Background Timing of initiation of kidney-replacement therapy (KRT) in critically ill patients remains controversial. The Standard versus Accelerated Initiation of Renal-Replacement Therapy in Acute Kidney Injury (STARRT-AKI) trial compared two strategies of KRT initiation (accelerated versus standard) in critically ill patients with acute kidney injury and found neutral results for 90-day all-cause mortality. Probabilistic exploration of the trial endpoints may enable greater understanding of the trial findings. We aimed to perform a reanalysis using a Bayesian framework. Methods We performed a secondary analysis of all 2927 patients randomized in multi-national STARRT-AKI trial, performed at 168 centers in 15 countries. The primary endpoint, 90-day all-cause mortality, was evaluated using hierarchical Bayesian logistic regression. A spectrum of priors includes optimistic, neutral, and pessimistic priors, along with priors informed from earlier clinical trials. Secondary endpoints (KRT-free days and hospital-free days) were assessed using zero–one inflated beta regression. Results The posterior probability of benefit comparing an accelerated versus a standard KRT initiation strategy for the primary endpoint suggested no important difference, regardless of the prior used (absolute difference of 0.13% [95% credible interval [CrI] − 3.30%; 3.40%], − 0.39% [95% CrI − 3.46%; 3.00%], and 0.64% [95% CrI − 2.53%; 3.88%] for neutral, optimistic, and pessimistic priors, respectively). There was a very low probability that the effect size was equal or larger than a consensus-defined minimal clinically important difference. Patients allocated to the accelerated strategy had a lower number of KRT-free days (median absolute difference of − 3.55 days [95% CrI − 6.38; − 0.48]), with a probability that the accelerated strategy was associated with more KRT-free days of 0.008. Hospital-free days were similar between strategies, with the accelerated strategy having a median absolute difference of 0.48 more hospital-free days (95% CrI − 1.87; 2.72) compared with the standard strategy and the probability that the accelerated strategy had more hospital-free days was 0.66. Conclusions In a Bayesian reanalysis of the STARRT-AKI trial, we found very low probability that an accelerated strategy has clinically important benefits compared with the standard strategy. Patients receiving the accelerated strategy probably have fewer days alive and KRT-free. These findings do not support the adoption of an accelerated strategy of KRT initiation.

Importance: Sepsis-associated acute kidney injury (AKI) adversely affects long-term kidney outcomes and survival. Administration of the detoxifying enzyme alkaline phosphatase may improve kidney function and survival.Objective: To determine the optimal therapeutic dose, effect on kidney function, and adverse effects of a human recombinant alkaline phosphatase in patients who are critically ill with sepsis-associated AKI.Design, Setting, and Participants: The STOP-AKI trial was an international (53 recruiting sites), randomized, double-blind, placebo-controlled, dose-finding, adaptive phase 2a/2b study in 301 adult patients admitted to the intensive care unit with a diagnosis of sepsis and AKI. Patients were enrolled between December 2014 and May 2017, and follow-up was conducted for 90 days. The final date of follow-up was August 14, 2017.Interventions: In the intention-to-treat analysis, in part 1 of the trial, patients were randomized to receive recombinant alkaline phosphatase in a dosage of 0.4 mg/kg (n = 31), 0.8 mg/kg (n = 32), or 1.6 mg/kg (n = 29) or placebo (n = 30), once daily for 3 days, to establish the optimal dose. The optimal dose was identified as 1.6 mg/kg based on modeling approaches and adverse events. In part 2, 1.6 mg/kg (n = 82) was compared with placebo (n = 86).Main Outcomes and Measures: The primary end point was the time-corrected area under the curve of the endogenous creatinine clearance for days 1 through 7, divided by 7 to provide a mean daily creatinine clearance (AUC1-7 ECC). Incidence of fatal and nonfatal (serious) adverse events ([S]AEs) was also determined.Results: Overall, 301 patients were enrolled (men, 70.7%; median age, 67 years [interquartile range {IQR}, 59-73]). From day 1 to day 7, median ECC increased from 26.0 mL/min (IQR, 8.8 to 59.5) to 65.4 mL/min (IQR, 26.7 to 115.4) in the recombinant alkaline phosphatase 1.6-mg/kg group vs from 35.9 mL/min (IQR, 12.2 to 82.9) to 61.9 mL/min (IQR, 22.7 to 115.2) in the placebo group (absolute difference, 9.5 mL/min [95% CI, -23.9 to 25.5]; P = .47). Fatal adverse events occurred in 26.3% of patients in the 0.4-mg/kg recombinant alkaline phosphatase group; 17.1% in the 0.8-mg/kg group, 17.4% in the 1.6-mg/kg group, and 29.5% in the placebo group. Rates of nonfatal SAEs were 21.0% for the 0.4-mg/kg recombinant alkaline phosphatase group, 14.3% for the 0.8-mg/kg group, 25.7% for the 1.6-mg/kg group, and 20.5% for the placebo group.Conclusions and Relevance: Among patients who were critically ill with sepsis-associated acute kidney injury, human recombinant alkaline phosphatase compared with placebo did not significantly improve short-term kidney function. Hintergrund: Thromboembolismus ist eine häufige und schwerwiegende Komplikation bei kritisch kranken Patienten mit einer Inzidenz von 40-80%. Daher ist eine effektive Antikoagulation unerlässlich. Die Standardtherapie für eine kontinuierlich intravenös verabreichte Gabe ist das Unfraktionierte Heparin (UFH). Allerdings entwickeln manche Patienten eine sogenannte Heparinresistenz (HR). HR ist so definiert, dass trotz hoher Dosen von UFH (1.200 IE/h) keine ausreichende Antikoagulation, welche anhand der aktivierten partiellen Thromboplastinzeit (aPTT) gemessen wird, erreicht werden kann. Es gibt keine alternative Medikation, die für diese Indikation registriert ist. Das Ziel dieser Studie ist herauszufinden, ob eine Gabe von Argatroban (Argatra®) besser wirkt als die maximale Dosis von Heparin, auch in Hinblick auf die Zeit bis zu einer adäquaten Antikoagulation. Methode: In die klinische Prüfung wurden 42 auswertbare kritisch kranke Patienten mit einer Heparinresistenz, welche an den Intensivstationen der Allgemeinen und Chirurgischen Intensivmedizin der Medizinischen Universität Innsbruck, Österreich, behandelt wurden, eingeschlossen und entweder in den Behandlungsarm mit der weiteren Gabe der Standardtherapie in Form einer Erhöhung von Heparin bis zur Maximaldosis (Gruppe H) oder in den Behandlungsarm mit dem Wechsel zu Argatroban (Gruppe A) randomisiert. 20 Patienten wurden in die Gruppe H und 22 in die Gruppe A randomisiert. Als Endpunkte waren das Erreichen des aPTT-Zielbereichs von 45-60 Sekunden innerhalb von 8 Stunden (Primärer Endpunkt) und von 24 Stunden (Sekundärer Endpunkt) definiert. Ergebnis: 60% der Patienten in der Gruppe H erreichten den aPTT-Zielbereich nach 8 Stunden, wobei nur 44% die angestrebte aPTT bis 24 Stunden halten konnten. In der Gruppe A konnten 64% den Zielbereich der aPTT innerhalb von 8 Stunden erreichen, nach 24 Stunden waren sogar 91% der Patienten mit einer Argatrobangabe im aPTT-Zielbereich. Der Behandlungserfolg (Primärer Endpunk, Background: Thromboembolism frequently occurs (incidence 40-80%) in critically ill patients. Therefore, a sufficient anticoagulation is crucial. The standard therapy for a continuous intravenous administration of anticoagulants is unfractionated heparin (UFH). However, some critically ill patients develop a so called heparin resistance (HR). HR is present, when, despite a high dose of heparin (1,200 IU/h), a sufficient anticoagulation, measured with the activated partial thromboplastin time (aPTT), cannot be achieved. There is no available anticoagulant registered for this indication. The aim of this study is to investigate, whether the administration of argatroban (Argatra®) is a superior antithrombotic therapy in comparison to a maximum dose of heparin regarding the elapsed time until sufficient anticoagulation. Methods: 42 evaluable critically ill patients with heparin resistance, treated at the General and Surgical Intensive Care Units of the Medical University Innsbruck, Austria, were included and randomized into either treatment with the standard therapy, which is increasing heparin to a maximum dose (group H), or with argatroban (group A). 20 patients were allocated to group H and 22 patients to group A. The endpoints were defined as achieving an aPTT-target range of 45 – 60 sec within 8 hours (primary endpoint) and 24 hours (secondary endpoint). Results: 60% of group H reached the target aPTT-level within 8 hours, whereas only 44% could maintain the target aPTT for 24 hours. 64% of group A achieved the target aPTT-range within 8 hours and after 24 hours 91% of the argatroban patients reached the target aPTT-level. The treatment success at 8 hours (primary endpoint) did not differ between groups (p=0.1000, two-sided Fisher’s exact test) whereas at 24 hours (secondary endpoint) group A was significantly superior (p=0.0021, two-sided Fisher’s exact test) to group H. Hintergrund: Thromboembolismus ist eine häufige und schwerwiegende Komplikation bei kritisch kranken Patienten mit einer Inzidenz von 40-80%. Daher ist eine effektive Antikoagulation unerlässlich. Die Standardtherapie für eine kontinuierlich intravenös verabreichte Gabe ist das Unfraktionierte Heparin (UFH). Allerdings entwickeln manche Patienten eine sogenannte Heparinresistenz (HR). HR ist so definiert, dass trotz hoher Dosen von UFH (1.200 IE/h) keine ausreichende Antikoagulation, welche anhand der aktivierten partiellen Thromboplastinzeit (aPTT) gemessen wird, erreicht werden kann. Es gibt keine alternative Medikation, die für diese Indikation registriert ist. Das Ziel dieser Studie ist herauszufinden, ob eine Gabe von Argatroban (Argatra®) besser wirkt als die maximale Dosis von Heparin, auch in Hinblick auf die Zeit bis zu einer adäquaten Antikoagulation. Methode: In die klinische Prüfung wurden 42 auswertbare kritisch kranke Patienten mit einer Heparinresistenz, welche an den Intensivstationen der Allgemeinen und Chirurgischen Intensivmedizin der Medizinischen Universität Innsbruck, Österreich, behandelt wurden, eingeschlossen und entweder in den Behandlungsarm mit der weiteren Gabe der Standardtherapie in Form einer Erhöhung von Heparin bis zur Maximaldosis (Gruppe H) oder in den Behandlungsarm mit dem Wechsel zu Argatroban (Gruppe A) randomisiert. 20 Patienten wurden in die Gruppe H und 22 in die Gruppe A randomisiert. Als Endpunkte waren das Erreichen des aPTT-Zielbereichs von 45-60 Sekunden innerhalb von 8 Stunden (Primärer Endpunkt) und von 24 Stunden (Sekundärer Endpunkt) definiert. Ergebnis: 60% der Patienten in der Gruppe H erreichten den aPTT-Zielbereich nach 8 Stunden, wobei nur 44% die angestrebte aPTT bis 24 Stunden halten konnten. In der Gruppe A konnten 64% den Zielbereich der aPTT innerhalb von 8 Stunden erreichen, nach 24 Stunden waren sogar 91% der Patienten mit einer Argatrobangabe im aPTT-Zielbereich. Background: Trauma-induced coagulopathy (TIC) substantially contributes to mortality in bleeding trauma patients., Objective: The aim of the study was to administer fibrinogen concentrate in the prehospital setting to improve blood clot stability in trauma patients bleeding or presumed to bleed., Design: A prospective, randomised, placebo-controlled, double-blinded, international clinical trial., Setting: This emergency care trial was conducted in 12 Helicopter Emergency Medical Services (HEMS) and Emergency Doctors' vehicles (NEF or NAW) and four trauma centres in Austria, Germany and Czech Republic between 2011 and 2015., Patients: A total of 53 evaluable trauma patients aged at least 18 years with major bleeding and in need of volume therapy were included, of whom 28 received fibrinogen concentrate and 25 received placebo., Interventions: Patients were allocated to receive either fibrinogen concentrate or placebo prehospital at the scene or during transportation to the study centre., Main Outcome Measures: Primary outcome was the assessment of clot stability as reflected by maximum clot firmness in the FIBTEM assay (FIBTEM MCF) before and after administration of the study drug., Results: Median FIBTEM MCF decreased in the placebo group between baseline (before administration of study treatment) and admission to the Emergency Department, from a median of 12.5 [IQR 10.5 to 14] mm to 11 [9.5 to 13] mm (P = 0.0226), but increased in the FC Group from 13 [11 to 15] mm to 15 [13.5 to 17] mm (P = 0.0062). The median between-group difference in the change in FIBTEM MCF was 5 [3 to 7] mm (P < 0.0001). Median fibrinogen plasma concentrations in the fibrinogen concentrate Group were kept above the recommended critical threshold of 2.0 g l-1 throughout the observation period., Conclusion: Early fibrinogen concentrate administration is feasible in the complex and time-sensitive environment of prehospital trauma care. 45. A Prospective Pilot Trial to Assess the Efficacy of Argatroban (Argatra ® ) in Critically Ill Patients with Heparin Resistance.

The current study aims to evaluate whether prophylactic anticoagulation using argatroban or an increased dose of unfractionated heparin (UFH) is effective in achieving the targeted activated partial thromboplastin time (aPTT) of more than 45 s in critically ill heparin-resistant (HR) patients. Patients were randomized either to continue receiving an increased dose of UFH, or to be treated with argatroban. The endpoints were defined as achieving an aPTT target of more than 45 s at 7 h and 24 h. This clinical trial was registered on clinicaltrials.gov (NCT01734252) and on EudraCT (2012-000487-23). A total of 42 patients, 20 patients in the heparin and 22 in the argatroban group, were included. Of the patients with continued heparin treatment 55% achieved the target aPTT at 7 h, while only 40% of this group maintained the target aPTT after 24 h. Of the argatroban group 59% reached the target aPTT at 7 h, while at 24 h 86% of these patients maintained the targeted aPTT. Treatment success at 7 h did not differ between the groups ( p = 0.1000), whereas at 24 h argatroban showed significantly greater efficacy ( p = 0.0021) than did heparin. Argatroban also worked better in maintaining adequate anticoagulation in the further course of the study. There was no significant difference in the occurrence of bleeding or thromboembolic complications between the treatment groups. - Abstract
Background and Aim : Although fibrinogen has been established as a key player in the process of coagulation, many questions about the role of fibrinogen under specific conditions remain. Confocal microscopic assessment of clot formation, and in particular the role that fibrinogen plays in this process, is commonly investigated using pre-labeled fibrinogen. This has a number of drawbacks, primarily that it is impossible to stain fibrin networks after their formation. The aim of the present study is to present an alternative for conveniently post-staining fibrin in a wide range of models/situations, in real time and with high resolution. Methods : We combined a peptide known to bind fibrin and linked it to fluorescein isothiocyanate (FITC), creating the FITC-Fibrin-Binding Peptide (FFBP). We subsequently tested its fibrin-binding capability in vitro under static conditions, as well as under simulated flow, using real-time live confocal microscopy. Results : Fibrin stained with FFBP overlaps with fibrin stained with fibrinogen pre-labeled with Alexa Fluor 647 following coagulation induction. In contrast to pre-labeled fibrinogen, FFBP also stains already formed fibrin networks. By combining FFBP with real-time live confocal microscopy even the visualization of single fibrin fibers is possible. Conclusions : These data indicate that FFBP is a valid and valuable tool for real-time live confocal assessment of clot formation. Relevance for patients: Our findings present a valuable alternative for the visualization of fibrin even after its formation, and we believe this approach will be particularly valuable for future investigations of important, but previously overlooked, aspects of clot formation., Competing Interests: C.
