Richter's syndrome represents the progression of chronic lymphocytic leukemia (CLL) to more aggressive diseases, most frequently diffuse large B-cell lymphoma, while Hodgkin's lymphoma (HL) and hairy cell leukemia (HCL) are rarely described. The first case involved a 67-year-old man with a diagnosis of a high-risk stage-II CLL treated with rituximab and ibrutinib, developed a HL nodular sclerosis variant after three months of therapy for CLL. After achieving a complete remission for HL and ibrutinib cessation because of drug-related cardiotoxicity, the patient relapsed after five months off-therapy and died due to disease progression after two cycles of brentuximab-vedotin. The second case involved an 83-year-old female with a diagnosis of stage-IV CLL treated with rituximab plus bendamustine who developed a HCL eight years later. Pentostatin was unsuccessfully employed as upfront HCL therapy, and the patient was then switched to rituximab while in remission for CLL. In conclusion, Richter's transformation risk rate might be higher in patients treated with novel targeted therapies, and multiparametric flow cytometry and lymph node biopsy at relapse could help in early identifying small clones. The treatment of predominant neoplasia is mandatory, and disease-specific drugs are administered; however, clinical efficacy might be lower in these patients. [ABSTRACT FROM AUTHOR]

Endometrioid endometrial cancer is the most common gynaecological tumor in developed countries, and its incidence is increasing. The definition of subtypes, based on clinical and endocrine features or on histopathological characteristics, correlate to some extent with patient's prognosis, but there is substantial heterogeneity within tumor types. The search for molecules and mechanisms implied in determining the progression and the response to therapy for this cancer is still ongoing. BAG3 protein, a member of BAG family of co-chaperones, has a pro-survival role in several tumor types. BAG3 anti-apoptotic properties rely on its characteristic to bind several intracellular partners, thereby, modulating crucial events such as apoptosis, differentiation, cell motility, and autophagy. BAG3 expression in human endometrial cancer tissues was not investigated so far. Here, we show that BAG3 protein levels are elevated in tumoral and hyperplastic cells in respect to normal glands. Furthermore, BAG3 subcellular localization appears to be changed in tumoral compared to normal cells. Our results indicate a possible role for BAG3 protein in the maintenance of cell survival in endometrioid endometrial cancer and suggest that this field of studies is worthy of further investigations. 13. Intra- and inter-rater agreement of a new classification system of gingival recession defects.

Purpose: Several classification systems have been proposed in the literature, but none of them has been validated by means of an appropriate statistical analysis and, therefore, it is unknown whether these systems work in the same manner among different clinicians. The aim of the present study is to investigate the intra- and inter-rater agreement of a new system to classify gingival recession defects and to evaluate its agreement among different clinicians. Materials and methods: A new classification system was proposed and tested considering the following three factors: the amount of keratinised tissue (<2 or ≥2 mm), the presence/absence of noncarious cervical lesions and the presence/absence of interproximal attachment loss. Kappa-statistics were performed to analyse the intra-rater and inter-rater agreement among three blinded examiners. Results: A total of 120 gingival recessions were evaluated using the new classification system. The intra-rater agreement ranged from 0.74 to 0.96 for the variable keratinised tissue, from 0.67 to 0.94 for the variable non-carious cervical lesions and from 0.70 to 0.92 for the variable interproximal attachment loss. The inter-rater agreement values ranged from 0.70 to 0.85 for keratinised tissue, from 0.54 to 0.59 for non-carious cervical lesions and from 0.54 to 0.77 for the interproximal attachment loss. Conclusions: Based on the results of this study, the proposed new classification system showed moderate to substantial agreement among investigators when used in the present study population and was therefore able to diagnose the severity of gingival recession defects. 14. The Treatment of Gingival Recession Associated with Deep Corono-Radicular Abrasions (CEJ step) -- a Case Series.

The present case series study illustrates two different mucogingival techniques for treating gingival recessions associated with deep dental abrasions. The therapeutic strategies depend on the depth of the abrasion measured at the mid most coronal point at the CEJ level (CEJ step). When the CEJ step was <1 mm, a connective tissue graft associated with an extremely positioned coronal flap was performed. In case of a CEJ step ≥1 mm, a double connective tissue graft with an extremely positioned coronal flap was used. The dental abrasions and/or CEJ were not mechanically treated in either situation. One year later, the results showed a recession reduction of 97.5%, with complete root coverage obtained in 82% of sites, and dental hypersensitivity reduction obtained in 5 out of the initial 7 sites. There were no new cases of dental hypersensitivity at the end of follow-up period as a consequence of the treatment. This case series report shows that optimal clinical results in terms of complete root coverage of gingival recessions associated with deep cervical abrasions can be achieved by performing an appropriate surgical procedure without removing dental tissue. Endometrioid endometrial cancer is the most common gynaecological tumor in developed countries, and its incidence is increasing. The definition of subtypes, based on clinical and endocrine features or on histopathological characteristics, correlate to some extent with patient's prognosis, but there is substantial heterogeneity within tumor types. The search for molecules and mechanisms implied in determining the progression and the response to therapy for this cancer is still ongoing. BAG3 protein, a member of BAG family of co-chaperones, has a pro-survival role in several tumor types. BAG3 anti-apoptotic properties rely on its characteristic to bind several intracellular partners, thereby, modulating crucial events such as apoptosis, differentiation, cell motility, and autophagy. BAG3 expression in human endometrial cancer tissues was not investigated so far. Here, we show that BAG3 protein levels are elevated in tumoral and hyperplastic cells in respect to normal glands. Furthermore, BAG3 subcellular localization appears to be changed in tumoral compared to normal cells. Our results indicate a possible role for BAG3 protein in the maintenance of cell survival in endometrioid endometrial cancer and suggest that this field of studies is worthy of further investigations. 32. A rare case of squamous cell carcinoma of a proliferating trichilemmal tumour

Squamous Cell Carcinoma (SCC) includes several subtypes and these can be classified according to their clinical behaviours from the least invasive to the most aggressive. Moreover, it is appropriate to analyse their clinicalpathological patterns 1. In light of its wide histopathological variability, we encountered a rare malignant cutaneous lesion: a SCC of a proliferating trichilemmal tumour (PTT) nature.We describe the clinical and pathologic findings of an 82-year-old woman, who developed a SCC with areas of trichilemmal differentiation. It has a surprising thickness of 25 mm, which passes through the dermis, infiltrates the hypodermis and evolves quickly.Considering its exophytic growth and the negative results of the post operative tests (ultrasounds confirmed absence of metastasis), the tumour was eradicated thanks to the surgical approach (a 1 cm border wide excision, including the periosteum in the central part of the lesion, followed by its reconstruction using a full-thickness skin graft) and the prompt intervention, without need for further complimentary treatments. The overall aesthetic results were pleasing.Adnexal tumour, PPT, SCC, Surgery.Il carcinoma sqaumocellulare annovera diversi sottotipi classificabili in base alle loro aggressività. In virtù di questa grande variabilità istopatologica riportiamo il caso clinico di un’ottantaduenne caucasica affetta da una rara forma di carcinoma a cellule squamose, con aree di differenziazione trichilemmale/pilare. Tale paziente giunge alla nosta osservazione con una lesione ulcerata, esofitica, nodulare, di 4 cm x 4 cm, localizzata in regione frontale, già precedentemente sottoposta a biopsia incisionale presso altra struttura e con diagnosi di “adenocarcinoma moderatamente differenziato con immunofenotipo compatibile con istogenesi annessiale cutanea”. Considerate l’età della paziente, le comorbidità nonchè la sua storia clinica, effettuate le dovute indagini preoperatorie, si procede all’intervento chirurgico: ampia escissione en bloc della lesione con un margine radiale di 1 cm e approfondimento fino al periostio, incluso nella porzione centrale della lesione. Chiusura della perdita di sostanza post-chirurgica a mezzo di innesto a tutto spessore prelevato dalla regione anteriore del braccio destro. L’intervento chirurgico è privo di complicanze. Il referto anatomo-patologico diagnostica, con valutazione definitiva, “un carcinoma a cellule squamose, moderatamente differenziato, con aree di differenziazione trichilemmale/ pilare, infiltrante l’ipoderma con uno spessore di 25mm. Margini chirurgici indenni”. Sebbene sia noto un comportamento non invasivo di questo tipo di lesione, la letteratura evidenzia molteplici casi di aggressività locoregionale e presenza di potenziale metastatico, strettamente legati all’infiltrazione dei tessuti adiacenti la neoplasia. Si rendono quindi opportuni follow-up e consulenza oncologica. Alla rivalutazione della paziente, non vi è evidenza di recidiva locale clinicamente nonchè a mezzo di indagine ecografica negativa si per recidia locale che per presenza di metastasi locoregionali. Ecco che nonstante lo spessore, l’invasività della lesione e la rapidità della sua evoluzione, dato anche il suo atteggiamento esofitico, la negatività delle indagini diagnostiche e la prontezza dell’approccio terapeutico, è stato possibile eradicare il tumore grazie al solo approccio chirurgico, scongiurando la necessità di terapie complementari. L’esito estetico è stato soddisfacente.
