2. Feasibility and reproducibility of new technique for measurement of transverse diameter of levator ani muscle hiatus using two‐dimensional transperineal ultrasound in nulliparous women with term pregnancy.

Objectives: The aim of the present study was two‐fold. Firstly, we aimed to develop and describe a technique for measurement of the transverse diameter (TD) of the levator ani muscle (LAM) hiatus in the coronal view using two‐dimensional (2D) transperineal ultrasound (TPUS) in nulliparous women with a term pregnancy. Secondly, we aimed to evaluate the feasibility and reproducibility of 2D‐TPUS assessment of LAM hiatal TD and assess intermethod agreement between 2D‐TPUS and three‐dimensional (3D) TPUS measurement of TD in the axial plane, which is considered the gold standard in nulliparous women with term pregnancy. Methods: We recruited a group of nulliparous women with term pregnancy before the onset of labor. The study was conducted in two phases: Phase 1 involved developing and describing the 2D‐TPUS technique for measuring LAM hiatal TD, and Phase 2 focused on assessing the technique's feasibility, reproducibility and intermethod agreement with 3D‐TPUS measurement of LAM hiatal TD. In Phase 1, we enrolled 30 women. Each woman underwent acquisition of a 3D‐TPUS volume, which was analyzed using multiplanar mode to identify and determine the appearance of the lateral borders of the LAM in the coronal plane, at the level of the plane of minimal hiatal dimensions. These borders were used as landmarks for TD measurement. Additionally, we measured the distance between the plane used for TD measurement and the center of the urethra in the axial view. In Phase 2, we recruited 100 women. Each woman underwent acquisition of three 2D‐TPUS videoclips in the coronal plane, each encompassing a sweep of the entire LAM hiatus, and a 3D volume, all obtained during rest. On the 2D videoclips, TD was measured twice by one operator and once by another operator. In the 3D volume, TD was measured once, by one operator, in the axial plane; this measurement was considered the gold standard. Each operator was blinded to all other measurements during their assessments. We analyzed intraobserver and interobserver reproducibility and performed an intermethod (2D vs 3D) comparison. Bland–Altman analysis was conducted, and Levene's W0 test and Student's t‐test were performed to explore clinical factors that might contribute to systematic differences. Results: In Phase 1, we identified successfully the landmarks denoting the lateral borders of the LAM hiatal TD in the coronal view. These appeared as two symmetrical hypoechogenic indentations located at the inner border of the hyperechogenic structure of the LAM, at the point of maximum distance between the two sides of the LAM. The distance between the urethra and the plane where TD should be measured using 3D‐TPUS in the axial plane had a median of 4 mm and varied from 0 to 9 mm. This enabled us to describe a method for assessing LAM hiatal TD in the coronal plane using 2D‐TPUS. In Phase 2, LAM hiatal TD was measured successfully in all 2D and 3D acquisitions from the entire group of 100 women. The analyses for intraobserver and interobserver reproducibility and the intermethod comparison (2D vs 3D) revealed almost perfect agreement in TD measurements using 2D‐TPUS, with intraclass correlation coefficients of 0.95 (95% CI, 0.92–0.96), 0.87 (95% CI, 0.78–0.92) and 0.85 (95% CI, 0.78–0.90), respectively. The average differences between measurements were 0.1 mm for intraobserver, 1.0 mm for interobserver and 0.2 mm for intermethod repeatability. No systematic differences were observed in any of the measurement sets, except in the interobserver analysis, although this difference was clinically not significant (38.2 vs 37.2 mm, P = 0.01). None of the examined clinical factors (maternal body mass index and maternal age) exhibited a statistically significant impact on intraobserver, interobserver or intermethod reliability. Conclusions: Utilizing our technique, described herein, to measure the LAM hiatal TD in the coronal view using 2D‐TPUS is not only feasible but also highly reproducible and accurate in nulliparous women with term pregnancy. Moreover, it yields measurements that are comparable to those obtained in the reconstructed axial plane generated by 3D‐TPUS. © 2024 International Society of Ultrasound in Obstetrics and Gynecology. Because of their peculiar genomic and chromosomal characteristics, reptiles are extraordinary model organ-isms to study karyotype and sex chromosome evolution, but despite the growing interest in their evolutionary cytoge-netics, only a small fraction of species have a known karyotype. We performed a comparative cytogenetic analysis on Elaphe quatuorlineata and Zamenis lineatus, using classic and molecular techniques. We provide the karyotype of these two species and an assessment of their chromosomal features. Chromosome analysis was performed with standard kar-yotyping, C-banding, sequential C-banding + CMA3 + DAPI and Ag-NOR staining. On E. quatuorlineata, we also per-formed CMA3-methyl green staining and Fluorescence in situ Hybridization mapping NOR loci (NOR-FISH). Elaphe quatuorlineata and Z. lineatus show a very similar karyotype of 2n = 36, with 8 macro- and 10 microchromosome pairs, but differ in the morphology of the pair 8, which resulted submetacentric in the former and metacentric in the latter species. By comparing our data to those available from the literature on congeneric species, we analysed the occurrence of primitive and derivate chromosomal characters and provide cytotaxonomic insights, which further support the spe-cies status of Z. lineatus. In both species, the 4th pair was identified as the sex chromosome pair (ZZ/ZW) and NORs were localized on a microchromosome pair. We finally highlight in both genera Elaphe and Zamenis different stages of heterochromatinization of the W chromosome, in agreement with the progressive diversification model of sex chromo-some as already shown in different reptile taxa.

AbstractDespite the growing interest in the evolutionary cytogenetics of squamates, chromosomal data are lacking for most taxa. We performed a preliminary molecular taxonomic analysis and a comparative cytogenetic study on Hemorrhois hippocrepisand Malpolon monspessulanus. We used a combination of standard karyotyping, Chromomycin A3/Methyl green staining, C-banding, Ag-NOR staining and NOR-FISH to provide the first karyotype description of H. hippocrepisand a re-description of the karyotype of M. monspessulanus, including chromosome markers, heterochromatin patterns and sex chromosome systems. Our results show that H. hippocrepishas 2 n = 36 chromosomes, with 16 macro- and 20 microchromosomes and NORs on the 6thpair. The 4thpair represents homomorphic (metacentric) ZW sex chromosomes, but the W chromosome is completely heterochromatic. Malpolon monspessulanushas 2 n = 44 chromosomes, with 20 macro- and 24 microchromosomes, NORs on the 6thtelocentric pair. The 4thpair represents the sex chromosomes (ZZ/ZW), with a W chromosome smaller than the Z and completely heterochromatic. Comparing our cytogenetic data to those available from the literature, we note the occurrence and distribution of primitive and derived chromosomal characteristics and discuss the chromosome diversification in two snake clades belonging to Colubridae and Psammophiidae, respectively. We highlight that these two families followed different chromosome diversification pathways, characterised by a highly conserved karyotype structure in Colubridae and a higher chromosome variability in Psammophiidae, mostly driven by a progressive reduction of the chromosome number by means of chromosome fusions. We also provide cytotaxonomic insights supporting the distinction between M. monspessulanusand M. insignitus.

This article discusses a study that aimed to evaluate the reproducibility of measurements of breech progression angle (BPA) using transperineal ultrasound (US). The study involved providing 3D ultrasound volumes to eight operators from four maternity units in Spain and Italy. The operators measured the BPA using 3D US volume processing software, and the interobserver reproducibility was assessed. After an online review and the development of an image-based score for BPA measurement, the reproducibility improved. The study concludes that the measurement of BPA is feasible and reproducible when using a standardized image-based score. [Extracted from the article]

This article, titled "Perinatal outcomes of early Twin–twin transfusion syndrome treated with laser: an international multicentre study," presents the results of a study on the perinatal outcomes of monochorionic diamniotic (MCDA) twin pregnancies with early Twin–twin transfusion syndrome (TTTS) that were treated with laser surgery. The study included 542 pregnancies diagnosed with early TTTS, with 12.2% of the women diagnosed at or before 16 weeks. The majority of cases (85.4%) were at stage II or III at diagnosis, and 81.5% of the cases were treated with laser before 18 weeks. The study found that laser surgery, even when performed before 18 weeks, was associated with a good outcome, with at least one surviving fetus in 67.4% of pregnancies and two surviving fetuses in 43.6% of pregnancies. Introduction: The nasal obstruction is potentially severe when affecting newborns, preferential nasal breathers. The newborns with nasal obstruction may present from an asymptomatic affection up to a severe situation of airway obstruction, with cyclical cyanosis. The cyanosis worsens with feeding and improves with crying. Despite the most common cause of obstruction in the newborn is mucosa edema secondary to viral rhinitis or idiopathic rhinitis of the child, it is important to be attentive to the diagnosis of the nasal anatomic alterations. Although not much frequent, they represent affections in which the early diagnosis and management are basic to prevent airway obstruction and feeding difficulties with recurrent aspiration. Objective: The objective of this case report is to describe the three most common causes of anatomic nasal obstruction in newborns: the choanal atresia, dacryocystocele and anterior piriform opening stenosis. Final Comments: The clinical characteristics, diagnostic investigation and treatment of these three pathologies are presented. Therefore, we seek to alert as to the importance that the nasal probe passage be part of the physical exam of every newborn, specially when it has breathing disorder upon birth and/or cyanosis when breastfeeding.

OBJETIVO: Revisar evidências sobre fatores de risco modificáveis para otite média aguda recorrente. FONTE DOS DADOS: MEDLINE sem restrição de linguagem de janeiro de 1966 até julho de 2005, utilizando descritores "acute otitis media/risk factors". Obtiveram-se 257 artigos. Desses, incluíram-se ensaios clínicos randomizados, coortes, estudos de caso-controle e transversais que tiveram análise dos fatores de risco modificáveis para desenvolvimento de otite média aguda recorrente como objetivo principal e com amostras de indivíduos de até 18 anos. Excluíram-se, exceto quando relevantes, revisões não-sistemáticas, relatos de casos e série de casos, além de diretrizes de sociedades médicas. SÍNTESE DOS DADOS: Identificaram-se nove fatores de risco ligados ao hospedeiro e oito ligados ao ambiente. Do primeiro grupo, classificaram-se como modificáveis alergia, anormalidades craniofaciais, refluxo gastroesofágico e presença de adenóides. Na segunda categoria, incluíram-se infecção de vias aéreas superiores, cuidados em creches, presença de irmãos/tamanho da família, fumo passivo, aleitamento materno e uso de chupetas. Posteriormente, classificaram-se os fatores de risco de acordo com níveis de evidência. CONCLUSÕES: Os fatores de risco estabelecidos para otite média aguda recorrente e passíveis de intervenção foram uso de chupetas e cuidados em creche. Os fatores de risco prováveis foram privação do leite materno, presença de irmãos, anormalidades craniofaciais, fumo passivo e presença de adenóides. Nenhum fator modificável foi classificado como pouco provável. Entre os que precisam ser melhor estudados estão alergia, refluxo gastroesofágico e fumo passivo na gestação.OBJECTIVE: Review evidence about modifiable risk factors for recurrent acute otitis media. SOURCE OF DATA: MEDLINE with no language restriction, from January 1966 to July 2005, using descriptors "acute otitis media/risk factors". Two hundred and fifty-seven articles were obtained. These included randomized clinical trials, cohorts, case-control and cross-sectional studies that contained analyses of modifiable risk factors for the development of recurrent acute otitis media as the main objective and with samples of individuals up to the age of 18 years. Except when relevant, the following were excluded: non-systematic reviews, reports of cases, series of cases, and medical society guidelines. SUMMARY OF DATA: Nine risk factors linked to the host and eight linked to the environment were identified. Of the first group, allergy, craniofacial abnormalities, gastroesophageal reflux and the presence of adenoids were classified as modifiable. In the second category, upper airway infections, day care center attendance, presence of siblings/family size, passive smoking, breastfeeding and use of pacifiers were included. Afterwards, the risk factors were classified in accordance with levels of evidence. CONCLUSIONS: The risk factors established for recurrent acute otitis media and capable of being modified were the use of pacifiers and care in daycare centers. The probable risk factors were privation of mother's milk, presence of siblings, craniofacial abnormalities, passive smoking and presence of adenoids. No modifiable factor was classified as unlikely. Among those that need further study are allergy, gastroesophageal reflux and passive smoking during gestation.

- Abstract
Objective: We estimated the cost consequence of Italian National Health System (NHS) investment in direct-acting antiviral (DAA) therapy according to hepatitis C virus (HCV) treatment access policies in Italy. Methods: A multistate, 20-year time horizon Markov model of HCV liver disease progression was developed. Fibrosis stage, age and genotype distributions were derived from the Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. The treatment efficacy, disease progression probabilities and direct costs in each health state were obtained from the literature. The break-even point in time (BPT) was defined as the period of time required for the cumulative costs saved to recover the Italian NHS investment in DAA treatment. Three different PITER enrolment periods, which covered the full DAA access evolution in Italy, were considered. Results: The disease stages of 2657 patients who consecutively underwent DAA therapy from January 2015 to December 2017 at 30 PITER clinical centres were standardized for 1000 patients. The investment in DAAs was considered to equal €25 million, €15 million, and €9 million in 2015, 2016, and 2017, respectively. For patients treated in 2015, the BPT was not achieved, because of the disease severity of the treated patients and high DAA prices. For 2016 and 2017, the estimated BPTs were 6.6 and 6.2 years, respectively. The total cost savings after 20 years were €50.13 and €55.50 million for 1000 patients treated in 2016 and 2017, respectively. Conclusions: This study may be a useful tool for public decision makers to understand how HCV clinical and epidemiological profiles influence the economic burden of HCV.

OBJECTIVE:\ud We estimated the cost consequence of Italian National Health System (NHS) investment in direct-acting antiviral (DAA) therapy according to hepatitis C virus (HCV) treatment access policies in Italy.\ud \ud METHODS:\ud A multistate, 20-year time horizon Markov model of HCV liver disease progression was developed. Fibrosis stage, age and genotype distributions were derived from the Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. The treatment efficacy, disease progression probabilities and direct costs in each health state were obtained from the literature. The break-even point in time (BPT) was defined as the period of time required for the cumulative costs saved to recover the Italian NHS investment in DAA treatment. Three different PITER enrolment periods, which covered the full DAA access evolution in Italy, were considered.\ud \ud RESULTS:\ud The disease stages of 2657 patients who consecutively underwent DAA therapy from January 2015 to December 2017 at 30 PITER clinical centres were standardized for 1000 patients. The investment in DAAs was considered to equal €25 million, €15 million, and €9 million in 2015, 2016, and 2017, respectively. For patients treated in 2015, the BPT was not achieved, because of the disease severity of the treated patients and high DAA prices. For 2016 and 2017, the estimated BPTs were 6.6 and 6.2 years, respectively. The total cost savings after 20 years were €50.13 and €55.50 million for 1000 patients treated in 2016 and 2017, respectively.\ud \ud CONCLUSIONS:\ud This study may be a useful tool for public decision makers to understand how HCV clinical and epidemiological profiles influence the economic burden of HCV.

This article, titled "EP19.03: Narrower subpubic arch angle is associated with breech presentation in nulliparous women at term," explores the correlation between the subpubic arch angle (SPA) and fetal breech presentation in nulliparous women at term. The study involved 124 nulliparous women at term with singleton pregnancies, and a three-dimensional transperineal ultrasound volume was acquired for each woman. The results showed that women with a fetus in breech presentation had significantly narrower SPA values compared to women with a cephalic fetus. Additionally, women with a fetus in breech presentation had a lower body mass index (BMI) compared to women with a fetus in cephalic presentation. The study concludes that breech presentation is associated with a narrower subpubic arch angle in nulliparous women at term. [Extracted from the article]

This article, published in the journal Ultrasound in Obstetrics & Gynecology, examines the correlation between levator ani muscle coactivation and breech presentation in nulliparous women at term. The study found that women with fetuses in breech presentation had smaller anteroposterior diameter (APD) of the levator ani muscle (LAM) under pushing, a higher prevalence of LAM coactivation, and a smaller change in APD from rest to maximum pushing compared to women with fetuses in cephalic presentation. The results suggest that proper pelvic floor relaxation may favor fetal cephalic presentation, while levator ani muscle coactivation and smaller increase in APD at maternal pushing are associated with an increased risk of breech presentation. This article discusses a case study of a 37-year-old woman who was referred to a medical center during her third trimester of pregnancy. An ultrasound scan revealed the presence of an intra-abdominal mass near the abdominal wall and umbilical cord insertion. The mass was diagnosed as an isolated fetal intra-abdominal umbilical vein varix (FIUVV) using color Doppler ultrasound. Three-dimensional reconstruction of the varix was performed, confirming its continuity with the umbilical vein and ruling out complications such as thrombosis. The woman gave birth to a healthy baby without any complications, who is now 9 months old and asymptomatic. [Extracted from the article]

This article examines the agreement between measurements of the anteroposterior diameter (APD) of the levator ani muscle hiatus using 2D and automated 3D transperineal ultrasound techniques. The study recruited nulliparous women at term before labor and measured APD using both techniques. The results showed a high agreement between the two methods, with an intraclass correlation coefficient (ICC) of 0.91. Although there was a small systematic difference between the measurements, it was clinically irrelevant. This article discusses a study on early Twin-Twin Transfusion Syndrome (TTTS) and the factors that predict single and double survival after laser surgery for TTTS. The study analyzed data from 484 cases of early TTTS undergoing laser surgery and found that coexisting selective fetal growth restriction (sFGR) and postoperative preterm premature rupture of membranes (PPROM) were independent predictors of survival. However, factors such as placental location, Quintero stage, and technique of laser ablation did not significantly impact survival.
