Abstract

Objectives: To assess the effectiveness of bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF) among people poorly represented in clinical trials and potentially at higher risk of suboptimal response to ART. Methods: Observational cohort study on persons with HIV (PWH) enrolled in ICONA who started BIC/FTC/TAF as initial therapy or as switching regimen while virologically suppressed. Primary endpoint was time to treatment failure (TF): new AIDS/death or virological failure (VF) or discontinuation for toxicity/failure. Secondary endpoints were time to treatment discontinuation for toxicity (TDT) and to VF. Groups of interest were those aged >50 years, female sex, and advanced HIV disease at first ART start. Probability of the events overall and according to groups and adjusted HR for every endpoint were calculated by Kaplan-Meier curves and Cox regression models. Results: Nine hundred and thirty-three ART-naive and 1655 ART-experienced PWH initiated BIC/FTC/TAF. Over a median follow-up of 69.8 weeks, 89 (9.6%) PWH at their first regimen experienced TF. PWH aged >50 years had 1.83-fold (95% CI: 1.19-2.83) higher risk of TF; PWH with advanced HIV disease had 2.21-fold (95% CI: 1.53-3.82) higher risk; there were no differences in TF according to sex.Over a median follow-up of 146.3 weeks, 109 (6.6%) out of 1655 switching PWH experienced TF; no differences were found in the risk of TF, TDT and VF according to groups of interest. Conclusions: Overall, BIC/FTC/TAF is well tolerated and virologically effective in the real-world scenario for ART-naive and -experienced PWH. Older ART-naive PWH and those with advanced HIV disease may respond less well as the burden of diseases might compromise treatment efficacy.

Abstract

Background and aims: Severe liver disease markers assessed before HCV eradication are acknowledged to usually improve after the SVR. We prospectively evaluated, in the PITER cohort, the long-term HCC risk profile based on predictors monitored after HCV eradication by direct-acting antivirals in patients with cirrhosis. Methods: HCC occurrence was evaluated by Kaplan-Meier analysis. Cox regression analysis identified the post-treatment variables associated with de-novo HCC; their predictive power was presented in a nomogram. Results: After the end of therapy (median follow-up:28.47 months), among 2064 SVR patients, 119 (5.8%) developed de-novo HCC. The HCC incidence was 1.90%, 4.21%, 6.47% at 12-, 24- and 36-months from end-of-therapy, respectively (incidence rate 2.45/100 person-years). Age, genotype 3, diabetes, platelets (PLT)≤120,000/µl and albumin ≤3.5g/dl levels were identified as pre-treatment HCC independent predictors. Adjusting for age, the post-treatment PLT≤120,000/µl (AdjHR 1.92; 95%CI:1.06-3.45) and albumin≤3.5g/dl (AdjHR 4.38; 95%CI 2.48-7.75) values were independently associated with HCC occurrence. Two different risk profiles were identified by combining long-term post-therapy evaluation of PLT ≤ vs. >120,000/µl and albumin ≤ vs. >3.5g/dl showing a significant different HCC incidence rate of 1.35 vs. 3.77/100 p-y, respectively. Conclusions: The nomogram score based on age, PLT and albumin levels after SVR showed an accurate prediction capability and may support the customizing management for early HCC detection.

Abstract

Recent technological advancements such as the Internet of Things (IoT) and machine learning (ML) can lead to a massive data generation in smart environments, where multiple sensors can be used to monitor a large number of processes through a wireless sensor network (WSN). This poses new challenges for the extraction and interpretation of meaningful data. In this spirit, age of information (AoI) represents an important metric to quantify the freshness of the data monitored to check for anomalies and operate adaptive control. However, AoI typically assumes a binary representation of the information, which is actually multi-structured. Thus, deep semantic aspects may be lost. In addition, the ambient correlation of multiple sensors may not be taken into account and exploited. To analyze these issues, we study how correlation affects AoI for multiple sensors under two scenarios of (i) concurrent and (ii) time-division multiple access. We show that correlation among sensors improves AoI if concurrent transmissions are allowed, whereas the benefits are much more limited in a time-division scenario. Furthermore, we discuss how ML can be applied to extract relevant information from data and show how it can further optimize the transmission policy with savings of resources. Specifically, we demonstrate, through simulations, that ML techniques can be used to reduce the number of transmissions and that classification errors have no influence on the AoI of the system.

4. Modeling Value of Information in remote sensing from correlated sources

Abstract

This paper investigates data correlation in remote sensing networks and how it can be characterized through diverse models quantifying the Value of Information (VoI), a metric that describes how informative the data transmitted by the sensors are. For each sensor, the VoI evaluations comprise the average node-specific Age of Information (AoI), the average cost spent for sending updates, and the AoI of neighbor nodes, assumed to be correlated sources of information and therefore benefiting the VoI of other sensors nearby. We discuss how this metric can be tracked through a two-dimensional Markov chain, but we also show how this representation can be simplified by including the impact of neighbor nodes within the transition probabilities, so as to obtain a simpler model that gives the same insight in terms of VoI evaluations.

Abstract

Objectives: The study measures trends in the profile of patients with chronic hepatitis B virus linked to care in Italy. Methods: A cross-sectional, multicenter, observational cohort (PITER cohort) of consecutive patients with hepatitis B surface antigen (HBsAg) over the period 2019-2021 from 46 centers was evaluated. The reference was the MASTER cohort collected over the years 2012-2015. Standard statistical methods were used. Results: The PITER cohort enrolled 4583 patients, of whom 21.8% were non-Italian natives. Compared with those in MASTER, the patients were older and more often female. The prevalence of hepatitis B e antigen (HBeAg) declined (7.2% vs 12.3; P <0.0001) and that of anti-hepatitis D virus (HDV) remained stable (9.3% vs 8.3%). In both cohorts, about 25% of the patients had cirrhosis, and those in the PITER cohort were older. HBeAg-positive was 5.0% vs 12.6% (P <0.0001) and anti-HDV positive 24.8% vs 17.5% (P <0.0017). In the logistic model, the variables associated with cirrhosis were anti-HDV-positive (odds ratio = 10.08; confidence interval 7.63-13.43), age, sex, and body mass index; the likelihood of cirrhosis was reduced by 40% in the PITER cohort. Among non-Italians, 12.3% were HBeAg-positive (vs 23.4% in the MASTER cohort; P <0.0001), and 12.3% were anti-HDV-positive (vs 11.1%). Overall, the adherence to the European Association for the Study of the Liver recommendations for antiviral treatment increased over time. Conclusion: Chronic hepatitis B virus infection appears to be in the process of becoming under control in Italy; however, HDV infection is still a health concern in patients with cirrhosis and in migrants.

Abstract

The Anthropocene is the present time of human‐caused accelerating global change, and new forms of Anthropocene risk are emerging that society has hitherto never experienced. Science and policy are grappling with the temporal and spatial magnitude of these changes. However, there is a gap in the transparency—and perhaps even in the awareness—of the profound role that Anthropocene science plays in shaping the structure and possibility of our future world. In this work, we explore three key features of Anthropocene scenarios, including: worlding capacity, values shaping what is possible, and refusal to look. We explore these three features using three cases of Anthropocene science including international energy scenarios, climate change projections, and the possibility of social collapse. We discuss how Anthropocene science modulates new risks and systematically, though perhaps inadvertently, entrains certain social‐ecological futures. We find that clarity in these three attributes of Anthropocene science could enhance its integrity and build trust, not least in the arena of public policy. We conclude with recommendations for improving the interpretability and scope of Anthropocene science in the context of a growing urgency for accurate information to inform our collective future. Plain Language Summary: Humanity has entered a new geologic epoch—the Anthropocene—as we unequivocally change the rhythms of planetary, ecological, and social dynamics. In the 21st century, civic society is asking science to provide unprecedented types of information to inform the present and guide future policy. The rapid uptake of Anthropocene scenarios as a part of this science has numerous, under‐explored implications that require much greater attention. In this work, we argue that there are critical aspects of this Anthropocene scenario science that require introspection, particularly on the part of the scenario science community. By examining three case studies of Anthropocene scenario science, we draw out key characteristics that require greater attention. These are: (a) the unacknowledged capacity of some scientific actors to effectively steer the future; (b) the unarticulated inclusion of value preferences in scenarios; and, (c) the persistent unwillingness to consider certain types of socially‐turbulent future possibilities. We conclude this analysis with recommendations for how the Anthropocene scenario science community can move forward. Abstract

Functional imaging methods such as resting-state fMRI allow to describe interactions among different areas of the brain, thus deriving a functional connectivity matrix of the entire brain network. Tracking functional relationships among different regions of interest can be applied, besides a pure modelling perspective, also to discovering procedures to detect brain diseases and anomalies, or pursuing rehabilitation of subjects with structural damages. However, network characterization is often regarded as frequency-independent, so that the frequency at which interactions take place among different regions is ignored. In this paper, we show how simple filtering procedures over different bands, applied to the resting-state fMRI signals, result in highly different connectivity matrices. Thus, it is highlighted that the functional network can be significantly dependent on the considered frequency range for the fMRI signal. This both justifies the need for a careful filtering of the signals, that avoids filtering out relevant frequencies, and also hints the possibility of classifying functional interactions according to the frequency where the connectivity among two areas is the strongest.

Abstract

Performance metrics such as Age of Information are used to represent data freshness, which is a key element to track in sensing scenarios with sporadic reporting, as typical for ex-ample of cyberphysical platforms in industry, health monitoring, agriculture. However, when multiple sensors are employed, all tracking the same scenario, the presence of correlation in the sensed metrics results in the collection of redundant data, which implies interesting quantitative trends. This paper leverages on analytical derivations of age of information for queueing systems, to investigate how this metric behaves in a system of correlated sources, in particular for what concerns the number of sources, their correlation, and their offered traffic. The quantitative results that we obtain can offer interesting insights for planning and managing large scale systems where information is expected to be correlated, such as sensor networks for smart industrial and agricultural applications.

11. Shapley Value as an Aid to Biomedical Machine Learning: a Heart Disease Dataset Analysis

Abstract

This paper investigates the decision making process aided by machine learning for biomedical problems and how to improve it through meta assessments of the most relevant features. Classification algorithms are usually trained and exploited with high dimensional datasets (i.e., with an extremely large number of features), which is inefficient and costly. It would be beneficial to identify the most meaningful features that contribute the most to assigning a category to a subject, and in particular, diagnosing a pathological condition. A helpful support can come from cooperative game theory, through the computation of the Shapley value, an indicator of desirable properties according to which the players, in our case the input features, can be ranked. We apply such a framework to a supervised machine learning scenario of a random forest tree classifier applied to heart disease detection. From a publicly available dataset, we identify the most relevant features that can affect the decision, thus obtaining practical guidelines for a compact yet efficient description based on an analytical rationale.

Abstract

Background: Direct-acting antivirals are highly effective for the treatment of hepatitis C virus (HCV) infection, regardless race/ethnicity. We aimed to evaluate demographic, virological and clinical data of HCV-infected migrants vs. natives consecutively enrolled in the PITER cohort. Methods: Migrants were defined by country of birth and nationality that was different from Italy. Mann-Whitney U test, Chi-squared test and multiple logistic regression were used. Results: Of 10,669 enrolled patients, 301 (2.8%) were migrants: median age 47 vs. 62 years, (p < 0.001), females 56.5% vs. 45.3%, (p < 0.001), HBsAg positivity 3.8% vs. 1.4%, (p < 0.05). Genotype 1b was prevalent in both groups, whereas genotype 4 was more prevalent in migrants (p < 0.05). Liver disease severity and sustained virologic response (SVR) were similar. A higher prevalence of comorbidities was reported for natives compared to migrants (p < 0.05). Liver disease progression cofactors (HBsAg, HIV coinfection, alcohol abuse, potential metabolic syndrome) were present in 39.1% and 47.1% (p > 0.05) of migrants and natives who eradicated HCV, respectively. Conclusion: Compared to natives, HCV-infected migrants in care have different demographics, HCV genotypes, viral coinfections and comorbidities and similar disease severity, SVR and cofactors for disease progression after HCV eradication. A periodic clinical assessment after HCV eradication in Italians and migrants with cofactors for disease progression is warranted.

This both justifies the need for a careful filtering of the signals, that avoids filtering out relevant frequencies, and also hints the possibility of classifying functional interactions according to the frequency where the connectivity among two areas is the strongest.

Abstract

The problem of multi-agent robotic survey of an unknown area is approached through a game theoretic frame-work. This is meant to enable cooperation in the group of robotic explorers reflecting their common objectives to minimize the effort in the surveying task, without requiring expensive exchanges of signaling. The game theoretic approach can be applied to avoid any preliminary planning, but just exploiting the ability of the robots to take smart actions based on the state of the environment and the behaviors of other neighboring agents. We discuss how the choice of different utility functions can improve the collaboration among the robots and lead to more efficient results.

Abstract

This paper investigates remote sensing networks and discusses different models to characterize the Value of Information (VoI), a metric that describes how informative the data transmitted by the sensors are. For each sensor, the VoI evaluations comprise the average node-specific Age of Information (AoI), the average cost spent for sending updates, and the AoI of neighbor nodes, assumed to be correlated sources of information and therefore benefiting the VoI of other sensors nearby. We discuss how this metric can be tracked through a two-dimensional Markov chain, but we also show how this representation can be simplified by including the impact of neighbor nodes within the transition probabilities, so as to obtain a simpler model that gives the same insight in terms of VoI evaluations.

17. Challenges of the Age of Information Paradigm for Metrology in Cyberphysical Ecosystems

Abstract

We are facing a transition towards interconnection of computing systems, people, and things, where boundaries are disappearing and new challenges are emerging. This trend also applies to smart living environments, which are becoming a cyberphysical ecosystem of devices and individuals. Generally, meta-descriptors such as age of information are exploited to obtain efficient content representation and semantic characterization, with the advantage of better data handling. However, the strong relevance of living support in the involved applications imposes to rethink of this approach whenever it is important to factor the human-in-the-loop. In this paper, we discuss how the investigations related to age of information, in particular aimed at statistical descriptions and/or network operation modeling, can be influenced in such scenarios, for what concerns overarching machine learning for data classification and its impact on the sensing frequency, as well as the presence of data correlation that allows for a parsimonious handling of the updates.

Abstract

Background Since the beginning of the pandemic, the epidemiology of coronavirus disease 2019 (COVID-19) in Italy has been characterized by the occurrence of subnational outbreaks. The World Health Organization recommended building the capacity to rapidly control COVID-19 clusters of cases in order to avoid the spread of the disease. This study describes a subregional outbreak of COVID-19 that occurred in the Emilia Romagna region, Italy, and the intervention undertaken to successfully control it. Methods Cases of COVID-19 were defined by a positive reverse transcriptase polymerase chain reaction (RT-PCR) for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on nasopharyngeal swab. The outbreak involved the residential area of a small town, with ~10 500 inhabitants in an area of 9 km2. After the recognition of the outbreak, local health care authorities implemented strict quarantine and a rearrangement of health care services, consisting of closure of general practitioner outpatient clinics, telephone contact with all residents, activation of health care units to visit at-home patients with symptoms consistent with COVID-19, and a dedicated Infectious Diseases ambulatory unit at the nearest hospital. Results The outbreak lasted from February 24 to April 6, 2020, involving at least 170 people with a cumulative incidence of 160 cases/10 000 inhabitants; overall, 448 inhabitants of the municipality underwent at least 1 nasopharyngeal swab to detect SARS-CoV-2 (positivity rate, 38%). Ninety-three people presented symptoms before March 11 (pre-intervention period), and 77 presented symptoms during the postintervention period (March 11–April 6). Conclusions It was possible to control this COVID-19 outbreak by prompt recognition and implementation of a targeted local intervention.

Abstract

We reported the efficacy and safety data for daclatasvir (DCV)-based all-oral antiviral therapy in patients treated in the Italian compassionate-use program. 275 patients were included (202 male-73.5%, mean age: 57.4 years, 62 HIV-coinfected, 94 with recurrence of hepatitis C post-OLT). Forty-nine patients (17.8%) had Child-Pugh B, Genotype(G) distribution was: G1a:72 patients (26.2%), G1b:137 (49.8%); G3:40 (14.5%) and G4:26 (9.5%). Patients received DCV with sofosbuvir(SOF) (n = 221, 129 with ribavirin(RBV) or with simeprevir (SMV) or asunaprevir (ASU) (n = 54, 19 with RBV) for up to 24 weeks. Logistic regression was used to identify baseline characteristics associated with sustained virological response at week 12 post-treatment (SVR12). Liver function changes between baseline and follow up were assessed in 228 patients. 240 patients achieved SVR12 (87.3%), post transplant and HIV co-infected patients were equally distributed among SVR and no SVR (35% vs 34.3%; p = 0.56 and 24.2% vs 11.4%, p = 0.13, respectively). SVR rate was significantly higher with the combination DCV + SOF compared with DCV + SIM or ASU (93.2% vs 63.0%, p < 0.0001). Bilirubin value (OR: 0.69, CI95%: 0.54–0.87, p = 0.002) and regimen containing SOF (OR: 9.99, CI95%: 4.09–24.40; p < 0.001) were independently related with SVR. Mean albumin and bilirubin values significantly improved between baseline and follow-up week 12. DCV-based antiviral therapy was well tolerated and resulted in a high SVR when combined with SOF either in pre-transplant and in OLT patients and in “difficult to treat” HCV genotypes. Regimens containing DCV in combination with NS3 protease inhibitors obtained suboptimal results.

Abstract

Background: Direct-acting antivirals are highly effective for the treatment of hepatitis C virus (HCV) infection, regardless race/ethnicity. We aimed to evaluate demographic, virological and clinical data of HCV-infected migrants vs. natives consecutively enrolled in the PITER cohort. Methods: Migrants were defined by country of birth and nationality that was different from Italy. Mann-Whitney U test, Chi-squared test and multiple logistic regression were used. Results: Of 10,669 enrolled patients, 301 (2.8%) were migrants: median age 47 vs. 62 years, (p < 0.001), females 56.5% vs. 45.3%, (p < 0.001), HBsAg positivity 3.8% vs. 1.4%, (p < 0.05). Genotype 1b was prevalent in both groups, whereas genotype 4 was more prevalent in migrants (p < 0.05). Liver disease severity and sustained virologic response (SVR) were similar. A higher prevalence of comorbidities was reported for natives compared to migrants (p < 0.05). Liver disease progression cofactors (HBsAg, HIV coinfection, alcohol abuse, potential metabolic syndrome) were present in 39.1% and 47.1% (p > 0.05) of migrants and natives who eradicated HCV, respectively. Conclusion: Compared to natives, HCV-infected migrants in care have different demographics, HCV genotypes, viral coinfections and comorbidities and similar disease severity, SVR and cofactors for disease progression after HCV eradication. A periodic clinical assessment after HCV eradication in Italians and migrants with cofactors for disease progression is warranted.

Abstract

Objectives: Portal vein thrombosis (PVT) is a frequent complication of cirrhosis. Benefit, safety, and duration of anticoagulant treatment in this setting are controversial issues. The aim of this study was to analyze the course of PVT in a large cohort of cirrhotic patients undergoing or not anticoagulation therapy. Methods: The data of 182 patients who presented between January 2008 and March 2016 with cirrhosis and PVT with at least 3 months of follow-up after the first PVT detection were analyzed. Eighty-one patients received anticoagulants and 101 were untreated per physician discretion. Results: The extension of the thrombosis decreased by >50% in 46 (56.8%, with complete recanalization in 31/46) patients under anticoagulation and in 26 (25.7%) untreated patients. Of the 46 patients who underwent recanalization, 17 (36%) suffered recurrent thrombosis after stopping anticoagulation therapy. Kaplan–Meier analysis showed a higher survival rate in the treated group (p = 0.010). At multivariate analysis, anticoagulation was an independent factor associated with longer survival (HR:0.30, CI:0.10–0.91, p = 0.014). The Child–Turcotte–Pugh classes B/C negatively influenced survival (hazard ratio, (HR):3.09, confidence interval (CI):1.14–8.36, p = 0.027 for Child–Turcotte–Pugh B and HR:9.27, CI:2.67–32.23, p < 0.001 for Child–Turcotte–Pugh C). Bleeding complications occurred in 22 (21.8%) untreated and 16 (19.7%) treated patients, but in only four cases was it judged to be related to the anticoagulant treatment. No death was reported as a consequence of the bleeding events. Conclusions: Anticoagulant treatment is a safe and effective treatment leading to partial or complete recanalization of the portal venous system in 56.8% of cases, improving the survival of patients with cirrhosis and PVT. Discontinuation of the therapy is associated with a high rate of PVT recurrence.

Abstract

This retrospective study reports the data of sofosbuvir-based anti-hepatitis C virus treatment in 24 candidates and 24 recipients of liver transplantation coinfected with human immunodeficiency virus. Sustained virologic response was cumulatively 85% (90% and 100% in those treated with optimal schedules pre- and posttransplant, respectively).

Abstract

In this paper, we propose a new method for jointly compressing EEG and EMG biosignals based on the so-called cortico-muscular coherence, a function that takes into account the simultaneous frequency changes of the brain and the muscles activity, and can be used, e.g., to classify different kinds of movement. It is shown that this method increases the achievable compression rate compared to transmitting EEG and EMG samples separately, while trading-off with the accuracy of the classification. This can be exploited in several kinds of life and health applications e.g., motor rehabilitation and drivers attention monitoring; it could be especially useful for low-power wireless technologies, such as Bluetooth Low Energy or IEEE 802.15.6, whose transmission resources are limited.

Abstract

This work presents an innovative application of the well-known concept of cortico-muscular coherence for the classification of various motor tasks, i.e., grasps of different kinds of objects. Our approach can classify objects with different weights (motor-related features) and different surface frictions (haptics-related features) with high accuracy (over 0.8). The outcomes presented here provide information about the synchronization existing between the brain and the muscles during specific activities; thus, this may represent a new effective way to perform activity recognition.

Abstract

The prevalence of Rotavirus (RV)-related gastroenteritis in Cameroon was monitored in this study, identifying the circulating G-P types, and the presence of co-infection with intestinal parasites. The stool samples were examined for rotaviruses using chromatographic and molecular techniques, and parasites were identified by macroscopic and microscopic examination. 38% of samples were RV positive. Data revealed the presence of three different G-genotypes (G1, G2 and G8) and three Pgenotypes (P [8], P[6] and P[4]). The main G-P combination was G1+G2P[8]. Entamoeba histolytica was identified in 44.4% of stool samples. The G2P[8] genotype identified is an unusual strain with the G1P[8] or G2P[4] more common. Genotype G8 strains, also associated with animals, have been sporadically recovered from humans and have been considered for inclusion in bovine-human reassortant vaccine. Epidemiological RV strain surveillance should be continued in order to support RV vaccination campaigns. The results also show a high prevalence of intestinal parasite infections.

43. Cyber security of smart grids modeled through epidemic models in cellular automata

Abstract

Due to their distributed management, smart grids can be vulnerable to malicious attacks that undermine their cyber security. An adversary can take control of few nodes in the network and spread digital attacks like an infection, whose diffusion is facilitated by the lack of centralized supervision within the smart grid. In this paper, we propose to investigate these phenomena by means of epidemic models applied to cellular automata. We show that the common key parameters of epidemic models, such as the basic reproductive ratio, are also useful in this context to understand the extent of the grid portion that can be compromised. At the same time, the lack of mobility of individuals limits the spreading of the infection. In particular, we evaluate the role of the grid connectivity degree in both containing the epidemics and avoiding its spreading on the entire network, and also increasing the number of nodes that do not get any contact with the cyber attacks.
