- Abstract
Purpose: Delays in the diagnosis and treatment of infantile hypertrophic pyloric stenosis (IHPS) may lead to laboratory and clinical decompensation in infants. The aim of this study was to determine whether pyloric measurement parameters can be effectively used in the clinical prognosis of IHPS by evaluating the relationship between pyloric dimensions on ultrasonographic evaluation at the time of presentation and clinical and laboratory parameters in infants with pyloric stenosis. Methods: A retrospective observational study was conducted on 122 infants who were followed up with the diagnosis of IHPS in our neonatal clinic and operated on by a pediatric surgeon between January 2005 and December 2020. Results: The mean pyloric dimensions of the infants were pyloric length 20 mm (17–22), pyloric thickness 5 mm (4–5.8), and pyloric index 96.5 (72.2–124). Babies with preoperative laboratory values of hypokalemia, hypochloremia and metabolic alkalosis had statistically higher pyloric thickness >5 mm (4.1–6) and pyloric index >100 (86–130) parameters. Conclusion: Pyloric thickness and pyloric index parameters can predict clinical prognosis in babies diagnosed with IHPS in the perioperative period. [ABSTRACT FROM AUTHOR]

Amaç: Mevsimsel alerjik konjonktivit (MAK) tanısı ile takip ve tedavi edilen pediatrik yaş grubu hastalarda, dönemsel olarak kullanılan ilaçların uzun dönemde göz içi basıncı (GİB) üzerine olan etkisini araştırmak. Gereç ve Yöntemler: Çalışmaya 16 yaş altında, 5 yıl veya daha fazla süredir MAK tedavisi alan 230 olgu (Grup I) ve kontrol grubuna 16 yaş altında sağlıklı 255 olgu (Grup II) dahil edildi. Hastaların GİB ölçümleri non-kontakt tonometri ile yapıldı. Her iki grubun ortalama GİB değerleri istatistiksel olarak kıyaslandı. Bulgular: Çalışmaya alınan olguların yaş ortalaması grup I'de 9.3±2.0, grup II'de 9.1±2.1 bulundu. Grup I'de hastaların MAK nedeni ile tedavi aldıkları süre ortalama 5.3±0.7 yıldı. Grup I'de ortalama GİB sağ gözde 13.7±3.1 mmHg, sol gözde 13.7±2.8 mmHg bulunurken, grup II'de ortalama GİB sağ gözde 13.8±3.1 mmHg, sol gözde 13.9±3.0 mmHg olarak bulundu. İki grubun ortalama GİB değerleri arasında istatistiksel olarak anlamlı fark olmadığı görüldü (P>0.05). Grup I'de 11 hastada (% 4.8) tedavi esnasında kortikosteroid kullanımına bağlı geçici GİB yükselmesi olduğu gözlendi. Bu hastaların tümünde kortikosteroid kesildikten sonra GİB normale döndü. Sonuç: Pediatrik olgularda MAK tanısıyla dönemsel olarak verilen medikal tedavinin uzun dönemde GİB üzerine kalıcı bir etkisinin olmadığı görüldü. Bu hastalarda olası akut GİB yükselmesini fark etmek için yakın takibin önemli olduğunu düşünmekteyiz. Objective: To investigate the effect of seasonal drugs on intraocular pressure (IOP) in the long term in pediatric age group who are followed up and treated with seasonal allergic conjunctivitis (SAC) diagnosis. Materials and Methods: The study included 230 cases under the age of 16 who received SAC treatment for 5 years or more (Group I), and 255 healthy cases under the age of 16 in the control group (Group II). IOP measurements of the patients were made with non-contact tonometry. Mean IOP values of both groups were statistically compared. Results: The mean age of the patients included in the study was 9.3±2.0 in group I and 9.1±2.1 in group II. The mean duration of treatment in SAC patients was 5.3±0.7 years. In group I, the mean IOP was 13.7±3.1 mmHg in the right eye, 13.7±2.8 mmHg in the left eye, while in group II the mean IOP was 13.8±3.1 mmHg in the right eye and 13.9±3.0 mmHg in the left eye. There was no statistically significant difference between the mean IOP values of two groups (P>0.05). Temporary IOP elevation due to corticosteroid treatment was observed in 11 patients (4.8%) in group I. In all these patients, IOP lowered to normal limits after corticosteroid treatment discontinued. Conclusion: It was observed that medical treatment, which is given periodically with the diagnosis of SAC in pediatric cases, does not have a permanent effect on IOP in the long term. We think that close follow-up is important to realize possible acute IOP elevation in these patients.

Aim: To evalResults: The most common cause was traumatic brain injury comprising 25% (n=6) of all cases. Materials and Methods: A total of 24 children admitted to a PICU for acute encephalopathy were enrolled into this study. The etiology, treatment, duration of stay in the PICU, their demographic information and their past medical history were recorded. cEEG was initiated as quickly as possible following admission to the PICU and continued for at least 24 hours. Their short-term prognosis was evaluated by the Pediatric Cerebral Performance Category score (PCPC) at PICU discharge. Conclusion: cEEG in critically ill children is useful for detecting both epileptic and non-epileptic events. The use of cEEG in PICUs can be helpful for the better management of cases. Purpose: To assess retinal and choroidal thickness changes in eyes with a first episode of unilateral non granulomatous acute anterior uveitis (AU). Methods: Thirty-one patients with acute unilateral non granulomatous AU who had undergone spectral domain optical coherence tomography (SD-OCT) of both eyes were included in this retrospective study. Retinal thickness (RT) as defined by the Early Treatment Diabetic Retinopathy Study (ETDRS) and subfoveal choroidal thickness (SFCT) obtained by SD-OCT were recorded. Healthy fellow eyes of the patients served as the control. Results: A significant thickening in 1 mm of ETDRS subfield and in subfoveal choroid was observed in eyes with acute AU when compared with the unaffected fellow eyes (P = 0.005 and P < 0.00001, respectively). RT and SFCT of affected eyes did not show a significant difference in terms of HLA-B27 antigen presence or absence (P > 0.05, for all). Conclusions: Significant posterior segment changes were detected in eyes with acute unilateral non granulomatous Objectif. — Évaluer les changements d’épaisseur rétinienne et choroïdienne dans les yeux lorsdu premier épisode d’uvéite antérieure aiguë unilatérale non granulomateuse (UA).Patients et méthodes. — Trente et un patients atteints d’UA aiguë unilatérale non granuloma-teuse qui avaient bénéficié d’une tomographie par cohérence optique en domaine spectral(SD-OCT) des deux yeux ont été inclus dans cette étude rétrospective. L’épaisseur rétinienne(ER) définie par le groupe ETDRS (Early Treatment Diabetic Retinopathy Study) et l’épaisseurde la choroïde sous-fovéale (ECSF) obtenue par SD-OCT ont été évaluées. Les yeux sains despatients ont été acceptés comme yeux témoins.Résultats. — Un épaississement significatif dans 1 mm de sous-champ ETDRS et dans la choroïdesous-fovéolaire a été observé dans les yeux atteints d’UA aiguë par rapport aux yeux témoins(p = 0,005 et p < 0,00001, respectivement). Les ER et ECSF des yeux affectés n’ont pas montré dedifférence significative en termes de présence ou d’absence de l’antigène HLA-B27 (p > 0,05).Conclusions. — Des modifications significatives du segment postérieur ont été détectées dansles yeux atteints d’UA aiguë unilatérale non granulomateuse.

20. Investigation of demodex parasite existence in treatment-resistant chronic blepharitis cases

Amaç: Demodex akarı, zorunlu bir ektoparazit olup, insanda özellikle kıl folikülleri, kirpik kökleri ve yağ salgı bezlerine yerleşmektedir. Bu çalışmanın amacı tedaviye dirençli kronik blefaritli olgularda Demodex enfestasyonu varlığını araştırmaktır. Yöntemler: Çalışmaya İstanbul Medipol Üniversitesi Tıp Fakültesi Hastanesi Göz Hastalıkları Polikliniği’ne başvuran kronik blefarit tanısı alan olgular dahil edildi. Tüm olgular daha önce en az üç kez blefarit tedavisi almış idi. Hastalardan blefaritin yoğun olduğu alanda bulunan kirpiklerinden 3-4 adet örnek alınarak ışık mikroskobu altında incelendi. Işık mikroskobunda 10x ve 40x büyütmede incelenen alanda bir veya daha fazla Demodex parazitinin görülmesi pozitif olarak kabul edildi. Bulgular: Çalışmaya 153 olgu dahil edildi. Olguların 79’u (%51,6) erkek, 74’ü (%48,4) kadın idi. Ortalama yaş 43,1±9,7 olarak bulundu. Işık mikroskopisi ile yapılan inceleme sonucunda 153 kronik blefaritli olgunun 69’unda (%45,1) kirpik foliküllerinde Demodex paraziti varlığı tespit edildi. Sonuç: Özellikle tedaviye dirençli kronik blefaritli olgularda Demodex enfestasyonunun mutlaka göz önüne alınması gerektiğini, parazitin tedavi öncesinde rutin olarak aranmasının faydalı olacağını düşünmekteyiz. OBJECTIVE: Demodex acari is an obligate ectoparasite, and it is usually located in the human hair follicles, eyelash roots, and sebaceous glands. The aim of this study was to investigate the presence of Demodex infestation in chronic blepharitis cases that are resistant to therapy. METHODS: Patients who were admitted at the Istanbul Medipol University School of Medicine Hospital with a diagnosis of chronic blepharitis were included. All cases received conventional therapy at least three times. Three or four eyelash samples from patients with blepharitis were collected and examined under light microscopy. For the diagnosis, the presence of one or more Demodex parasites at 10× and 40× magnification by a light microscope was considered as positive for infestation. RESULTS: Overall, 153 cases were included in the study. Of the cases, 79 (51.6%) were males and 74 (48.4%) were females. The mean age was 43.1±9.7 years. The presence of D. acari in the follicles of the eyelashes in patients with chronic blepharitis was found in 69 (45.1%) cases. CONCLUSION: D. acari should be considered in patients with chronic blepharitis, especially in treatment-resistant cases. We believe that it would be useful to search for the parasite in patients with blepharitis prior to treatment on a routine basis.

WOS: 000425546600010 PubMed ID: 29269183 We introduce a new technique for the management of encapsulated non filtering blebs which are unresponsive to needling procedure. A 21 year-old man with a visual acuity of 20/30 presented with encapsulated bleb in the left eye 4 weeks after a successful trabeculectomy operation with adjunctive mitomycin C for medically uncontrolled glaucoma. The internal ostium was patent on gonioscopic examination. Intraocular pressure was 35 mmHg despite multiple therapies with bimatoprost, brimonidine, and dorzalomide-timolol fixed combination. He underwent two consecutive bleb needling procedures with adjunctive subconjunctival 5-fluorouracil (5-FU) injections which were successful initially but encapsulated bleb recurred. The 5-FU augmented bleb needling was performed at the slit lamp after cleaning the patient’s eyelid and periorbital skin with 10% providone iodine followed by instillation of a drop of 5% providone iodine. Needling of the bleb was performed using a 27-gauge needle. The aim was to remove episcleral scarring which was obstructing the intrascleral pathway. This was followed by an injection of 0.1 mL of 5-FU solution (50 mg/mL) along the far posterior margins of the bleb area.

- Abstract
Background to evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition. Methods: a prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA)<= 32 weeks and those with a BW> 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses. Results: the TR-ROP study included 6115 infants: 4964 (81%) with a GA <= 32 weeks and 1151 (19%) with a GA>32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. Of all infants, 414 needed treatment and 395 (95.4%) of the treated infants had a BW <= 1500 g. Sixty-six (16%) of the treated infants did not fulfil the Early Treatment for Retinopathy of Prematurity requirements for treatment. Conclusions: screening of infants with a GA <= 34 weeks or a BW<1700 g appears to be appropriate in Turkey. ÖZETİSTANBUL İLİ ÜMRANİYE İLÇESİ ALEMDAĞ ORMANLARI'NDA YETİŞEN YENEBİLEN MANTAR FLORASI ÜZERİNE ARAŞTIRMALAR Makromantarlar denildiğinde, akla ilk önce zehirli mantarlar, daha sonra yenilebilir mantarlar gelir. Ancak bu canlıların doğadaki pozisyonları göz önüne alındığında, onları asıl önemli kılan, ölü veya canlı organik maddeleri parçalamaları ve böylece karbon azot devrinin sürdürülmesinde önemli bir rol oynamalarıdır.Türkiye'de, gerek iklim farlılıkları gerekse ağaç türlerinin zenginliği, mantar florasının da cins ve türleri bakımından çeşitli ve zengin olmasını sağlamaktadır. Türkiye'de çeşitli yörelerde yetişen, zehirli ve yenilebilen mantarların, halk sağlığını ilgilendirmesinin yanı sıra, halk için ekonomik anlamda da bir değer taşıdığı anlaşılmaktadır.Bu çalışmada, İstanbul ili Ümraniye ilçesi Alemdağ Ormanlarında yetişen yenilebilen mantar florasını araştırılmıştır.Çalışmamızda kullanılan mantar örnekleri, 2003 Eylül ayından 2004 Haziran ayına kadar değişik tarihlerde yapılan araştırma gezileri ile, özellikle yaz sonu, yaz başı ve sonbahar aylarında toplandı. Arazi çalışmaları sırasında toplanan mantarların yetiştiği ortam ve substratı, büyüme özellikleri, toplandığı tarih, yetiştiği ortamdaki yüksek yapılı bitkilerle olan ilişkisi kaydedildi. Arazi ve laboratuvar çalışmalarıyla elde edilen verilerin ilgili literatürlerden faydalanılarak karşılaştırılması sonucu toplanan mantar örneklerinin tür seviyesinde teşhisi yapıldı.Araştırmamız sonucunda, Alemdağ Ormanlarında gelişen çok çeşitli mantarlar arasında yenilebilen 12 adet mantar tespit edilmiştir. Örneklerin 1 adedi Ascomycota bölümünden Helvella locunosa'dır. 11 Adedi ise Basidiomycota mensubu olan Auricularia auricular-judae, Agaricus bitorquis, Agaricus silvaticus, Agaricus silvaticus var. pallidus, Leucopaxillus gentianeus, Marasmius cohaerens, Pholiota squarrosa, Megacollybia platyphlla, Melanoleuca strictipes, Bovista plumbea, Pleurotus ostreatus'dır. Dolayısıyla Basidiomycota mensubu yenen mantarları bu bölgede daha fazla geliştiği saptanmıştır. Ayrıca bazı mantarlar için literatürde "yenme değeri" fazla gösterilmezken, halk arasında bunların sevilerek yendiği gözlemlenmiştir. Haziran / 2004ÖZLEM BALCIABSTRACTRESEARCHES ON EDIBLE MUSHROOM FLORA OF THE ALEMDAG FORESTS IN UMRANIYE DISTRICT, ISTANBUL The first thing the word macro fungus brings to mind is the "poisonous mushrooms", while the second being "edible mushrooms". However, regarding the position of these organisms in nature, their most important aspects is that they break down dead or living organic substances, and thus play an important role in the maintenance of carbon-nitrogen cycle. In Turkey both and the climate varieties and richness of tree species, enable the mushroom flora to grow abundantly and rich in variety. It is obvious that, poisonous and edible mushrooms, which grow in various regions of Turkey, comprise an economical value for the people in addition to their being an important public health issue.In this study, a research has been made on the edible mushroom flora of the Alemdag Forests in Umraniye District, Istanbul. The mushroom samples we used in the study have been picked at research expeditions made in various dates from September 2003 to June 2004, especially around late summer, early summer, and autumn. The mushrooms picked during the field research have been recorded in terms of their environment and substrata, their growth characteristics, the time of their being picked up, and their relations to the higher plants in their surroundings. Benefiting from the related scientific literature, comparisons have been made in the data obtained from field and laboratory studies, and thus the mushroom samples have been classified.As a result of our research, 12 species of edible mushrooms have been identified among the huge variety of mushrooms found in Alemdag Forests. One of the examples is Helvella locunosa from Ascomycota family. The other 11 are Auricularia auricular-judae, Agaricus bitorquis, Agaricus silvaticus, Agaricus silvaticus var. pallidus, Leucopaxillus gentianeus, Marasmius cohaerens, Pholiota squarrosa, Megacollybia platyphlla, Melanoleuca strictipes, Bovista plumbea, Pleurotus ostreatus, all of which are members of Basidiomycota. So, edible mushrooms of Basidiomycota family are proven to thrive in this region. Urethral duplications are rare lower urinary tract anomalies. Here we present an unusual presentation of urethral duplication in which, completely patent accessory urethra was coursing laterally on perianal region, near the anus, and presented as perianal fistula. A 2-month-old boy presented with discharge from the perianal region. On physical examination a fistula was observed on perianal region, in the 1 o'clock position and only 1 cm near the anus. Fistulography showed an opening to the prostatic urethra. The opening to the urethra was seen left superior part of the verumontanum on cystourethroscopy by giving methylene blue from the fistula. The accessory urethra was dissected from the skin to the prostatic part of the orthotopic urethra and excised completely with anterior sagittal approach. His postoperative course was uneventful. In unusual form of urethral duplications of Effmann Type IIA2, as in our case, orthotopic urethra is normal and ventral urethra opens to the perineum. It should be kept in mind that especially laterally placed ones can be misdiagnosed as anal fistula. [ABSTRACT FROM AUTHOR]

Background To evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition., Methods A prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA) 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses., Results The TR-ROP study included 6115 infants: 4964 (81%) with a GA 32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. - Abstract
Objective: Demodex acari is an obligate ectoparasite, and it is usually located in the human hair follicles, eyelash roots, and sebaceous glands. The aim of this study was to investigate the presence of Demodex infestation in chronic blepharitis cases that are resistant to therapy., Methods: Patients who were admitted at the Istanbul Medipol University School of Medicine Hospital with a diagnosis of chronic blepharitis were included. All cases received conventional therapy at least three times. Three or four eyelash samples from patients with blepharitis were collected and examined under light microscopy. For the diagnosis, the presence of one or more Demodex parasites at 10× and 40× magnification by a light microscope was considered as positive for infestation., Results: Overall, 153 cases were included in the study. Of the cases, 79 (51.6%) were males and 74 (48.4%) were females. The mean age was 43.1±9.7 years. The presence of D. acari in the follicles of the eyelashes in patients with chronic blepharitis was found in 69 (45.1%) cases., Conclusion: D. acari should be considered in patients with chronic blepharitis, especially in treatment-resistant cases. We believe that it would be useful to search for the parasite in patients with blepharitis prior to treatment on a routine basis.
