Objective. One third of epilepsy patients cannot be helped using conventional medication. Selective hippocampal suppression using Designer Receptors Exclusively Activated by Designer Drugs (DREADDs) have potential to address this unmet need. We evaluated whether DREADD-mediated seizure suppression could be obtained in the intraperitoneal kainic acid (IPKA) rat model for temporal lobe epilepsy. Methods. The inhibitory DREADD hM4Di was selectively expressed in excitatory neurons in one hippocampus of IPKA rats. Naïve IPKA rats were included as control group. The effect of clozapine-mediated DREADD activation on dentate gyrus evoked potentials (DGEPs) was evaluated. Next, the effect of DREADD activation on spontaneous seizures was examined using continuous video-electroencephalography. Animals were systemically treated with single (0.1 mg/kg/24h) and repeated (0.1 mg/kg/6h) clozapine injections. Finally, long-term continuous release of clozapine and olanzapine (both 2.8 mg/kg/7d) using implantable osmotic minipumps was evaluated. Results. In the DREADD group, inhibition of DGEPs was observed after clozapine treatment, whereas no effect was observed in control animals. Only in DREADD-expressing animals, a single dose of clozapine reduced seizure frequency during the first six hours post injection. When clozapine was administered every six hours, seizures were suppressed the entire day. Long term treatment resulted in a significant seizure-suppressing effect during the first four days, after which tolerance developed. Significance. This study shows that unilateral inhibition of hippocampus using chemogenetics results in potent seizure suppressing effects in the IPKA rat model, even months after vector injection. It indicates that chemogenetic neuromodulation could contribute to a more optimal treatment for temporal lobe epilepsy.

RATIONALE: Gene therapy holds promise for a curative mutation-independent treatment applicable to all cystic fibrosis (CF) patients. The various viral vector-based clinical trials conducted in the past have demonstrated safety and tolerance of different vectors, but none have led to a clear and persistent clinical benefit. Recent clinical breakthroughs in adeno-associated virus-(rAAV) based gene therapy encouraged us to re-explore a rAAV approach for CF. OBJECTIVES: We evaluated the preclinical potential of rAAV gene therapy for CF to restore chloride and fluid secretion in two complementary models: intestinal organoids derived from CF subjects and a CF mouse model, an important milestone towards the development of a clinical rAAV candidate for CF gene therapy. METHODS: We engineered a rAAV vector containing a truncated CFTR (CFTRΔR) combined with a short promoter (CMV173) to ensure optimal gene expression. A rescue in chloride and fluid secretion after rAAV-CFTRΔR treatment was assessed by forskolin-induced swelling in CFTR-deficient organoids and by nasal potential differences in ΔF508 mice. MEASUREMENTS AND MAIN RESULTS: rAAV-CFTRΔR transduction of human CFTR-deficient organoids resulted in forskolin-induced swelling indicating a restoration of CFTR function. Nasal potential differences demonstrated a clear response to low chloride and forskolin perfusion in the majority of rAAV-CFTRΔR treated CF mice. CONCLUSIONS: Our study provides robust evidence that rAAV-mediated gene transfer of a truncated CFTR functionally rescues the CF phenotype across the nasal mucosa of CF mice and in patient-derived organoids. These results underscore the clinical potential of rAAV-CFTRΔR in offering a cure for all CF patients in the future.

Dopamine dysregulation syndrome shares some core behavioral features with psychostimulant addiction, suggesting that dopamine replacement therapy can acquire psychostimulantlike properties in some patients with Parkinson disease (PD). We here report strong experimental evidence supporting this hypothesis in an α-synuclein rat model of PD. Although levodopa had no effect in controls, it acquired 2 prominent psychostimulantlike properties in Parkinsonian rats: (1) it produced intense reward on its own and in parallel (2) decreased interest in other nondrug reward. These 2 effects may combine to explain the addictive use of levodopa after loss of midbrain dopamine neurons in some PD patients.

Two hallmarks of Parkinson’s disease (PD) are dopaminergic cell loss and the presence of cytoplasmic inclusions (Lewy bodies). Different point mutations in alpha-synuclein, the main constituent of Lewy bodies, have been identified in familial PD. Alpha-synuclein also constitutes one of the main components of Lewy bodies in sporadic cases of PD. Moreover, oxidant stress and generation of free radicals from both mitochondrial impairment and dopamine metabolism are considered to play critical roles in PD etiopathogenesis. Melatonin, a known potent antioxidant secreted by the pineal gland, may protect against the effect of several Parkinsonogenic compounds that are associated with progressive impairment of mitochondrial function and increased oxidative damage. However, the neuroprotective effect of melatonin has never been tested in the newly available genetic models of PD based on the viral expression of mutated alpha-synuclein. Lentiviral vectors encoding A30P mutant human alphasynuclein (lenti-A30P) were stereotactically injected into the right substantia nigra of adult male Sprague-Dawley rats and neuroprotection was examined by administration of melatonin or vehicle from two days before nigral administration of lenti-A30P until eight weeks after injection. It was found that lenti-A30P induced a significant TH+ cell-loss both in the medial and lateral substantia nigra versus the contrallateral side injected with lenti-eGFP. However, melatonin administration showed a total neuroprotective effect in both regions of the substantia nigra. In conclusion, the data here show that melatonin is neuroprotective against mutant alpha-synuclein-induced injury in the substantia nigra.

The human immunodeficiency virus type 1 (HIV-1)-specific reverse transcriptase (RT) inhibitors [tetrahydroimidazo[4,5,1-jk] [1,4]benzodiazepin-2(1H)-one and -thione (TIBO), 1-[(2-hydroxyethoxy)methyl]-6-(phenylthio)thymine, nevirapine, pyridinone, bis(heteroaryl)piperazine, etc.] are potent inhibitors of HIV-1 replication in cell culture. The rapid emergence of drug-resistant escape mutants in vitro (cell culture) and in vivo (patients) is predominantly linked to the Y181C mutation. Because amino acids Y181 and Y188 appear to be located within the drug binding site of the enzyme, we studied the impact of mutations of both amino acids on the enzyme kinetics and on the susceptibility of the enzyme to different HIV-1-specific RT inhibitors. Mutations Y181C, Y181I, and Y188L led to reduced sensitivity, albeit of varying extents, to all HIV-1-specific RT inhibitors. No resistance was observed to 2',3'-dideoxyguanosine 5'-triphosphate or phosphonoformic acid. The kcat of the Y181C mutant was similar to that of the wild-type RT (18 sec-1 x 10(-3)). The catalytic constant of the Y181I mutant was 6-fold higher and that of the Y188L mutant was 6-fold lower. Whereas TIBO displayed a linear mixed-type (noncompetitive) inhibition with respect to the deoxynucleotide substrate when wild-type p66/p51 was used, the pattern of inhibition became competitive or uncompetitive with Y181C or Y181I, respectively. Thus, the TIBO binding site of HIV-1 RT seems to be functionally and/or spatially related to the natural deoxynucleoside triphosphate binding site.
