Amaç: Üçüncü basamak kadın hastalıkları ve doğum kliniğimizde 2013 yılı içerisindeki toplam vajinal doğum ile sezaryen doğum sayılarını karşılaştırdık ve sezaryen doğumlarındaki endikasyon dağılımlarını, sezaryen saatleri ve tercih edilen anestezi şekillerini belirleyerek WHO standartlarının neresinde olduğumuzu bulmayı hedefledik. Gereç ve yöntem: 2013 yılı içerisinde hastanemizde doğum yapmış 6765 hastadan sezaryenle doğum yapmış olan 2736 tane olgunun endikasyonları, uygulanan anestezi metodları ve operasyon saatleri retrospektif olarak tarandı. Bulgular: Bir yıl içerisindeki 6765 doğum olgusunun 4029'u (%60) vajinal doğum, 2736'sı (%40) sezaryen doğum idi. Sezaryen endikasyonları içinde ilk sırada geçirilmiş uterin cerrahi (%46), 2. sırada fetal distress (%16), 3. sırada ise başpelvis uyumsuzluğu (%11) tespit edildi. Olguların %89'una spinal anestezi uygulanırken sadece %11'inde genel anestezi tercih edilmişti. Sonuç: Hastanemiz üçüncü basamak referans bir hastane olmasına rağmen, sezaryen ile doğum oranlarımız (%40) Türkiye genelinin (%58,6) altındadır. Sezaryen endikasyonları içinde ilk sırada geçirilmiş uterin cerrahinin olması sezaryen oranlarını düşürmek için asıl çözümün primer sezaryen oranlarının düşürülmesi olduğunu bir kez daha gözler önüne sermiştir, Objective: We compared the number of vaginal deliveries with cesarean section deliveries in 2013 in our tertiary care clinic and determined the rate of cesarean indications, lengths of operations, and anesthesia technique chosen for the cesarean deliveries to ascertain our fit within the WHO standards. Methods: We retrospectively evaluated 2736 patients whose babies were delivered by cesarean section out of a total of 6765 births. The indications for cesarean section, lengths of operations, and anesthesia methods were recorded. Results: Of the 6765 births, 4029 (60%) normal vaginal births and 2736 (40%) cesarean births occurred in the one-year period. The most common indication for cesarean delivery was previous uterine surgery (46%), followed by fetal distress (16%) and cephalo-pelvic disproportion (11%). Regional anesthesia was performed in 89% and general anesthesia was used in 11% of the cesarean births. Conclusion: Although our hospital is a reference tertiary care clinic, our cesarean rate (40%) is lower than that in Turkey (58.6%). Most cesarean deliveries were indicated by previous uterine surgery, so a key way to reduce them is to decrease the number of primary cesarean deliveries

Sağlık kuruluşları pekçok riskin yer aldığı tehlikeli ve çok tehlikeli bir iş koludur. Bu çalışmada, tıp fakültesi öğrencilerinin iş sağlığı ve güvenliği konusunda bilgi, tutum ve davranışlarının araştırılması amaçlanmıştır. . Kesitsel tipte planlanan bu çalışmada, Gülhane Askeri Tıp Fakültesi'nde öğrenim gören bütün öğrenciler çalışmanın evrenini oluşturdu (N=754). Açık ve kapalı uçlu 20 adet sorudan oluşan bir soru formu öğrencilere uygulandı. Çalışmaya katılım oranı %82,4 (622 kişi), katılımcıların yaş ortalaması 21,5±1.84 yıl, %94,1'i erkek idi. Öğrencilerin %41,0'i iş sağlığı ve güvenliği hakkındaki bilgi düzeyini `az/çok az` olarak nitelendirdi. Öğrencilerin %95,0'i şu ana kadar herhangi bir meslek hastalığına yakalanmadığını, %76,7'si ise herhangi bir iş kazası geçirmediğini ifade etti. Katılımcıların %29,7'sinin, tıp fakültesi eğitimine başladığından beri en az bir kez aşı yaptırdığı saptandı. Aşı yaptırma durumu 1. sınıflara göre 5.sınıf öğrencilerinde 92 kat, 6.sınıf öğrencilerinde 63 kat daha fazla idi [sırasıyla OR=92,66 %95 GA (26,70 – 321,59), OR=63,01 %95 GA (17,96 – 221,01)]. Öğrencilerine göre hastane ortamında çalışan sağlığı açısından en sık karşılaşılan 5 riskin bulaşıcı bir hastalığa yakalanma (%32,6), iğne ucu batması (%12,2), hijyen/asepsi eksikliği (%8,7), şiddete maruz kalma (%7,4) ve dikkatsizlik (%5,0) olduğu belirlendi. Tıp fakültesi öğrencilerinin iş sağlığı ve güvenliği ile ilgili bilgi düzeyleri geliştirilmeli, öğrenciler arasında düşük düzeyde olan bağışıklanma durumları iyileştirilmelidir. Health institutions, which have many risks, have been described as in the field of dangerous and very dangerous work. In this study, it was aimed to investigate knowledge, attitudes and behaviors of medical students regarding occupational health and safety. In this cross-sectional study, all the medical students studying at Gulhane Military Medical Faculty consisted of the study population (N = 754). Open and closed-ended questionnaire consisting of 20 questions was performed to the students. Mean age of the medical students, 94.1% of whom male, was 21.5 ± 1.84 years with a participation rate of 82.4%. Of all students, 41.0% described their level of knowledge as `low / very low` regarding occupational health and safety. The 95.0% of the students have not been caught any occupational disease, and the 76.7% of them stated that they had not have any work accident, until now. Of all participants, 29.7% had been vaccinated at least one shot since the start of training in the faculty of medicine. According to the state, Grade 5 and Grade 6 students had been vaccinated 92 times, and 63 times more likely than the Grade 1 students, respectively [OR = 92.66, 95% CI (26.70 to 321.59), OR = 63.01, 95% CI (17.96 to 221.01)]. According to the medical students, most frequently encountered five risks with regards to occupational health terms were infectious diseases (32.6%), needlestick injury (12.2%), deficiency of hygiene/asepsis (8.7%), exposure to violence (7.4%) and lack of attention (5.0%) in the hospital environment. Medical students' knowledge toward occupational health and safety should be developed, and low immunization status among students should be improved. 149

Objective: This study evaluated the current state of anxiety and depression levels in patients with HG and TA and compared the prevalence of anxiety and depression levels with healthy pregnant controls. The aim of this prospective casecontrol study was to investigate the possible relationship between anxiety, depression and HG and TA and compare the results with healthy pregnant controls. Patients and Methods: A prospective case–control study was performed at our tertiary referral centre. 84 consecutive women with HG and 88 consecutive women with TA constituted our study group and 98 healthy pregnant women constituted our control group. Beck Anxiety Inventory BAI and Beck Depression Inventory BDI were administered to patients during the psychiatric interview for the anxiety and depression evaluation. Results: The mean BAI scores in HG, TA and healthy control groups were 17.34 ± 8.97, 17.23 ± 8.71 and 7.03 ± 5.45, respectively. The mean BDI scores in HG, TA and healthy control groups were 15.54 ± 7.81, 16.27 ± 6.72 and 6.68 ± 5.28, respectively. Conclusion: The findings of this study indicated a potential link between HG, TA and anxiety and depression disorders. Therefore, patients with HG and TA during pregnancy should be evaluated in terms of anxiety and depression disorders as much as their medical conditions. Medical professionals should be sensitive to psychological consequences of HG and TA., Amaç: Bu çalışma hiperemezis HG ve düşük tehtidi Aİ olan hastaların mevcut anksiyete ve depresyon seviyelerini değerlendirmekte ve ayrıca anksiyete ve depresyon prevelanslarını sağlıklı gebe kontrolleri ile karşılaştımaktadır. Bu prospektif vaka-kontrol çalışmasının amacı anksiyete ve depresyon ile HG ve Aİ arasındaki olası ilişkiyi araştırmak ve bu sonuçları sağlıklı gebe kontrolleri ile karşılaştırmaktır. Hastalar ve Yöntemler: Bu prospektif vaka-kontrol çalışması üçüncü basamak referans hastenemizde gerçekleştirildi. Hiperemezisli 84 hasta ve düşük tehtidi olan 88 hasta çalışma grublarımız oluşturken; 98 sağlıklı gebe kadın ise kontrol grubumuzu oluşturdu. Hastalara anksiyete ve depresyon değerlendirmesi için psikiyatrik değerlendirme sırasında Beck Anksiyete Ölçeği BAI ve Beck Depresyon Ölçeği BDI uygulandı. Sonuçlar: Ortalama BAI skoru HG, Aİ ve sağlıklı kontrol grubunda sırasıyla 17.34 ± 8.97, 17.23 ± 8.71 ve 7.03 ± 5.45 idi. Ortalama BDI skoru HG, Aİ ve sağlıklı kontrol grubunda sırasıyla 15.54 ± 7.81, 16.27 ± 6.72 ve 6.68 ± 5.28 idi. Sonuç: Bu çalışmanın bulguları HG ve Aİ ile anksiyete ve depresyon arasında potansiyel bir bağlantı göstermektedir. Bu nedenle, gebelikte HG ve Aİ hastaları tıbbi durumları kadar anksiyete ve depresyon açısından da değerlendirilmelidirler. Klinisyenler HG ve Aİ’in psikolojik sonuçları konusunda duyarlı olmalıdırlar.
