra, shows the first results of increased EPO production after intermittent hyperoxia through breath-hold diving, a process also known as the normobaric oxygen paradox. After the dive, the return back from the increased oxygen partial pressure is considered by the body as a drop of oxygen in the tissues and thus leading to an increase in EPO (similar to a stay at high altitudes). Dr. Balestra was amongst the first scientists to report the possibility to increase EPO with a single non hypoxic stimulus.

© by the Aerospace Medical Association, Alexandria, VA. BACKGROUND: Divers try to limit risks associated with their sport, for instance by breathing enriched air nitrox (EANx) instead of air. This double blinded, randomized trial was designed to see if the use of EANx could effectively improve cognitive performance while diving. METHODS: Eight volunteers performed two no-decompression dry dives breathing air or EANx for 20 min at 0.4 MPa. Cognitive functions were assessed with a computerized test battery, including MathProc and Ptrail. Measurements were taken before the dive, upon arrival and after 15 min at depth, upon surfacing, and at 30 min postdive. After each dive subjects were asked to identify the gas they had just breathed. RESULTS: Identification of the breathing gas was not possible on subjective assessment alone, while cognitive assessments showed significantly better performance while breathing EANx. Before the dives, breathing air, mean time to complete the task was 1795 ms for MathProc and 1905 ms for Ptrail. When arriving at depth MathProc took 1616 ms on air and 1523 ms on EANx, and Ptrail took 1318 ms on air and and 1356 ms on EANx, followed 15 min later by significant performance inhibition while breathing air during the ascent and the postdive phase, supporting the concept of late dive/postdive impairment. DISCUSSION: The results suggest that EANx could protect against decreased neuro-cognitive performance induced by inert gas narcosis. It was not possible for blinded divers to identify which gas they were breathing and differences in postdive fatigue between air and EANx diving deserve further investigation.

Abstract

Introduction Decompression sickness (DCS) is a complex and poorly understood systemic disease caused by inadequate desaturation after a decrease of ambient pressure. Strong variability between individuals is observed for DCS occurrence. This raises questions concerning factors that may be involved in the interindividual variability of DCS occurrence. This study aimed to experimentally assess the existence of heritable factors involved in DCS occurrence by selectively breeding individuals resistant to DCS from a population stock of Wistar rats. Methods Fifty-two male and 52 female Wistar rats were submitted to a simulated air dive known to reliably induce about 63% DCS: compression was performed at 100 kPa·min-1up to 1000 kPa absolute pressure before a 45-min long stay. Decompression was performed at 100 kPa·min-1with three decompression stops: 5 min at 200 kPa, 5 min at 160 kPa, and 10 min at 130 kPa. Animals were observed for 1 h to detect DCS symptoms. Individuals without DCS were selected and bred to create a new generation, subsequently subjected to the same hyperbaric protocol. This procedure was repeated up to the third generation of rats. Results As reported previously, this diving profile induced 67% of DCS, and 33% asymptomatic animals in the founding population. DCS/asymptomatic ratio was not initially different between sexes, although males were heavier than females. In three generations, the outcome of the dive significantly changed from 33% to 67% asymptomatic rats, for both sexes. Interestingly, survival in females increased sooner than in males. Conclusions This study offers evidence suggesting the inheritance of DCS resistance. Future research will focus on genetic and physiological comparisons between the initial strain and the new resistant population.

Abstract

Bubbles can form in the body during or after decompression from pressure exposures such as those undergone by scuba divers, astronauts, caisson and tunnel workers. Bubble growth and detachment physics then becomes significant in predicting and controlling the probability of these bubbles causing mechanical problems by blocking vessels, displacing tissues, or inducing an inflammatory cascade if they persist for too long in the body before being dissolved. By contrast to decompression induced bubbles whose site of initial formation and exact composition are debated, there are other instances of bubbles in the bloodstream which are well-defined. Gas emboli unwillingly introduced during surgical procedures and ultrasound microbubbles injected for use as contrast or drug delivery agents are therefore also discussed. After presenting the different ways that bubbles can end up in the human bloodstream, the general mathematical formalism related to the physics of bubble growth and detachment from decompression is reviewed. Bubble behavior in the bloodstream is then discussed, including bubble dissolution in blood, bubble rheology and biological interactions for the different cases of bubble and blood composition considered., REVIEW, info:eu-repo/semantics/published

49. The 'Normobaric Oxygen Paradox'. a new tool for the anaesthetist?

Abstract

Hypoxia is the natural trigger for endogenous EPO production but recently the use of intermittent hyperoxia to stimulate EPO has been postulated and this phenomenon has been called the "normobaric oxygen paradox" (NOP). The "NOP" is a mechanism by which oxygen regulates the expression of the Hypoxia Inducible Factor 1 alpha (HIF-1α). The HIF-1α-depending gene regulation is responsible for many different genetic expressions including EPO and VEGF. It has been proposed that relative changes of oxygen availability rather than steady state hypoxic or hyperoxic conditions, play an important role in HIF transcriptional effects. According to this hypothesis, the cell interprets the return to normoxia after a hyperoxic event as an oxygen shortage, and induces HIF-1-regulated gene synthesis, including EPO. Being both a hormone and a cytokine, the actual actions of EPO are complex; its clinical utility has been postulated for neuroprotection and cardioprotection. The precise level of inspired oxygen and the exact timeframe for its iterative administration are not totally known. N-Acetyl-L-Cysteine (NAC) supplementation has been shown to help. All the reported data demonstrate how hyperoxic and hypoxic states can potentially be manipulated if oxygen is been considered as a multifaceted molecule more than just a gas.
