Abstract

Introduction: Palliative care (PC) refers to interventions aimed to improve the quality of life (QoL) of children and their families by the prevention and relief of suffering. The purpose of this study was to explore the knowledge and views of parents of children with cancer regarding PC in Greece. Methods: A descriptive cross-sectional study was performed in a sample of 100 parents of hospitalized children in oncology departments and a bone narrow transplant unit of two pediatric hospitals in Attica, from February to June 2021. The "Evaluating Supportive Care for Children" tool was used. Data analysis was performed with SPSS v.23.0. All tests were performed at a statistical significance level of p≤0,05. Results: Only 19% of the participants know about PC while no one reports personal PC provision experience. Highest level of education and monthly income were statistically significantly correlated with the knowledge of PC, (p=0,040) and (p=0,036) respectively, and the positive parental attitude towards PC inclusion in the child's care (p=0,013). The majority of parents rated the QoL of their children as good (46%) or very good (31%). Thirty two percent reported no or limited discussion with their child about their disease and treatment. Moreover, parents reported occurrence of pain in 95% of cases. Conclusions: It is clear that Greek parents are unaware of PC, they overestimate their children's QoL, and there is a noticeable lack of adequate discussion regarding cancer and its treatment between parents and their children. Abstract

Primary thyroid lymphoma (PTL) is a rare thyroid gland cancer, with diffuse large B-cell lymphomas (DLBCL) being extremely rare in children and adolescents. Thus, optimal therapy is debatable. We describe a rare case of thyroid DLBCL in an adolescent girl with a history of Hashimoto thyroiditis (HT), the difficulty in diagnosis and the outcome of treatment. A 12-year-old girl with a nine-year history of HT was admitted with a right-sided painless progressive swelling of the neck. Physical examination and imaging including ultrasound (US), computed tomography (CT) and positron emission tomography/CT revealed an enlarged thyroid gland with right side lymphadenopathy and no metastasis. Two fine needle aspirations were done showing suspected lymphoblastic lesions for non-Hodgkin lymphoma without precise diagnosis. US guided core needle biopsy was finally performed confirming the diagnosis of DLBCL. She was treated according to LMB 96-group B protocol with no surgical removal of thyroid. The patient responded very well to treatment and 14 months later there is no evidence of relapse or metastases. PTL is an extremely rare cause of thyroid malignancy in children. However, it should be considered in the differential diagnosis of a thyroid mass in adolescents presenting with a rapidly enlarging neck mass and a history of HT. It is a treatable condition with a good prognosis, even in aggressive histological subtypes, with no need for thyroidectomy. [ABSTRACT FROM AUTHOR]

27. Predictive Factors for Gram-negative Versus Gram-positive Bloodstream Infections in Children With Cancer

Abstract

Background: Identifying potential predictive factors for the type of bacteremia (Gram-negative vs. Gram-positive) in children with cancer would be crucial for the timely selection of the appropriate empiric antibiotic treatment. Materials and Methods: Demographic, clinical, and laboratory characteristics of children with cancer and a bacterial bloodstream infection (BSI) (February 1, 2011 to February 28, 2018) in a tertiary pediatric oncology department were retrospectively examined and were correlated with the type of isolated bacteria. Results: Among 224 monomicrobial bacterial BSI episodes, Gram-negative and Gram-positive bacteria were isolated in 110 and 114 episodes, respectively. Gram-negative bacteria were isolated significantly more frequently in girls (Gram-negative/Gram-positive ratio 1.7:1) versus boys (Gram-negative/Gram-positive ratio 0.72:1), P=0.002, in patients with previous BSI episodes (1.4:1) versus those without (0.8:1), P=0.042, and in children with hematologic malignancy (1.3:1) versus those who suffered from solid tumors (0.52:1), P=0.003. Gram-negative BSI episodes were more frequently correlated with a lower count of leukocytes, P=0.009, neutrophils, P=0.009 and platelets, P=0.002, but with significantly higher C-reactive protein (CRP) levels, P=0.049. Female sex, hematologic malignancy, and higher CRP levels remained independent risk factors for Gram-negative BSI in the multivariate analysis. Among neutropenic patients, boys with solid tumors and a recent central venous catheter placement appear to be at increased risk for Gram-positive BSI in the multivariate analysis. Conclusions: Although Gram-negative and Gram-positive BSIs are close to balance in children with cancer, Gram-negative bacteria are more likely to be isolated in girls, children with hematologic malignancies and those with higher CRP level at admission. In contrast, neutropenic boys with solid tumors and a recently placed central venous catheter may be at increased risk for Gram-positive BSI indicating probably the need for initially adding antibiotics targeting Gram-positive bacteria.

34. Overexpression of the GR Riborepressor LncRNA GAS5 Results in Poor Treatment Response and Early Relapse in Childhood B-ALL

Simple Summary Although childhood acute lymphoblastic leukemia (chALL) management is considered as one of the success stories in modern clinical oncology, the increased incidence of relapse in high-risk patients and the severe toxicity/long-term health effects due to chemotherapy intensity highlight the need for further improvements in patients’ risk stratification and personalized management. Synthetic glucocorticoids (GCs) are the core agents in chALL chemotherapy, exerting their role through the nuclear glucocorticoid receptor (GR), while GAS5 lncRNA suppresses the GCs-GR axis through binding to GR’s DNA binding domain. The objective of the study was the evaluation of GAS5 prognostic utility in chALL. GAS5 overexpression was strongly associated with a higher risk for short-term relapse and poor treatment outcome, independently of patients’ clinicopathological data. Moreover, “GAS5-including” multivariate models resulted in superior risk stratification and clinical benefit for disease prognosis, supporting precision medicine decisions in chALL. Glucocorticoids (GCs) remain the cornerstone of childhood acute lymphoblastic leukemia (chALL) therapy, exerting their cytotoxic effects through binding and activating of the glucocorticoid receptor (GR). GAS5 lncRNA acts as a potent riborepressor of GR transcriptional activity, and thus targeting GAS5 in GC-treated chALL could provide further insights into GC resistance and support personalized treatment decisions. Herein, to study the clinical utility of GAS5 in chALL prognosis and chemotherapy response, GAS5 expression was quantified by RT-qPCR in bone marrow samples of chB-ALL patients at diagnosis (n = 164) and at end-of-induction (n = 109), treated with ALL-BFM protocol. Patients’ relapse and death were used as clinical end-points for survival analysis. Bootstrap analysis was performed for internal validation, and decision curve analysis assessed the clinical net benefit for chALL prognosis. Our findings demonstrated the elevated GAS5 levels in blasts of chALL patients compared to controls and the significantly higher risk for short-term relapse and poor treatment outcome of patients overexpressing GAS5, independently of their clinicopathological data. The unfavorable prognostic value of GAS5 overexpression was strongly validated in the high-risk/stem-cell transplantation subgroup. Finally, multivariate models incorporating GAS5 levels resulted in superior risk stratification and clinical benefit for chALL prognostication, supporting personalized prognosis and precision medicine decisions in chALL.

Abstract

IL-23 is an IL-12 cytokine family member with pleiotropic functions that regulates tumour growth in various cancer types, exhibiting both anti-tumorigenic and pro-tumorigenic properties. Preclinical studies have shown a potential anti-leukemic action on childhood B-ALL cells. The study involved 65 children with acute leukemia [59 patients with acute lymphoblastic leukemia (ALL) and 6 patients with acute myeloid leukemia (AML)] and 27 healthy controls. Using an enzyme-linked immunosorbent assay, we aimed to determine the IL-23 levels in the peripheral blood (PB) and bone marrow (BM) of patients at diagnosis and at the end of the induction therapy (EIT). PB IL-23 levels were lower in leukemia patients compared to the healthy controls. In all acute leukemia patients, IL-23 levels were significantly lower at diagnosis both in PB (P = .015) and in BM (P = .037) compared to the PB and BM concentrations at the EIT. The same pattern was present in both subgroups of ALL and AML patients. The high leukemic burden at diagnosis was related with lower IL-23 levels, which were increased with the disease remission. Considering the anti-leukemic potential of this cytokine, the elevation of the IL-23 concentration at the disease remission indicates a beneficial role of IL-23 in paediatric acute leukemia.

Abstract

Background: Nursing has been characterized as one of the most stressful professions that affect nurses' quality of work life negatively. Aim: This study aimed to assess the Quality of Nursing Work Life in a sample of pediatric oncology nurses in Greece. Methodology: This is a cross-sectional comparative study. We assessed the quality of work life in a convenience sample of pediatric oncology and pediatric nurses working in the two major pediatric public hospitals in Greece, with the use of the Quality of Nursing Work Life (QNWL) Questionnaire. Data analysis was performed using Statistical Package SPSS version 23.0 (SPSS, Inc., Chicago, IL). The significance level was set at 0.05. Results: The study participated in total 119 nurses (79 pediatric oncology nurses and 40 pediatric nurses). The quality of work life was assessed as moderate (138.87 ± 24.198). The vast majority were female nurses (N=103, 86.6%) working for more than ten years in a pediatric hospital setting (N = 74, 62.2%). Pediatric oncology nurses showed statistically significant differences in subscale "Work Life / Home Life" (p = 0.003) and "Work Design" (p = 0.002) as well as in the overall QNWL scale score, with a significantly increased mean score (145.08 ± 27,064) against non-oncology pediatric nurses (135.72 ± 22.132). Conclusions: The quality of work life is a factor that affects the professional performance of nurses as well as their individual family life. Therefore, its regular assessment is important in order to address problems in work life that may result in provision of poorer quality services to patients and directly affect nurses' personal quality of life. Abstract

Background: Children with cancer face many challenges and commonly develop somatic, emotional and behavioral symptoms that negatively affect their quality of life (QoL) and their psychosocial functioning. Aim: The aim of this study was to assess the QoL of children with cancer and to address their emotional and behavioral symptoms. Methodology: This is a non-invasive descriptive study. We assessed children's QoL using proxy reports from one of their parents, in a sample of 100 children with cancer, during their hospitalization in one of the two major pediatric public hospitals in Greece. The proxy form of "Quality of Life Scale" (QoLS) was used to assess QoL and the PSC-35 scale for the reporting of their children's emotional and behavioral symptoms, as reported by their parents. Data analysis was performed using Statistical Package SPSS version 23.0 (SPSS, Inc., Chicago, IL). The significance level was set at 0.05. Results: The study participated 100 parents (24 fathers, 76 mothers) with a mean age of 41.94 ± 5.27 years. Their children (51 boys, 49 girls) with a mean age of 10.74 ± 4.21 years were hospitalized during the interviews in an oncology unit. Parents reported a quite high overall score in QoLS (Mean ± SD: 97.85 ± 22.84, Median: 94, IQR: 52-151) indicating that their children experience an overall acceptable quality of life. Moreover, mothers rated higher QoL of their children than fathers in the Social Dimension and the subscales of Communication Ability and Self-assessment. The PSC-35 scale was used to assess the psychosocial dysfunction of the children (attention, externalizing, and internalizing symptoms) as rated by their parents. The mean score was lower than the cut off level (Mean ± SD: 18.01 ± 6.78) and only 12% of the participants states scores >24, indicating dysfunction. However, despite the PSC-35 score, the 25% of parents believe that their child has emotional or behavioral problems and 84.1% of them expressed their will for psychological support for their child. Conclusions: The QoL in Greek children with cancer is rated as high according to their parents' reports. Abstract

Background: Pediatric oncology nurses (PONs) present higher levels of occupational stress (OS) related to the nature of the care they provide, along with the extreme workload and the emotional distress related to the loss of a patient. Aim: The aim of this study was to assess the level of OS that PONs experience in Greece in comparison to the stress experienced by general pediatric nurses (PNs). Methodology: This is a non-invasive comparative cross-sectional study. We assessed the stress level in a sample of 60 PNs and 60 PONs, working in the two biggest pediatric public hospitals in Greece, using the Nurse Stress Scale (NSS). The self-reported stress levels among these groups were compared. X² test and t-test were used to investigate the relationship between variables. The significance level was set at 0.05. Results: In all NSS' subscales PONs stated higher levels of stress in comparison to PNs, but this was statistically significant only in the death & suffering (p = 0.033) and lack of staff support subscales (p = 0.027). The majority of PNs are experiencing occasional OS on almost all NSS questions. Workload, death & suffering, conflicts with other nurses and supervisors, with medical staff, treatment uncertainty were noted as particularly stressful factors in both groups. Conclusions: PONs experience greater stress than PNs, but more evidence is needed to support the hypothesis and reveal the related factors. However, younger age, exposure to patients' death and suffering, lack of support from colleagues seem to be the core determinants related to PONs stress. [ABSTRACT FROM AUTHOR]

Abstract

Background: Parental age has been associated with several childhood cancers, albeit the evidence is still inconsistent. Aim: To examine the associations of parental age at birth with acute myeloid leukemia (AML) among children aged 0-14 years using individual-level data from the Childhood Leukemia International Consortium (CLIC) and non-CLIC studies. Material/methods: We analyzed data of 3182 incident AML cases and 8377 controls from 17 studies [seven registry-based case-control (RCC) studies and ten questionnaire-based case-control (QCC) studies]. AML risk in association with parental age was calculated using multiple logistic regression, meta-analyses, and pooled-effect estimates. Models were stratified by age at diagnosis (infants < 1 year-old vs. children 1-14 years-old) and by study design, using five-year parental age increments and controlling for sex, ethnicity, birthweight, prematurity, multiple gestation, birth order, maternal smoking and education, age at diagnosis (cases aged 1-14 years), and recruitment time period. Results: Adjusted odds ratios (ORs) and 95% confidence intervals (CIs) derived from RCC, but not from the QCC, studies showed a higher AML risk for infants of mothers >= 40-year-old (OR = 6.87; 95% CI: 2.12-22.25). There were no associations observed between any other maternal or paternal age group and AML risk for children older than one year. Conclusions: An increased risk of infant AML with advanced maternal age was found using data from RCC, but not from QCC studies; no parental age-AML associations were observed for older children.
