Abstract

Accurate and robust recording and decoding from the central nervous system (CNS) is essential for advances in human-machine interfacing. However, technologies used to directly measure CNS activity are limited by their resolution, sensitivity to interferences, and invasiveness. Advances in muscle recordings and deep learning allow us to decode the spiking activity of spinal motor neurons (MNs) in real time and with high accuracy. MNs represent the motor output layer of the CNS, receiving and sampling signals originating in different regions in the nervous system, and generating the neural commands that control muscles. The input signals to MNs can be estimated from the MN outputs. Here we argue that peripheral neural interfaces using muscle sensors represent a promising, non-invasive approach to estimate some neural activity from the CNS that reaches the MNs but does not directly modulate force production. We also discuss the evidence supporting this concept, and the necessary advances to consolidate and test MN-based CNS interfaces in controlled and real-world settings.

Abstract

Metal additive manufacturing (AM) by laser powder bed fusion (L-PBF) builds upon fundamentals established in the field of laser welding which include the influence of gas and plume dynamics on weld depth and quality. L-PBF demands a thorough investigation of the complex thermophysical phenomena that occur where the laser interacts with the metal powder bed. In particular, melt pool turbulence and evaporation are influenced by the ambient gas chemistry and pressure. This paper presents the design and validation of high pressure laser melting (HPLM) testbed; this accommodates bare metal plate samples as well as manually-coated single powder layers, and operates at up to 300 psig. The open architecture of this testbed allows for full control of all relevant laser parameters in addition to ambient gas pressure and gas flow over the build area. Representative melt tracks and rasters on bare plate and powder are examined in order to validate system performance, and preliminary analysis concludes that pressure has a significant impact on melt pool aspect ratio. The HPLM system thus enables careful study pressure effects on processing of common L-PBF materials, and can be applied in the future to materials that are challenging to process under ambient pressure, such as those with high vapor pressures., Comment: 25 pages, 16 figures, submitted to Additive Manufacturing

17. Major psychiatric comorbidity among the critically ill: a multi-centred cohort study in Queensland.

Abstract

Background: Although comorbid medical diseases are important determinants of outcome among the critically ill, the role of psychiatric comorbidity is not well defined. The objective of this study was to determine the occurrence of psychiatric comorbidity and its effect on the outcome of patients admitted to adult intensive care units (ICU) in Queensland. Methods: Admissions among adults to 12 ICUs in Queensland during 2015–2021 were included and clinical and outcome information was obtained through linkages between the ANZICS Adult Patient Database, the state-wide Queensland Hospital Admitted Patient Data Collection, and death registry. Results: A total of 89,123 admissions were included among 74,513 individuals. Overall, 7,178 (8.1%) admissions had psychiatric co-morbidity with 6,270 (7.0%) having one major psychiatric diagnosis and 908 (1%) having two or more. Individual diagnoses of mood, psychotic, anxiety, or affective disorders were present in 1,801 (2.0%), 874 (1.0%), 3,241 (3.6%) and 354 (0.4%) admissions respectively. Significant differences were observed among the main groups (mood, affective, anxiety, psychotic, or multiple disorders) and those without psychiatric comorbidity with respect to main diagnosis, Acute Physiology and Chronic Health Evaluation (APACHE II) score, sex, age, and medical comorbidity. Crude 30-day case-fatality rates were significantly lower (5.1%) compared to the general ICU population (10.1%) (p < 0.001). After controlling for confounding variables in the logistic regression model, patients with psychiatric comorbidity were at lower odds of death. Conclusions: Psychiatric comorbidity is common among ICU presentations and is associated with a lower risk of death. This association is likely to be more complex than being a simple protective factor, and future research needs to further delineate how psychiatric comorbidity informs outcomes of specific ICU presentations. 18. Quantifying Overdiagnosis for Multicancer Detection Tests: A Novel Method.

Abstract

Multicancer detection (MCD) tests use blood specimens to detect preclinical cancers. A major concern is overdiagnosis, the detection of preclinical cancer on screening that would not have developed into symptomatic cancer in the absence of screening. Because overdiagnosis can lead to unnecessary and harmful treatments, its quantification is important. A key metric is the screen overdiagnosis fraction (SOF), the probability of overdiagnosis at screen detection. Estimating SOF is notoriously difficult because overdiagnosis is not observed. This estimation is more challenging with MCD tests because short‐term results are needed as the technology is rapidly changing. To estimate average SOF for a program of yearly MCD tests, I introduce a novel method that requires at least two yearly MCD tests given to persons having a wide range of ages and applies only to cancers for which there is no conventional screening. The method assumes an exponential distribution for the sojourn time in an operational screen‐detectable preclinical cancer (OPC) state, defined as once screen‐detectable (positive screen and work‐up), always screen‐detectable. Because this assumption appears in only one term in the SOF formula, the results are robust to violations of the assumption. An SOF plot graphs average SOF versus mean sojourn time. With lung cancer screening data and synthetic data, SOF plots distinguished small from moderate levels of SOF. With its unique set of assumptions, the SOF plot would complement other modeling approaches for estimating SOF once sufficient short‐term observational data on MCD tests become available. Abstract

Current microelectrodes designed to record chronic neural activity suffer from recording instabilities due to the modulus mismatch between the electrode materials and the brain. We sought to address this by microfabricating a novel flexible neural probe. Our probe was fabricated from parylene-C with a WTi metal, using contact photolithography and reactive ion etching, with three design features to address this modulus mismatch: a sinusoidal shaft, a rounded tip and a polyimide anchoring ball. The anchor restricts movement of the electrode recording sites and the shaft accommodates the brain motion. We successfully patterned thick metal and parylene-C layers, with a reliable device release process leading to high functional yield. This novel reliably microfabricated probe can record stable neural activity for up to two years without delamination, surpassing the current state-of-the-art intracortical probes. This challenges recent concerns that have been raised over the long-term reliability of chronic implants when Parylene-C is used as an insulator, for both research and human applications. Abstract

To assess reticulospinal tract excitability, high‐intensity transcranial magnetic stimulation (TMS) has been used to elicit ipsilateral motor‐evoked potentials (iMEPs). However, there is no consensus on robust and valid methods for use in human studies. The present study proposes a standardized method for eliciting and analysing iMEPs in the biceps brachii. Twenty‐four healthy young adults participated in this study. Electromyography (EMG) electrodes recorded contralateral MEPs (cMEPs) from the right and iMEPs from the left biceps brachii. A dynamic preacher curl task was used with ~15% of the subject's one‐repetition maximum load. The protocol included maximal compound action potential (M‐max) determination of the right biceps brachii muscle, TMS hotspot determination, and four sets of five repetitions where 100% stimulator output was delivered at an elbow angle of 110° of flexion. We normalized cMEP amplitude by M‐max (% M‐max) and iMEP by cMEP amplitude ratio (ICAR). Clear iMEPs above background EMG were observed in 21 subjects (88%, ICAR =.31 ±.19). Good‐to‐excellent agreement (intraclass correlation coefficient [ICC] =.795–1.000) and low bias (.01–.08 mV and.60–1.11 ms) were demonstrated when comparing two different analysis methods (i.e. fixed time‐window vs. manual onset detection) to determine the cMEP and iMEP amplitude and latency, respectively. Most subjects demonstrated clear iMEPs above background EMG triggered at a pre‐determined joint angle during a light‐load dynamic preacher curl exercise. Similar results were obtained when comparing a single‐trial manual identification of iMEP and a semi‐automated time‐window data analysis approach. [ABSTRACT FROM AUTHOR]

Abstract

This article explores the importance of calibration in prediction models when applied to new populations. It compares different calibration approaches and introduces Elkan's general updating approach (GUA), which accounts for differences in base rates between populations without re-estimation. The article explains how calibrated probability can be expressed as a function of base rates, which are estimated using proportions of subjects with a certain outcome in training and calibration data. The text discusses two approaches for estimating base probability in a training data set: Elkan's GUA calculates the average of the target variable, while Baker's GUA suggests using the average of the conditional probability. Both approaches assume that the only difference between populations is the change in base rate and require a calibration data set. A simulation study comparing different calibration methods for machine learning models found that beta calibration performed the best, while isotonic calibration and IVAP calibration were similar. Abstract

The StartReact test, increasingly popular for assessing cortico‐reticular functioning, is a valid method to influence the firing of reticulospinal tract neurons noninvasively. However, there remains limited evidence on how different stimuli employed in the StartReact test impact motor output in humans. The present study tested elbow flexor responses of 33 adults (aged 26–48 years) to visual stimuli only (LED light), audio‐visual (80 dB) stimuli, and startle‐inducing audio‐visual (120 dB) stimuli sitting with the arm supinated in an electromechanical dynamometer. Surface electromyogram (EMG) recorded muscle activity from the right biceps brachii muscle. Participants were presented with 20 stimuli for each of the three conditions in pseudorandom order with interstimulus intervals of ~8 s. Reaction times were calculated from the stimulus trigger to the initial rise in the EMG signal above 7 × SD from baseline. Rate of torque development (RTD) and EMG signals were recorded throughout and analyzed over their initial 50 ms and 100 ms time‐windows. Reaction times were reduced from visual (169 ± 23) to audio‐visual (140 ± 23) and further reduced to startle‐inducing audio‐visual stimuli (108 ± 19, p < 0.001). While RTD and EMG were consistently greatest following startle‐inducing stimuli (p < 0.001), they were also enhanced following all audio‐visual stimuli over 100 ms (p < 0.05). It appears that startle‐inducing audio‐visual stimuli result in shorter reaction times, increased RTD, and enhanced muscle activity within the initial 50 ms, likely from subcortical upregulation. However, the 100 ms time‐window suggests cortical upregulation following all audio‐visual stimuli considering the longer transmission times. [ABSTRACT FROM AUTHOR]

46. Working in the present moment : a phenomenological enquiry into the impact of mindfulness practice on trainee psychological therapists' experience of therapeutic practice

This study explored the lived-experience and impact of a brief mindfulness meditation training intervention on trainee psychological therapists. Particular attention is given to participants’ experiences of relational depth, the cultivation of therapeutic qualities and their use of the self in the therapeutic relationship; as well as how they integrated mindfulness into their clinical practice. The qualitative methodology of interpretative phenomenological analysis was used to investigate participants’ lived-experience of mindfulness. Fifteen (out of nineteen) participants completed a two-month MBSR/MBCT mindfulness training, contextualised for therapists; followed by two focus groups, then eight individual interviews four months later. The findings suggested that a brief mindfulness training programme can have a significant impact, with participants experiencing a range of positive outcomes and meaningful effects at both a personal and professional level. The study indicated the potential of mindfulness as a complementary clinical training, in developing ‘being versus doing’ qualities and the potential to experience relational depth. Mindfulness facilitated participants’ confidence and ability to access and trust their own embodied experience or felt-sense of the emergent therapeutic process, in comparison to their core trainings where the emphasis was more on conceptual knowledge, theory and protocols. Although this research did not set out to build a model of mindfulness in the therapeutic encounter the findings were suggestive of a process model, where therapist intra-personal attunement facilitates therapist-client interpersonal attunement that in turn promotes client self-attunement, in a ‘co-created co-meditation space’. Participants experienced mindfulness as providing an embodied developmental opportunity that complemented their core trainings and modalities. Although participants proposed that mindfulness should be a training requirement for all psychological therapists the question of fit needs to be considered, with some participants finding this challenging at times both personally and in the context of jobs where adherence to a particular modality or process was required. It also seems that this training attracted those with a predisposition toward mindfulness; in contrast the four drop-outs indicate that this approach may not suit all. Thus future research needs to address these aspects. In a political context where therapist training is becoming more protocol driven, mindfulness appears to provide a way to redress this imbalance, providing a complementary training route for the practical cultivation of relational qualities that allows participants a greater sense of authenticity, empowerment and trust in themselves and the therapeutic process.
