ObjectiveElectronic cigarettes are addictive and harmful, and flavour is a key factor determining their abuse liability. Both adult smokers and young non-smokers like sweet and fruity flavours in particular. In order to discourage e-cigarette use among youth, the Dutch government announced in 2020 to only allow tobacco flavours in e-liquids. We propose a restrictive list of flavourings that will only enable the production of e-liquids with a tobacco flavour.MethodsWe used e-liquid ingredient data notified via the European Common Entry Gate system before the government’s announcement. First, we classified all e-liquids into flavour categories, and continued with the set of flavourings present in tobacco e-liquids. Five selection criteria related to prevalence of use, chemical composition, flavour description and health effects were defined to compile a restrictive list of tobacco flavourings.ResultsE-liquids marketed as having tobacco flavour contained 503 different flavourings, some with tobacco flavour, but also other (such as sweet) flavours. We excluded (1) 330 flavourings used in ConclusionsImplementing this restrictive list will likely discourage e-cigarette use among youth, but could also make e-cigarettes less attractive as smoking cessation aid.

De overheid wil e-sigaretten minder aantrekkelijk maken, vooral voor jongeren. Smaakstoffen bepalen voor een groot deel de aantrekkelijkheid van dit product. Zowel volwassen als jongere niet-rokers houden vooral van zoete smaken en fruitsmaken. Daarom heeft de Nederlandse regering besloten alleen nog smaakstoffen toe te staan die vooral in tabaksmaken voorkomen. Om dit uit te kunnen voeren stelt het RIVM voor om 23 smaakstoffen met de smaak van tabak toe te staan. Waarschijnlijk zal het gebruik van e-sigaretten minder aantrekkelijk worden als alleen deze smaakstoffen erin mogen zitten. Het RIVM heeft het voorstel in opdracht van het ministerie van VWS gemaakt. Het ministerie beslist uiteindelijk welke stoffen worden toegestaan. Voor het onderzoek zijn de gegevens van fabrikanten over de samenstelling van hun producten gebruikt. Vloeistoffen met tabaksmaak bevatten smaakstoffen met tabaksmaak, maar vaak ook andere smaakstoffen, zoals ethylmaltol (zoet, fruit-karamel-achtig). Er worden in totaal 503 verschillende smaakstoffen gebruikt in vloeistoffen met tabaksmaak. Om ervoor te zorgen dat er alleen stoffen overblijven die naar tabak smaken of in tabak zitten, heeft het RIVM vier criteria opgesteld. De 23 smaakstoffen die het RIVM voorstelt voldoen hieraan. Als eerste moet de stof in minimaal 0,5 procent van alle vloeistoffen met tabaksmaak zitten. Als tweede moeten ze vaker in vloeistoffen met tabaksmaak voorkomen dan in andere vloeistoffen. Ten derde mogen het geen extracten van plantaardige grondstoffen zijn. De samenstelling daarvan is niet constant en is daarom moeilijk te controleren. Ten slotte moet de smaak van een stof sterk lijken op tabak(smaak) en mogen het geen zoete smaken zijn. Het RIVM merkt op dat volwassenen e-sigaretten als hulpmiddel gebruiken om te stoppen met roken. Dit hulpmiddel kan voor hen minder aantrekkelijk worden als de smaken worden ingeperkt. Verder zouden gebruikers van e-sigaretten zelf smaak kunnen gaan toevoegen, zoals los verkrijgbare aroma’s, bij een sterke beperking van het aantal smaken. Het RIVM adviseert om hier rekening mee te houden bij het opstellen van de regelgeving.

Purpose: Potential drug-drug interactions (pDDIs) may harm patients admitted to the Intensive Care Unit (ICU). Due to the patient's critical condition and continuous monitoring on the ICU, not all pDDIs are clinically relevant. Clinical decision support systems (CDSSs) warning for irrelevant pDDIs could result in alert fatigue and overlooking important signals. Therefore, our aim was to describe the frequency of clinically relevant pDDIs (crpDDIs) to enable tailoring of CDSSs to the ICU setting. Materials & methods: In this multicenter retrospective observational study, we used medication administration data to identify pDDIs in ICU admissions from 13 ICUs. Clinical relevance was based on a Delphi study in which intensivists and hospital pharmacists assessed the clinical relevance of pDDIs for the ICU setting. Results: The mean number of pDDIs per 1000 medication administrations was 70.1, dropping to 31.0 when con -sidering only crpDDIs. Of 103,871 ICU patients, 38% was exposed to a crpDDI. The most frequently occurring crpDDIs involve QT-prolonging agents, digoxin, or NSAIDs. Conclusions: Considering clinical relevance of pDDIs in the ICU setting is important, as only half of the detected pDDIs were crpDDIs. Therefore, tailoring CDSSs to the ICU may reduce alert fatigue and improve medication safety in ICU patients. ? 2020 The Authors. Published by Elsevier Inc. Gelatin is one of the most prominent biopolymers in biomedical material research and development. It is frequently used in hybrid hydrogels, which combine the advantageous properties of bio‐based and synthetic polymers. To prevent the biological component from leaching out of the hydrogel, the biomolecules can be equipped with azides. Those groups can be used to immobilize gelatin covalently in hydrogels by the highly selective and specific azide-alkyne cycloaddition. In this contribution, we functionalized gelatin with azides at its lysine residues by diazo transfer, which offers the great advantage of only minimal side‐chain extension. Approximately 84-90% of the amino groups are modified as shown by 1H‐NMR spectroscopy, 2,4,6‐trinitrobenzenesulfonic acid assay as well as Fourier‐transform infrared spectroscopy, rheology, and the determination of the isoelectric point. Furthermore, the azido‐functional gelatin is incorporated into hydrogels based on poly(ethylene glycol) diacrylate (PEG‐DA) at different concentrations (0.6, 3.0, and 5.5%). All hydrogels were classified as noncyctotoxic with significantly enhanced cell adhesion of human fibroblasts on their surfaces compared to pure PEG‐DA hydrogels. Thus, the new gelatin derivative is found to be a very promising building block for tailoring the bioactivity of materials.

In today’s clinical practice, magnetic resonance imaging (MRI) is routinely accelerated through subsampling of the associated Fourier domain. Currently, the construction of these subsampling strategies - known as experimental design - relies primarily on heuristics. We propose to learn experimental design strategies for accelerated MRI with policy gradient methods. Unexpectedly, our experiments show that a simple greedy approximation of the objective leads to solutions nearly on-par with the more general non-greedy approach. We offer a partial explanation for this phenomenon rooted in greater variance in the non-greedy objective's gradient estimates, and experimentally verify that this variance hampers non-greedy models in adapting their policies to individual MR images. We empirically show that this adaptivity is key to improving subsampling designs.

Nicotineproducten zonder tabak (NZT) nemen toe in populariteit. Een voorbeeld van NZT zijn nicotinezakjes, zakjes met een poeder en nicotine die onder de (boven)lip worden geplaatst. Gebruikers krijgen nicotine binnen via het slijmvlies en speeksel in de mond. De nicotine geeft hen een lekker gevoel. De hoeveelheid nicotine verschilt per soort. Nicotine is verslavend en schadelijk voor de gezondheid. Ook kan het een nicotineverslaving bij rokers in stand houden. Zeker bij hoge doseringen is het schadelijk voor het zenuwstelsel en kan het hartritmestoornissen veroorzaken. Daarom adviseert het RIVM het ministerie van VWS om het gebruik van nicotinezakjes te ontmoedigen. Strengere regelgeving en voorlichting kan daar bij helpen. Nicotinezakjes vallen nu onder de Warenwet. Het RIVM heeft bekeken onder welke andere bestaande wetgeving de NZT kunnen vallen. Deze producten vallen nu niet onder de Tabaks- en rookwarenwet. Beleidsmakers zouden kunnen overwegen om NZT hier als aanverwant product in op te nemen. Dit kan bijvoorbeeld door de definitie van tabaks- en aanverwante producten te verbreden. Het aantal mensen dat de nicotinezakjes ooit heeft gebruikt, is nu nog laag: 0,6 procent van de Nederlanders, 0,3 procent onder 13-17-jarigen en 1,3 procent onder 18-24-jarigen. Van de gebruikers is 48 procent een roker of ex-roker. Vanwege de schadelijke en verslavende effecten is het belangrijk om te voorkomen dat mensen, vooral nooit-tabak- of nicotinegebruikers en jongeren, beginnen met nicotinezakjes te gebruiken. Er zijn meerdere redenen die nicotinezakjes aantrekkelijk maken om te gebruiken. Ze zijn overal te gebruiken, ook op plaatsen waar roken is verboden. Mensen in de omgeving hebben geen last van rooklucht. Bovendien zijn ze, in tegenstelling tot tabaksproducten, in veel smaakjes beschikbaar. Dat kan ze aantrekkelijk maken om ze te gaan gebruiken, vooral voor jongeren.

ObjectivesFlavoured products are especially appealing to youth and contribute to the onset of waterpipe smoking and continued use of waterpipe tobacco. The goal of database and chemical analysis was to provide a clear overview of commonly used flavours and flavourings in tobacco and related waterpipe products, that is, herbal molasses and steam stones.MethodsIn 2019, 249 waterpipe tobacco products were registered in the European Common Entry Gate by manufacturers to be marketed in The Netherlands. Flavour categories were assigned to the registered products based on their brand names and product descriptions. Nicotine and eleven 1111 flavourings were identified and quantified in waterpipe tobacco (n=8), herbal molasses (n=7) and steam stones (n=4) by extraction and gas chromatography–mass spectrometry (GC–MS) analysis.ResultsFlavour categories could be assigned to 237 of 249 registered waterpipe tobacco products. Eight flavour main categories and 48 unique subcategories were identified and presented in a flavour wheel. All registered waterpipe tobacco products were flavoured, and the majority (78%) was fruit flavoured. Herbal molasses contained similar median flavouring levels, and steam stones contained lower median levels compared with waterpipe tobacco. Flavourings in waterpipe products were almost exclusively fruity and sweet, often in combination with menthol/mint flavourings.ConclusionsThis study is the first to present a waterpipe tobacco flavour wheel, providing a quick overview of waterpipe tobacco flavours and thereby aiding communication among experts around the globe. GC–MS analysis revealed that the most prevalent flavourings are present in similar levels in herbal and tobacco waterpipe products. Banning flavourings in all waterpipe products would be a good strategy to reduce waterpipe smoking among youth.
