When multilingual students face the challenge of writing a doctoral capstone or dissertation, delays in academic progress may occur. The aim of this study was to identify writing challenges multilingual doctoral students face and provide recommendations regarding learner-centered resources to support timely academic success, as literature regarding multilingual students and language diversity in the doctoral environment is limited. A qualitative descriptive design was used for this study, and six multilingual DNP and PhD alumni participated. Data were collected using semi-structured audio interviews and analyzed using iterative content analysis. The findings support the need for community and culture to support language identity and doctoral writing development. Findings also support that early diagnostics of writing issues and opportunities for editorial feedback help support multilingual doctoral students. Findings also suggest a need for faculty and academic team development regarding the impacts of language diversity and culture on academic writing. Ultimately the goal is to help all graduate students preserve and share their identity in their writing.

We aimed to compare the GOLD 2011, GOLD 2017 ABCD, and GOLD 2017 1A-4D classifications in terms of their distribution and prediction of mortality and hospitalizations.Methods: In the GenKOLS study, 912 COPD patients with FEV1 less than 80% of the predicted answered questionnaires and performed lung function testing in 2003–2005. The patients were recruited from a hospital patient registry (n=662) and from the general population (n=250), followed up until 2011 with respect to all-cause and respiratory mortality, and all-cause and respiratory hospitalizations. We performed logistic regression and receiver operating curve (ROC) analyses for the different classifications with estimations of area under the curve (AUC) for comparisons.Results: Mean age at baseline was 60 years (SD 11), 55% were male. Mean duration of follow-up was 91 months. By GOLD 2011, 21% were classified as group A, 29% group B, 6% group C, and 43% as group D, corresponding percentages for GOLD 2017 were: 25%, 52%, 3%, and 20%. The GOLD 2011 classification had higher AUC values than the GOLD 2017 group ABCD classification for respiratory mortality and hospitalization, but after inclusion of airflow limitation severity in GOLD 2017 groups 2A–4D, AUC values were significantly higher with GOLD 2017.Conclusion: In a clinically relevant sample of COPD patients, the GOLD 2017 classification doubles the prevalence of group B and halves the prevalence of groups C and D as compared to the GOLD 2011 classification. The prediction of respiratory mortality and respiratory hospitalization was better for GOLD 2017 2A–4D taking airflow limitation severity into account, as compared to GOLD 2017 ABCD and GOLD 2011.Keywords: respiratory, hospitalization, mortality, Cox regression, ABCD classification, airflow limitation

The present study explored the association of within-breath reactance measured over multiple breaths and EFLT with 6-minute walk distance (6MWD), exacerbations, and mortality.Methods: In 425 patients, spirometry and forced oscillation technique measurements were obtained on eight occasions over 3 years. 6MWD was assessed at baseline and at the 3-year visit. Respiratory symptoms, exacerbations, and hospitalizations were recorded. A total of 5-year mortality statistics were retrieved retrospectively. We grouped patients according to the mean within-breath reactance (ΔXrs), measured over several breaths at baseline, calculated as mean inspiratory–mean expiratory reactance over the sampling period. In addition to the established threshold of EFLT, an upper limit of normal (ULN) was defined using the 97.5th percentile of ΔXrs of the healthy controls in the study; 6MWDs were compared according to ΔXrs, as normal, ≥ ULN < EFLT, or ≥ EFLT. Annual exacerbation rates were analyzed using a negative binomial model in the three groups, supplemented by time to first exacerbation analysis, and dichotomizing patients at the ULN.Results: In patients with COPD and baseline ΔXrs below the ULN (0.09 kPa·s·L-1), 6MWD was stable. 6MWD declined significantly in patients with ΔXrs ≥ ULN. Worse lung function and more exacerbations were found in patients with COPD with ΔXrs ≥ ULN, and patients with ΔXrs ≥ ULN had shorter time to first exacerbation and hospitalization. A significantly higher mortality was found in patients with ΔXrs ≥ ULN and FEV1 >50%.Conclusion: Patients with baseline ΔXrs ≥ ULN had a deterioration in exercise performance, more exacerbations, and greater hospitalizations, and, among those with moderate airway obstruction, a higher mortality. Abstract Atlantic cod has a peculiar immune system, characterized by the loss of Major Histocompatibility Complex (MHC) class II pathway, and an extreme expansion of the MHC class I gene repertoire. This has led to the hypothesis that some of the MHC I variants have replaced MHC II by presenting exogenous-peptides in a process similar to cross-presentation. In mammals, MHC I loads endogenous antigens in the endoplasmic reticulum, but we recently found that different Atlantic cod MHC I gene variants traffic to endolysosomes. There, they colocalize with Tapasin and other components of the peptide-loading complex, indicating a plausible peptide-loading system outside the endoplasmic reticulum. In this study, we further characterize the identity of the Atlantic cod MHC I compartment (cMIC). We found that, similarly to mammalian MHC II compartment, cMIC contains late endosomal markers such as Rab7, LAMP1 and CD63. Furthermore, we identified Hsp90b1 (also known as grp94) and LRP1 (also known as CD91) as interactors of MHC I by mass spectrometry. As these two proteins are involved in cross-presentation in mammals, this further suggests that Atlantic cod MHC I might use a similar mechanism to present exogenous peptides, thus, compensating for the absence of MHC II.

Abstract Background Life-sustaining therapy, including heart and lung resuscitation and transfer to the intensive care department, is demanding for patients and relatives and utilizes large amounts of healthcare resources. For older patients acutely admitted to the hospital, very sparse data exist on decision making about life-sustaining therapy. Methods Retrospective data were extracted from patients ≥ 70 years old who were acutely admitted to the hospital. Age, sex, clinical frailty scale score and Charlson comorbidity index were manually extracted from patients’ files. Furthermore, data about life-sustaining treatment decisions were extracted. This was further divided into decisions documented within 24 h from admission or during the hospital stay. Results Data were extracted for 200 patients with a median age of 80 years. Patients had a Charlson Comorbidity Index of 6 (5–8 IQR) and a Clinical Frailty Scale (CFS) score of 5 (3–6 IQR). During the first 24 h, 61 patients (30.5%) had a written decision about heart and cardiopulmonary resuscitation (CPR), and 52 patients (26%) had written information about intensive care therapy. A total of 93 patients (46.5%) had a written decision about cardiopulmonary resuscitation (CPR), intensive care therapy or both during their hospital stay. With increasing Charlson Comorbidity Index and Clinical Frailty Scale scores, more patients had decisions about limitations in therapy documented in their files. Conclusions Within the first 24 h, 30.5% of the patients had a written decision about cardiopulmonary resuscitation (CPR), and 26% had written information about intensive care therapy. These numbers increased to 46.5% of patients who had a decision made during their hospital stay whether they were candidates for either cardiopulmonary resuscitation (CPR), intensive care therapy or both. These data suggest that further work should be done to determine the limitations of therapy early on the admission for all older frail acutely admitted patients.

Marta Erdal1,2 Ane Johannessen3 Tomas Mikal Eagan1,2 Per Bakke2 Amund Gulsvik2 Rune Grønseth1,2 1Department of Thoracic Medicine, Haukeland University Hospital, 2Department of Clinical Science, University of Bergen, 3Department of Global Public Health and Primary Care, University of Bergen, Bergen, Norway Objectives: The objectives of this study were to estimate the impact of recruitment source and outcome definition on the incidence of acute exacerbations of COPD (AECOPD) and explore possible predictors of AECOPD.Patients and methods: During a 1-year follow-up, we performed a baseline visit and four telephone interviews of 81 COPD patients and 132 controls recruited from a population-based survey and 205 hospital-recruited COPD patients. Both a definition based on health care utilization and a symptom-based definition of AECOPD were applied. For multivariate analyses, we chose a negative binomial regression model.Results: COPD patients from the population- and hospital-based samples experienced on average 0.4 utilization-defined and 2.9 symptom-defined versus 1.0 and 5.9 annual exacerbations, respectively. The incidence rate ratios for utilization-defined AECOPD were 2.45 (95% CI 1.22–4.95), 3.43 (95% CI 1.59–7.38), and 5.67 (95% CI 2.58–12.48) with Global Initiative on Obstructive Lung Disease spirometric stages II, III, and IV, respectively. The corresponding incidence rate ratios for the symptom-based definition were 3.08 (95% CI 1.96–4.84), 3.45 (95% CI 1.92–6.18), and 4.00 (95% CI 2.09–7.66). Maintenance therapy (regular long-acting muscarinic antagonists, long-acting beta-2 agonists, inhaled corticosteroids, or theophylline) also increased the risk of AECOPD with both exacerbation definitions (incidence rate ratios 1.65 and 1.73, respectively). The risk of AECOPD was 59%–78% higher in the hospital sample than in the population sample.Conclusion: If externally valid conclusions are to be made regarding incidence and predictors of AECOPD, studies should be based on general population samples or adjustments should be made on account of a likely higher incidence in other samples. Aim/Purpose: This paper is part of a multi-case study that aims to test whether generative AI makes an effective coding assistant. Particularly, this work evaluates the ability of two AI chatbots (ChatGPT and Bing Chat) to generate concise computer code, considers ethical issues related to generative AI, and offers suggestions for how to improve the technology. Background: Since the release of ChatGPT in 2022, generative artificial intelligence has steadily gained wide use in software development. However, there is conflicting information on the extent to which AI helps developers be more productive in the long term. Also, whether using generated code violates copyright restrictions is a matter of debate. Methodology: ChatGPT and Bing Chat were asked the same question, their responses were recorded, and the percentage of each chatbot's code that was extraneous was calculated. Also examined were qualitative factors, such as how often the generated code required modifications before it would run. Contribution: This paper adds to the limited body of research on how effective generative AI is at aiding software developers and how to practically address its shortcomings. Findings: Results of AI testing observed that 0.7% of lines and 1.4% of characters in ChatGPT's responses were extraneous, while 0.7% of lines and 1.1% of characters in Bing Chat's responses were extraneous. This was well below the 2% threshold, meaning both chatbots can generate concise code. However, code from both chatbots frequently had to be modified before it would work; ChatGPT's code needed major modifications 30% of the time and minor ones 50% of the time, while Bing Chat's code needed major modifications 10% of the time and minor ones 70% of the time. Recommendations for Practitioners: Companies building generative AI solutions are encouraged to use this study's findings to improve their models, specifically by decreasing error rates, adding more training data for programming languages with less public documentation, and implementing a mechanism that checks code for syntactical errors. Developers can use the findings to increase their productivity, learning how to reap generative AI's full potential while being aware of its limitations. Recommendation for Researchers: Researchers are encouraged to continue where this paper left off, exploring more programming languages and prompting styles than the scope of this study allowed. Impact on Society: As artificial intelligence touches more areas of society than ever, it is crucial to make AI models as accurate and dependable as possible. If practitioners and researchers use the findings of this paper to improve coders' experience with generative AI, it will make millions of developers more productive, saving their companies money and time. Future Research: The results of this study can be strengthened (or refuted) by a future study with a large, diverse dataset that more fully represents the programming languages and prompting styles developers tend to use. Moreover, further research can examine the reasons generative AI fails to deliver working code, which will yield valuable insights into improving these models.

The objective of this study is to evaluate the spatio-temporal dynamics of land vulnerability and pasture areas in the dairy basins of the states of Pernambuco and Alagoas, which are part of the Ipanema River Watershed (IRW) in the Northeast Region of Brazil. Maps of the Land Use and Land Cover (LULC); the Index of Vulnerability to Degradation (IVD); the Land Vulnerability Index (LVI); time series of Effective Herd (EH), Milked Cows (MC), and Milk Production (MP); and Pasture Cover (PC) and Quality (PCQ) were created as parameters. An opposite pattern was observed between the land use classes of Livestock, Agriculture, and Forest. The IRW area has predominantly flat terrain with a very high risk of degradation. The analysis of MC was consistent with the information from the EH analysis as well as with MP. When assessing Pasture Quality, Severe Degradation areas increased from 2010 to 2014, decreased after 2015, and rose again in 2020. Moderate Degradation areas remained high, while Not Degraded pasture areas were consistently the lowest from 2012 to 2020. Over the 10 years analyzed (2010–2020), the area showed a strong degradation process, with the loss of approximately 16% of the native vegetation of the Caatinga Biome and an increase in pasture areas and land vulnerability.

IntroductionKrill oil is a dietary supplement derived from Antarctic krill; a small crustacean found in the ocean. Krill oil is a rich source of omega-3 fatty acids, specifically eicosapentaenoic acid and docosahexaenoic acid, as well as the antioxidant astaxanthin. The aim of this study was to investigate the effects of krill oil supplementation, compared to placebo oil (high oleic sunflower oil added astaxanthin), in vivo on energy metabolism and substrate turnover in human skeletal muscle cells.MethodsSkeletal muscle cells (myotubes) were obtained before and after a 7-week krill oil or placebo oil intervention, and glucose and oleic acid metabolism and leucine accumulation, as well as effects of different stimuli in vitro, were studied in the myotubes. The functional data were combined with proteomic and transcriptomic analyses.ResultsIn vivo intervention with krill oil increased oleic acid oxidation and leucine accumulation in skeletal muscle cells, however no effects were observed on glucose metabolism. The krill oil-intervention-induced increase in oleic acid oxidation correlated negatively with changes in serum low-density lipoprotein (LDL) concentration. In addition, myotubes were also exposed to krill oil in vitro. The in vitro study revealed that 24 h of krill oil treatment increased both glucose and oleic acid metabolism in myotubes, enhancing energy substrate utilization. Transcriptomic analysis comparing myotubes obtained before and after krill oil supplementation identified differentially expressed genes associated with e.g., glycolysis/gluconeogenesis, metabolic pathways and calcium signaling pathway, while proteomic analysis demonstrated upregulation of e.g., LDL-receptor in myotubes obtained after the krill oil intervention.ConclusionThese findings suggest that krill oil intervention promotes increased fuel metabolism and protein synthesis in human skeletal muscle cells, with potential implications for metabolic health.

Governing in Times of Crisis is, as the title suggests, an anthology about coalition governments in Central and Eastern Europe, defined therein as the ten post-communist countries that became members of the European Union (EU) in 2004 and 2007: Estonia, Latvia, Lithuania, Poland, Czech Republic, Slovakia, Hungary, Slovenia, Romania and Bulgaria. The anthology covers the period from 2008 to 2021 and is based on the research project Party Government in Europe Database (PAGED).

Objectives: The aim of this case-control study was to examine upper airway by acoustic reflection in class II children with large horizontal maxillary overjet compared to children with neutral occlusion. Material and Methods: The study group included children of 9 to 14 years with class II and large horizontal maxillary overjet (≥ 6 mm) with indication for growth adaptive treatment of the mandible compared to children with neutral occlusion (controls). Acoustic pharyngometry and rhinometry were performed in natural head position. Differences between groups were tested by chi-square test, general linear model (adjusted for age, gender and body mass index [BMI]), and Mann-Whitney test. Results: The study and control group consisted of 37 (male 19, female 18) and 32 (male 16, female 16) participants, respectively. No significant differences in age, gender, and BMI were found between the groups. For the acoustic rhinometry measurements significantly increased resistance (P = 0.04), reduced volume (P = 0.03) and distance to minimal cross-section area (MCA) (P = 0.035) were found in the study group, but only for the right nostril. However, significantly reduced MCA for both nostrils was found in the study group compared to controls (P = 0.025 to 0.04). No significant differences in acoustic pharyngometry measurements were found. Conclusions: Nasal airway dimensions were significantly reduced and nasal resistance was significantly increased in the study group compared to controls. Thus, class II and large overjet with indication for growth adaptive treatment may be a risk factor for sleep-disordered breathing in children. The results may prove valuable in non-invasive risk assessment of narrow upper airway in children with class II and large overjet.

Gjennomføring av videregående opplæring er avgjørende for tilknytning til og deltakelse i arbeidslivet. Likevel fullfører en av fem elever i Norge ikke videregående skole. I denne studien undersøkte vi hvordan helsesykepleiere kan legge til rette for tverrprofesjonelt arbeid for å motvirke skolefrafall. Basert på tre fokusgruppeintervjuer med 13 helsesykepleiere i videregående skole, ble kvalitative utsagn analysert ved hjelp av en stegvis-deduktiv-induktiv metode. Funnene indikerer at helsesykepleierne tilstreber å se hele eleven for å forstå fraværsårsaker, men har utfordringer med å synliggjøre sin egen kompetanse for samarbeidspartnere. Implisitte maktstrukturer, inkludert skoleledelsen, spiller en betydelig rolle i helsesykepleiernes inkludering i tverrprofesjonelle team. For å fremme tverrprofesjonelt samarbeid og styrke sin egen rolle, bør helsesykepleierne aktivt synliggjøre seg selv og sin kompetanse.

Our aim was to investigate long-term mortality and associated risk factors in COPD patients previously hospitalized for a COPD exacerbation.Methods: A total of 256 patients from the Nordic countries were followed for 8.7 ± 0.4 years after the index hospitalization in 2000–2001. Prior to discharge, the St George’s Respiratory Questionnaire was administered and data on therapy and comorbidities were obtained. Information on long-term mortality was obtained from national registries in each of the Nordic countries.Results: In total, 202 patients (79%) died during the follow up period, whereas 54 (21%) were still alive. Primary cause of death was respiratory (n = 116), cardiovascular (n = 43), malignancy (n = 28), other (n = 10), or unknown (n = 5). Mortality was related to older age, with a hazard risk ratio (HRR) of 1.75 per 10 years, lower forced expiratory volume in 1 second (FEV1) (HRR 0.80), body mass index (BMI)

Abstract Background Video streaming in emergency medical communication centers (EMCC) from caller to medical dispatcher has recently been introduced in some countries. Death by trauma is a leading cause of death and injuries are a frequent reason to contact EMCC. We aimed to investigate if video streaming is associated with recognition of a need for first aid during calls regarding injured patients and improve quality of bystander first aid. Methods A prospective observational study including patients from three health regions in Norway, from November 2021 to February 2023 (registered in clinical trials 10/25/2021, NCT05121649). Cases where video streaming had been used as a supplement during the medical emergency call were compared to cases where video streaming was not used during the call. Patients were included by ambulance personnel on the scene of accident if they met the following criteria: 1. Ambulance personnel arrived at a patient who had an injury, 2. One or more bystanders had been present before their arrival, 3. One or more of the following first aid measures had been performed by bystander or should have been performed: airway management, control of external bleeding, recovery position, and hypothermia prevention. Ambulance personnel assessed quality of first aid performed by bystander, and information concerning use of video streaming and patient need for first aid measures recognized by dispatcher was collected through EMCC audio logs and patient charts. We present descriptive data and results from a logistic regression analysis. Results Data was collected on 113 cases, and dispatchers used video streaming in addition to standard telephone communication in 12/113 (10%) of the cases. The odds for the dispatcher to recognize a need for first aid during a medical emergency call were more than five times higher when video streaming was used compared to no use of video streaming (OR 5.30, 95% CI 1.11-25.44). Overall quality of bystander first aid was rated as “high”. The odds ratio for the patient receiving first aid of higher quality were 1.82 (p-value 0.46) when video streaming was used by dispatcher during the call. Conclusion Our findings show that video streaming is not frequently used by dispatchers in calls regarding patients with injuries, but that video streaming is associated with improved recognition of patients’ first aid needs. We found no statistically significant difference in first aid quality comparing the calls where video streaming as a supplement were used with the calls with audio only.

ABSTRACTCareer guidance counsellors’ competence requirements in Norwegian schools have been the subject of decades-long discussion. Practitioners, experts, and stakeholders have called for precise requirements and professionalized status for counsellors, but with little results. In the Finnish context, however, career guidance is more professionalized, and the counsellor competence requirements are defined by law. Why have the two systems developed differently? In this research, we used ‘historical institutionalism’ as our lens and compared the two developmental histories of the counsellor role and the reasoning behind these differentiating systems. By utilizing Toulmin’s model of argumentation, we analysed rationales in policy documents from ministries to expert groups to reveal how the policy rationales have formed the different statuses of the career counsellor profession in the 1950s, 1960s and 1990s. Norway and Finland saw the counsellor role similarly, but eventually, decisions on qualifications and competency were made from different perspectives.

We evaluate different methods to detect benzene at a parts-per-billion level regarding their potential to be used in a wearable sensor. Benzene is a carcinogenic molecule, regarded as a major health threat by the World Health Organization. A wearable sensor is necessary to detect leaks immediately, but it is challenging to achieve such low limits of detection and quantification, even with laboratory equipment. A wearable sensor must, in addition to good selectivity and sensitivity, meet stricter requirements of size, weight, temperature, repeatability, and power consumption. We conclude that the most promising techniques for a wearable sensor are either infrared photoacoustic spectroscopy near 14.8 μm, or a photoionization detector combined with one of three selective devices: micro-gas chromatography, cavitands, or catalytic filters (WO3, for example). Ultraviolet photoacoustic spectroscopy may also be a suitable future technique for a wearable benzene sensor when efficient LEDs and lasers become available at many UV-C wavelengths.

This research explores the effects of listening to music during exams for engineering college students. Students were given the option to listen to self-selected music while completing exams in three undergraduate engineering courses over four years. It was found that listening to music during an exam had no significant effect on the mean exam scores or completion times. However, when analyzing data from students who had taken exams both with and without music, it was found that these students performed significantly better in the music condition but at par with the mean score. Results indicate that allowing music during exams for students who study with music does not represent an advantage but rather an opportunity for equity in exam-taking environments.

This study investigates differences in focal adhesion (FA) morphology and Talin cleavage levels between transformed and non-transformed cell lines. Utilizing fluorescently tagged wild-type Talin and Talin mutants with calpain cleavage site mutations, FA structures were visualized. Mutations in different Talin cleavage sites showed varying impacts on FA morphology and distribution across melanoma cell lines (Meljuso, A375P, A2058) and a non-transformed cell line (HFF). Western blot analysis, ratiometric fluorescence intensity-based measurements, and FRAP experiments revealed higher Talin cleavage levels within FAs of transformed cell lines compared to non-transformed cells. Additionally, growth assays indicated that reducing calpain cleavage levels attenuated transformed cell growth. These findings suggest that Talin cleavage level is crucial for FA morphology and assembly, with higher levels observed in transformed cells, influencing their growth dynamics.

Gut microbial communities are highly dynamic and respond to factors such as diet and host metabolism. However, there is limited information about how these responses can generate community dynamics at different time scales, such as circadian rhythms. The aim of the present study was to characterize variations along a daily cycle in the gut microbiota of fish on different feeding regimes. In addition, an estimation of the potential contribution of the gut bacterial community to the digestive process of the fish was assessed. The greater amberjack (Seriola dumerili) was used as experimental species, a promising species for Mediterranean aquaculture. Fish were randomly distributed in two different feeding regimes, in triplicate rearing tanks. Feeding regimes were continuous feeding and time-restricted feeding (three meals per day). Fish feces for RNA-based amplicon sequencing and bacterial proteolytic capacity were sampled along a 24h cycle. Time-restricted fed fish exhibited changes in the composition of the active bacterial community promoted by feeding, thus revealing two types of bacterial community profiles: “pre-feeding” and “active-feeder” profiles. Continuous-fed fish maintained an “active-feeder” community profile throughout the whole day. Bray-Curtis and Sørensen similarity indicated that differences in the composition between the two community profiles were mainly driven by the absence/presence of infrequent ASVs (amplicon variant sequences). The potential proteolytic bacterial activity tended to be higher before feeding. Thus, gut microbiota could be a key factor to understanding fish digestion, playing a role in interactions with host metabolism. In conclusion, our results suggest that feeding pattern and time of sampling are important parameters when assessing the microbiome’s contribution to host metabolism.
