1. Assessment of Working Conditions and Job Satisfaction of Neurology Specialists

Abstract

Objective: This study aimed to examine the demographic characteristics and working conditions of neurology specialists working in Türkiye and to determine the factors that play a role in neurologists' resignations. Materials and Methods: This study was designed as a multiple-choice survey. The study included 472 neurology specialists who were actively practicing in clinics. The data collected through the questionnaire were analyzed. Results: Most (78.2%) of neurologist work in public hospitals, while 15.7% work in private hospitals, 3.6% work in foundation hospitals, and 2.5% work in private practices. 70.1% of the participants examine 41 or more patients per day. 37.7% of neurologists take on-call duty for ten or more days per month. 93.8% think their payment is not at a deserved level. 70% of the participants (n = 328) reported experiencing verbal violence. The rate of those who reported being exposed to mobbing is 62.5% (n = 295). 56% of the participants (n = 264) frequently feel burned out or depressed. Only 10% of them are satisfied with their working conditions. We used Logistic regression analysis to investigate factors for resigning. Accordingly, exposure to verbal or physical violence (P = 0.033), working in a particular institution (P = 0.038), and the number of emergency shifts affect the likelihood of resignation (P = 0.046). The probability of resigning for lecturers is significantly lower (OR: 0.05). The likelihood of resigning for those who report earning less than what they deserve is about 16 times higher than those who feel they are adequately paid. Conclusion: The workload of neurology specialists is high. Neurologists have a high rate of exposure to violence and low job satisfaction. The workload of physicians varies significantly across different units and regions and is unequally distributed. To increase job satisfaction among healthcare workers and prevent physician migration, there is a need to improve working conditions, distribute workload and resources more equally among units.

2. Polypharmacy and Potential Drug Interactions Causing Neurological Symptoms in Geriatric Patients with Epilepsy

Abstract

Objective:Polypharmacy brings with it the risk of potential drug interaction. This study aimed to investigate polypharmacy, potential drug interactions, the drug interactions causing neurological symptoms in geriatric patients with epilepsy, and diffenrences between drug interaction databases.Methods:The study included patients with epilepsy aged 65 and over (the demographic information, antiepileptic drug use, other chronic diseases, and medications of the patients were retrospectively recorded from their files). The use of 5 or more drugs was accepted as polypharmacy. Potential drug interactions were checked from 2 open access databases (database-1; database-2), and interacting drugs, interaction types, clinical results, and differences between databases were determined.Results:This study included 126 patients (56 females/70 males), the mean age was 73.13 ± 7.42 (65-92), and the mean duration of antiepileptic drug (69.8% monotherapy) use was 9.08 ± 13.68 (0.5-58) years. The most commonly used antiepileptic drug was levetiracetam (69.8%). Totally 88 patients had at least 1 central nervous system disease (except epilepsy), and 116 had a chronic diseases other than central nervous system disease. Polypharmacy frequency was 75.4%. The most commonly used drug groups with the highest potential drug interaction risk were antihypertensives (69%), antia ggreg ant-a ntico agula nts (67%), statins (44%), proton pump inhibitors (41%), and antidepressants (39%). The major, intermediate, and minor potential drug interactions for database-1 and database-2 were 44 versus 53; 586 versus 428, and 127 versus 70, respectively. The most important potential drug interactions were the increased risk of hemorrhage and thrombotic events, arrhythmia, and blood pressure changes.Conclusion:It is important to know about potential drug interactions (especially involving cardiac drugs, antidepressants, statins, and proton pump inhibitors) in the evaluation of the entire neurological picture of geriatric epilepsy patients.

Objective:It has been shown that pain, depression, and fatigue related to Obstructive Sleep Apnea syndrome (OSAS) results in a significant socioeconomic burden. We aimed to determine the prevalence, demographic, and clinical properties of pain symptoms in OSAS patients.Materials and Methods:We evaluated 84 OSAS patients diagnosed after full-night polysomnography with the Brief Pain Inventory, Beck Depression Rating scale (BDS), Pittsburg Sleep Quality index (PSQI), Epworth Sleepiness scale (ESS), Fatigue Impact scale (FIS), and Fatigue Severity scale (FSS).Results:A total of 84 patients (62 males, 22 females) with a mean age of 54.37±11.03 years were included. Headache in 25 (43.1%), myofascial pain in 22 (37.9%), and other types of pain in 11 patients were defined. ESS, FSS, FIS total, physical, cognitive, and psychosocial scores were found to be significantly higher in patients with pain. The presence of pain did not differ according to the severity of OSAS, and no significant difference was found in any parameters between mild, moderate, and severe OSAS except for minimal O2 saturation. A positive correlation between last week pain severity and BDS scores, a negative correlation between PSQI scores and the decrease in pain rate as a result of the treatment, and a weak positive correlation between FIS-cognitive sub-score and pain intensity scores were found.Conclusion:In our study, excessive daytime sleepiness and the impact and severity of fatigue were found to be higher in patients with pain than in those without pain. The most obvious difference was seen in terms of cognitive, and psychosocial fatigue. Contrary to expectations, it was observed that OSAS severity did not affect pain according to our data.

Amac: Cinsel islev bozuklugu (CIB), multipl sklerozda (MS) sik gorulen multifaktoryel bir durumdur. Bu calisma, MS’de CIB sikligini degerlendirmek ve ilgili faktorleri arastirmak amaciyla duzenlenmistir. Yontemler: Kesitsel ozellikteki bu calismaya, 2018-2019 yillarinda,18 ile 60 yaslari arasinda Mc Donald kriterlerine gore kesin relapsing remitting multipl skleroz (RRMS) tanisi alan 96 hasta gonullu olarak calismaya dahil edildi. Multiple sclerosis intimacy and sexuality questionnaire-19 (MSISQ-19), Arizona Sexual Experience Scale (ASEX), Multiple Sclerosis Inventory of Cognition (MUSIC) ve Beck Depresyon Olcegi (BDI) anketleri uygulandi. Bulgular: Hastalarin yas ortalamasi 38,46 (8,47) (min-maks: 20 - 60 yil) yildi; ortalama hastalik suresi 77,02 (70,62) (3 - 324 ay) aydi. Ortalama Genisletilmis Ozurluluk Durumu Olcegi (EDSS) puani 2,1 (1,33) (0-5) ve standart sapma %80,5 idi. Sosyodemografik özellikleri, hastalık başlangıç yaşları, hastalık süreleri, özürlülük düzeyleri, Beck Depresyon Ölçeği, Yorgunluk Şiddet Ölçeği ve Kısa Form Yaşam Kalitesi Ölçeği skorları kaydedildi. Bulgular: 65’i kadın, 35’i erkek 100 hastanın yaş ortalaması 35,21 ± 8,86 (min-max:18-63) yıldı. Abstract

Cerebral amyloid angiopathy (CAA) is a common but often asymptomatic disease, characterized by the deposition of amyloid proteins within brain parenchyma and leptomeningeal-cortical vessels. It can occur as a sporadic disorder or accompany Alzheimer disease (AD). The usual presentation of CAA is spontaneous lobar hemorrhage, Unlike CAA, cerebral amyloid angiopathy-related inflammation (CAA-ri), a subtype of CAA, can show various presentations and responds to steroid (or immuno suppressive) treatment. We report a patient with mixed dementia who showed subacute progression in cognitive impairment and had a mass lesion on brain magnetic resonance imaging (MRI). The lesion was identified as CAA related inflammation and the cognitive status of the patient improved significantly after steroid treatment. In this case report, we aimed to emphasize that CAA-ri is one of the possible diagnoses that should be considered in demented patients with rapid cognitive deterioration and showing brain lesions resembling neoplasms on the MRI. Therefore, steroids or other immunosuppresive treatments, which may lead to a dramatic clinical improvement, could be administered without delay.

Abstract

The effect of angiotensin II antagonist, losartan, on chronic stress-induced elevation of blood glucose levels was investigated in rats. Chronic immobilization stress caused an increase in blood glucose levels in rats. Administration of losartan (3 mg/kg, po) before stress exposure significantly prevented this increment. We suggest that losartan showed this effect by decreasing the excessive sympathetic response to stress. In conclusion, there is a relationship between stress, sympathetic nervous system, and renin-angiotensin system.
