4. Innovative and cost-effective SesZen-Bio™ with enriched Biotin and improved superior dissolution efficiency

Background: Micronutrient deficiencies, including biotin, iron, zinc, and vitamin D, can lead to hair problems such as loss and thinning. This study explores the improvement of SesZen-BioTM, a natural biotin supplement derived from Sesbania grandiflora leaves, to give NI- SesZen-BioTM. Materials: The nature and structure of biotin in SesZen-BioTM were evaluated using C14 analysis and 1H NMR. Further, Sesbania grandiflora was subjected to an improved economic process to give NI-SesZen-BioTM. HPLTC-aided phytochemical profiling was performed for SesZen-BioTM, NI- SesZen-BioTM, and standard S. grandiflora leaf extract. The purity of d-biotin was quantified using HPLC. The dissolution profile of SesZen-BioTM and NI- SesZen-BioTM was tested to determine bioequivalence and compared versus synthetic biotin. Results: NMR and C14 analysis revealed that SesZen-BioTM contained 100% natural d-biotin. The yield for SesZen-BioTM and NI- SesZen-BioTM was found to be 17% and 18%, respectively, and both exhibited phytochemical profiles similar to standard extract. The release rate for SesZen-BioTM was similar till a dose of 500 mg and slowly decreased at higher concentrations (800-100mg). A 350 mg NI- SesZen-BioTM showed bioequivalence to 250 mg and 500 mg for SesZen-BioTM. Percent d-biotin content was 0.5% in SesZen-BioTM and 0.81% in NI- SesZen-BioTM. Conclusion: This new economically driven process yielded NI-SesZen-BioTM, with similar yield, higher biotin (100%natural) content, and similar dissolution profile but at a reduced dosage as compared to SesZen-BioTM and surpassed synthetic biotin with its sustained release format. Thus, NI- SesZen-BioTM is a promising solution for individuals seeking natural supplements for maintaining hair and skin heath.

IntroductionDue to its distinctive fatty acid makeup, which makes it simple to utilize in various applications, palm oil is one of the most widely used fats in the food business. However, the increased palm oil production is causing global environmental issues, including the loss of rainforests. As a result, companies that use palm oil, particularly the food industry, must hunt for alternate sources of fat that can replace palm oil. Oleogels, which are created from liquid oils, are one of the potential substitutes. Another alternative is to combine solid fat and liquid oil, however, research in this area is scarce.MethodsTo fill this gap, we used a fully hydrogenized rapeseed oil and sunflower oil mixture in three different ratios for our research. We examined the rheological behavior of the mixtures using oscillatory and rotational viscometry.Results and discussionAccording to our results, 35% FH rapeseed oil sample yielded results most closely resembling the control palm mid fraction. Our findings show that replacing palm oil with a combination of completely hydrogenated rapeseed oil and sunflower oil is only viable to a limited degree from a rheological point of view.

8. Determination of Probiotic Viability in Yoghurts Produced with Acid Adapted Lactobacillus acidophilus ATCC 4356 and Bifidobacterium bifidum ATCC 11863 During Refrigerated Storage

Microorganisms have various stress response systems to maintain their viability when exposed to different stress conditions. In this study, Lactobacillus acidophilus ATCC 4356 and Bifidobacterium bifidum ATCC 11863 strains, used in probiotic yoghurt production, were subjected to acid (lactic and hydrochloric acid) stress to induce acid tolerance response (ATR). Yoghurts produced with both acid-adapted and non-adapted strains were stored at +4°C for 21 days. During the storage period, the pH and titratable acidity values of the yoghurts were measured, and the viability levels of the probiotic strains in the yoghurts were determined. In all yoghurt groups, a decrease in pH values and an increase in titratable acidity were observed during storage. The highest viability levels of the probiotic strains were detected on the first day of storage. Lactic acid-adapted Lb. acidophilus ATCC 4356 and B. bifidum ATCC 11863 in yoghurt showed growth at a level of 8.08 ± 0.12 and 8.08 ± 0.09 log10 Cfu/g at the first day of storage, respectively. Additionally, hydrochloric acid-adapted Lb. acidophilus ATCC 4356 and B. bifidum ATCC 11863 in yoghurt exhibited growth at levels of 7.90 ± 0.08 and 5.99 ± 0.03 log10 Cfu/g, respectively. The viability of acid-adapted Lb. acidophilus ATCC 4356 and B. bifidum ATCC 11863 showed a decrease similar way to that of the control group (non-acid adapted) during the storage period.

9. Extra-lymphatic Filariasis: A Study of Three Interesting Cases

Filariasis is a chronic disabling parasitic disease that causes a major public health problem in tropical countries like India. Wuchereria bancrofti is associated with almost 99.4% of cases. Filariasis not only affects the structure and function of lymphatic vessels but is also associated with extra-lymphatic pathology and diseases. Lymphatic filariasis is commonly found throughout the tropics and subtropics. While lymph nodes are the common sites, unusual sites include the breast, spleen, subcutaneous tissue, thyroid, bone marrow, urinary tract, sputum, bronchial washing, pleural and pericardial fluid. Patients from endemic areas presenting with swelling should be evaluated for filariasis. Here in present case series, the authors discussed three cases: filariasis of the breast with fibroadenoma, soft-tissue nodule, and splenic lesions, due to their extreme rarity and unusual sites. In the first case, 20 years old female patient presented with a lump in the left breast. The Complete Blood Cell (CBC) revealed eosinophilia, but peripheral smears did not show the presence of microfilariae. On Fine Needle Aspiration Cytology (FNAC), smears showed Wuchereria bancrofti microfilaria in the background of fibroadenoma. The patient was treated with Diethylcarbamazine (DEC) for 21 days. Repeat CBC revealed a decrease in the absolute eosinophil count. Lumpectomy for fibroadenoma was performed, which showed fragments (dead) of the microfilarial parasite. The association of filariasis with fibroadenoma is possibly due to pre-existing subclinical filariasis when the neoplasm developed, as the patient hails from an endemic area. Filariasis presenting as a soft-tissue nodule is an uncommon incidence. In the second case discussed here, 30 years old female patient had soft-tissue swelling over the left elbow. Peripheral smear only revealed eosinophilia without any parasites. On FNAC, smears showed Wuchereria bancrofti microfilaria, along with lymphoid cells and a few eosinophils. The swelling subsided with DEC treatment, and repeat CBC showed a decrease in the absolute eosinophil count. The third case discussed here, involved an unknown 35 years old male, who was brought dead to the hospital. No clinical details were available. On autopsy, the spleen showed multiple white patches, ranging in size from 0.2 to 0.5 cm in diameter. All other organs were unremarkable. Microscopy showed many granulomas and numerous dead fragmented microfilariae. It was a case of isolated splenic filariasis, which is a rarely diagnosed entity. These unique cases will raise awareness of diagnosing and instituting proper therapy.

11. Gender Differences in Periprocedural and Long-term Outcomes in Patients with Hypertrophic Cardiomyopathy Treated with Alcohol Septal Ablation Therapy: A Single Center Retrospective Study

Background and Aim:We aim to demonstrate the periprocedural and long-term results of alcohol septal ablation (ASA) treatment in patients with hypertrophic cardiomyopathy (HCM) and specify the differences between female and male patients.Materials and Methods:We enrolled 53 consecutive patients with HCM who underwent ASA treatment. Preprocedural demographic data, pre-and postprocedural characteristics and complications, echocardiographic data, and long-term results, including all-cause mortality and major adverse cardiovascular events (MACE), were recorded. MACE was defined as sudden cardiac death due to ventricular arrhythmias or heart failure (HF) and rehospitalizations due to HF or atrial fibrillation after the procedure.Results:The mean age was 56.4 ± 12.1 years and 29 (54.7%) of the patients were female. Age at the time of ablation was higher (P = 0.04), and the presentation New York Heart Association functional class (P = 0.03) was worse in female patients. The median volume of ethanol usage was higher in male patients (P = 0.03) and the median duration of intensive care unit stay was higher in female patients (P = 0.02). The overall survival rates after ASA at 1, 5, 10, and 12 years were 96%, 87%, 76%, and 76%, respectively. There was no difference in the overall survival rates between genders (log-rank P = 0.4) and MACE was significantly higher in women patients (log-rank P = 0.03).Conclusion:Women patients with HCM were older and had a worse functional capacity during the ASA procedure. Despite the similar mortality rates between genders, MACE was higher in women after the procedure. Earlier evaluation and treatment in female patients might decrease MACE during follow-up after ASA treatment.

Yaşanan pandemi ile küresel ticarette ulaşım ve lojistik kabiliyetlerin ne denli önemli olduğu bir kez daha gözler önüne serilmiştir. Sektördeki işletmeler için nitelikli eleman istihdamı son derece önemli hale gelmiştir. Sektöre başarılı çalışanların kazandırılması ise doğru ve etkin eğitimden geçmektedir. Bu çalışmada Türkiye’de denizcilik ve lojistik sektörüne en fazla ara eleman kaynağı sağlayan deniz ve liman işletmeciliği ön lisans programları ele alınmıştır. Türkiye’de ve Kuzey Kıbrıs Türk Cumhuriyeti’nde 21 üniversitedeki bu ön lisans programları ve dersleri içerik analizi yöntemiyle incelenmiştir. Araştırma sonucunda 21 üniversitedeki deniz ve liman işletmeciliği programları arasında yer alan dersler açısından genel bir uyum olduğu ve sektörün ihtiyacına yönelik olan derslerin çoğunun programlarda zorunlu olarak bulunduğu belirlenmiştir. Ayrıca yabancı dil eğitimiyle ilgili verilen derslerin de yeterli olduğu görülmüştür. Ancak sektör açısından önemli olan dış ticaret işlemleri ve freight forwarder işletmeciliği derslerinin birkaç programda bulunması önemli bir eksiklik olarak tespit edilmiştir.

14. The Effect of Antioxidant Astaxanthin on Intestinal Ischemia Reperfusion Damage in Rats

Background Mesenteric ischemia is a frequently encountered disease in surgical clinics, difficult to diagnose, and very mortal if not treated. Our study investigated the effects of astaxanthin, which is known to have potent antioxidant properties and is also known to have anti-inflammatory effects on ischemia-reperfusion (I/R) injury. Methods A total of 32 healthy Wistar albino female rats were used in our study. Subjects were randomized and equally divided into 4 groups; control (laparotomy group only), I/R (transient mesenteric ischemia group only), astaxanthin 1 mg/kg and 10 mg/kg doses. The transient ischemia time was 60 minutes and the reperfusion time was 120 minutes. Tissue samples were taken from intracardiac blood and terminal ileum after reperfusion. Superoxide dismutase (SOD), catalase (CAT), malondialdehyde (MDA) from blood samples, interleukin-1 (IL-1), IL-6, tumor necrosis factor-α (TNFα), Caspase-3, P53 tests from terminal ileum were studied. Tissue samples were also taken for histopathological evaluation. Results At the end of the study, both doses of astaxanthin were found to significantly reduce MDA level, CAT, and SOD enzymatic activity, whereas higher doses of astaxanthin significantly reduced MDA level, CAT, and SOD enzyme activities. In addition, cytokines such as TNFα, IL-1 and IL-6 were found to be reduced at both doses of astaxanthin, but only significantly inhibited at higher doses. We observed that inhibition of apoptosis reduced caspase-3 activity and P53 and deoxyribonucleic acid (DNA) fragmentation. Conclusion Astaxanthin, a potent antioxidant, and anti-inflammatory, significantly reduces ischemia and reperfusion injury, especially when used at a dose of 10 mg/kg. These data need to be confirmed by larger animal series and clinical studies.

Influenza, pneumococcal, tetanus, and shingles vaccines are recommended for older adults routinely. This study aimed to show the knowledge and attitudes of the physicians to older adults’ vaccination schemes.Patients and Methods: An electronic self-reported questionnaire was sent to physicians between March and July 2021 in Turkey. Sociodemographic characteristics, professional experience, area of expertise, and practice setting of the participants were recorded. As multiple-choice questions; the routinely recommended vaccines, and vaccines suggested in their daily practice before and after the COVID-19 pandemic were enquired.Results: A total of 435 participants were included in the study. 43.9% of the patients were primary family physicians, and 36.8% were internists. 63.4% of the participants had reported reviewing the National Vaccination Scheme. 94.5% of the medical doctors indicated that they had recommended any vaccination to their patients. 20.9% of the practitioners could select four or five of the routinely recommended vaccines. Reviewing the National Adult Vaccination Scheme and being an internist were positively related to predicting the recommended vaccines. The recommendation rates of influenza and pneumococcal conjugate (PCV13) were seen at 88% and 78%, respectively. Except for PCV13, recommendation rates of other routine vaccines were decreased after the pandemic.Conclusion: Awareness of routine vaccination schedules should be improved among health-care professionals, and reminders for immunization should be provided periodically in each health-care setting.Keywords: older adults, attitudes to vaccination, vaccine hesitancy, influenza, herpes zoster, preventive care

Background and Study Aim. This study aimed to investigate the effect of 8-weeks of wrestling training on unilateral muscle imbalances. Material and Methods. Eight male elite Greco-Roman wrestlers (wrestling group) (age 21.25±2.7 years, height 176.6±6.8 cm, body weight 81.00±9.3 kg) and seven recreationally active male (control group) (age 21.3±3.2 years, height 173.6±4.7 cm, body weight 67.3±5.3 kg) voluntarily participated in the study. Both group participants were students in the Selcuk university in the period of study. Wrestlers were in the off-season before the training period of 10 weeks. Before and after the 8-weeks training period, wrestling and control groups were tested for isokinetic torque measurement. In the training period, the wrestling group carried out only wrestling training without specific strength training. In this period the control group did not involve in any exercise. Results. In the wrestling group, all quadriceps torque parameters significantly increased for both legs but the torque parameters of the hamstring did not change, except the average torque of the left leg. The peak and average torque of the H/Q (Hamstring/quadriceps) significantly decreased after training periods for right leg in the wrestling group. No parameters significantly changed in the control group. Conclusions. Specific hamstring strength exercises can be put in training programs to avoid from a possible knee injury.

Animal blood is a valuable resource, which is usually not utilized in a value-added way by the industry like other animal by-products, even though it has plenty of benefits in terms of sustainability and human health, particularly against iron deficiency anemia. Animal blood is perfectly suitable for providing special functions, which are necessary for functional foods, and improving techno-functional properties based on the previous reports published in the literature. In this paper, egg powder was substituted by powdered animal blood products (whole blood powder, blood plasma powder, and hemoglobin powder) in sponge cake. Techno-functional and sensory properties (texture by texture profile analysis and three-point breaking test, water activity, dry matter content, and color) were instrumentally measured and then a sensory evaluation was carried out by unskilled panelists. Quality characteristics (texture, color, and dry matter content) were daily measured on the day of baking and then every 24 h for 3 additional days because freshly baked cakes are usually consumed within 3 days. Based on the results, powdered blood products are suitable for substituting the egg powder in sponge cakes and developing functional foods. Blood powders can increase the hardness, chewiness, and breaking force of cakes, giving them the ability to be stuffed with more fillings and molded into special shapes without compromising on the sensory characteristics. They can also increase the intensity of the cocoa flavor, which results in a richer, darker color without deceiving the consumers.

Background and Aim: Despite the advances in oral anticoagulation with NOACs, careful patient and dose selection is required with NOAC therapy. Our study aimed to assess treatment patterns of NOACs in AF along with patients’ continuity to NOAC treatments in first year, and their knowledge level of AF and NOAC treatment. Methods: ASPECT-NOAC was designed as an observational, prospective, and multicenter study. AF patients who were prescribed NOACs within last four months were recruited from 34 outpatient cardiology clinics covering all geographic regions of Turkey. Baseline data were collected initially whereas patient awareness was evaluated at 3 to 4 weeks. Final study visit was performed at 12 months. Results: In total, 991 patients were included to the study. Mean ± standard deviation of age was 69.4 ± 10.2 years and 53.0% of patients were female. Mean duration from AF diagnosis was 24.9 ± 50.9 months. Mean CHA2DS2-VASc and HAS-BLED scores were 3.1 ± 1.5 and 1.6 ± 1.1, respectively. AF disease and NOAC treatment knowledge levels were found to be 48.9 ± 23.1% and 73.0 ± 19.3%, respectively. Among reduced dose users 71.4% of patients were prescribed inappropriate reduced doses. Through the study follow-up, 32 patients (3.2%) deceased and NOAC therapy was discontinued in 74 patients (8.7%). Conclusion: AF patients who recently started NOAC treatment in Turkey were found to have variable knowledge about their disease and anticoagulation treatment. It was observed that most of the patients continued the NOAC treatment throughout the study. Reduced dosing of NOACs was common, which was associated with higher baseline risk for bleeding as well as stroke.

More frequent use of invasive procedures such as intubation, tracheostomy and mechanical ventilation increases the frequency of benign tracheoesophageal fistula development, especially in intensive care patients. In this case report, the patient with a history of total laryngectomy operation, aspirated the audio device secondary to the use of excessive force and the connection of the device to the gastrointestinal system due to the existing tracheoesophageal shunt. Introduction: Population needs health literacy (HL) in order to understand health-related topics. Short Assessment of Health Literacy for Spanish Adults (SAHLSA-50) could be a high-quality tool for assessing HL in adults. Unlike questionnaires in the English language, which are not easily modifiable for Croatian, translation of SAHLSA-50 from Spanish into Croatian could be an appropriate choice. There are no similar questionnaires to assess HL in Croatian to date. Aim: To translate SAHLSA-50 into Croatian using a validated procedure and to adapt it linguistically, so that it could be used in the Croatian-speaking area to measure HL in the Croatian population. Material and Methods: SAHLSA was developed for the Spanish-speaking population by Lee et al. It consists of 50 items and is based on multiple choice questions. A score between 0 and 37 suggests that the respondent has inadequate HL. After the protocolized translation process (translation/back-translation), the final version of the Croatian translation of SAHLSA-50 was produced, after we introduced several changes that do not represent a direct translation of the Spanish items. Most of the changes were made because of a cultural difference in common use or in the meaning of the term. Conclusion: SAHLSA-50 was translated and adapted into Croatian using the standard translation/back-translation procedure. The Croatian version will now be available as an important part of epidemiological and clinical studies, as a useful tool for evaluating the levels of HL in the Croatian-speaking population. Validation of the translated Croatian version in practice would be the next step.

50. Investigations into Flour Mixes of Triticum Monococcum and Triticum Spelta

Bread samples were made using flour mixes of Triticum monococcum (Tr. monococcum) and Triticum spelta (Tr. spelta). They were tested for their rheological behaviour over the first 3 days of storage at room temperature, and for their characteristics based on a Hungarian Standard. Parameters were set such as the volume of the baked product, baking loss, crumb characteristics and elasticity of crumbs. The behaviour of flour from einkorn wheat is different to that of Tr. spelta. The properties of the tested flour mixes measured by a farinograph show that Tr. spelta produces an acceptable dough, on the other hand, the dough of Tr. monococcum develops quickly but is very unstable so weakens within minutes of being kneaded. This also suggests that doughs composed of einkorn wheat flour require a different type of kneading than those of Tr. spelta (or Tr. aestivum, also referred to as common wheat) flours. Breads composed of Tr. spelta were comparable with those made with Tr. aestivum, the crumb elasticity was above 90 % on the day of baking, which indicates high quality. The Tr. monococcum breads, however, were of low grade: the volume of the breads decreased by increasing the ratio of Tr. monococcum to Tr. spelta and the elasticity reduced to unacceptable levels (less than 60 %). It should be mentioned that the grading was based on breads made purely from Tr. aestivum flours.
