As stomatognatic system dysfunctions, hypertonias are quite common. As premedication, miorelaxants, adjuvants for inflammatory processes (which result in antalgic hypertonic phenomena), and during local anesthesia through neuromuscular transmission blockage, miorelaxants are a class of pharmacological agents with various structures and mechanisms of action that are highly helpful in treating stomatognatic dysfunctions. The most current medication utilized for stomatognatic system miorelaxation is Botox, which has a favorable local administration impact in a variety of clinical circumstances and during stomatological operations. It also provides comfort during mandibulo-cranial repositioning and centric relation maneuvers. [ABSTRACT FROM AUTHOR]

Temporo-mandibular disorders (TMDs) represent a major cause of non-dental pain in the orofacial region, the effects of which may be presumed to form at the site where the greatest forces are exerted, and host resistance is least. The signs and symptoms of TMDs which are observed in patients with psychiatric easy disease observed and measured by anthropometrical methods and correct with specific treatment. The literature reviewed in the present work revealed that suggests a complex relationship between psychological status and treatment outcome and TMDs signs and symptoms in different groups of individuals persists and the specificity and morphological dates also. Timely identification and the management of this condition may improve the comfort and psychic status of the patients, underlined also in our research. [ABSTRACT FROM AUTHOR]

This article from the Romanian Journal of Oral Rehabilitation examines the relationship between hearing loss and mental health issues, as well as the treatment options for patients with hearing loss. The study focuses on the therapeutic implications of diagnosing and treating comorbid conditions such as hearing loss, mental disorders, and temporomandibular joint (TMJ) disorders. The research sample consisted of 28 patients with hearing loss who sought treatment for TMJ disorders. The article outlines the stages of the treatment plan and suggests that further testing is needed to determine the long-term effects of the treatment. It also discusses the potential role of orthognathic therapy in improving ear function and dental malocclusion. Temporomandibular disorders (TMDs) are a common condition associated with orofacial pain and can be linked to comorbid conditions such as headaches, tinnitus, and hearing loss. Physiotherapy treatments have been shown to be effective in treating TMDs and improving symptoms. Complex oral rehabilitation can lead to recovery and homeostasis in patients with TMDs. Management is the art to achieve goals using work, the intellect, the behavior of other people; it is a necessity imposed by the introduction and study of a new discipline that will contribute to a better organization of the health system and to the improvement of the quality of healthcare and of human health. Knowledge of management functions is the key to a successful activity. Pain is defined as an unpleasant sensory and emotional experience, generated by real or potential tissue damage, and having a multifactorial and multidimensional nature. The objectives of establishing effective analgesic therapy are: to improve the patient’s comfort and quality of care and to facilitate complete recovery. Human psyche, consciousness, takes shape gradually from birth to the end of life and one cannot know what one has not been taught or has not learned; one cannot understand something as long as the circumstances and education have not provided the appropriate means. The entire content of psychic, intellectual and emotional life is borrowed from social reality through activities and self-education. Skills are developed gradually through practice, their quality depending directly on the duration and quality of the practice. Necessities evolve in relation to the items we consume or use and the activities we undertake. Our feelings are modeled on the social relations in which we engage. Even goals, that would seem to have as a source the depths of our being, are actually learned in the context of human existence. The capacity of voluntary exercise also depends on the lifestyle and activity experienced by the human being as a child, a young person or an elderly one. Although mental disorders in the elderly have some special features, they do not differ substantially from the ones in younger adults. What distinguishes elderly psychiatric patients from other patients is their needs. Material and method: The sample investigation includes a total of 57 cases to have dental treatment at the University Clinic of Dentistry in Iasi. The sample includes 24 women (42.10%) and 33 men (57.89%). Results and discussion: Although finally realized, pain remains up to this day difficult to quantify, mainly due to the involvement of a major affective component with a high degree of subjectivity; it is a matter of evolutionary physiology; the sensitivity to pain is considered to be an acquisition of phylogenesis, even if limited in some lineages. Conclusions: The majority of anxious patients present to a dentist in the final stage of development of a dental periodontal disease or after repeated painful crises of a condition of the pulp; no anxious patient is willing to treat carious lesions.

The temporomandibular joint (TMJ) imbalances and dysfunctions can lead to a group of different affections named temporomandibular disorders (TMDs). Elderly with TMDs are experiencing severe phenomena due to instability centric relationship, craniomandibular malrelations, hypotonia, edentation and general chronic diseases. The aim of our study is to investigate the efficiency of the pharmacologic treatment followed by prosthetic-aesthetic and gnathological treatment in elderly patients. This prospective study is based on data obtained from 96 patients, aged 65-74, 67 female and 29 male. The data was analyzed by correlating the independent variables with the dependent ones and the prevalence of TMDs. Study results revealed a high prevalence of TMDs (27.35%). The prevalence of TMJ pain (81.25%) and depressive manifestations were high (47.92%). After the pharmacologic medication, in association with prosthetic-aesthetic and gnathological treatment, the prevalence of depressive manifestations decreased (at 32.29%). In conclusion, the pharmacologic medication followed by prosthetic-aesthetic and gnathological treatment determine improved outcomes in elderly patients with TMDs not only in terms of pain and neuropsychiatric manifestations, but also clinically, i.e., at TMJ function level and in all the facial and oral aspects. The image of the dental offices and their promotion has acquired an important role in creating an identity and in developing the praxis. Along with the increasing dental services market also appeared a strong need for differentiation of dental offices/dental clinics. The study has set the goal of identifying the main promotion means and methods used in many of dental offices/dental clinics faced with the negative effects of economic crisis such as the reduction of the budget and the significant erosion of profits. The study group included 360 subjects, graduates and young dentists, aged between 25 and 34, of which 183 (50.83%) female and 177 (49.17%) male gender. In terms of qualification, 186 (53.30%) of those surveyed are graduates, and 174 (46.70%) are young dentists with licence to practise. The tool used was an opinion survey, whose questions wanted to be relevant to the research aim and objectives. The opinion survey was conducted from April 2016 to May 2017. The competition that currently exists in the dental market has led to differentiation of dental offices/ dental clinics and to creating an identity for each one of them. The image of the dental office/ dental clinic and institutional visual identity influence the first impression that the patient remembers and ultimately its "business card". The efficiency and success in dentistry are mainly the result of the quality of dental services and patient satisfaction. This depends on the dentist's professional training and on the endowment with modern and quality equipment of the dental office. Optimizing dentist-patient relationship has become important because it determines the professional and financial success on a medium and long term. [ABSTRACT FROM AUTHOR]

SPEECH disorders in children ,SELF-esteem ,SPEECH disorders in adolescence ,SOCIAL psychology ,PERSONALITY development - Abstract
Society nowadays is governed by important principles like performance, esthetic requirements that are aspects based on self esteem, a real parameter resulting from harmonious somatic development, where the oral-maxillofacial field has a distinct significance. Any impediment in the child's and adolescent's ways of communication corroborated with the intraoral status and the personality type, ultimately affects the child's and adolescent's social insertion, setting the path for the future adult's behavior and their academic and future professional performance. The research study included the correlation of incidence and prevalence of speech disorders with the defects of the frontal area represented by edentation or carious lesions, determining the impact on the personality development and social insertion. The speech disorders were detected through specific tests. The influence on personality development for this group of patients was assessed using questionnaires: the 16PF personality test (Cattell & Kline, 1977) with dichotomous questions, the personality test "Star of Self-esteem" with open questions and a test of psycho-sociological nature. Speech impediments correlated with frontal edentation, as a defect, abnormality in basic teeth structure, intervene in the natural process of formation and crystallization of self-esteem, causing negative thoughts regarding own self and own development abilities. On behavioral level, these negative thoughts turn into ways of hiding the defect by covering the mouth with the hand, speaking with lips tight or facing downwards, avoiding eye contact, sometimes in a low voice or even turning the head away while speaking, going as far as refusing to talk to someone. The older adults present different combinations of chronic multi-morbidity and functional limitations which determines health and social problems. The aim of the study was to assess the health status (general and oral) of the older adults because it can offer valuable information for estimation of the necessary health services. The targeted objectives were: the evaluation of oral morbidity, oral health related behaviors and attitudes towards oral health; evaluation of general morbidity; study of the addressability to health services and evaluation supplying medicines for rural area. From January-December 2016, we conducted a prevalence study focused on the main indicators of morbidity (general and oral). In our study were included 196 subjects, aged 65-74 years, 45.41% males, 54.59% females, from the rural community of Iasi County, in North Eastern region of Romania. In the studied group, the general morbidity model was the same for both male and female gender: in male gender, the morbidity was dominated by the cardiovascular diseases (63.24%), chronic digestive diseases (33.82%), diabetes mellitus (29.41%), osteoarticular diseases (27.94%), accidents and traumatisms (26.47%) and chronic respiratory diseases (23.53%); in female gender, it was also dominated by the cardiovascular diseases (67.82%), followed by diabetes mellitus (32.18%), osteoarticular diseases (31.03%) and chronic digestive diseases (24.14%). The oral morbidity model was relatively different between genders. At male gender, the predominant diseases were: dental caries (57.30%), lip and mucosal diseases (28.09%, OR=1.51, p<0.05), trauma (24.24%, OR=2.68, p<0.05) and total edentulism (25.84%). At female gender, the predominant diseases were: caries disease (59.81%), non-carious dental disorders (42.67%), periodontal disease (41.33%, OR=1.48, p<0.05) and total edentulism (29.91%). The global crisis has affected the Romanian economy during 2008-2014 being a key factor with a significant negative impact on the health system and population health. The aim of this study is to assess the impact of the crisis upon the Romanian health system as the main socio-economic determinant of population health and to explore the results of the austerity policies meant to control the negative consequences of the crisis. We have investigated the crisis impact on socioeconomic determinants of health taking into account the main macro-economic indicators registered during crisis period of time. In our analysis we used the population health indicators as health system outcomes. Romania occupied the last place in the EU as regards the health spending share from the Gross Domestic Product, with 5.68% in 2009, while the EU health spending grew up towards 8.9%. We pinpoint the decrease of the nominal spending of hospitals in 2010 comparing with 2009 (-10.55%), after the closure of 69 public hospitals. In 2009, the family medicine received a budget 24% lower than in 2008. The allocated revenue for medicine consumption in 2009 was of 2.18 billion RON (with 29.5% less than in 2008). The degradation of the health of the population was caused by the Romanian significant decrease of the economy, the economic crisis and its negative impact upon the Romanian health system. These factors generated the depreciation of the population health, the underfunding of the health system and impairment of the quality of health services. [ABSTRACT FROM AUTHOR]

Due to particularities related to anatomy, fiziology, health and adaptability, adolescents, especially those coming from the rural environment, are an important population group, coping with specific problems and vulnerabilities. The aim of this study targetted the awareness of certain health and social aspects with regards to health education for adolescents in rural environment. The main objective is focused on the evaluation of the cognitive level of health education regarding general and oro-dental health in adolescents from the rural area. The investigated group, which was statistically representative for the studied population, comprised of 160 persons (88 males, 72 females). The maximum accepted error was ±1.68%. We are focused on main indicators of general health, oro-dental morbidity and addressability towards health services. The responses to the health education questionnaire revealed that cognitive educational level is precarious in fields: the general health and oro-dental health. The prevalence of general morbidity (51.40%), obesity (20.56%) and the prevalence of smoking (6.54%) and alcohol (3.74%) consumption was assessed for knowledge of general health status. The prevalence of the dental caries (51.55%) and periodontal disease (29.90%) was assessed in order to evaluate the oro-dental morbidity. Adolescents from the rural environment represent a highly vulnerable population group that requires the implementation of a health promotion program at community level. Adolescents' health ought to be one of the primary concerns for the decision factors, with the purpose of ensuring an acceptable health level for future generations. [ABSTRACT FROM AUTHOR]

PERSONALITY disorders ,BEHAVIOR ,INTERPERSONAL relations ,PSYCHOSOCIAL factors ,VIOLENCE - Abstract
Personality disorders are represented by a behavioural pattern associated with recurrent violation of moral principles and social rules. Personality disorders - especially antisocial and borderline, but also the others, usually in unpredictable ways - are frequent diagnostics among the individuals psychiatrically analyzed at the request of authorities for committing violent infractions. Through their features are centred on maladjustment and on the desire to alter the environment according to their own structure, personality disorders lead to violent acts, sometimes extremely serious. Maladjustment pattern is part of a real vicious circle, which leads almost invariably to a socio-familial integrative deficit and to professional adjustment issues. In their turn, they entail increased frustrations, aggravated impulsive tendencies, as well as higher interpretability (when it is already present). Consequently, all these aspects favour the commission of violent, criminal acts, which end up compromising irremediably the social integration of individuals who suffer from a personality disorder. It is necessary to pinpoint the certain predictors of criminal behaviour, by the characteristics of each personality disorder, in order to assess their aggravating or intensifying factors and to prevent it effectively, for both the protection of potential victims and a better social integration of individuals diagnosed with various psychopathies. Obesity and depression represent two fundamental problems of public health at a global level; paediatric obesity is alarming both per se and through the risk of maintaining the obese status in adulthood and of continuing to be exposed to comorbidity, implicitly. On the other hand, depression in children is a genuine diagnostic problem (considering its masked clinical symptomatology) and a diagnostic necessity (considering its severe consequences and mostly the pathological alterations of food-related behaviour). This paper seeks to elaborate a synthesis of the current scientific literature regarding the causes of obesity -- depression comorbidities in children, with a focus on the interrelation and common etiopathogenic origin. Both functional abdominal pain and irritable bowel syndrome are common causes of chronic abdominal pain in children and adolescents. Addressing these nosological entities is done according to the biopsychosocial model of functional gastrointestinal disorders, given their effect on quality of life and psychosocial status thereof. The cognitive-behavioral therapy, family psychotherapy, relaxation techniques or hypnotherapy have proved effective in achieving better coping mechanisms in alleviating psychological and somatic symptoms. However, antidepressant medication including selective serotonin reuptake inhibitors and tricyclic antidepressants, improve psychiatric symptomatology, especially depressive mood due to gastrointestinal illness, but also digestive symptoms due to mechanisms of action that depend on complex neurotransmitters. The bipolar disorder becomes more and more frequent in the pediatric field, raising numerous questions regarding the diagnosis. In most cases, the standard criteria of the disease are not fully met in the case of a child, requiring their particularization and a more accurate classification. Also, a central issue is the differentiation and/or detection of comorbidity with ADHD and conduct disorders in children. [ABSTRACT FROM AUTHOR]

The frequency of suicidal behavior in children and adolescents has increased significantly in recent years, representing a plurifactorial phenomenon, with determining factors in both psychiatric disorders, especially depression and conduct disorders, and also environmental factors including dysfunctional families, academic failure, faulty relationships, including an issue of increasing interest which lies on the border between illness and social factor, represented by the abuse of psychoactive substances. Approaches remain unorganized and inadequate, thus maintaining a real interest in the folowing subject, child and adolescent suicide, and furthermore raising the issue of improving the means of effective intervention for this situation of global importance.
