4. DIAGNOSIS OF HYPERTHYROIDISM WHEN SERUM-THYROXINE ALONE IS RAISED

Abstract

31 patients admitted to hospital with different non-thyroidal intercurrent diseases were found to have raised total serum-thyroxine (T4) and free T4 together with normal serum-triiodothyronine (T3) concentrations. At admission none was clinically hyperthyroid. Patients were assigned to 3 groups according to clinical course and the laboratory findings. In the first group (14 patients) classic hyperthyroidism developed after recovery from the intercurrent disease. 11 out of these 14 patients had recently received iodine-containing preparations. In a second group (11 patients) the initially raised serum-T4 rapidly returned to normal with recovery from the non-thyroidal disease. In a third group (6 patients) serum-T4 concentrations remained raised well after recovery from intercurrent disease. In this group, there were 2 cases of transient iodine-induced (Jod-Basedow) hyperthyroidism in which raised serum-T4 returned spontaneously to normal after several months as iodine was eliminated. These results indicate that increase in serum-T4 with normal serum- T3 in patients with intercurrent systemic disease is not always the result of hyperthyroidism and in many cases probably reflects changes in peripheral metabolism of T4. It is suggested that careful clinical follow-up is needed in patients with raised serum-T4 and normal serum-T3 for the early detection and treatment of classic hyperthyroidism.

10. Maternal and cord blood ferritin.

Abstract

The levels of plasma iron, transferrin saturation and ferritin in cord blood of 64 newborns were found to be elevated relative to the corresponding maternal blood. There was no correlation between the levels of plasma iron, transferrin saturation or ferritin in cord and maternal blood. No correlation was found in cord blood between the hemoglobin levels or red cell indices (MCV, MCH and MCHC) with the levels of ferritin. These results suggest that there is an active process of transfer for iron from mother to the fetus, and this transport is independent of fetal erythropoiesis.

Abstract

The levels of serum ferritin, folates, and vitamin B12 in relation to the hematologic values were measured in 66 pregnant women at term who had been supplemented with iron (100 mg/day) and folate (350 microgram/day). The levels of hemoglobin in pregnant women were similar to the controls;' however, levels of ferritin and transferrin saturation were decreased and showed a correlation with the hemoglobin. The MCV increased, whereas folate and vitamin B12 decreased, but no correlation was found between these values. These results suggest that in pregnant women (a) serum ferritin and transferrin saturation reflect the iron available for erythropoiesis, (b) the iron supplementation of 100 mg/day in the population studied is not sufficient, and (c) the macrocytosis is independent of the folate and vitamin B12 levels.

Abstract

The relation between the iron transmitted to infants by breast milk and the maternal iron stores was investigated in a group of 46 women. Total milk iron concentration was not correlated with the levels of serum ferritin or transferrin saturation. These results suggest that the iron transport from mother to the milk is irrespective to the maternal iron stores.

Abstract

The effect of thiamphenicol (a protein synthesis inhibitor) administration on iron absorption was studied in rabbits. Iron absorption was measured in a whole-body counter using 59Fe, and plasma iron turnover (PIT) was simultaneously determined with 55Fe. In comparison with the controls, the group receiving thiamphenicol showed no significant difference in PIT (p > 0.05), while iron absorption was increased (p < 0.0001).
