Abstract Backgrounds Dietary potassium can play an important role in decreasing inflammatory factors as a protective factor for cancers. In this case-control study, we aimed to assess the possible association between dietary potassium intake and the risk of breast cancer (BC) among Iranian adult women. Methods The present case-control study was conducted at Shohada and Imam Hossain hospitals, in Tehran. The study included 134 newly diagnosed cases of BC and 267 controls. A validated semi-quantitative 168-item food frequency questionnaire was used to compute the potassium intake. Logistic regression, adjusted for potential confounders, was used to estimate odds ratios(ORs) and 95% confidence intervals(CI) of BC according to tertiles of potassium intake. Results The mean(M) ± standard deviation(SD) of age and body mass index (BMI) were 47.9 ± 10.3 years and 29.4 ± 5.5 kg/m2, respectively. Also, the M ± SD of potassium intake for the control and case groups was 1616 ± 293 and 1542 ± 338 (mg/1000 Kcal), respectively. Summary: Background: Hyperemesis gravidarum (HG) is the most widespread reason for admission to the hospital during the first half of pregnancy. HG etiology is not clear in many cases. However, maternal body composition and pre-pregnancy diet are suggested as possible factors. This hospital-based case-control study aims to determine the link between the Mediterranean diet score (MDS) and pre-pregnancy anthropometric indicators with Hyperemesis gravidarum. Methods: A total of 45 women with incident HG and 126 controls were entered into this case-control study between April 2020 and June 2021. A repeatable and accurate 168-item semi-quantitative food frequency questionnaire was used to define the MDS. The association between MDS and HG odds was examined through adjustment by possible confounders by the estimation of multivariate regression models. Results: The median (interquartile range) pre-pregnancy body mass index of participants was 22.7 (21.7–25.9) and 22.9 (21.4–25) Kg/m2 in cases and controls, respectively (P=0.284). The adjusted analyses found that the MDS was associated with a 25% reduced odds of HG (odds ratio (OR): 0.75, 95% confidence interval (95%CI): 0.24–0.91). Body mass index and waist circumference before pregnancy were not significantly associated with HG odds in both regression models. Conclusions: The odds of HG during pregnancy are lower in women with a higher MDS before pregnancy. Also, a high intake of fruits and nuts, vegetables, legumes, and fish before pregnancy is liked with a lower HG odds.

Abstract Introduction The benefit of adherence to a plant-based diet concerning colorectal cancer (CRC) has not been investigated among Middle Eastern population. This study aimed to investigate how adherence to a plant-based diet influences the risk of CRC in this understudied population. Methods This case-control study was conducted in the CRC surgery departments of general hospitals in Tehran, Iran. A total of 71 individuals with newly diagnosed CRC (cases) and 142 controls subjects free of cancer and acute illness were concurrently recruited from the same hospital. Dietary information was collected using a semi-quantitative 168-item food frequency questionnaire. Dietary patterns were characterized using the plant-based diet index (PDI), unhealthy plant-based diet index (uPDI) and healthy plant-based diet index (hPDI). Multivariate logistic regression was employed to assess the association between these dietary patterns and the risk of CRC. Results After adjusting the potential confounders, the risk of CRC was significantly lower in the highest tertile of hPDI compared to the lowest tertile (odds ratio (OR) = 0.21; 95% confidence interval (CI): 0.07–0.56, representing 79% risk reduction). Conversely, the risk of CRC was significantly higher in the highest tertile of uPDI compared to the lowest tertile (OR = 6.76; 95% CI: 2.41–18.94). PDI was no significant associated with the risk of CRC. Conclusions This study found that higher scores on the hPDI was significantly associated with a decrease risk of CRC, while greater adherence to the uPDI contributed to a significantly increase risk.

Abstract Background Dietary patterns, encompassing an overall view of individuals’ dietary intake, are suggested as a suitable means of assessing nutrition’s role in chronic disease development. The aim of this study was to evaluate the validity and reproducibility of a food frequency questionnaire (FFQ) designed for use in the Prospective Epidemiological Research Studies in IrAN (PERSIAN), by comparing major dietary patterns assessed by the FFQ with a reference method. Methods Study participants included men and women who enrolled in the PERSIAN Cohort Study at seven of the eighteen centers. These centers were chosen to include dietary variations observed among the different Iranian ethnic populations. Two FFQ were completed for each participant over a one-year study period (FFQ1 upon enrollment and FFQ2 at the end of the study), with 24 interviewer-administered 24-hour dietary recalls (24 h) being completed monthly in between. Spearman correlation coefficients (SCC) were used comparing FFQs 1 and 2 to the 24 h to assess validity, while FFQ1 was compared to FFQ2 to assess reproducibility of the questionnaire. Results Three major dietary patterns—Healthy, Low Protein/High Carb and Unhealthy—were identified, accounting for 70% of variance in the study population. Corrected SCC ranged from 0.31 to 0.61 in the validity and from 0.34 to 0.57 in reproducibility analyses, with the first two patterns, which accounted for over 50% of population variance, correlated at above 0.5 in both parameters, showing acceptable findings. Conclusions The PERSIAN Cohort FFQ is suitable for identification of major dietary patterns in the populations it is used for, in order to assess diet-disease relationships.

Abstract Background Evidence from recent studies suggested that variation in the quantity and quality of macronutrients in the diet may potentially play a role in predicting the risk of breast cancer (BC). In the current study, we aimed to assess the association of different high-protein diet scores and replacing fats and carbohydrate (CHO) with protein in the diet with the BC risk among Iranian women. Methods The current hospital-based case-control study was conducted on 401 participants, aged ≥ 30 years old, including 134 women in the case group who had been diagnosed with histologically confirmed BC and 267 women in the control group. Dietary intake data was collected using a validated food frequency questionnaire, and high protein diet scores were determined. Logistic regression models were used to determine the odds ratios (OR) and 95% confidence interval (CI) of BC across tertiles of high protein diet scores. Also, we assessed how substituting protein with other macronutrients affected BC odds while adjusting for the various confounding variables. Results Participants’ mean ± SD of age and body mass index were 47.9 ± 10.3 years and 29.4 ± 5.5 kg/m2, respectively. The scores of high-protein-low-CHO and fat diet, high-protein and CHO-low-fat diet, and high-protein and fat-low-CHO diet in participants were 16.5 ± 3.8, 16.5 ± 6.7, and 16.4 ± 5.9, respectively. Abstract Background The incidence of colorectal cancer (CRC) has increased in Iran, and determining the dietary patterns that can contribute to reducing or increasing the risk of CRC will help better control this disease. Therefore, in the current study, we assessed the association between the empirical lifestyle index for hyperinsulinemia (ELIH) and the empirical dietary index for hyperinsulinemia (EDIH) with the CRC odds. Methods The present case (n = 71)-control (n = 142) study was carried out in several CRC surgical units of hospitals in Tehran, Iran. A semi-quantitative food frequency questionnaire containing 168 items was used to assess participants’ dietary intakes. The EDIH and ELIH scores were calculated by food groups and some variables such as body mass index and physical activity. Logistic regression models were applied to evaluate the association between the EDIH and ELIH scores with CRC odds. Results According to baseline features of the study participants, there were significant differences between the controls and cases in ELIH score, fiber intake, taking aspirin, and family history of CRC in first- and second-degree relatives. Also, we found that the odds of CRC increased significantly in the last tertile compared to the first tertile in EDIH and ELIH in the adjusted model (odds ratio (OR) = 3.12; 95% confidence interval (CI): 1.30–7.48 and OR = 4.72; 95% CI: 1.15–19.39, respectively). Conclusions In conclusion, the result of this study indicated that CRC odds was significantly greater in subjects with higher EDIH and ELIH scores. Also, according to the results of this study, lifestyle and diet with insulinemic potential can influence the CRC risk.

Abstract Background Few studies assess the link between plant-based diets and colorectal cancer (CRC) incidence. To our knowledge, no study has examined the association between pro-vegetarian dietary pattern (PDP) and CRC globally or among Iranians. Therefore, the objective of our matched case-control study was to evaluate the association between PDP and CRC in the Iranian population. Methods The present research was a hospital-based case (n = 71)-control (n = 142) study conducted in the same hospitals in Tehran, Iran. This study used a reliable and valid semi-quantitative food frequency questionnaire to evaluate the participants’ dietary intake. According to the residual method, the selected plant and animal foods have been adjusted in the total energy intake to calculate the PDP index. Odds ratios (ORs) and 95% confidence intervals (CIs) adjusted for confounding variables were also expressed using logistic regression by SPSS software. Results In the crude and adjusted models, we observed that the odds of CRC decreased significantly in the 3rd and last quartile of PDP compared to the 1st quartile (Q) (Crude model: Q3: OR = 0.36, 95% CI: 0.17 − 0.79, P-value = 0.011 and Q4: OR = 0.33, 95% CI: 0.14 − 0.79, P-value = 0.012 - Adjusted model: Q3: OR = 0.41, 95% CI: 0.18 − 0.94, P-value = 0.035 and Q4: OR = 0.35, 95% CI: 0.14 − 0.87, P-value = 0.025). Conclusions Based on the results of the present case-control study in the Iranian population, it was concluded that PDP, which involves consuming vegetables, fruits, cereals, dairy products, and low meat consumption, reduces the odds of CRC. In conclusion, adherence to PDP is associated with a decreased odds of CRC.

Abstract Background Dietary advanced glycation end products (AGEs) can play an important role in increasing inflammatory factors and oxidative stress as risk factors for cancers. In the present study, we aimed to assess the relationship between dietary AGEs and the risk of breast cancer (BC) in Iranian adult women. Methods This hospital-based case-control study includes 401 participants aged ≥ 30 years old. The cases group consisted of 134 women diagnosed with histologically confirmed BC. The control group included 267 women enrolled randomly from patients admitted to the same hospitals. Dietary intake information was determined using a validated food frequency questionnaire, and dietary AGEs intake was computed for all participants. Logistic regression models, adjusted for potential confounders, were used to determine the odds ratios (OR) and 95% confidence interval (CI) of BC across tertiles of dietary AGEs. Results The mean ± SD age and body mass index of the study population were 47.92 ± 10.33 years and 29.43 ± 5.51 kg/m2, respectively. The median (interquartile) of dietary AGEs in all individuals was 9251(7450, 11,818) kU/day. After adjusting for age, first pregnancy age, and energy intake, participants in the highest tertile of dietary AGEs intakes had higher odds of BC compared to those in the lowest tertile of dietary AGEs (OR:2.29;95%CI:1.19–4.39, Ptrend:0.012). Additionally, in the multivariable model, after adjusting for age, age at first pregnancy, energy, menopausal status, family history of cancer, anti-inflammatory drug use, Vitamin D supplementation, physical activity, body mass index, number of childbirths, and history of abortion, breastfeeding, and oral contraceptive pills use, the odds of BC were increased across tertiles of dietary AGEs intake (OR: 2.33; 95%CI: 1.18–4.60, Ptrend: 0.017). Conclusion The present findings suggest that a diet with high AGEs is associated with a higher likelihood of BC in adult women.

Abstract Background Diet quality is a significant determinant in the etiology of breast cancer (BrCa), but further studies are required to explore this relationship. Therefore, we tried to assess if diet quality, assessed using the Diet Quality Index-International (DQI-I), was related to BrCa among the Iranian population. Methods In the present case-control research, 134 women with a recent diagnosis of BrCa and 267 without BrCa were selected as case and control groups. Individual food intake data from a food frequency questionnaire was used to compute DQI-I. Also, the multivariable logistic regression models were utilized to evaluate the association between DQI-I and BrCa odds . Results We found a significant association between the last tertile of DQI-I and BrCa odds in the fully adjusted model (odds ratio (OR) = 0.30; 95% confidence interval (CI): 0.15–0.56). The subgroup analysis based on menopausal status also showed a significant decrease in BrCa odds in pre-and post-menopausal women (pre-menopausal: OR = 0.27; 95% CI: 0.10–0.70 – post-menopausal status: OR = 0.35; 95% CI: 0.13–0.92). Conclusions Our findings indicated that a higher DQI-I score was related to a lower chance of BrCa. According to our research, a healthy diet pattern is crucial for BrCa prevention.

Abstract Background No previous study has assessed the association between oxidative balance score (OBS) and dietary phytochemical index (DPI) with colorectal cancer (CRC) simultaneously. Therefore, this study investigated the association between OBS and DPI with the odds of CRC among the Iranian population. Methods This hospital-based age and sex-matched case–control study was conducted between September 2008 and January 2010 (142 controls and 71 cases were entered for analysis). New diagnosed CRC cases were selected from the Cancer Institute, Imam Khomeini Hospital of Tehran. Dietary intakes were determined by a semi-quantitative food frequency questionnaire (FFQ). Then, dietary indices were calculated by food items and nutrient intake. Logistic regression was utilized for assessing the tertiles of OBS and DPI. Results In multivariate analysis, OBS was associated with a 77% reduction in CRC odds in the last tertile than the first tertile (odds ratio (OR) = 0.23, confidence interval (CI): 0.07–0.72, Ptrend = 0.017). Also, we found a 64% reduction in CRC odds in the last tertile of DPI compared to the first tertile (OR = 0.36, CI: 0.15–0.86, Ptrend = 0.015). Conclusions A diet rich in phytochemicals and anti-oxidants, including fruits and vegetables (citrus fruits, colored berries, and dark-green leafy vegetables) and whole grains, may reduce the CRC odds.

Abstract Background Colorectal cancer (CRC) is the fourth and third most common cancer in Iran and the world, respectively. Carbohydrates can lead to the proliferation of cancer cells, including CRC. The current study aimed to investigate the association between glycemic load (GL), insulin load (IL), glycemic index (GI), insulin index (II), low-carbohydrate diet score (LCDS), and carbohydrate quality index (CQI) with CRC odds. Methods The present case-control study was performed on 71 CRC cases and 142 controls in the Hospital Cancer Organization and three general hospitals in Tehran, Iran. We calculated the dietary GI, GL, IL, II, CQI, and LCDS by a validated food frequency questionnaire. Results The results indicated that people who were in the highest tertile of the GI had higher odds of CRC compared to the lower tertile (in the adjusted model: odds ratio (OR) = 3.89; 95% confidence interval (CI): 1.71–8.84). On the contrary, people who were in the highest tertile of the CQI and LCDS had significantly lower odds of CRC compared to the lower tertile (in the adjusted model: tertile (T) 2-OR = 0.24; 95% CI: 0.11–0.53 and T3-OR = 0.15; 95% CI: 0.06–0.39 for CQI and T2-OR = 0.33; 95% CI: 0.13–0.79 and T3-OR = 0.28; 95% CI: 0.10–0.82 for LCDS). Also, IL was positively associated with the odds of CRC after adjusting for confounding factors (T2-OR = 2.46; CI: 1.08–5.61 and T3- OR = 2.80; 95% CI: 1.07–7.31). Regarding the GL, only individuals who were in the second tertile had significantly higher odds of CRC compared to the first tertile (OR = 2.42; CI: 1.07–5.47). Conclusion According to the findings, it is recommended to use a diet with high-quality carbohydrates and low GI and GL to minimize the odds of developing CRC. People should also be encouraged to have a balanced carbohydrate intake.

Abstract Background Breast cancer (BC) is the most prevalent cancer, with a higher mortality rate in women worldwide. We aimed to investigate the association of the insulinemic potential of diet and lifestyle with the odds of BC using empirical indices, including the empirical dietary index for hyperinsulinemia (EDIH), empirical lifestyle index for hyperinsulinemia (ELIH), the empirical dietary index for insulin resistance (EDIR), and empirical lifestyle index for insulin resistance (ELIR). Methods This hospital-based case-control study was conducted among Tehranian adult women aged≥30 years. The final analysis was performed on 134 women newly diagnosed with histologically confirmed BC as a case and 267 healthy women of the same age as control. A 168-food item food frequency questionnaire was used for assessing dietary intakes at baseline. The odds ratios (ORs) and 95% confidence intervals (CIs) of BC across tertiles of EDIH, ELIH, EDIR, and ELIR were determined using multivariable-adjusted logistic regression. Results The mean ± SD of age and BMI of participants were 47.9±10.3 years and 29.4±5.5 kg/m2, respectively. EDIH score was related to the higher risk of BC based on fully adjusted models (OR:2.24;95%CI:1.21–4.12, Ptrend=0.016). Furthermore, subgroup analysis showed a higher BC risk with increasing EDIH score in postmenopausal women (OR:1.74, 95%CI:1.13-2.69) and those without a history of the oral contraceptive pill (OCP) use (OR:1.44;95%CI:1.02–2.04). Moreover, ELIH scores were positively associated with an increased risk of BC in postmenopausal women (OR; 1.98; 95% CI: 1.35 – 2.89), those with a family history of cancer (OR:1.94;95%CI:1.10–3.42), and in individuals who did not use OCP (OR:1.46; 95% CI:1.00–2.12). Conclusion Our results showed a possible link between EDIH and higher BC risk. Also, higher EDIH and ELIH scores were strongly associated with a higher risk of BC in postmenopausal women, those with a family history of BC, and those who do not use OCP.

Background and aimsDifferent factors, such as environmental, epigenetic, genetic and immunological, have been identified as potential risks for developing endometriosis. However, the correlation between dietary patterns and endometriosis is currently unknown. The aim of this study was to explore the potential link between fertility diet score and the odds of endometriosis.MethodsThis study was a hospital-based case–control study that took place in a gynecology clinic in Tehran, Iran, between February 2021 and January 2022. A total of 107 newly diagnosed endometriosis cases and 210 controls were included. The participants' habitual diets were evaluated using a food frequency questionnaire, and their fertility diet score was estimated using a point system based on Chavarro et al.'s criteria. The logistic regression was utilized to calculate the odds ratios (OR) with 95% confidence intervals (CIs).ResultsThe study found that women who adherence to fertility diet have a lower odds of endometriosis. This was observed in both the base model and the adjusted model, with a significant decrease in odds of endometriosis by 66% (OR = 0.44, 95%CI = 0.27–0.71, p = 0.001) and 54% (aOR = 0.46, 95%CI = 0.23–0.90, p = 0.022), respectively. Additionally, consuming vegetable proteins and multivitamins were also associated with lower odds of endometriosis. On the other hand, consuming animal proteins, heme iron, and having a high glycemic load were associated with significantly higher odds of endometriosis.ConclusionOur research supports the hypothesis that following a fertility diet may decrease the odds of endometriosis in Iranian women. However, these findings should be verified through extensive, prospective studies.

Abstract Background: Changing the dietary pattern may be an alternative treatment for bacterial vaginosis, the prevalent vaginal infection in women. Methods: One hundred and forty-three bacterial vaginosis-affected women diagnosed by Amsel criteria and 151 healthy controls aged 18 to 45 entered the current case-control research. To calculate the alternative healthy eating index and dietary acid load score, food consumption was recorded with an accurate and precise food frequency questionnaire. The dietary acid load was measured by potential renal acid load (PRAL) and net endogenous acid production (NEAP) indices. Using logistic regression models, the association between the alternative healthy eating index and dietary acid load score with bacterial vaginosis was investigated. Results: The last tertile of the alternative healthy eating index had a 75% decreased odds of experiencing bacterial vaginosis in the adjusted model (adjusted odds ratio (aOR) = 0.25, 95% confidence interval (CI) = 0.12–0.53, P for trend = 0.001). Abstract Bacterial vaginosis (BV) is a predominant vaginal disturbance that affects about 25% of childbearing-aged women. Dietary consumption may have a crucial role in vaginal flora imbalances. This study was a hospital-based case–control study. In total, 144 incident BV cases and 151 healthy participants were recruited from the gynecology clinic in Tehran, Iran, between November 2020 and June 2021. Participants' typical diets were collected by a food frequency questionnaire. Vaginal flora was characterized based on the Amsel criteria. Factor analysis was used to pinpoint the principal dietary patterns. For logistic regression, the first tertile was assumed as a reference. Five principal dietary patterns emerged and were nominated as "Healthy diet," "Unhealthy diet," "Ovo-vegetarian diet," "Pseudo-Mediterranean diet," and "Western diet." The "Unhealthy diet" pattern were positively associated with BV (adjusted odds ratio (aOR) = 3.35; 95% confidence interval (CI) 1.41, 7.94; ptrend: 0.006), while adherence to the "Ovo-vegetarian diet" pattern was associated with a reduced odds of BV (aOR = 0.16; 95% CI 0.07, 0.34; ptrend

Abstract Background Bacterial vaginosis, BV, is a common inflammatory vaginal dysbiosis. The Mediterranean diet, MD, containing foods rich in antioxidant compounds, is shown to be beneficial for inflammatory conditions. This study aimed to investigate the association between MD adherence and dietary total antioxidant capacity (DTAC) with BV. Methods This case–control study was conducted on 143 BV-affected and 151 healthy individuals aged between 15 and 45 years. The Amsel criteria were used to detect newly diagnosed patients with BV by a gynecologist. The 168-item food frequency questionnaire (FFQ) was used to record participants' dietary intakes in the last year. The reported data in the FFQ was used to measure adherence to the MD by calculating the Medi-Lite score and to measure the DTAC by calculating ferric-reducing antioxidant power, FRAP, based on the related databases. Logistic regression models were used to determine the association between Medi-Lite and DTAC and BV odds. Results The highest tertile of Medi-Lite score was associated with a reduced odds of BV in the crude model (Odds Ratio, OR: 0.49, 95% Confidence Interval, 95% CI 0.25, 0.96, P for trend: 0.023). This significant inverse association was not observed in the last model adjusted for age, body mass index (kg/m2), waist circumferences (cm), cigarette smoking, frequency of pregnancy, and physical activity (MET/h/d). Introduction: Endometriosis is a multifactorial disease. Limited studies have been published on the relationship between nutritional factors and endometriosis. This study was performed with aim to determine the association between dietary intake and anthropometric indices with endometriosis.Methods: This case-control study was performed in 2020-2021 on 317 women aged 18 to 49 years in Tehran. Hyperemesis gravidarum was diagnosed using the American College of Obstetrics and Gynecology criteria, and participants were divided into the case and control groups. A semi-quantitative valid and reliable food frequency questionnaire containing 168 food items was used to assess food intake of participants in the past year before pregnancy. Data analysis was performed by SPSS statistical software (version 22), using Chi-square, Mann-Whitney and logistic regression tests. A p-value lower than 0.05 was considered statistically significantResults: The case group had significantly higher total fat and saturated fat intakes compared with controls. Median intakes of vitamin B groups, vitamin A and vitamin C were significantly higher among controls than cases. After adjusting for confounding variables, participants in the upper tertile of intakes of vegetables (OR=0.39, CI: 0.19-0.43) and fruits (OR=0.51, CI: 0.31-0.83) had a lower possibility of hyperemesis gravidarum. However, participants in the upper tertile of intakes of processed meats (OR=3.93, CI: 1.04-6.11) and organ meats (OR=2.39, CI: 1.23-5.03) were significantly more likely to be suffering from hyperemesis gravidarum.Conclusion: There was a significant inverse association between intake of vegetables, fruits, fish and seafood, whole grains, legumes, nuts, olives and olive oil with hyperemesis gravidarum. However, intake of high-fat dairy products, processed meats, organ meats, sweets, solid fats had a significant direct association with hyperemesis gravidarum.

Introduction: Bacterial vaginosis is the most common vaginal infection in women of childbearing age. Identifying nutritional risk factors as a potential therapeutic target can be important. This study was performed with aim to determine the relationship between the consumption of ultra-processed foods and bacterial vaginosis. Methods: This case-control study was performed in 2020 on 144 women with bacterial vaginosis and 151 healthy women aged 15-45 years referred to the gynecology clinic of Imam Hossein Hospital in Tehran. Bacterial vaginosis was diagnosed using the Amsel criteria and participants were divided into case and control groups based on it. Food intake of participants during 1 last year was collected using a valid semi-quantitative food frequency questionnaire with 168 food items. Ultra-processed foods were classified according to the NOVA classification system. A logistic regression test was used to investigate the association between the consumption of ultra-processed foods with bacterial vaginosis, and then the odds ratio was calculated. P< 0.05 was considered significant. Results: After adjusting for confounding variables, it was seen that participants in the upper tertile of intakes of the ultra-processed foods (OR=2.13, CI: 1.19-3.8, P=0.004), processed meats and fast foods (OR=2.27, CI: 1.23-4.2, P=0.008), oils and sauces (OR=2.47, CI: 1.34-4.54, P=0.004) and sweets (OR=2.82, CI: 1.5-5.3 P=0.001) were significantly more likely to be suffering from bacterial vaginosis than the group in first tertile. Conclusion: There was a direct relationship between the intake of ultra-processed foods, processed meats and fast foods, oils and sauces and sweets with bacterial vaginosis.

Background: Abdominal obesity (AO) is a global health problem. Although few studies indicated that Nordic diet (ND) was associated with AO, no evidence exists for the association between ND and AO outside the Nordic countries. In this study, we aimed to determine the relationship of ND with AO and anthropometric indices among Iranian women. Methods: In this cross-sectional study, we evaluated anthropometric indices and usual dietary intakes of 294 Iranian women aged 20-50 years. We defined AO as waist circumference (WC) ≥ 88 cm and WC ≥ 90 (according to Iranian criteria). Association between qualitative variables and health eating index-nordic nutrition recommendation (HEI-NNR) score was determined using Chi-square and Mann–Whitney tests. The relationship of HEI-NNR score with AO was analyzed by a logistic regression model after adjusting the effect of confounders. Results: The mean of HEI-NNR score was significantly higher in overweight and obese participants than participants without overweight (P = 0.04). No linear trend was found for body mass index, WC, and waist to hip ratio (WHR) through the HEI-NNR score even after adjustment for confounders. A significant positive association was found among HEI-NNR score, overweight, and obesity (OR = 1.85, 95%CI = 1.15-2.96, P = 0.01). However, the association did not remain significant after adjusting for the confounders. Conclusion: Adherence to the ND was not associated with AO and anthropometric measurement among Iranian women. Future evidences from RCT and prospective cohort studies are needed to confirm these findings.

Background. Inconsistent findings have been reported for associations between dietary indices and bacterial vaginosis (BV). The aim of this study was to examine the association of dietary glycemic index (DGI), glycemic load (DGL), insulin index (DII), and insulin load (DIL) with BV among Iranian women. Methods. The current case-control study consisted of 144 new cases of BV and 151 controls. The diagnosis of BV was made based on the Amsel criterion in hospital clinics in Tehran, Iran, from November 2020 until June 2021. DGI, DGL, DII, and DIL were calculated from a validated semiquantitative food frequency questionnaire. The association between dietary carbohydrate indices and odds of BV were assessed adjusting for potential confounders through an estimation of two multivariate regression models. Results. The multivariate adjusted odds ratio (OR) comparing the highest tertile of dietary DGI and DGL with the lower tertile was 2.99 (95% confidence interval (CI): 1.47–6.81; Ptrend=0.003) and 4.01 (95% CI: 1.22–5.91; Ptrend=0.029), respectively. Abstract Background Studying entire dietary patterns is a promising alternative approach to overcome limitations of the single food or nutrient approach. We evaluated the relationship between the scores of 4 established Dietary Approaches to Stop Hypertension (DASH) diet indexes and breast cancer risk among Iranian women. Methods This case-control study was carried out on 408 eligible women (136 cases and 272 hospital-based controls). A validated 168 item semi-quantitative food frequency questionnaire was used for assessing usual dietary intakes. DASH index scores were generated based on predefined algorithms for each of the 4 previously described indexes (Dixon’s, Mellen’s, Fung’s and Günther’s DASH diet index). Unconditional logistic regression analysis was performed to estimate odds ratio (OR) and 95% confidence intervals (CIs) for score categories or quintiles of DASH diet indexes and breast cancer risk in multivariate adjusted models. Results Women in the highest categories of the Mellen’s and Günther’s scores had lower odds of breast cancer than those in the lowest quintiles (Mellen’s OR:0.50; 95% CI:0.62–0.97; P-trend:0.02; Günther’s OR:0.48; 95% CI:0.25–0.93; P-trend:0.05). However, no significant associations were found between Dixon’s and Fung’s DASH score and breast cancer risk. Modification by menopausal status revealed that breast cancer risk was only reduced in postmenopausal women with higher scores on Mellen’s index (OR:0.24; 95% CI:0.08–0.68; P-trend:0.04). Conclusion A greater adherence to 2 of the 4 DASH indexes (Mellen’s and Günther’s indexes) was associated with decreased risk of breast cancer.

Background: The pattern of dietary intake reflects the preferences and dietary habits of individuals and their socio-economic and demographic status such as ethnicity and place of residence. So far, various studies have examined the dietary patterns of different populations in Iran, but the information on eating patterns of rural areas is limited in comparison with urban regions. The aim of this study was to identify major dietary patterns and to compare the scores of these patterns in women living in urban and rural areas of Dashtestan County, (Bushehr province), 2017. Materials and Methods: In this cross-sectional study, 302 women (168 urban and 134 rural) aged 20_59 years old living in Dashtestan county were sampled with multistage sampling method (stratified cluster sampling). Dietary intake was assessed by filling the standard semi-quantitative food frequency questionnaire in face-to-face interviews. Factor analysis method was used for identifying major dietary patterns. Introduction: Polycystic ovary syndrome (PCOS) is a common endocrinopathy among women and insulin resistance is one of the clinical features in this syndrome which plays an important role in improvement of lipid disorders. Dietary glycemic index and glycemic load can affect blood lipid disorders. Because of limited studies in this field, this study was performed with aim to evaluate the relationship between dietary glycemic index/glycemic load and serum lipids level in women with PCOS. Methods: This case-control study was conducted on 45 PCOS women and 45 healthy controls in Moheb Yas Women hospital of Tehran in 2016. Dietary intake was assessed using three 24-h recall questionnaires, and glycemic index and glycemic load were calculated by standard methods. Lipid profile was measured and insulin resistance was calculated by the formula. Anthropometric measurements were conducted in all participants. Data were analyzed by SPSS software (version 16), and Independent T-test and correlation test. P< 0.05 was considered statistically significant. Results: Mean of serum total cholesterol, insulin resistance and mean of glycemic index was significantly higher in the patients group than the control group (P0.05). Conclusion: Lifestyle modification like correct nutritional status and management of carbohydrate intake and reducing the dietary glycemic load may be effective in prevention or reducing the complications of disease such as lipid abnormalities.

Background and Aims: Although irritable bowel syndrome is one of the most common gastrointestinal disorders presented to gastroenterologists, therapeutic strategies are not yet well-established. Accordingly, we conducted a randomized, double-blind, placebo-controlled, clinical trial to evaluate the possible superiority of adding glutamine supplement to low fermentable oligo- di- monosaccharides and polyols (FODMAP) diet in patients with irritable bowel syndrome (IBS).Methods: Eligible adults were randomized to receive a low FODMAP diet either with glutamine (15 g/day) or a placebo for 6 weeks. The primary endpoint was a significant reduction in IBS-symptom severity score (IBS-SSS). Secondary endpoints were changes in IBS symptoms, stool frequency, consistency, and quality of life.Results: The study group enrolled 50 patients, among which 22 participants from each group completed the study protocol. The glutamine group had significant changes in total IBS-severity score, dissatisfaction of bowel habit and interference with community function (58% reduction; P < 0.001, 57% reduction; P < 0.001, 51% reduction; P = 0.043, respectively). Improvement in IBS-severity score of more than 45% was observed in 22 of 25 participants (88%) in the glutamine group, while it was only 15 of 25 participants (60%) in the control group (p = 0.015). No serious adverse events were observed.Conclusions: Our findings indicated the superiority of adding glutamine supplementation to a low FODMAP diet in amelioration of IBS symptoms while confirming the beneficial effects of a low FODMAP diet in IBS management.

AIM: To assess the relation between nutrient patterns and cataract risk. METHODS: This is a hospital-based case-control study with 97 cataract patients and 198 matched controls. Dietary consumption was collected through a valid food frequency questionnaire (FFQ). Nutrient patterns were detected by applying factor analysis. Unconditional logistic regression models were used to estimate odds ratio (ORs) and 95%CIs. RESULTS: We extracted 5 main nutrient patterns. Factor 1 included niacin, thiamin, carbohydrates, protein, zinc, vitamin B6 and sodium (sodium pattern). Factor 2 was characterized by oleic acid, monounsaturated fats, polyunsaturated fats, linoleic acid, trans fatty acid, linolenic acid, vitamin E and saturated fats (fatty acid pattern). The third factor represented high intake of vitamin B12, vitamin D, cholesterol and calcium (mixed pattern). The 4th pattern was high in intake of beta and alpha carotene, vitamin A and vitamin C (antioxidant pattern). Finally, the 5th pattern loaded heavily on docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA) (omega-3 pattern). In crude and multivariate analysis, the sodium pattern was associated with increased risk of cataract (OR=1.97, 95%CI: 1.09-3.96). The fatty acid pattern elevated the risk of cataract (OR=1.94, 95%CI: 1.1-3.86). Antioxidant pattern was associated with a significant 79% reduced risk (2nd category compared with the 1st). Omega-3 pattern was significantly negatively associated with risk of cataract (P=0.04). CONCLUSION: These findings imply that nutrient patterns reflecting a combined consumption of nutrients might be important in the etiology of cataract. Additional studies with more efficient designs are warranted to confirm our findings.

Background: The relation between dietary inflammation and risk of depression has not been widely explored. We examined the association between the inflammatory effect of the diet and the odds of depression among Iranian female adolescents.Methods: Using a stratified cluster sampling technique, 300 female adolescents aged 15–18 years were recruited from schools in Tehran between years 2014–2015. Depression was assessed using the Depression, Anxiety and Stress Scale (DASS)- a 21-point scale. The dietary inflammatory index (DII®) was used to evaluate the inflammatory potential of the diet. Dietary intake was assessed using a validated food frequency questionnaire. In addition to descriptive statistics, multivariable linear and logistic regression were used to calculate confounder-adjusted beta estimates and odds ratios.Results: In total, 88 females (30%) had at least a moderate level of depressive symptoms (DASS > 6). Females with the most pro-inflammatory diet had higher DASS depression score (β = 1.67; 95% CI = 0.03, 3.31) and were at 3.96 (95% CI = 1.12, 13.97) times higher odds of having at least moderate depressive symptoms, compared to females with the least anti-inflammatory diets.Conclusion: These data suggest that Iranian adolescent females eating a pro-inflammatory diet, as indicated by higher DII scores, had greater odds of having at least moderate depressive symptoms.

Background and Objectives: Multiple Sclerosis (MS) is the most prevalent autoimmune disease of the central nervous system, and it has been suggested that nutrition might play a role in the etiology of MS. This study was aimed to evaluate the relationship between MS risk and intake of some macro- and micro-nutrients in Tehran (Iran). Materials and Methods: In this hospital based, case-control study, a total of 60 newly diagnosed patients with MS and 140 controls underwent face-to-face interviews. Information regarding the usual dietary intake of each individual in the past year was collected by using a valid and reliable 168-item semi-quantitative food frequency questionnaire. Multivariate logistic regression was used to estimate the odds ratios and 95% confidence intervals. Results: Inverse significant associations were observed (P< 0.05) between the intake of protein (OR=0.19; 95% CI: 0.04-0.76) and micro-nutrients such as vitamin B1 (OR=0.10; 95% CI: 0.02-0.53) , vitamin B2 (OR=0.15; 95% CI: 0.04-0.50), cobalamin (OR=0.13; 95% CI: 0.04-0.38), vitamin C (OR=0.20; 95% CI: 0.07-0.58), vitamin A (OR=0.23; 95% CI: 0.09-0.59), vitamin D (OR=0.28; 95% CI: 0.11-0.72), vitamin E (OR=0.15; 95% CI: 0.05-0.41), β-carotene (OR=0.38; 95% CI: 0.15-0.97), zinc (OR=0.05; 95% CI: 0.01-0.27), magnesium (OR=0.12; 95% CI: 0.03-0.47) and calcium (OR=0.23; 95% CI: 0.08-0.67) and the risk of MS. Conclusions: The results suggest that intake of some macro- and micro-nutrients might be associated with reduced risk of MS. Background. Maternal dietary patterns play an important role in the progress of gestational diabetes mellitus (GDM). The aim of the present study was to explore this association. Method. A total of 388 pregnant women (122 case and 266 control) were included. Dietary intake were collected using a food frequency questionnaire (FFQ). GDM was diagnosed using a 100-gram, 3-hour oral glucose tolerance test. Dietary pattern was identified by factor analysis. To investigate the relation between each of the independent variables with gestational diabetes, the odds ratio (OR) was calculated. Results. Western dietary pattern was high in sweets, jams, mayonnaise, soft drinks, salty snacks, solid fat, high-fat dairy products, potatoes, organ meat, eggs, red meat, processed foods, tea, and coffee. The prudent dietary pattern was characterized by higher intake of liquid oils, legumes, nuts and seeds, fruits and dried fruits, fish and poultry whole, and refined grains. Western dietary pattern was associated with increased risk of gestational diabetes mellitus before and after adjustment for confounders (OR = 1.97, 95% CI: 1.27–3.04, OR = 1.68, 95% CI: 1.04–2.27). However, no significant association was found for a prudent pattern. Conclusion. These findings suggest that the Western dietary pattern was associated with an increased risk of GDM.

Background and Objectives: This study was designed to investigate the association between parental behavior and the dietary intake of Tehranian preschool children aged 2-6 years. Materials and Methods: In a cross-sectional study conducted on 310 children aged 2-6 years from the kindergartens of 22 districts of Tehran, a qualitative validated 85-item food frequency questionnaire was completed by interviewing with their parents. Also the effect of parental behavior on the children’s dietary intake was assessed using a validated comprehensive feeding practices questionnaire (CFPQ). Results: 47 and 53% of children were girls and boys, respectively. Among the parental behavior patterns, encouraging children to eat healthy foods, using food rewards, teaching and talking with children about the value of foods, encouraging the children to participate in food purchasing and preparation, making available the healthy foods, parental controlling behavior, child’s control, restricted behaviors because of having weight loss diet, and role modeling were significantly associated with the dietary intake of food groups (P

Background: The epidemic peak of obesity and overweight began in 1980. Obesity during childhood and adolescence, particularly in the second decade of life, is a strong predictor of obesity in adulthood. The significant risk factors for obesity and overweight in childhood include breastfeeding, obese parents, socioeconomic status, birth weight, physical activities and diet. Breastfeeding is one of the most important strategies for weight loss and reduction in childhood obesity. Objectives: The purpose of this study is to investigate the relationship between overweight, obesity, breastfeeding patterns and birth weight among first grade elementary school students in Shiraz, Iran. Patients and Methods: This case-control study included 200 obese (case group) and 200 normal weight (control group) children. A sample size of 400 children was determined. Study participants were selected from among elementary schools in Shiraz by multistage sampling. First, schools were chosen according to a stratified random sampling method. Then, we chose all overweight or obese first grade students. For each case, we chose a control student matched in terms of age, sex, grade and elementary school. The General Information Questionnaire, Socioeconomic Questionnaire, Questionnaire of Breastfeeding during Infancy, Physical Activity Questionnaire and 24-Hour Dietary Recall Questionnaire were completed. Mothers’ height and weight were also measured. Data analysis was carried out by SPSSversion 15. Conclusion : Obesity and overweight in first grade elementary school students is related to breastfeeding pattern; however, this may not be an independent relationship. Results: There was a significant difference in breastfeeding duration between the case and control groups. No relationship was found among birth weight, overweight and obesity in the case group. Mother’s body mass index (BMI) and obesity in immediate family members showed a significant difference between case and control groups. There was no significant difference in the socioeconomic status of the two groups. Conclusion: Obesity and overweight in first grade elementary school students is related to breastfeeding pattern; however, this may not be an independant relationship.

Background: Chronic inflammation is implicated in the development of colorectal cancer (CRC) and its precursor; colorectal adenomatous polyps (CAP). Some dietary factors are important triggers for systemic inflammation. Therefore, the present study aimed to investigate the association between the dietary inflammatory index (DII®) and the risk of CRC and CAP in an Iranian case-control study. Methods: 134 newly diagnosed CRC patients, 130 newly diagnosed CAP patients, and 240 hospitalized controls were recruited using convenience sampling. Energy-adjusted DII (E-DII) scores were computed based on dietary intake assessed using a reproducible and valid 148-item food frequency questionnaire. Logistic regression models were used to estimate odds ratios (ORs) and 95% confidence intervals (CI) after adjusting for confounders. Results: The E-DII score ranged between −4.23 (the most anti-inflammatory score) to +3.89 (the most pro-inflammatory score). The multivariable-adjusted ORs for participants in the 3rd tertile compared to the 1st tertile was 5.08 (95%CI: 2.70−9.56; P-trend < 0.0001) for CRC and 2.33 (95% CI: 1.30−4.02; P-trend = 0.005) for CAP. Conclusions: Our findings suggest that more pro-inflammatory diets, indicated by higher E-DII scores, might increase the risk of both CRC and CAP. Future steps should include testing these associations in a prospective setting in Iran.

BACKGROUND: The dietary approaches to stop hypertension (DASH) dietary pattern reduces blood pressure. However, there is little information about the relationship between DASH and coronary heart diseases. This study aimed to assess the relationship between a DASH-style diet adherence score and coronary heart diseases (CHD) in patients referring for coronary angiography. METHODS: In this study, 201 adults (102 males, 99 females) within the age range of 40-80 years who referred for coronary angiography were selected. Diet was evaluated using a validated food frequency questionnaire. DASH score was calculated based on 8 food components (fruits, vegetables, whole grains, nuts and legumes, low fat dairy, red/processed meats, soft drinks/sweets, and sodium). The relationship between DASH score and CHD was assessed using logistic regression analysis. RESULTS: Mean of DASH score was 23.99 ± 4.41. Individuals in the highest quartile of DASH score were less likely to have CHD [odds ratio (OR) = 0.38, 95% confidence interval (CI): 0.16-0.86]. However, after adjustment for gender or smoking, there was little evidence that coronary heart disease was associated with DASH diet score. There was a significant negative correlation between DASH score and diastolic blood pressure (P ≤ 0.05). CONCLUSION: In conclusion, having a diet similar to DASH plan was not independently related to CHD in this study. This might indicate that having a healthy dietary pattern, such as DASH pattern, is highly related to gender (dietary pattern is healthier in women than men) or smoking habit (non-smokers have healthier dietary pattern compared to smokers). Objective: Major depression is a mood disorder that causes changes in physical activity, appetite, sleep and weight. Regarding the role of zinc in the pathology of depression, the present study was aimed to investigate the effects of zinc supplementation in the treatment of this disease.Methods:This study was a double-blind randomized clinical trial. Forty four patients with major depression were randomly assigned to groups receiving zinc supplementation and placebo. Patients in Zinc group received daily supplementation with 25 mg zinc adjunct to antidepressant; Selective Serotonin Reuptake Inhibitors (SSRIs), while the patients in placebo group received placebo with antidepressants (SSRIs) for twelve weeks. Severity of depression was measured using the Beck Depression Inventory at baseline and was repeated at the sixth and twelfth weeks. Background and Aim: Nearly 50% of the elderly have insomnia, with difficulty in going to sleep, oversleeping, or feeling unrefreshed on waking up. With aging, several changes can occur that place one at risk of insomnia. The objective of the present study was to determine the efficacy of magnesium supplementation -the natural NMDA antagonist and GABA agonist- to improve insomnia in the elderly. Materials and Methods: A double blind randomized clinical trial was conducted on 46 elderly subjects, randomly allocated into the magnesium and the placebo group and received 500 mg magnesium and placebos daily for 8 weeks, respectively. Questionnaires of insomnia severity index, physical activity, and sleep-log were completed at baseline and after the intervention period. Anthropometric and dietary confounding factors were obtained using 24h recall for 3 days. Blood samples were taken at baseline and after the intervention period for analysis of serum magnesium, renin, melatonin, and cortisol. Background & Aim: Diet rich in fruits and vegetables is shown to be an important factor in prevention of chronic diseases. Adolescence is a critical period in adopting eating behaviors. Schools are important social environments in adolescentschr('39') lives and are important settings for interventions that aim to improve eating behaviors, including fruit and vegetable intake. This study aimed to assess effectiveness of a school-based intervention based on social cognitive theory on fruit and vegetable (F&V) intake of female adolescents in Tehran. Methods & Materials: This experimental school based field trial was conducted on 296 female students aged 10-13 years old (157 in the intervention and 139 in the control groups). The intervention included classroom nutrition education on F&V and increasing availability of F&V through school buffet. Psychosocial characteristics of the students were assessed using a questionnaire and F&V intake were measured using 24-hr recall method, at baseline and after three months intervention. Descriptive and inferential statistics were used for data analysis. Results: After the intervention, vegetable and fruit intake were 0.6 and 0.63 servings higher in the intervention group compared to the controls. Knowledge and self-efficacy scores were significantly higher in the intervention group compared to the control group. Home availability and observational learning scores were significantly decreased in the intervention group. Conclusion: Through the intervention process, many valuable experiences were gained to be used in future interventions.

Materials and Methods: A double-blind randomized clinical trial was conducted in 46 elderly subjects, randomly allocated into the magnesium or the placebo group and received 500 mg magnesium or placebo daily for 8 weeks. Questionnaires of insomnia severity index (ISI), physical activity, and sleep log were completed at baseline and after the intervention period. Anthropometric confounding factors, daily intake of magnesium, calcium, potassium, caffeine, calories form carbohydrates, and total calorie intake, were obtained using 24-h recall for 3 days. Blood samples were taken at baseline and after the intervention period for analysis of serum magnesium, renin, melatonin, and cortisol. Statistical analyses were performed using SPSS 19 and P values < 0.05 were considered as statistically significant. Results: No significant differences were observed in assessed variables between the two groups at the baseline. As compared to the placebo group, in the experimental group, dietary magnesium supplementation brought about statistically significant increases in sleep time ( P = 0.002), sleep efficiency ( P = 0.03), concentration of serum renin ( P < 0.001), and melatonin ( P = 0.007), and also resulted in significant decrease of ISI score ( P = 0.006), sleep onset latency ( P = 0.02) and serum cortisol concentration ( P = 0.008). Supplementation also resulted in marginally between-group significant reduction in early morning awakening ( P = 0.08) and serum magnesium concentration ( P = 0.06). Although total sleep time ( P = 0.37) did not show any significant between-group differences. Conclusion: Supplementation of magnesium appears to improve subjective measures of insomnia such as ISI score, sleep efficiency, sleep time and sleep onset latency, early morning awakening, and likewise, insomnia objective measures such as concentration of serum renin, melatonin, and serum cortisol, in elderly people.

Background & Aim: Diet rich in fruits and vegetables is shown to be an important factor in prevention of chronic diseases. Adolescence is a critical period in adopting eating behaviors. Schools are important social environments in adolescents' lives; and are important settings for interventions that aim to improve eating behaviors, including fruit and vegetable intake. This study aimed to assess effectiveness of a school-based intervention based on social cognitive theory on fruit and vegetable (F&V) intake of female adolescents in Tehran.Methods & Materials: This experimental school based field trial was conducted on 296 female students aged 10-13 years old (157 in the intervention and 139 in the control groups). The intervention included classroom nutrition education on F&V and increasing availability of F&V through school buffet. Psychosocial characteristics of the students were assessed using a questionnaire and F&V intake were measured using 24-hr recall method, at baseline and after three months intervention. Descriptive and inferential statistics were used for data analysis.Results: After the intervention, vegetable and fruit intake were 0.6 and 0.63 servings higher in the intervention group compared to the controls. Knowledge and self-efficacy scores were significantly higher in the intervention group compared to the control group. Home availability and observational learning scores were significantly decreased in the intervention group.Conclusion: Through the intervention process, many valuable experiences were gained to be used in future interventions.

BACKGROUND: Using dietary pattern analysis method could provide more information about nutritional etiology of chronic disease such as obesity. The aim of this study is to determine the association between major dietary patterns and general and central obesity among adult women living in Tehran. METHODS: A cross-sectional study was conducted in Tehran, Iran, with 460 women aged 20-50 years. Dietary intake in last year was collected by a semi-quantitative food frequency questionnaire. Weight, height and waist circumstance (WC) were measured with standard methods and body mass index (BMI) was calculated. General obesity was defined as BMI ≥ 30 kg/m2 and central obesity as WC ≥ 88 cm. Factor analysis was used for identifying major dietary patterns. The association between major dietary patterns and general and central obesity were assessed by logistic regression analysis. RESULTS: Two major dietary patterns were extracted: "Healthy" and "Unhealthy" dietary pattern. After adjusting for confounders, individuals in the highest quartile of the unhealthy dietary pattern score were more likely to be generally (OR = 7.33, 95% CI: 2.39-22.51) and centrally obese (OR = 4.99, 95% CI: 2.08-11.94); whereas, those in the upper quartile of healthy dietary pattern were less likely to be generally (OR = 0.38, 95% CI: 0.15-0.98) and centrally obese (OR = 0.33, 95% CI: 0.16-0.71). CONCLUSION: Major dietary patterns of Tehrani women had a significant association with general and central obesity. Further prospective researches are required to confirm such associations.
