In this study, a nanofiber surface loaded with apheresis platelet concentrate (APC) was produced for the first time to develop a bioactive wound dressing design. Nanofiber surface (n=5) consisting of polyurethane polymer outer layer, polyvinyl alcohol polymer middle layer, and polycaprolactone polymer matrix inner layer were produced via the electrospinning method. The surface morphologies of the produced nanofiber surfaces were examined by scanning electron microscopy. Quantitative analyzes of growth factors released from the APC-loaded composite nanofiber surfaces into phosphate-buffered saline at certain time intervals were performed with the ELISA. When the release amounts between bFGF, EGF, and PDGF-AA groups were compared, a significant difference was found in all periods (p<0.05). When the time-dependent release changes of each group were examined, there was no statistically significant difference in the bFGF group (p>0.05), but there was a significant difference between the EGF and PDGF-AA growth factors (p<0.05). Treatment of psoriasis differs in special cases such as in women who wish to become pregnant, during pregnancy and lactation, in pediatric and geriatric populations, in the presence of obesity or chronic viral diseases, and during the COVID-19 pandemic. Due to their well-known teratogenic effect, neither topical nor systemic use of retinoids is recommended in any period of pregnancy and lactation. Literature data on treatment approaches in psoriasis in children is less than in adults, and safety data usually relate to short-term use. There is a multifactorial relationship between psoriasis and obesity; response to treatment is lower in obese patients with a body mass index greater than 30. It should be noted that the elderly have comorbidities and concomitant drug use, which increases the likelihood of side effects and drug interactions of psoriasis medications. In the presence of a viral disease, many agents used in the treatment of psoriasis carry the risk of exacerbating the infection due to their immunosuppressive properties. In such a case, the option that can be considered apart from topical therapies is narrow band phototherapy. It was observed during the COVID-19 pandemic that there had been no increase in the risk of infection-related mortality and morbidity solely due to biological therapy. [ABSTRACT FROM AUTHOR]

Topical corticosteroids and vitamin D analogs can be combined with other topical, systemic, and biological agents. The effect of methotrexate can be enhanced with ultraviolet B (UVB) or psoralen ultraviolet A (PUVA). When a biological agent remains insufficient, the first agent to be added to the treatment is methotrexate. The use of acitretin in combination with phototherapy (both UVB and PUVA) has a synergistic effect. A UVB therapy may be combined with topical tar and anthralin as well as with etanercept and adalimumab, which are anti-tumor necrosis factor-α therapies. [ABSTRACT FROM AUTHOR]

Assessment of the severity of psoriasis is versatile, and unfortunately, there is no single tool that can assess the severity of this disease in every aspect. As well as psoriasis area and severity index, other tools such as physician's global assessment and body surface area are also used in assessing the severity of psoriasis. The index used to assess quality of life is the dermatology life quality index. For a drug to be defined as a biosimilar, it should be fairly similar to the previously approved original or reference product. The minor variances in its inactive ingredients should not pose clinically significant differences from the reference product in terms of safety and efficacy potency. Biosimilars of infliximab and adalimumab have received reimbursements in our country. Background and Design: Bullous pemphigoid (BP) is an autoimmune disease characterised by widespread itchy plaques and subepithelial blisterings and usually affects the elderly population. Due to the chronic nature of the disease, to prevent the side effects of chronic steroid treatment, adjuvant immunosuppressive (mycophenolate mofetil, azathioprine, methotrexate) and anti-inflammatory (tetracycline, nicotinamide, dapsone) agents may be used. In this study, we retrospectively evaluated the clinical and demographic characteristics and applied treatments of 46 patients with the diagnosis of BP and compared with literature data. Materials and Methods: We retrospectively evaluated the records of 46 patients who received clinical and histopathological diagnosis of BP and followed up in our clinic between 2006 and 2013. Results: Of the 46 patients, 22 were female and 24 male. The mean age of onset was 69.54 years (range: 18-105). The duration of the lesion ranged from 1 week to 10 months with a median duration of 8 weeks. The most frequent comorbid chronic disease was hypertension detected in 28 (60.8%) patients. Only four patients had a history of malignancy before the onset of the disease. Nineteen patients (42%) used more than 5 agents for comorbid diseases. Thirty-two patients (69.5%) used systemic corticosteroids alone and ten (22%) patients needed additional adjuvant therapies. Conclusion: BP is a major cause of morbidity in the elderly population receiving multiple drug treatment. To avoid the side effects of steroid therapy, especially in patients with severe disease, short-term use of additional immunosuppressive agents appears to be safe and effective. Background and Design: The Turkish Society of Dermatology, Dermatoallergy Study Group planned a survey study to search sources of knowledge and practices of dermatologists in the field of dermatoallergy and establish a basis for further educational activities and projects. Materials and Methods: In this survey, a questionnaire consisting of 42 questions was completed by the participants for detailed evaluation of their practices of patch, photopatch, prick, autologous serum skin testing, drug tests and occupational dermatitis, indications, number of applications, background sources, and allergen panel used. The survey was carried on either under the supervision of the group members in their area or by distributing and collecting the questionnaire during the Dermatoallergy panel of the 21st Prof. Dr. Lütfü Tat Symposium held in Ankara on 13-17 November 2013. Results: The results of our survey revealed that the most commonly used tests in dermatoallergy practice were prick test, patch test, drug test, autologous serum skin testing, and photopatch test, respectively. Conclusion: We found out that most of the dermatologists in Turkey have the sufficient knowledge and practice to perform dermatoallergy tests. Amaç: Psoriasisli hastalarda PsA taraması için değişken performansa sahip çeşitli araçlar geliştirilmiştir. PsA için optimal bir tarama aracı hala karşılanmamış bir ihtiyaçtır. Bu çalışma ile ilgili farklı tutulum tiplerini tespit edebilecek Türkçe yeni bir tarama aracı geliştirmeyi amaçladık. Yöntem: On bir romatolog/on dermatologdan oluşan bir çekirdek grup belirlendi ve PubMed'de 15 Ağustos 2020'ye kadar "psoriatik artrit" anahtar kelimesi kullanılarak sistematik bir literatür taraması yapıldı. İnceleme, PEST, PASE, EARP, STRIPP, SIPAS, SIPAT, TOPAS-II, GEPARD, PASQ, CONTEST, Roman, kısa ve basit tarama anketi adlı araçları ortaya çıkardı. Bu araçların her bir parçası Delphi setine dahil edildi. Delphi'nin 3 turundan sonra, yeni bir dizi tarama sorusu geliştirildi. Bulgular: Delphi'nin ilk turu için eklem, daktilit, entezit, omurga, deri-tırnak bölgelerinin yanı sıra sabah tutukluğu, işlev, tedavi ve diğerleri ile ilgili sorular da dahil olmak üzere toplam 85 soru soruldu. İlk tura katılan on yedi uzman (çekirdek gruptan 9 dermatolog/8 romatolog) ve on beş hasta (kadın/erkek= 9/6), ortalama (SS) yaşı 39,3 (10,9), tutulum tipleri periferik (%73,4), aksiyel (%40), enteseal (%33,4) ve daktilit %14) Delfi sorularını yanıtladı. Bu ilk değerlendirme sonucunda 85 sorudan 44'ü seçilerek ikinci tura taşındı. Bu sorular romatoloji ve dermatoloji dernekleri aracılığıyla üyelere iletildi. İkinci turda toplam 85 romatoloji uzmanı ve 48 dermatoloji uzmanı soruları yanıtladı. İkinci turda soru sayısı 44'ten 22'ye düşürüldü. İlk çekirdek grup içinde ikinci tur sonunda belirlenen 22 soruyu tartışmak için bir uzlaşma toplantısı yapıldı. Her soru tek tek ele alındı, bazı sorular birleştirilerek gerekirse Türkçeye uyarlandı ve soru setine son şekli verildi. Anketin son hali 6 sorudan oluşmaktadır (Tablo). Sonuç: Türkçe olarak psoriatik hastalıkta farklı tutulum alanlarını hedefleyen yeni bir tarama aracı geliştirildi. Taramada kültürel farklılıklar önemli bir rol oynamakla birlikte, Türkçe olarak geliştirilen ilk aracın klinik uygulama ve araştırma ortamlarında yardımcı olacağına inanıyoruz. Background and Design: Despite the development of effective systemic treatments and new biological agents for psoriasis nowadays, topical medications are still the mainstay of treatment. Topical calcineurin inhibitors are currently used in various skin diseases. We investigated the efficacy and safety of tacrolimus, which is an alternative in topical treatment, in comparison with the present medications in plaque psoriasis. Materials and Methods: This prospective double-blind placebo-controlled study was conducted in 24 patients with the diagnosis of plaque psoriasis who were seen in Uludağ University Hospital dermatology outpatient clinic. 0.1% tacrolimus ointment, 0.1% mometasone furoate ointment, 0.005% calcipotriol ointment and placebo in encrypted bottles were randomly applied under occlusion to 27 psoriatic plaques in test chambers, every other day, for a period of 19 days. Clinic sum scores, side effects, and epidermal thickness measured by superficial ultrasound were noted before and after treatment in all microplaques. Besides, histopathologic scoring and epidermal thickness were measured in 9 patients at the end of the study. Results: The clinic sum scores and ultrasonographic epidermal thickness at the end were both significantly lower than the beginning values in all microplaques (p<0.05). The reduction in these two values with tacrolimus were significantly higher than that with placebo (p<0.001), but no difference was observed with calcipotriol (p=0.287, p=0.813, respectively). On the other hand, the reduction in these values with mometasone was significantly higher than with tacrolimus (p<0.05). Mometasone furoate was the most effective when the three ointments were compared with placebo in terms of total histopathological score and epidermal thickness. Conclusion: Tacrolimus ointment applied under occlusion is an alternative topical medication in the treatment of plaque psoriasis. Occlusion in practice requires patient compliance.
