4. Between past and future: what the everyday lives of beneficiaries of brazil's Back Home Program teach us about the program.

This study investigates the everyday lives of six beneficiaries of the Programa de Volta para Casa (the "Back Home Program" - PVC) with the aim of analyzing its effects and identifying lessons that can be applied to help improve and ensure the continuity of the deinstitutionalization process in dialogue with the current political, social and economic reality. Using participant observation and narratives, we conducted a qualitative study in two cities that have been implementing the PVC over the last 15 years. The results were organized into the following core themes: the participants - who they are and how they live; the challenges of being back in the city; using money and the challenge of shifting towards social capital; and the guarantee of rights. The findings of the hermeneutic-dialectic analysis show that the beneficiaries have moved away from a situation of zero contractuality and ruptured affective relations towards one of access to goods and services, housing and free movement in public spaces, limited by urban violence, lack of money for leisure activities, aging, precarious housing conditions, and lack relations of trust and solidarity with ordinary people living in the neighborhood. Making the city an enabling environment and promoting actions in coordination with other sectors such as housing, income, employment, security and justice are steps of resistance in the face a regressive political, economic and social landscape. This research highlights housing as one of the dimensions of the lives of individuals who experience severe psychological suffering, which reverberates in their care and unique therapeutic plan. This paper aims to grasp the Psychosocial Care Centers (CAPS) users' meanings of their experiences of living in Santos (SP), Brazil. This qualitative research was conducted with users of different services through semi-structured interviews submitted to the thematic content analysis. This study identified how the conception of housing relates to protection, individuality, and affective memories that corroborate the construction of an expanded concrete life regarding living in freedom in the city, presenting plural housing located in different territories. Taking ownership of spaces reveals the possibilities and challenges inherent to social exchanges and daily experiences. Being and living in a house means having access to material and subjective goods, besides assuring care in freedom. 7. Saúde Mental em Contextos Rurais: o Trabalho Psicossocial em Análise

- Abstract
Dwellers of agrarian reform settlements have a life conditioned by poor living and work conditions; they have difficulties accessing health programs, social assistance and other public policies and this exacerbate their vulnerability, which has an impact on their mental health. This research investigates the care given by the health and social assistance staff, regarding the mental health demands of rural settlements of Rio Grande do Norte, Brazil. 53 experts from different professional categories were interviewed individually or in groups. For the analysis of the results we used the Content Analysis as a proposal for categorization of data. TR results indicate that the workers experience poor working conditions and that staff has little knowledge of the local conditions and of the mental health needs. The implemented health care still corresponds to the biomedical logic, with little participation of the dwellers, disregarding the traditional knowledge and practices of local health care. The psychosocial care is not well coordinated, presenting problems with follow-up and continuity. Due to the lack of specific knowledge of the specifics of the life conditions of the dwellers and the fragmentation of the psychosocial health care network, the staff does not abide and is not ready to face the mental health needs in order to change these health iniquities., Moradores de assentamentos de reforma agrária têm uma vida marcada por condições de vida e trabalho precárias, que, aliadas às barreiras no acesso às políticas e programas de saúde e assistência social, agravam as situações de vulnerabilidade que impactam a saúde mental. Este trabalho pretende discutir o cuidado desenvolvido por equipes de saúde e assistência social referente às demandas de saúde mental de assentados do Rio Grande do Norte, Brasil. Foram entrevistados 53 profissionais de forma individual ou por grupo de diferentes categorias profissionais. Para a análise dos resultados utilizou-se a Análise de Conteúdo como proposta para categorização dos dados. Resultados indicam que os trabalhadores vivenciam condições precárias de trabalho e que as equipes têm pouco conhecimento do território e das necessidades de saúde mental. O cuidado implementado ainda corresponde à lógica biomédica, com pouca participação dos moradores e desconsideração dos saberes e práticas de cuidado tradicionais. A atenção psicossocial não funciona de forma articulada apresentando problemas quanto ao seguimento e à continuidade de cuidados. Em função do desconhecimento das especificidades da população assentada e da fragmentação da rede de atenção psicossocial, essas equipes não conseguem acolher e responder às necessidades em saúde mental de modo a interferir nas iniquidades em saúde., Residentes de asentamientos de reforma agraria tienen una vida marcada por condiciones de vida y trabajo precario, que unida a las barreras para tener acceso a las políticas y los programas sanitarios y de asistencia social agravan las situaciones de vulnerabilidad que tienen un impacto en la salud mental. Este artículo analiza el cuidado desarrollado por equipos de salud y atención social, relativos a las necesidades de salud mental de 9 (nueve) asentamientos rurales del Rio Grande do Norte, Brasil. Fueron entrevistados 53 (cincuenta y tres) de distintas categorías profesionales individualmente o en grupos. Utilizamos el Análisis de Contenido como propuesta para la categorización de los datos. Los resultados muestran que los trabajadores viven condiciones laborales precarias y los equipos tienen poco conocimiento del territorio y de las necesidades de salud mental. El cuidado implementado todavía corresponde a la lógica biomédica, con poca participación de los residentes y la desconsideración de los saberes y prácticas tradicionales de cuidado. En función del desconocimiento de las especificidades de la población asentada y la fragmentación de la red de atención psicosocial, esos equipos no logran acoger y responder a las necesidades de salud mental para cambiar las inequidades en salud.

8. Práticas de Cuidado Integral às Pessoas em Sofrimento Mental na Atenção Básica

With this study we aim to know the practices of integral health care performed in the Family Health Unities (FHU) of a health region in Vitória – ES, Brazil, that are offered to people with mental disorders, and to point the difficulties and possibilities faced by health professionals at the development of these practices in Basic Care. We organized six focus group sessions, one session in each one of the six researched Health Unities (HU); after identifying the HU that performed integral health care (according to respondents), we used systematic observation and semi-structured interviews with the professionals responsible for these practices to analyze the actions. We concluded that there are still important difficulties to be faced in the sense of making effective the integration of people with mental disorder in HU spaces of integral health care. However, there were also positive results related to giving access to these participants to health care and to qualified hearing, contributing to their autonomy and social integration, Objetivou-se com este estudo conhecer as práticas de cuidado integral ofertadas às pessoas em sofrimento mental, efetivadas nos espaços das Unidades de Saúde da Família (USF) em uma região de saúde do município de Vitória, Espírito Santo, bem como apontar as dificuldades e possibilidades enfrentadas pelos profissionais de saúde no desenvolvimento dessas práticas na Atenção Básica. Para tanto, foram realizadas seis sessões de grupo focal, uma sessão em cada uma das seis Unidades de Saúde pesquisadas; em seguida, identificadas as Unidades que referiam realizar ação de cuidado integral, foi utilizada a técnica da observação sistemática para acompanhamento de tais ações e realizada entrevista semiestruturada com os profissionais responsáveis por essas práticas. São grandes as dificuldades enfrentadas no sentido de fazer valer uma inserção efetiva destas pessoas em sofrimento mental nos espaços das Unidades de Saúde para um cuidado integral. Contudo, na tentativa de garantia de acesso destas pessoas à Atenção Básica, com vistas ao acolhimento, à escuta qualificada e ao cuidado em saúde, houve resultados no sentido de ampliação de autonomia e inserção social dos sujeitos participantes., El objetivo de este estudio fue conocer las prácticas de cuidado integral ofrecidas a personas con trastornos mentales en los espacios de las Unidades de Salud de la Familia (USF) en una región de la ciudad de Vitória-ES, así como señalar las dificultades y posibilidades que enfrentan los profesionales de la salud en el desarrollo de estas prácticas en la atención primaria. Con este fin, se llevaran a cabo seis sesiones de grupos focales, una sesión por cada una de las seis unidades de salud encuestadas; en seguida, fueron identificadas las unidades que reportaron realizar acciones de atención integral. Se utilizó la técnica de la observación sistemática para rastrear este tipo de acciones y se realizaron entrevistas semiestructuradas con los profesionales responsables de estas prácticas. Existen grandes dificultades para hacer cumplir una integración efectiva de personas con problemas mentales en los espacios de las Unidades de Salud para la atención integral. Sin embargo, en un intento de garantizar que estas personas tengan acceso a la atención primaria, con vistas al acogimiento, a la escucha cualificada y al cuidado en salud, hubo resultados relacionados a ampliar la autonomía e inserción social de los sujetos participantes.
