20. Méningite à streptocoque β-hémolytique du groupe A chez une femme enceinte = Group A streptococcal meningitis in a pregnant woman

Abstract

Cet article décrit la prise en charge d'une méningite à streptocoque β-hémolytique du groupe A (pyogenes), apparue chez une jeune patiente enceinte en bonne santé habituelle. Ce cas permet de discuter la littérature publiée concernant cette situation. Dans le cadre de méningite bactérienne, ce germe est rarement retrouvé. La plupart du temps les patients sont immunocompétents. Ils présentent parfois une atteinte neurologique sévère. Le pronostic est bon sous traitement de pénicilline. Vu la fréquence des effets invasifs et graves retrouvés lors des infections par le Streptococcus pyogenes, ce germe pourrait dans le futur, assumer un rôle plus important dans la méningite bactérienne. Cette recrudescence pourrait être rapprochée de celle présente dans l'ère d'avant les antibiotiques.

Abstract

We report about an 80-year-old patient, who underwent the extraction of an upper molar tooth because of facial pain. In the course of time the patient developed a maxillary sinusitis in presence of an ectopic tooth. Given that the patient got fever, neck pain and -stiffness, a purulent meningitis was first suspected. The liquor analysis was normal and the CT-scan showed a calcification around the dens axis. We finally diagnosed a “Crowned Dens”-syndrome.
