Diagnosis at an advanced stage and in younger women are the most concerning issues of breast cancer in India. Lack of awareness and social taboos related to cancer diagnosis make women feel hesitant to seek timely medical advice. As almost half of women develop breast cancer at an age younger than 50 years, breast cancer diagnosis poses a huge financial burden on the household and impacts the entire family. Moreover, inaccessibility, unaffordability, and high out-of-pocket expenditure make this situation grimmer. Women find it difficult to get quality cancer care closer to their homes and end up traveling long distances for seeking treatment. Significant differences in the cancer epidemiology compared to the west make the adoption of western breast cancer management guidelines challenging for Indian women. In this article, we intend to provide a comprehensive review of the management of breast cancer from diagnosis to treatment for both early and advanced stages from the perspective of low-middle-income countries. Starting with a brief introduction to epidemiology and guidelines for diagnostic modalities (imaging and pathology), treatment has been discussed for early breast cancer (EBC), locally advanced, and MBC. In-depth information on loco-regional and systemic therapy has been provided focusing on standard treatment protocols as well as scenarios where treatment can be de-escalated or escalated. Abstract

Alveolar soft part sarcoma (ASPS) is a rare malignant soft tissue tumor of uncertain histogenesis. (1) It accounts for less than 1% of all soft tissue sarcomas. (1,2) It was [...], * Context.--Alveolar soft part sarcoma is a rare soft tissue tumor involving mainly deep soft tissue of the lower extremities. Primary bone involvement is extremely rare. Objective.--To discuss histopathologic and immunohistochemical features of alveolar soft part sarcoma along with challenges in diagnosis and management in the context of primary bone origin. Design.--Our study constituted 8 patients. Data were collected from the Tata Memorial Hospital, Mumbai, India, during a 10-year period. Results.--Five patients in our study were female and 3 were male. Their ages ranged from 13 to 56 years. Primary bone involvement was seen in the humerus, tibia, fibula, radius, calcaneus, and rib. Radiologic impression was that of a primary malignant bone tumor in all patients. Conventional histopathologic features were seen in 7 of 8 patients. Positive immunohistochemical expression for TFE3 was demonstrated in 6 patients. All patients had distant metastasis either at presentation or later in the course of the disease. Surgical excision of the primary tumor and adjuvant chemotherapy formed the basis of treatment. Four patients succumbed to disease and 2 were alive with disease. Conclusions.--Rare occurrences of primary bone alveolar soft part sarcoma posed a diagnostic challenge in the form of differential diagnoses of metastatic tumors resembling it either morphologically or immunohistochemically. The combined application of clinical and radiologic features along with characteristic histomorphology and immunohistochemistry helps to arrive at a definite diagnosis of alveolar soft part sarcoma. Aggressive behavior of this tumor and its refractoriness to conventional chemotherapy were evident from our series.

Abstract

Background: The care of adolescents and young adults (AYAs) with bone sarcomas involves unique challenges. The objectives of this study were to identify challenges and evaluate long‐term outcomes of these patients from India who received treatment with novel protocols. Methods: This prospective cohort study included AYA patients (aged 15–39 years) with osteosarcoma and Ewing sarcoma (ES), who were treated uniformly at the authors' institute using unique protocols (OGS‐12 and EFT‐2001) from 2011 to 2021 and from 2013 to 2018, respectively. Results: The cohorts included 688 of 748 (91.9%) treatment‐naive AYA patients with osteosarcoma and 126 of 142 (88.7%) treatment‐naive AYA patients with ES. Among 481 of 561 patients (85.7%) who had nonmetastatic osteosarcoma treated according to protocol, at a median follow‐up of 59.7 months, the 5‐year event‐free survival (5‐EFS) rate was 58.6% (95% confidence interval, 54.1%–63.5%) and for 142 patients (20.6%) who had metastatic osteosarcoma, the 5‐EFS rate was 18.7%. The 5‐EFS rate was 66.4% and 21.9% for 104 patients (73%) with nonmetastatic ES and 38 patients (27%) with metastatic ES, respectively. Treatment‐naive patients had better outcomes, similar to compliance in the form of protocol completion (hazard ratio, 1.93 [p =.0043] and 2.66 [p <.0001], respectively. Only 230 of 377 (61.0%) male patients and 10 of 134 (7.4%) female patients reached out to fertility specialists. In addition, 17 of 161 (10.6%) eligible male survivors and 14 of 61 (22.9%) eligible female survivors got married posttreatment. Furthermore, 14 of 17 (82.4%) males and 14 of 14 (100%) females conceived. Among 311 patients who were working or attending school during diagnosis, greater than 90% had interruptions. Conclusions: Homogenous treatment with the OGS‐12 and EFT‐2001 protocols resulted in internationally comparable long‐term outcomes in the cohorts with nonmetastatic and metastatic AYA bone sarcomas. Treatment compliance, timely referral to sarcoma reference centers (avoiding prior inadvertent treatment), and streamlining fertility‐preservation practices constitute unmet needs that demand prioritization. Adolescents and young adults who have bone sarcomas receive prior inadvertent treatment at peripheral centers and commonly present with a significant delay, resulting in a high baseline tumor burden and relatively higher stage disease. Superior outcomes with the indigenous OGS‐12 and EFT‐2001 protocols in treatment‐naive and compliant patients who have osteosarcoma and Ewing sarcoma, respectively, compared with inadvertently treated adolescent and young adult patients, underscores the need for the timely referral of these patients to sarcoma reference centers and effective patient navigation systems. Abstract

Background: Obstructive airway disease (OAD) causes significant morbidity and healthcare expenses. Smoking cessation, drug therapy, and pulmonary rehabilitation reduce morbidity and improve outcomes. However, each strategy requires knowledge, positivity, adherence, compliance, and technique to improve OAD clinical outcomes. Aim and objective: To assess the knowledge and attitude of OAD patients and to evaluate their medication adherence in a tertiary care hospital. Methodology: A north Indian tertiary care hospital conducted this six-month prospective, observational, questionnaire-based investigation. Subjects were asked questions on their disease knowledge, attitude, and drug adherence. Results: 152 patients were analyzed for this study. Here, 80 (51.9%) patients were male and 74(48.1%) were female. Among the subjects, 46.9% (n=72, p=0.0001) have adequate knowledge about their treatment and ailment. Out of the total patients, 71.2% (n=110, p=0.007) have an optimistic attitude towards their treatment, while 42.5% (n=65) of patients adhere to their therapy. Conclusion: OAD patients have low drug adherence. Simplified treatment regimens, self-management education, and provider abilities in patient education, communication, and adherence counseling can solve shortcomings. [ABSTRACT FROM AUTHOR]

Abstract

The main issue in Cross Language Information Retrieval (CLIR) is the poor performance of retrieval in terms of average precision when compared to monolingual retrieval performance. The main reasons behind poor performance of CLIR are mismatching of query terms, lexical ambiguity and un-translated query terms. The existing problems of CLIR are needed to be addressed in order to increase the performance of the CLIR system. In this paper, we are putting our effort to solve the given problem by proposed an algorithm for improving the performance of English-Hindi CLIR system. We used all possible combination of Hindi translated query using transliteration of English query terms and choosing the best query among them for retrieval of documents. The experiment is performed on FIRE 2010 (Forum of Information Retrieval Evaluation) datasets. The experimental result show that the proposed approach gives better performance of English-Hindi CLIR system and also helps in overcoming existing problems and outperforms the existing English-Hindi CLIR system in terms of average precision., Comment: International Journal on Natural Language Computing (IJNLC) Vol. 2, No.6, December 2013 http://airccse.org/journal/ijnlc/index.html

Abstract

Background: As the pulmonary system and cardiovascular system are intimately linked, patients with chronic obstructive pulmonary disease (COPD) and asthma have high risk for developing cardiovascular diseases (CVDs) and altered central hemodynamic. Objective: We aim to assess the central aortic blood pressure (CABP) indices, pulse wave velocity (PWV) and other indicators of arterial stiffness in Indian patients with COPD and bronchial asthma. Methods: This is a single‐center, cross‐sectional study conducted in outpatients diagnosed with either chronic stable phase of COPD or bronchial asthma. CABP indices, vascular age, arterial stiffness and central hemodynamics were measured in patients. Results: Of 193 patients with obstructive airway disease who were enrolled, (n = 81 had COPD and n = 112 had partially‐controlled bronchial asthma) the proportion of male patients was higher in both groups. The PWV, augmentation index (AI) and vascular age (VA) were significantly higher in patients with COPD compared to those with bronchial asthma (all, p < 0.05). Conclusion: The study showed that PWV, AI and VA were higher in patients with stable COPD without any cardiac comorbidities compared to bronchial asthma. [ABSTRACT FROM AUTHOR]

In this paper, we define and explore several properties of Fermatean fuzzy soft Ti, i = 0,1,2, Fermatean fuzzy soft regular, Fermatean fuzzy soft T3, Fermatean fuzzy soft normal, and Fermatean fuzzy soft T4 axioms using Fermatean fuzzy soft points. We also discuss some Fermatean fuzzy soft invariance properties namely Fermatean fuzzy soft topological property and Fermatean fuzzy soft hereditary property.
