Purpose: To describe age- and sex-related differences in COVID-19 symptoms in patients admitted to emergency departments during the four waves of the pandemic in Central Northern Bulgaria Materials and methods: Retrospective analysis of data collected from 5382 outpatients with positive result for SARS-CoV-2 by RT-PCR, admitted to 11 emergency departments in Central Northern Bulgaria, was carried out. Results: The median age of patients was 64 years, and 60.6% of them were old adults (≥ 60 years). Overall, more patients were males (51.8%) than females (48.2%). The most frequent symptoms were fever (84.8%), dyspnea (72.3%), and cough (69.9%) without significant difference in both sexes. Less common symptoms, such as sore throat, headache, loss of taste/smell, and fatigue, were found in 1.3% to 4.0% of patients and were more prevalent in women (p≤0.01). Fever remained constant, leading symptom in all ages. The incidence of cough and dyspnoea was less in young patients and showed an increase in middle-aged and old adults. Loss of taste and smell, muscle pain and fatigue displayed the opposite trend. The neutron-rich rare isotope 190W is discussed as a candidate for a prolate-oblate transitional nucleus with maximum γ-softness. The collectivity of this isotope is assessed for the first time by the measurement of the reduced E2 transition probability of its first 2+ state to the ground state. The experiment employed the FAst TIming Array (FATIMA), comprised of 36 LaBr3(Ce) scintillators, which was part of the DESPEC setup at GSI, Darmstadt. The 41+ and 21+ states of 190W were populated subsequently to the decay of its 127(12) μs isomeric Jπ=10− state. The mean lifetime of the 21+ state was determined to be τ=274(28) ps, which corresponds to a B(E2;21+→01+) value of 95(10) W.u. The results motivated a revision of previous calculations within an energy-density functional-based interacting boson model-2 approach, yielding E2 transition properties and spectroscopic quadrupole moments for tungsten isotopes. From comparison to theory, the new data suggest that 190W is at the transition from prolate to oblate structure along the W isotopic chain, which had previously been discussed as a nuclear shape-phase transition.

Reduced transition probabilities have been extracted between excited, yrast states in the N=Z+2 nucleus 94Pd. The transitions of interest were observed following decays of the Iπ=14+, Ex=2129-keV isomeric state, which was populated following the projectile fragmentation of a 124Xe primary beam at the GSI Helmholtzzentrum für Schwerionenforschung accelerator facility as part of FAIR Phase-0. Experimental information regarding the reduced E2 transition strengths for the decays of the yrast 8+ and 6+ states was determined following isomer-delayed Eγ1−Eγ2−△T2,1 coincidence method, using the LaBr3(Ce)-based FATIMA fast-timing coincidence gamma-ray array, which allowed direct determination of lifetimes of states in 94Pd using the Generalized Centroid Difference (GCD) method. The experimental value for the half-life of the yrast 8+ state of 755(106) ps results in a reduced transition probability of B(E2:8→+6+) = 205−25+34 e2fm4, which enables a precise verification of shell-model calculations for this unique system, lying directly between the N=Z line and the N=50 neutron shell closure. The determined B(E2) value provides an insight into the purity of (g9/2)n configurations in competition with admixtures from excitations between the (lower) N=3pf and (higher) N=4gds orbitals for the first time. The results indicate weak collectivity expected for near-zero quadrupole deformation and an increasing importance of the T=0 proton-neutron interaction at N=48.

Objective: Early diagnosis of nosocomial infections in newborns is a great challenge, because in the initial phase of systemic infection, clinical symptoms are often non-specific, and routinely used hematological markers are not sufficiently informative. The aim of this study was to determine the potential of early inflammatory markers to diagnose late-onset neonatal sepsis—procalcitonin (PCT), interleukin 6 (IL-6), interleukin 8 (IL-8) and endocan (ESM-1). Material and methods: A prospective clinical–epidemiological study was conducted in a third-level NICU in Pleven, Bulgaria. Patients with suspected late-onset sepsis and healthy controls were tested. A sandwich ELISA method was used to measure the serum concentrations of biomarkers. Results: Sixty newborns were included, of which 35% symptomatic and infected, 33.3% symptomatic but uninfected and 31.7% asymptomatic controls. The mean values of PCT, IL-6, I/T index and PLT differ significantly in the three groups. For ESM-1, IL-8 and CRP, the difference was statistically insignificant. The best sensitivity (78%) and negative predictive value (84%) was found for IL-6. The combinations of PCT + IL-6 and PCT + IL-6+ I/T+ PLT showed very good diagnostic potential. Conclusion: The introduction into the routine practice of indicators such as PCT and IL-6 may provide an opportunity to promptly optimize the diagnostic and therapeutic approach to LOS.

The g factors of 11/2− isomers in semimagic 109Sn and 111Sn (isomeric lifetimes τ=2.9(3) ns and τ=14.4(7) ns, respectively) were measured by an extension of the Time Differential Perturbed Angular Distribution technique, which uses LaBr3 detectors and the hyperfine fields of a gadolinium host to achieve precise measurements in a new regime of short-lived isomers. The results, g(11/2−;109Sn)=−0.186(8) and g(11/2−;111Sn)=−0.214(4), are significantly lower in magnitude than those of the 11/2− isomers in the heavier isotopes and depart from the value expected for a near pure neutron h11/2 configuration. Broken-symmetry density functional theory calculations applied to the sequence of 11/2− states reproduce the magnitude and location of this deviation. The g(11/2−) values are affected by shape core polarization; the odd 0h11/2 neutron couples to Jπ=2+,4+,6+... configurations in the weakly-deformed effective core, causing a decrease in the g-factor magnitudes.

The uterine microbiota has been the subject of increasing study, but its interaction with the local immune system remains unclear. Successful embryo implantation relies on endometrial receptivity, which is pivotal for immunological tolerance to fetal antigens and precise regulation of inflammatory mediators. Emerging data suggest a dynamic interplay between endometrial microflora and the immune system, making dysbiosis a potential determinant of pregnancy outcomes. Imbalances in the regulation of immune cells in the endometrium and decidua have been associated with infertility, miscarriage, and obstetric complications. A thorough comprehension of the immune system in the female reproductive tract shows potential for improving women’s health and pregnancy outcomes. The objective of this study was to evaluate the patterns of endometrial microbiota in patients with recurrent implantation failure (RIF) and recurrent pregnancy loss (RPL) and to explore their implications for endometrial immune cells and chronic endometritis (CE). Immune cells in biopsies from 107 RIF and 93 RPL patients were examined using flow cytometry. The endometrial microbial composition was analyzed using real-time polymerase chain reaction (RT-PCR). The research uncovered disrupted endometrial microbiota in most women with RIF and RPL, which was often associated with significant effects on lymphocytes, T cells, and uNK cells.

The increased glycation of elastin is an important factor in vascular changes in diabetes. Using the ELISA method, we determined serum levels of IgM and IgG autoantibodies to advanced glycation end products of vascular elastin (anti-AGE EL IgM and anti-AGE EL IgG) in 59 hypertensive patients with type 2 diabetes (T2D) and 20 healthy controls. Serum levels of matrix metalloproteinases-2 and -9 (MMP-2 and MMP-9) and the C-reactive protein (CRP) were also determined. The levels of anti-AGE EL IgM antibodies in the T2D group were similar to those in the control group, while those of anti-AGE EL IgG antibodies were significantly higher (p = 0.017). Significant positive correlations were found between the levels of anti-AGE EL IgM antibodies and MMP-2 (r = 0.322; p = 0.013) and between the levels of anti-AGE EL IgG antibodies and CRP (r = 0.265; p = 0.042). Our study showed that elevated anti-AGE EL IgG antibody levels may be an indicator of the enhanced AGE-modification and inflammatory-mediated destruction of vascular elastin in hypertensive patients with T2D. Anti-AGE EL IgM antibodies may reflect changes in vascular MMP-2 activity, and their elevated levels may be a sign of early vascular damage.

Direct mass measurements of neutron-deficient nuclides around the N=50 shell closure below 100Sn were performed at the FRS Ion Catcher (FRS-IC) at GSI, Germany. The nuclei were produced by projectile fragmentation of 124Xe, separated in the fragment separator FRS and delivered to the FRS-IC. The masses of 14 ground states and two isomers were measured with relative mass uncertainties down to 1×10−7 using the multiple-reflection time-of-flight mass spectrometer of the FRS-IC, including the first direct mass measurements of 98Cd and 97Rh. A new QEC=5437±67 keV was obtained for 98Cd, resulting in a summed Gamow-Teller (GT) strength for the five observed transitions (0+⟶1+) as B(GT)=2.94−0.28+0.32. Investigation of this result in state-of-the-art shell model approaches accounting for the first time experimentally observed spectrum of GT transitions points to a perfect agreement for N=50 isotones. The excitation energy of the long-lived isomeric state in 94Rh was determined for the first time to be 293±21 keV. This, together with the shell model calculations, allows the level ordering in 94Rh to be understood.

Purpose: To describe clinical characteristics of patients positive simultaneously for RdRp, N and E genes of SARS-CoV-2 and to analyse dynamics of symptoms and Ct values in the interval from day 0 to day 10 after symptoms onset. Materials and methods: Retrospective analysis of data obtained on the day of specimen collection from 1044 individuals was carried out. Detection of RdRp, E and N genes of SARS-CoV-2 was conducted by RT-PCR. Only patients positive for all three genes were included. Results: Out of all patients, 47.8% were female, and 52.2% were male with a median age of 60 years, and the elder (>50 years) population constituted 71.5%. The most common comorbidities were cardiovascular disease (44.5%), diabetes mellitus (11.0%), and chronic pulmonary disease (8.6%). The prevalent symptoms at the time of specimen collection were fever (74.7%), cough (67.1%), and dyspnea (39.6%). Fever and cough were leading symptoms in the time interval between zero and the tenth day since the onset of COVID-19. A linear trend for dry cough and dyspnea, with a daily increase of 2.2% and 2.3%, respectively, was observed. Dry cough was the primary discriminator in distinguishing mild and moderate infection. Gradual increase of Ct values of all genes was observed from day 0 to day 10, and day 3 was essential for separating two time intervals. Conclusions: Flu-like symptoms were leading over the study period. Male sex, older age, cardiovascular disease, diabetes mellitus, and chronic pulmonary disease were risk factors for infection. Cough, age over 65 years and male sex in the group 33-63 years were predictors of more severe disease.

Introduction. The decrease in serum estrogens concentration at menopause disrupts the metabolic balance and leads to visceral obesity, which causes an increase in serum estradiol levels, through aromatase activity. Also, estrogen deficiency is a reason for the development of osteoporosis. The objective of the study was to investigate the serum estradiol levels and changes in bone alpha estrogen receptor expression in an experimental model of osteoporosis. Materials and methods. The study included 20 female Wistar rats at a reproductive age of two months, divided into two groups: group 1 (G1) – 10 ovariectomized rats, and group 2 (G2) -10 rats sham-operated. The role of endothelin-1 (ET-1) in the pathogenesis of hypertension (HTN) is not clearly established. There is evidence that its circulating levels are elevated in some forms of experimental and human HTN, but this was not a consistent finding. Based on these controversial data, we tested serum levels of ET-1 and Big ET-1 (the precursor of ET-1) in patients with essential HTN, comparing the results with those of healthy normotensive controls. The levels of ET-1 and Big ET-1 were measured by ELISA. Our results in patients with essential HTN showed that the mean levels of ET-1 (5.01 ± 2.1 pg/mL) were significantly higher (F = 6.34, p = 0.0144) than the mean levels in the control group (3.2 ± 1.0 pg/mL). The levels of Big ET-1 in patients with essential HTN (0.377 ± 0.1 pmol/L) were similar to those in the control group (0.378 ± 0.07 pmol/L) and did not differ significantly (F = 0.00, p = 0.9531). These data suggest that ET-1, but not Big ET-1, may play an important role in the pathogenesis of primary HTN.

Introduction: Collagen type I is a major structural component of human uterus. Recent studies have found that collagen type I turnover is altered in preeclampsia and imbalance occurs between collagen synthesis and degradation processes. This acts as a stimulus for abnormal changes in the concentrations of collagen type I derived products in circulating blood.Aim: Having in mind the pathological collagen type I turnover in preeclampisa, the aim of the present study was to determine the levels of N-terminal propeptide of collagen type I (PINP), C-terminal propeptide of collagen type I (PICP): both markers of collagen type I synthesis and levels of matrix metalloproteinase-1 (MMP-1): marker of collagen type I degradation in sera of women with early-onset preeclampsia.Materials and methods: Thirty-two patients with early-onset preeclampsia were examined. The mean age of the patients was 28.8±5.5 years; and the mean age of 22 age-matched healthy pregnant women was 28.5±6.0 years. The enzyme-linked immunosorbent assay (ELISA) was used for measuring serum levels of PINP, PICP and MMP-1.Results: There were no statistically significant differences between levels of PINP, PICP and MMP-1 in the sera of women with preeclampsia and healthy pregnancies: 16.991 (15.41÷21.143) vs. 17.189 (15.805÷20.747) µg/L (KW=0.02; p>0.05), 10.929 (8.7÷13.937) vs. 11.474 (7.369÷11.932) µg/L (KW=0.026; p>0.05), and 2.097 (1.384÷2.923) vs. 2.589 (1.41÷4.533) µg/L (KW=0.238; p>0.05), respectively. Both nuclei are populated (i) in Xe136+Pb208 MNT reactions employing the high-resolution Advanced GAmma Tracking Array (AGATA) coupled to the magnetic spectrometer PRISMA, (ii) in the Xe136+Pt198 MNT reaction employing the GAMMASPHERE spectrometer in combination with the gas-detector array CHICO, and (iii) as an evaporation residue after a Te130(α,xn)Xe134-xn fusion-evaporation reaction employing the HORUS γ-ray array at the University of Cologne. The high-spin level schemes are considerably extended above the Jπ=(7-) and (10+) isomers in Xe132 and above the 11/2- isomer in Xe133. The results are compared to the high-spin systematics of the Z=54 as well as the N=78 and N=79 chains. Furthermore, evidence is found for a long-lived (T1/2â‰1μs) isomer in Xe133 which closes a gap along the N=79 isotones. Shell-model calculations employing the SN100PN and PQM130 effective interactions reproduce the experimental findings and provide guidance to the interpretation of the observed high-spin features.

Background: Malignant pleural mesothelioma (MPM) is a rare cancer type with an increasing incidence worldwide. We aimed to develop a rational gene expression-based prognostic score in MPM using publicly available datasets. Methods: We developed and validated a two-gene prognostic score (2-PS) using three independent publicly available gene expression datasets. The 2-PS was built using the Robust Likelihood-Based Survival Modeling with Microarray Data method. Results: We narrowed down the model building to the analysis of 179 genes, which have been shown previously to be of importance to MPM development. Our statistical approach showed that a model including two genes (GOLT1B and MAD2L1) was the best predictor for overall survival (OS) (p < 0.0001). The binary score based on the median of the continuous score stratified the patients into low and high score groups and also showed statistical significance in uni- and multivariate models. The 2-PS was validated using two independent transcriptomic datasets. Furthermore, gene set enrichment analysis using training and validation datasets showed that high score patients had distinct gene expression profiles. Our 2-PS also showed a correlation with the estimated infiltration by some immune cell fractions such as CD8+ T cells and M1/2 macrophages. Finally, 2-PS correlated with sensitivity or resistance to some commonly used chemotherapeutic drugs. Conclusion: This is the first study to demonstrate good performance of only two-gene expression-based prognostic scores in MPM. Our initial approach for features selection allowed for an increased likelihood for the predictive value of the developed score, which we were also able to demonstrate.

We present results from an experiment dedicated to measure the lifetimes of the low-lying excited states of 205Bi. This nucleus was studied in the one-proton transfer reaction 204Pb(16O,15N)205Bi and the lifetimes of the 11 / 2 1 - , 7 / 2 1 - , 5 / 2 1 - , 7 / 2 2 - states were determined by utilizing the Recoil Distance Doppler Shift method. Upon comparing the results with the expectations of the particle-core coupling model, it indicates a low quadrupole collectivity in the structure of these states. [ABSTRACT FROM AUTHOR]

The interaction between CoII and 5-methyl-4-(2-thiazolylazo)-resorcinol (MTAR) was studied in a water-chloroform system, in the presence or absence of benzalkonium chloride (BZC) as a cationic ion-association reagent. The optimum pH, concentration of the reagents and extraction time for the extraction of Co were found. In the presence of BZC, the extracted ion-associate could be represented by the formula (BZ+)[CoIII(MTAR2–)2], where MTAR is in its deprotonated form. The following extraction-spectrophotometric characteristics were determined: absorption maximum, molar absorptivity, Sandell’s sensitivity, limit of detection, limit of quantification, constant of extraction, distribution ratio and fraction extracted. In the absence of BZC, the extraction is incomplete and occurs in a narrow pH range. The extracted chelate contains one deprotonated and one monoprotonated ligand: [CoIII(MTAR2–)(HMTAR–)].

Detailed spectroscopic information on the N∼82 nuclei is necessary to benchmark shell-model calculations in the region. The nuclear structure above long-lived isomers in Xe134 is investigated after multinucleon transfer (MNT) and actinide fission. Xenon-134 was populated as (i) a transfer product in Xe136+U238 and Xe136+Pb208 MNT reactions and (ii) as a fission product in the Xe136+U238 reaction employing the high-resolution Advanced Gamma Tracking Array (AGATA). Trajectory reconstruction has been applied for the complete identification of beamlike transfer products with the magnetic spectrometer PRISMA. The Xe136+Pt198 MNT reaction was studied with the γ-ray spectrometer GAMMASPHERE in combination with the gas detector array Compact Heavy Ion Counter (CHICO). Several high-spin states in Xe134 on top of the two long-lived isomers are discovered based on γγ-coincidence relationships and information on the γ-ray angular distributions as well as excitation energies from the total kinetic energy loss and fission fragments. The revised level scheme of Xe134 is extended up to an excitation energy of 5.832 MeV with tentative spin-parity assignments up to 16+. Previous assignments of states above the 7- isomer are revised. Latest shell-model calculations employing two different effective interactions reproduce the experimental findings and support the new spin and parity assignments.

Galectin-3 (gal-3) is lectin which is presumed to interact with extracellular matrix proteins and cell surface glycoproteins in normal and pathophysiological conditions. The expression of gal-3 at the fetal-maternal interface partially overlaps that of gal-1, suggesting that an interplay between them might be important for hypertensive disorders in pregnancy like preeclampsia. The aim of our study was to test the hypothesis whether galectin-3 could be used as a predictive marker for early-onset preeclampsia development. 32 patients with early-onset preeclampsia were examined, mean age 28.8 ± 5.5; and 22 age matched normal pregnancies mean age 28.5 ± 6.0. The enzyme-linked immunosorbent assay (ELISA) was used for measuring serum galectin-3 levels. There were no significant differences between serum levels of galectin-3 in sera of preeclampsia patients compared to normal pregnant women – 14.1 ± 4.77 vs. 15.7 ± 5.95 ng/ml (p>0.05). Serum galectin-3 levels correlated with maternal age (r=0.33; p=0.03) and BMI (body mass index) (r=0.52; p=0.01). Our data suggest that determination of serum galectin-3 levels may not be a useful method for prediction of early-onset preeclampsia. Studies should be aimed to other categories of biomarkers.

Background and Aims: Proteins containing advanced glycation end products are highly immunogenic and anti-advanced glycation end products antibodies (anti-AGEs antibodies) are found in the sera of diabetics.Materials and methods: Enzyme-linked immunosorbent assay (ELISA) was used for measuring levels of anti-advanced glycation end products antibodies in sera of 93 patients with type 2 diabetes mellitus and arterial hypertension (mean age 61.4±11.3 years, diabetes duration 9.88±3.12 years; hypertension duration 9.28±4.98). These values were compared to serum anti-AGEs antibodies in 42 age and sex matched controls. Introduction. Matrix metalloproteinases (MMPs) are important target genes for angiotensin II (AII), which subsequently alters MMP expression in extracellular matrix (ECM) and lead to remodeling process. Objective. To investigate whether an association between serum levels of MMP-12, AII and left ventricular hypertrophy exists in patients with heart failure with mid-range ejection fraction (HFmrEF) and arterial hypertension (AH). Material and methods. 56 patients with HFmrEF and AH were examined, mean age 65.62±9.69 years, and 22 age and sex-matched healthy subjects, mean age 56.4±5.53 years. 41 patients had hypertension-mediated organ damage and 15 did not. Patients were divided in two subgroups: subjects with left ventricular hypertrophy (n=32), (HFmrEF+LVH), and subjects without left ventricular hypertrophy (n=24), (HFmrEF-LVH). ELISA was used for measuring AII and MMP-12. Results: Serum MMP-12 levels were statistically significant lower in patients, than in controls: 0.0033 ng/mL (0.0022 ÷ 0.0071) vs. 0.0075 ng/mL (0.0068 ÷ 0.016) (KW=7.37; p=0.006). MMP-12 showed correlation with grade of AH (r=0.28; p=0.03). Patients with HFmrEF+LVH showed statistically significant higher levels of AII: 8.533 pg/mL (1.477÷13.009) than HFmrEF-LVH 1.333 pg/mL (0.477÷6.932) and healthy controls 1.539 pg/mL (0.274÷5.218); (KW=3.48; p=0.04). A water-isobutanol extraction-chromogenic system for NiII, based on the azo dye 5-methyl-4-(2-thiazolylazo)resorcinol (MTAR; H2L) and the ionic liquid Aliquat 336 (A336), was studied. Under the optimal conditions (cMTAR = 2.0 × 10–4 mol dm–3, cA336 = 5.6 × 10–3 mol dm–3, pH 8.5 and extraction time t = 1 min), NiII is extracted as a ternary complex which can be represented by the formula (A336+)2[Ni(L2-)2]. In the absence of A336, or in a slightly acidic medium, a binary complex, [Ni(HL-)2], with an absorption maximum at l = 548 nm and a shoulder at 590 nm is formed. The following extraction-spectrophotometric characteristics were determined at the above-mentioned optimal conditions: lmax (545 nm), molar absorptivity (5.0 × 104 dm3 mol–1 cm–1), Sandell’s sensitivity (1.2 × 10–3 μg cm–2), Beer’s law limits (0.05–3.1 mg cm–3), constant of extraction (Log K = 6.1) and fraction extracted (99.2%). The effect of foreign ions was studied; the most serious interferences were caused by CoII, CuII and CrIII.

Four azo dyes known to form anionic complexes with V(V) were investigated as potential liquid–liquid extraction–spectrophotometric reagents for the antihistamine medication hydroxyzine hydrochloride (HZH). A stable ion-association complex suitable for analytical purposes was obtained with 6-hexyl-4-(2-thiazolylazo)resorcinol (HTAR). The molar absorption coefficient, limit of detection, linear working range, and relative standard deviation in the analysis of real pharmaceutical samples (tablets and syrup) were 3.50 × 104 L mol−1 cm−1, 0.13 μg mL−1, 0.43–12.2 μg mL−1, and ≤2.7%, respectively. After elucidating the molar ratio in the extracted ion-association complex (HZH:V = 1:1), the ground-state equilibrium geometries of the two constituent ions—HZH+ and [VO2(HTAR)]−—were optimized at the B3LYP level of theory using 6-311++G** basis functions. The cation and anion were then paired in four different ways to find the most likely structure of the extracted species. In the lowest-energy structure, the VO2 group interacts predominantly with the heterochain of the cation. A hydrogen bond is present (V–O···H–O; 1.714 Å) involving the terminal oxygen of this chain.

The aim of the study was to determine prevalence of Anaplasmataceae-infected ticks in the Black Sea Coast and the Pleven regions of Bulgaria. A total of 350 ticks from different tick species were collected. Two hundred fifty-five ticks were removed from dogs in the Black Sea Coast region, and 95 Ixodes ricinus ticks were collected by flagging vegetation with a white flannel cloth in two areas in the region of Pleven. After the DNA isolation of the ticks, a genus-specific polymerase chain reaction (PCR) was performed to identify Anaplasmataceae. Second PCRs were performed with species-specific primers to identify Ehrlichia canis (E. canis) and Anaplasma phagocytophilum (A. phagocytophilum). The results showed that 26.9% of the Ixodes ricinus ticks were infected with Anaplasmataceae in the Black Sea Coast region and 36.8% in the Pleven region. The infection with E. canis was detected in 35.7% and A. phagocytophilum in 25.0% of positive ticks from the Black Sea Coast region. In the Pleven region, 22.9% of ticks were positive for E. canis, while 42.9% were positive for A. phagocytophilum. The molecular identification of E. canis in ticks collected from Bulgaria was performed for the first time. In conclusion, the present study revealed a higher prevalence of ticks infected with Anaplasmataceae, particularly A. phagocytophilum, in the Pleven region than in the Black Sea Coast region.
