Abstract

Summary: Diffuse large B‐cell lymphoma (DLBCL) patients with relapsed or refractory (RR) disease have poor outcomes with current salvage regimens. We conducted a phase 2 trial to analyse the safety and efficacy of adding lenalidomide to R‐ESHAP (LR‐ESHAP) in patients with RR DLBCL. Subjects received 3 cycles of lenalidomide 10 mg/day on days 1–14 of every 21‐day cycle, in combination with R‐ESHAP at standard doses. Responding patients underwent autologous stem‐cell transplantation (ASCT). The primary endpoint was the overall response rate (ORR) after 3 cycles. Centralized cell‐of‐origin (COO) classification was performed. Forty‐six patients were included. The ORR after LR‐ESHAP was 67% (35% of patients achieved complete remission). Patients with primary refractory disease (n = 26) had significantly worse ORR than patients with non‐refractory disease (54% vs. 85%, p = 0.031). No differences in response rates according to the COO were observed. Twenty‐eight patients (61%) underwent ASCT. At a median follow‐up of 41 months, the estimated 3‐year PFS and OS were 42% and 48%, respectively. The most common grade ≥3 adverse events were thrombocytopenia (70% of patients), neutropenia (67%) and anaemia (35%). There were no treatment‐related deaths during LR‐ESHAP cycles. In conclusion, LR‐ESHAP is a feasible salvage regimen with promising efficacy results for patients with RR DLBCL. [ABSTRACT FROM AUTHOR]

Abstract

The properties of the cast aluminium alloys 6061/(20%Al2O3)p are studied. This material is obtained by Duralcan procedure after being submitted to an extrusion and forging process. These materials are submitted to T6 heat treatments in order to study their effect on mechanical properties. In this work the matrix characteristics are compared with the reinforced material in different production stages, using an optical and electronic microscopy, EDS, tensile, fatigue and ultramicrohardness tests. The mechanical properties indicate that the extruded reinforced material presents ultimate tensile strength values lower than the alloy 6061 under the same conditions, and this is related to the presence of spinel in the interface matrix-reinforcement. The fracture mechanisms are established by static and dynamic tensions in different cases, studying the possible interactions matrix-reinforcement and their repercussion in the mechanical behaviour of the alloy.Se estudian las propiedades de las aleaciones de aluminio 6061/(20%Al2O3)p obtenidas por colada y conformadas posteriormente por extrusión y forja. Estos materiales son sometidos a tratamientos térmicos de envejecimiento (T6) para estudiar su efecto en las propiedades mecánicas. En este trabajo se comparan las características de la matriz con las del material reforzado en las distintas etapas de producción utilizando técnicas de microscopía óptica y electrónica, EDS y ensayos de tracción, fatiga y ultramicroindentación. Los ensayos mecánicos indican que el material compuesto extruido presenta valores de resistencia mecánica más bajos que la aleación 6061 tratada en las mismas condiciones y ello se relaciona con la presencia de espinela en la interfase alúmina-matriz. Se establecen los mecanismos de fractura por tensiones estáticas y dinámicas en los distintos casos, estudiando las posibles interacciones matriz-refuerzo y su influencia en el comportamiento mecánico de la aleación.

Abstract

Simple Summary We have tested the effects of IGF-1R tyrosine kinase inhibitors BMS-754807 (BMS) and OSI-906 (OSI) on human colon, pancreatic carcinoma cell, and glioblastoma cell lines and primary cultures. Although OSI and BMS are able to inhibit IGF-1R activity at low doses, the differential effect on cell proliferation and cell-cycle phase distribution shown by both compounds probes that many effects observed are mediated by BMS off-target interactions. Using MAPKs ELISAs and phospho-RTK array analysis, we have identified several BMS regulated putative kinases able to mediate BMS off-target effects. Interestingly, molecular docking assays suggest that BMS could affect these kinases not only by blocking their ATP-binding domain, but also by means of allosteric interactions. Since BMS has an important antineoplastic effect on these poor prognosis types of cancer, these compounds could be taken in consideration for treatment independently of IGF-1R status. We have determined the effects of the IGF-1R tyrosine kinase inhibitors BMS-754807 (BMS) and OSI-906 (OSI) on cell proliferation and cell-cycle phase distribution in human colon, pancreatic carcinoma, and glioblastoma cell lines and primary cultures. IGF-1R signaling was blocked by BMS and OSI at equivalent doses, although both inhibitors exhibited differential antiproliferative effects. In all pancreatic carcinoma cell lines tested, BMS exerted a strong antiproliferative effect, whereas OSI had a minimal effect. Similar results were obtained on glioblastoma primary cultures, where HGUE-GB-15, -16 and -17 displayed resistance to OSI effects, whereas they were inhibited in their proliferation by BMS. Differential effects of BMS and OSI were also observed in colon carcinoma cell lines. Both inhibitors also showed different effects on cell cycle phase distribution, BMS induced G(2)/M arrest followed by cell death, while OSI induced G(1) arrest with no cell death. Both inhibitors also showed different effects on other protein kinases activities. Taken together, our results are indicative that BMS mainly acts through off-target effects exerted on other protein kinases. Given that BMS exhibits a potent antiproliferative effect, we believe that this compound could be useful for the treatment of different types of tumors independently of their IGF-1R activation status.

Abstract

The International Prognostic Index (IPI) is the most widely used score for non-Hodgkin lymphoma but lacks the ability to identify a high-risk population in diffuse large B-cell lymphoma (DLBCL). Low absolute lymphocyte count and high monocytes have proved to be unfavourable factors. Red-cell distribution width (RDW) has been associated with inflammation and beta-2 microglobulin (B2M) with tumour load. The retrospective study included 992 patients with DLBCL treated with R-CHOP. In the multivariate analysis, age, Eastern Cooperative Oncology Group performance status (ECOG-PS), stage, bulky mass, B2M, RDW, and lymphocyte/monocyte ratio (LMR) were independently related to progression-free survival (PFS). A new prognosis score was generated with these variables including age categorized into three groups (0, 1, 2 points); ECOG >= 3-4 with two; stage III/IV, bulky mass, high B2M, LMR 0 center dot 96 with one each; for a maximum of 9. This score could improve the discrimination of a very high-risk subgroup with five-year PFS and overall survival (OS) of 19% and 24% versus 45% and 59% of R (revised)-IPI respectively. This score also showed greater predictive ability than IPI. A new score is presented including complete blood cell count variables and B2M, which are readily available in real-life practice without additional tests. Compared to R-IPI, it shows a more precise high-risk assessment and risk discrimination for both PFS and OS.

Abstract

Chronic lymphocytic leukemia (CLL) is a disease of the elderly, characterized by immunodeficiency. Hence, patients with CLL might be considered more susceptible to severe complications from COVID-19. We undertook this retrospective international multicenter study to characterize the course of COVID-19 in patients with CLL and identify potential predictors of outcome. Of 190 patients with CLL and confirmed COVID-19 diagnosed between 28/03/2020 and 22/05/2020, 151 (79%) presented with severe COVID-19 (need of oxygen and/or intensive care admission). Severe COVID-19 was associated with more advanced age (≥65 years) (odds ratio 3.72 [95% CI 1.79–7.71]). Only 60 patients (39.7%) with severe COVID-19 were receiving or had recent (≤12 months) treatment for CLL at the time of COVID-19 versus 30/39 (76.9%) patients with mild disease. Hospitalization rate for severe COVID-19 was lower (p < 0.05) for patients on ibrutinib versus those on other regimens or off treatment. Of 151 patients with severe disease, 55 (36.4%) succumbed versus only 1/38 (2.6%) with mild disease; age and comorbidities did not impact on mortality. Abstract

Chronic lymphocytic leukemia (CLL) is a disease of the elderly, characterized by immunodeficiency. Hence, patients with CLL might be considered more susceptible to severe complications from COVID-19. We undertook this retrospective international multicenter study to characterize the course of COVID-19 in patients with CLL and identify potential predictors of outcome. Of 190 patients with CLL and confirmed COVID-19 diagnosed between 28/03/2020 and 22/05/2020, 151 (79%) presented with severe COVID-19 (need of oxygen and/or intensive care admission). Severe COVID-19 was associated with more advanced age (≥65 years) (odds ratio 3.72 [95% CI 1.79–7.71]). Only 60 patients (39.7%) with severe COVID-19 were receiving or had recent (≤12 months) treatment for CLL at the time of COVID-19 versus 30/39 (76.9%) patients with mild disease. Hospitalization rate for severe COVID-19 was lower (p < 0.05) for patients on ibrutinib versus those on other regimens or off treatment. Of 151 patients with severe disease, 55 (36.4%) succumbed versus only 1/38 (2.6%) with mild disease; age and comorbidities did not impact on mortality. In CLL, (1) COVID-19 severity increases with age; (2) antileukemic treatment (particularly BTK inhibitors) appears to exert a protective effect; (3) age and comorbidities did not impact on mortality, alluding to a relevant role of CLL and immunodeficiency.

Abstract

In this work the microstructural changes that occurs during the solution and aging steps of heat treated of AlSi alloys, conformed in semi-solid state, were analysed. The study allowed developing a new T6 heat treatment, with solution times down to 30 min. With this new short heat treatment, the alloy showed better mechanical properties in comparison with the same alloy heat treated in standard conditions (solution times between 6 h and 8 h). This new heat treatment, carried out at 540 ºC, allowed complete magnesium dissolution and, at the same time, minimizes the grain and eutectic silicon growth. Although this experimentation was carried out with A356 and A357 aluminum alloys, conformed by Sub-Liquidus-Casting process, these results may be applicable to components produced with other semi-solid technologies and with others AlSi hardenable alloys that form coherent magnesium precipitates. The characterization of samples was carried out by micrographic analysis, by hardness and ultramicrohardnes tests. Se estudian los cambios microestructurales que tienen lugar en los tratamientos térmicos de las aleaciones AlSi, conformadas en estado semisólido, durante las etapas de puesta en solución y envejecimiento. El estudio ha permitido proponer nuevos tratamientos T6, con tiempos de puesta en solución inferiores a los 30 min, manteniendo o mejorando las propiedades mecánicas que se obtienen con los actuales procesos de tratamiento de 6 a 8 h. Estos tratamientos de corta duración, realizados a 540 ºC, permiten una completa disolución del magnesio, minimizando al mismo tiempo el crecimiento de los granos y del silicio eutéctico. Si bien la experimentación se ha realizado con componentes producidos por Sub-Liquidus Casting (SLC) con aleaciones A356 y A357, se considera que los resultados obtenidos pueden ser aplicables a los componentes producidos en estado semisólido por otras tecnologías, con diferentes aleaciones AlSi que se endurecen por formación de precipitados coherentes de magnesio. La caracterización del material se ha realizado mediante análisis micrográficos, ensayos de dureza y ultramicrodureza.

[ES] Se realiza un estudio comparativo del comportamiento frente a la corrosión de la aleación de aluminio 2124 reforzada con partículas de carburo de silicio, obtenida por pulvimetalurgia y con tratamientos térmicos T4 y T6. Los resultados experimentales muestran que el tratamiento térmico T6 induce la formación de numerosos intermetálicos que reducen la resistencia a la corrosión. Las partículas de carburo de silicio no tienen un comportamiento catódico con respecto a la matriz de aluminio, sin embargo, disminuyen la resistencia a la corrosión, ya que generan zonas más activas en la interfase matriz/ refuerzo, debido a la acumulación de dislocaciones, tensiones residuales y a la precipitación de intermetálicos., [EN] A comparative study was performed on the corrosion behaviour of an aluminium matrix composite reinforced with silicon carbide particles, obtained by powder metallurgic. The 2124/SiC material was heat treated using T4 and T6 procedures. The T6 heat treatment induced the formation of several intermetallics and reducing the corrosion resistance. The silicon carbide particles did not have a cathodic behaviour as compared with the aluminium matrix. However, these particles produced a diminution in the corrosion resistance due to the formation of more active zones in the matrix/reinforced interface. These ceramic particles caused intermetallic precipitation and deformation originating in the surrounding zones localized corrosion.

Abstract

This paper studies the pitting corrosion of a structural component of A357 aluminium alloy obtained by a semisolid metal forming process. The mechanical properties of the A357 alloy were improved by applying standard heat treatments T5 and T6. Impedance measurements were conducted at the rest potential and polarisation curves were plotted using a 3% NaCl test solution. After polarisation experiments the specimens were analysed by scanning electron microscopy. The corrosion process is favoured through the eutectic regions. The results show that T6 heat treatment improved the corrosion resistance of the A357 aluminium alloy. © WILEY-VCH Verlag GmbH, 2001.
