Rationale: Cardiac monitoring has often been identified as an area of overutilization and remains a limited resource in many hospitals. With the aim of reducing telemetry overuse, we added clinical decision support to our health system's telemetry order with guidance on appropriate indications for monitoring. The new order requires selection of an appropriate clinical indication. Aims and Objectives: In this study, we aimed to understand provider engagement with this tool by assessing concordance between selected indications within the order and the clinical presence of those conditions as documented within the patient chart. Methods: We randomly selected 100 telemetry orders from July to October 2022 across four different hospitals at NYU Langone Health. Two independent, blinded reviewers used a structured protocol to identify documentation of actual indications for telemetry in each selected chart. We calculated the rate of concordance between indications selected in the order and indications that were determined to be clinically present on chart review. Results: There were 30,839 telemetry orders placed during the study timeframe. Overall concordance between the selection within the order and the actual indication was 48% (95% confidence interval [CI], 38.21%−57.79%). We observed especially low concordance rates for vague indications, such as 'Other', and for 'Confirmed Stroke', which was the only indication allowing for indefinite telemetry. Conclusion: The overall low concordance suggests a disconnect between the support tool and clinical practice. In this paper, the design and validation of an implantable antenna which is applicable to biotelemetry services is presented. This proposed antenna operates in the wireless medical telemetry service (WMTS) frequency band of 1.39-1.4 GHz. As compared to other contemporary antennas, this design provides better gain of-31 dB and reflection coefficient of-20.2 dB with better safe limit of specific absorption rate (SAR). At the resonating frequency of 1.4 GHz, the intended antenna provides good radiation and gain characteristics. The VSWR parameter for this designed antenna has been obtained as 1.25 which promises for proper impedance match. The designed antenna has been fabricated and validated with tissue mimic liquid-phantom to make sure the suitability for implantation. Background: Breakthrough cancer pain (BTCP) is primarily managed at home and can stem from physical exertion and emotional distress triggers. Beyond these triggers, the impact of ambient environment on pain occurrence and intensity has not been investigated. This study explores the impact of environmental factors on the frequency and severity of breakthrough cancer pain (BTCP) in the home context from the perspective of patients with advanced cancer and their primary family caregiver. Methods: A health monitoring system was deployed in the homes of patient and family caregiver dyads to collect self-reported pain events and contextual environmental data (light, temperature, humidity, barometric pressure, ambient noise.) Correlation analysis examined the relationship between environmental factors with: 1) individually reported pain episodes and 2) overall pain trends in a 24-hour time window. Machine learning models were developed to explore how environmental factors may predict BTCP episodes. Results: Variability in correlation strength between environmental variables and pain reports among dyads was found. Light and noise show moderate association (r = 0.50–0.70) in 66% of total deployments. The strongest correlation for individual pain events involved barometric pressure (r = 0.90); for pain trends over 24-hours the strongest correlations involved humidity (r = 0.84) and barometric pressure (r = 0.83). Machine learning achieved 70% BTCP prediction accuracy. Conclusion: Our study provides insights into the role of ambient environmental factors in BTCP and offers novel opportunities to inform personalized pain management strategies, remotely support patients and their caregivers in self-symptom management. This research provides preliminary evidence of the impact of ambient environmental factors on BTCP in the home setting. We utilized real-world data and correlation analysis to provide an understanding of the relationship between environmental factors and cancer pain which may be helpful to others engaged in similar work. Due to rapid technological advancements, remote patient monitoring (RPM) technology has gained traction in recent years. While the effects of specific RPM interventions are known, few published reviews examine RPM in the context of care transitions from an inpatient hospital setting to a home environment. In this systematic review, we addressed this gap by examining the impacts of RPM interventions on patient safety, adherence, clinical and quality of life outcomes and cost-related outcomes during care transition from inpatient care to a home setting. We searched five academic databases (PubMed, CINAHL, PsycINFO, Embase and SCOPUS), screened 2606 articles, and included 29 studies from 16 countries. These studies examined seven types of RPM interventions (communication tools, computer-based systems, smartphone applications, web portals, augmented clinical devices with monitoring capabilities, wearables and standard clinical tools for intermittent monitoring). RPM interventions demonstrated positive outcomes in patient safety and adherence. RPM interventions also improved patients' mobility and functional statuses, but the impact on other clinical and quality-of-life measures, such as physical and mental health symptoms, remains inconclusive. In terms of cost-related outcomes, there was a clear downward trend in the risks of hospital admission/readmission, length of stay, number of outpatient visits and non-hospitalisation costs. Future research should explore whether incorporating intervention components with a strong human element alongside the deployment of technology enhances the effectiveness of RPM. The review highlights the need for more economic evaluations and implementation studies that shed light on the facilitators and barriers to adopting RPM interventions in different care settings. The characteristic mode method is used to design a miniaturized dual-band dual circularly polarized (CP) implantable antenna operating in ISM bands. The miniaturization and dual-band characteristics are gained by using the slotting method and by inserting a short-circuit probe between the radiation patch and the ground plane. We use the characteristic mode method to study the current distribution of circular radiation patches with T-shaped slots in different modes. After opening a cross-shaped slot at the center of the radiation patch and the ground plane, we obtained two orthogonal modes with equal amplitude and phase difference of 90° in two operating frequency bands, ultimately achieving CP characteristics of the antenna. Its overall size is only π ×(0.014 λ 0)2 × 0.0027 λ 0, smaller than other CP implantable antennas with similar performances, and it has satisfactory radiation efficiency and gain characteristics. Measurements show that it can operate in the ISM bands of 0.9 and 2.4 GHz with an effective 3 dB axial ratio bandwidth greater than 220 MHz (0.87 to 1.09 GHz, 22.45%) and 230 MHz (2.31 to 2.54 GHz, 9.48%), and its peak gain is − 29.5 dBi and − 19.2 dBi, respectively. Interspecific interactions, including predator–prey, intraguild predation (IGP) and competition, may drive distribution and habitat use of predator communities. However, elucidating the relative importance of these interactions in shaping predator distributions is challenging, especially in marine communities comprising highly mobile species.We used individual‐based models (IBMs) to predict the habitat distributions of apex predators, intraguild (IG) prey and prey. We then used passive acoustic telemetry to test these predictions in a subtropical marine predator community consisting of eight elasmobranch (i.e. shark and ray) species in Bimini, The Bahamas.IBMs predicted that prey and IG prey will preferentially select habitats based on safety over resources (food), with stronger selection for safe habitat by smaller prey. Elasmobranch space‐use patterns matched these predictions. Species with predator–prey and asymmetrical IGP (between apex and small mesopredators) interactions showed the clearest spatial separation, followed by asymmetrical IGP among apex and large mesopredators. Competitors showed greater spatial overlap although with finer‐scale differences in microhabitat use.Our study suggests space‐use patterns in elasmobranchs are at least partially driven by interspecific interactions, with stronger spatial separation occurring where interactions include predator–prey relationships or IGP. Continuous electrocardiographic (ECG) monitoring was first introduced into hospitals in the 1960s, initially into critical care, as bedside monitors, and eventually into step-down units with telemetry capabilities. Although the initial use was rather simplistic (ie, heart rate and rhythm assessment), the capabilities of these devices and associated physiologic (vital sign) monitors have expanded considerably. Current bedside monitors now include sophisticated ECG software designed to identify myocardial ischemia (ie, ST-segment monitoring), QT-interval prolongation, and a myriad of other cardiac arrhythmia types. Physiologic monitoring has had similar advances from noninvasive assessment of core vital signs (blood pressure, respiratory rate, oxygen saturation) to invasive monitoring including arterial blood pressure, temperature, central venous pressure, intracranial pressure, carbon dioxide, and many others. The benefit of these monitoring devices is that continuous and real-time information is displayed and can be configured to alarm to alert nurses to a change in a patient's condition. I think it is fair to say that critical and high-acuity care nurses see these devices as having a positive impact in patient care. However, this enthusiasm has been somewhat dampened in the past decade by research highlighting the shortcomings and unanticipated consequences of these devices, namely alarm and alert fatigue. In this article, which is associated with the American Association of Critical-Care Nurses' Distinguished Research Lecture, I describe my 36-year journey from a clinical nurse to nurse scientist and the trajectory of my program of research focused primarily on ECG and physiologic monitoring. Specifically, I discuss the good, the not so good, and the untapped potential of these monitoring systems in clinical care. Free‐ranging otariids are routinely captured for data and sample collection. To achieve this, anesthesia may be used to facilitate handling, decrease stress, and improve human and animal safety. Injectable anesthetics are widely used for such endeavors; however, certain disadvantages to this approach warrant further exploration of alternative anesthetic techniques. Inhalant anesthesia, commonly utilized for otariids in a clinical setting, is used more sparingly in the field, with few studies assessing safety and efficacy in free‐ranging otariids. During 2016–2020, 175 Guadalupe fur seals were net‐captured and anesthetized with isoflurane and oxygen on Guadalupe Island, Mexico, for satellite telemetry attachment and biological sampling. To contribute to the body of knowledge surrounding the use of inhalants in the field, physiologic and anesthetic parameters (time to induction, total oxygen use, heart rate, respiratory rate, time to recovery, and anesthetic depth) were assessed for effects of biometric and logistical factors (pursuit and holding time, sex, age class, body weight, year, oxygen flow rate, and total anesthesia time). This anesthetic technique provided rapid induction and recovery times and rare side effects in Guadalupe fur seals, serving as a practical means of field immobilization for minimally invasive procedures in this imperiled species. Algorithms for the detection of COVID-19 illness from wearable sensor devices tend to implicitly treat the disease as causing a stereotyped (and therefore recognizable) deviation from healthy physiology. In contrast, a substantial diversity of bodily responses to SARS-CoV-2 infection have been reported in the clinical milieu. This raises the question of how to characterize the diversity of illness manifestations, and whether such characterization could reveal meaningful relationships across different illness manifestations. Here, we present a framework motivated by information theory to generate quantified maps of illness presentation, which we term "manifestations," as resolved by continuous physiological data from a wearable device (Oura Ring). We test this framework on five physiological data streams (heart rate, heart rate variability, respiratory rate, metabolic activity, and sleep temperature) assessed at the time of reported illness onset in a previously reported COVID-19-positive cohort (N = 73). We find that the number of distinct manifestations are few in this cohort, compared to the space of all possible manifestations. In addition, manifestation frequency correlates with the rough number of symptoms reported by a given individual, over a several-day period prior to their imputed onset of illness. These findings suggest that information-theoretic approaches can be used to sort COVID-19 illness manifestations into types with real-world value. This proof of concept supports the use of information-theoretic approaches to map illness manifestations from continuous physiological data. Tracking of animal migrations using telemetry technologies needs to take into consideration the burden that the tag exerts on the animal. Here, we examined the potential impacts of acoustic tags of two sizes (nominally a ‘V6’ [smaller] and ‘V7’ [larger]) on the downstream riverine migration success and behaviour of wild Atlantic salmon (Salmo salar L.) smolts. One hundred fish were tagged with either a V6 or V7 tag. Tag burden (tag: fish weight) ranged from 1.88% to 7.39% and differed significantly between fish tagged with the V6 (mean [SD] = 3.63% [0.51%]) and the V7 tags (mean [SD] = 5.84% [0.95%]). There was no significant difference in the in‐river migration failure between the two groups when tested with a time‐to‐event analysis. There were also no differences in other elements of the migratory behaviour (rate of movement, time of detection and residency time) between the two tagging groups. These data support the use of acoustic tracking for monitoring smolt migration and highlight that tagging of smaller smolts at up to 7.39% tag burden to gain a more representative understanding of migration success and behaviours across a smolt population. A dual‐band bio‐implantable compact antenna with a meander‐line structure is presented. The proposed meander‐line antenna resonates at the industrial, scientific, and medical (2.4 GHz) and wireless medical telemetry (1.4 GHz) bands. The meander‐line structure is selected as a radiating patch given its versatile and effective design. With a dimension of only 10mm×10mm×0.635mm, the designed antenna is compact. Considering a skin phantom, the proposed antenna was designed, optimized, and simulated. The Rogers RT/duroid 6010 substrate material with high dielectric constant was used to fabricate the meander‐line dual‐band implantable antenna, which was validated experimentally. The superstrate was made of the same material. Experiments were conducted on skin‐mimicking gel. The designed meander‐line antenna has a high peak gain of −21 dBi at 2.4 GHz, and its maximum specific absorption rate is compliant with IEEE safety standards. [ABSTRACT FROM AUTHOR]

Background: Guidelines recommended remote monitoring (RM) in managing patients with Cardiac Implantable Electronic Devices. In recent years, smart device (phone or tablet) monitoring-based RM (SM-RM) was introduced. This study aims to systematically review SM-RM versus bedside monitor RM (BM-RM) using radiofrequency in terms of compliance, connectivity, and episode transmission time. Methods: We conducted a systematic review, searching three international databases from inception until July 2023 for studies comparing SM-RM (intervention group) versus BM-RM (control group). Results: Two matched studies (21 978 patients) were retrieved (SM-RM arm: 9642 patients, BM-RM arm: 12 336 patients). There is significantly higher compliance among SM-RM patients compared with BM-RM patients in both pacemaker and defibrillator patients. Manyam et al. found that more SM-RM patients than BM-RM patients transmitted at least once (98.1% vs. 94.3%, p < .001), and Tarakji et al. showed that SM-RM patients have higher success rates of scheduled transmissions than traditional BM-RM methods (SM-RM: 94.6%, pacemaker manual: 56.3%, pacemaker wireless: 77.0%, defibrillator wireless: 87.1%). There were higher enrolment rates, completed scheduled and patient-initiated transmissions, shorter episode transmission time, and higher connectivity among SM-RM patients compared to BM-RM patients. Younger patients (aged <75) had more patient-initiated transmissions, and a higher proportion had ≥10 transmissions compared with older patients (aged ≥75) in both SM-RM and BM-RM groups. Conclusion: SM-RM is a step in the right direction, with good compliance, connectivity, and shorter episode transmission time, empowering patients to be in control of their health. Brugada syndrome is an inherited genetic disorder known to cause a variety of patient complaints but may ultimately cause ventricular fibrillation and sudden cardiac death. In the Laurentian Great Lakes, the issue of barrier removal is complicated by the presence of non‐native species below barriers. A fish tracking study was conducted to guide efforts for barrier remediation decisions for the restoration of fish populations with a focus on Walleye (Scander vitreus) and Lake Sturgeon (Acipenser fulvescens) in the Black Sturgeon River, a river system fragmented by a dam which blocks access of fishes to the majority of a large, otherwise barrier‐free watershed. Data from 3 years of spawning migrations (2018–2020) indicated that the Walleye population in Black Bay likely consists of both river (65%) and lake spawners (27%), with the remaining individuals spawning in the bay or river in different years. Walleye and Lake Sturgeon showed consistent differences in the extent to which individuals migrated upstream in the river during the spawning season, despite expectations that both species would spawn at the base of the dam when prevented from further migration. The dam was presumably a barrier to migration for Lake Sturgeon, as nearly all Lake Sturgeon that entered the river migrated to the base of the dam. In contrast, few Walleye entering the river during the spawning season migrated to the dam annually. These findings suggest that Walleye and Lake Sturgeon may not benefit equally, at least in the short term, from barrier remediation or dam removal. [ABSTRACT FROM AUTHOR]

Anadromous rainbow smelt (Osmerus mordax, [Mitchill 1814]) are found along the northeast Atlantic coastline of North America, with their range now limited to north of Cape Cod, Massachusetts, USA. Although their anadromous life cycles are described broadly, gaps remain regarding how adult rainbow smelt use estuaries post‐spawning, including movement behaviors, habitats used, and specific timing of emigration to coastal waters. In spring 2021, we used acoustic telemetry to characterize movements during and after the spawning season of rainbow smelt captured in tributaries to Great Bay, New Hampshire, USA, a large estuarine system near the southern edge of their range. Forty‐four adult rainbow smelt (n = 35 male, n = 9 female) were tagged with Innovasea V5 180‐kHz transmitters and an array of 22,180 kHz VR2W receivers were deployed throughout Great Bay to detect movements of tagged fish from March to October 2021. Rainbow smelt were detected 14,186 times on acoustic telemetry receivers, with 41 (93%) of the tagged individuals being detected at least once post‐tagging. Individuals were detected moving between tributaries, revealing that rainbow smelt can use multiple rivers during the spawning season (March–April). Mark‐recapture Cormack‐Jolly‐Seber models estimated 83% (95% confidence interval 66%–92%) of rainbow smelt survived to the mainstem Piscataqua River, and a minimum of 50% (22 of 44) reached the seaward‐most receivers and were presumed to have survived emigration. Most individuals that survived remained in the estuary for multiple weeks (average = 19.47 ± 1.99 standard error days), displaying extended use of estuarine environments. Downstream movements occurred more frequently during ebb tides and upstream movements with flood tides, possibly a mechanism to reduce energy expenditures. Fish emigrated from the estuary by mid‐May to the coastal Gulf of Maine. Our results underscore that rainbow smelt need access to a variety of habitats, including multiple tributaries and high‐quality estuarine habitat, to complete their life cycle. Background: Worldwide, up to 20 % of hospitalised patients have diabetes mellitus. In-hospital dysglycaemia increases patient mortality, morbidity, and length of hospital stay. Improved in-hospital diabetes management strategies are needed. The DIATEC trial investigates the effects of an in-hospital diabetes team and operational insulin titration algorithms based on either continuous glucose monitoring (CGM) data or standard point-of-care (POC) glucose testing. Methods: This is a two-armed, two-site, prospective randomised open-label blinded endpoint (PROBE) trial. We recruit non-critically ill hospitalised general medical and orthopaedic patients with type 2 diabetes treated with basal, prandial, and correctional insulin (N = 166). In both arms, patients are monitored by POC glucose testing and diabetes management is done by ward nurses guided by in-hospital diabetes teams. In one of the arms, patients are monitored in addition to POC glucose testing by telemetric CGM viewed by the in-hospital diabetes teams only. The in-hospital diabetes teams have operational algorithms to titrate insulin in both arms. Outcomes are in-hospital glycaemic and clinical outcomes. Discussion: The DIATEC trial will show the glycaemic and clinical effects of in-hospital CGM handled by in-hospital diabetes teams with access to operational insulin titration algorithms in non-critically ill patients with type 2 diabetes. The DIATEC trial seeks to identify which hospitalised patients will benefit from CGM and in-hospital diabetes teams compared to POC glucose testing. This is essential information to optimise the use of healthcare resources before broadly implementing in-hospital CGM and diabetes teams. The brain's sensory lateralization involves the processing of information from the sensory organs primarily in one hemisphere. This can improve brain efficiency by reducing interference and duplication of neural circuits. For species that rely on successful interaction among family partners, such as geese, lateralization can be advantageous. However, at the group level, one-sided biases in sensory lateralization can make individuals predictable to competitors and predators. We investigated lateral preferences in the positioning of pair mates of Greater white-fronted geese Anser albifrons albifrons. Using GPS-GSM trackers, we monitored individual geese in flight throughout the year. Our findings indicate that geese exhibit individual lateral biases when viewing their mate in flight, but the direction of these biases varies among individuals. We suggest that these patterns of visual lateralization could be an adaptive trait for the species with long-term social monogamy, high levels of interspecies communication and competition, and high levels of predator and hunting pressure. Biomedical telemetry is, therefore, significant considering it facilitates prompt telecommunication as well as tracking of medical devices between centralized systems and patients. The availability and quality of communication of information are determined by the performance and selection of the telemetry antenna. This article analyzes the current state of BMA technology, aiming to extend the communication range and transmission speed of the data. The research article intends to contribute to the development of wireless technology. A plethora of antenna sizes are tackled from wearable to insertable antennas in addition to the improvements in materials and fabrication methods. The present review paper puts the thesis on only a few of the numerous biomedical telemetry antenna applications in healthcare, and these are the Internet of Medical Things (IoMT) and remote patient monitoring applications. It discusses case studies where better antennas had led to the creation of new therapeutic strategies, and diagnostic capacities, and had overall improved the quality of services. Therefore, the architectural problems of the existing designs are scrutinized, and this gives the other research areas the chance to be explored. A biological telemetry antenna is set to be the mobile edge computing solution that combines artificial intelligence, a 5G network, and edge computing. It also improves capital effectiveness over the transition period. Presentation makes it evident, why antennas are the essential component of the connected healthcare system and how antennas might redefine individualized care and the healthcare ecosystem. In conclusion, this research provides an extensive overview of the developments, uses, and future directions of biomedical telemetry antenna technology. It is an invaluable resource for academics, engineers, and medical professionals who seek to understand more about the evolving nature of this crucial component of modern healthcare systems. The biologger is a widely used tool in biotelemetry for investigating marine fish. However, studies reported that the attachment of biologgers has a negative effect on the target organisms. We assessed the health status of spotted sea bass with attached biologgers by analyzing changes in their serum biochemistry and biomarker gene expression at varying biologger/fish body weight ratios. Especially, the assessment of the health status using biomarker genes offers the rapid evaluation of the condition of an individual. The genes bax, hsp70-2, and Cx32.7, associated with apoptosis, stress, and immunity, were selected as biomarker genes to assess the fish stress levels and overall health. The experimental groups included a control group without the biologger (C) and biologger-equipped groups, each carrying a biologger whose weight was 2.0–3.0% (W2), 5.0–6.0% (W5), and 10.0–12.0% (W10) of the fish body weight. Blood samples were collected on days 0, 1, 7, 14, and 21 post-attachment. The expression of bax, hsp70-2, and Cx32.7 was analyzed in the liver and muscle on day 21. The W10 group showed significantly higher levels of superoxide dismutase on day 1 and of glutamic oxaloacetic transaminase and glutamic pyruvic transaminase on day 7 than the other groups. On day 21 post-attachment, regardless of the biologger/fish body weight ratio, the liver and muscle tissues from groups W2, W5, and W10 showed significantly higher expression of bax, hsp70-2, and Cx32.7 than those from group C, respectively. However, there was no significant change in blood parameters depending on the weight of the attached biologger on day 21 post-attachment. Nonsustained ventricular tachycardia (NSVT) is a common arrhythmia associated with heart failure, cardiomyopathy, coronary artery disease, electrolyte imbalances, and congenital heart disorders (Foth et al., 2023). NSVT is often asymptomatic depending on its burden percentage. However, typical NSVT presentation in the emergency department (ED) includes palpitations, near-syncope, dizziness, skipped beats, chest pain, and/or dyspnea (Katritsis et al., 2012). In some instances, NSVT can present with elevated or slightly elevated troponin from demand ischemia.A definite diagnosis of NSVT is not of high complexity; nevertheless, it is not always identified on electrocardiogram (ECG) by the time the patient arrives to the ED. Identification of NSVT usually requires prolonged cardiac monitoring, mobile cardiac telemetry (MCT), and in some instances internal loop recorder placement. Wireless body area networks (WBANs) have emerged as a promising solution for addressing challenges faced by elderly individuals, limited medical facilities, and various chronic medical conditions. WBANs consist of wearable sensing and computing devices interconnected through wireless communication channels, enabling the collection and transmission of vital physiological data. However, the energy constraints of the battery-powered sensor nodes in WBANs pose a significant challenge to ensuring long-term operational efficiency. Two-hop routing protocols have been suggested to extend the stability period and maximize the network's lifetime. These protocols select appropriate parent nodes or forwarders with a maximum of two hops to relay data from sensor nodes to the sink. While numerous energy-efficient routing solutions have been proposed for WBANs, reliability has often been overlooked. Our paper introduces an energy-efficient routing protocol called a Hybrid Clustering Approach for Extending WBAN Lifetime (HCEL) to address these limitations. HCEL leverages a utility function to select parent nodes based on residual energy (RE), proximity to the sink node, and the received signal strength indicator (RSSI). The parent node selection process also incorporates an energy threshold value and a constrained number of serving nodes. The main goal is to extend the overall lifetime of all nodes within the network. Through extensive simulations, the study shows that HCEL outperforms both Stable Increased Throughput Multihop Protocol for Link Efficiency (SIMPLE) and Energy-Efficient Reliable Routing Scheme (ERRS) protocols in several key performance metrics. The specific findings of our article highlight the superior performance of HCEL in terms of increased network stability, extended network lifetime, reduced energy consumption, improved data throughput, minimized delays, and improved link reliability. Conflicts can arise in developmental pathways that prevent an individual entering different developmental life stages that result in the expression of different phenotypes within a specific time period. In salmonids, theory suggests that sexual maturation may inhibit subsequent smolting within the same 12‐month period and that this is partly the result of the time and the apparently conflicting physiological changes for these processes to occur, and partly because of the energy requirements for these physiologically taxing processes. This study tested whether sexually mature male Atlantic salmon (Salmo salar L.) parr, caught in the autumn, would subsequently smolt the following spring. Through individual identification using PIT telemetry, minimum estimates of 3.0% (n = 6/203) and 5.9% (n = 1/17) of Atlantic salmon parr that were sexually mature in two river catchments during the autumn were subsequently identified as smolts in the following spring. We therefore suggest that, in line with previous studies on domesticated Atlantic salmon and laboratory‐based experiments, there is no developmental conflict but that life‐history expression is mediated by environmental and genetic processes. Aims In-hospital telemetry monitoring has been an integrated part of arrhythmia monitoring for decades. A substantial proportion of patients require arrhythmia monitoring during stays in non-intensive care units. However, studies exploring patients' experiences of telemetry monitoring are scarce. Therefore, the aim was to explore and describe patients' experiences of in-hospital telemetry monitoring in a non-intensive care setting. Methods and results Twenty face-to-face, semi-structured interviews were conducted. Interviews were conducted before discharge at two university hospitals in Norway. The patients were purposively sampled, resulting in a well-balanced population comprising 11 men and nine women, mean age 62 years (range 25–83). Average monitoring time was 9 days (range 3–14). Data were audiotaped, transcribed verbatim, and coded using NVivo software. Qualitative content analysis using an inductive approach was performed. Patients expressed a need for individualized information during telemetry monitoring. Their feelings of safety were related to responses from nurses from the central monitoring station when alarms from the telemetry were triggered. Despite perceived physical restrictions and psychological limitations associated with telemetry monitoring, they found monitoring to be beneficial because it facilitated the diagnosis of arrhythmia. Moreover, they expressed a need for improvements in wearable monitoring equipment. Patients expressed ambivalent feelings about discontinuing the telemetry and their readiness for discharge. Conclusion Patients need individualized information about the results of their telemetry monitoring in order to better understand the arrhythmia management and to increase their experience of safety after discharge. In 2022, highly pathogenic avian influenza (HPAI) virus resulted in mass mortality throughout seabird colonies in the North Atlantic Ocean. While mortalities and transmission of the virus are being documented, other effects are less understood. We documented nest abandonment and unprecedented reproductive failure by Northern Gannets Morus bassanus at Cape St. Mary's, Newfoundland and Labrador, Canada, the species' southernmost colony. Reproductive success in 2022 was the lowest on record at 17%, which was likely a result of adults abandoning the nest and succumbing to avian influenza virus (AIV) infection. Following the AIV outbreak at Cape St. Mary's, we also documented extremely long foraging trips, including the second known instance of inter-colony movement by an adult Northern Gannet, who embarked on two trips to the gannet colony on Baccalieu Island. This inter-colony movement represents a possible means of viral transmission between colonies. The effects of HPAI may have been exacerbated as a result of heat stress during a marine heatwave, which coincided with the major outbreak at this colony. As this HPAI virus could continue to circulate in seabirds, ongoing monitoring of mortality and other indirect effects of AIV outbreaks are needed to inform future impacts. [ABSTRACT FROM AUTHOR]

This paper proposes a slotted patch antenna with wide bandwidth covering ISM frequency band (2.40-2.48 GHz) for implantable biotelemetry applications. A homogeneous skin phantom (HSP) model proves the usability of the proposed antenna in in-body environments. At a resonance frequency of 2.42 GHz, the design shows an S11 parameter of-35.56 dB, a percentage impedance bandwidth of 66.6% (2-4 GHz), and the maximum peak gain of-24.80 dBi. To validate the simulated results, the designed antenna was fabricated and measured, showing good compliance with the expected results. To ensure tissue safety, a specific absorption rate (SAR) is simulated for the proposed antenna which satisfies the requirements of IEEE standards, with a value of 87.75 W/kg for 10 g of tissue. The proposed antenna shows a telemetry range of 11 and 6.3 m at 7 kbps and 100 kbps data rates, respectively. BackgroundMovement is central to understanding the ecology of animals. The most robustly definable segments of an individual's lifetime track are its diel activity routines (DARs). This robustness is due to fixed start and end points set by a 24-h clock that depends on the individual's quotidian schedule. An analysis of day-to-day variation in the DARs of individuals, their comparisons among individuals, and the questions that can be asked, particularly in the context of lunar and annual cycles, depends on the relocation frequency and spatial accuracy of movement data. Here we present methods for categorizing the geometry of DARs for high frequency (seconds to minutes) movement data.MethodsOur method involves an initial categorization of DARs using data pooled across all individuals. We approached this categorization using a Ward clustering algorithm that employs four scalar "whole-path metrics" of trajectory geometry: 1. net displacement (distance between start and end points), 2. maximum displacement from start point, 3. maximum diameter, and 4. maximum width. We illustrate the general approach using reverse-GPS data obtained from 44 barn owls, Tyto alba, in north-eastern Israel. We conducted a principle components analysis (PCA) to obtain a factor, PC1, that essentially captures the scale of movement. We then used a generalized linear mixed model with PC1 as the dependent variable to assess the effects of age and sex on movement.ResultsWe clustered 6230 individual DARs into 7 categories representing different shapes and scale of the owls nightly routines. Five categories based on size and elongation were classified as closed (i.e. returning to the same roost), one as partially open (returning to a nearby roost) and one as fully open (leaving for another region). Our PCA revealed that the DAR scale factor, PC1, accounted for 86.5% of the existing variation. It also showed that PC2 captures the openness of the DAR and accounted for another 8.4% of the variation. We also constructed spatio-temporal distributions of DAR types for individuals and groups of individuals aggregated by age, sex, and seasonal quadrimester, as well as identify some idiosyncratic behavior of individuals within family groups in relation to location. Finally, we showed in two ways that DARs were significantly larger in young than adults and in males than females.ConclusionOur study offers a new method for using high-frequency movement data to classify animal diel movement routines. Insights into the types and distributions of the geometric shape and size of DARs in populations may well prove to be more invaluable for predicting the space-use response of individuals and populations to climate and land-use changes than other currently used movement track methods of analysis.

Despite many benefits, the extensive deployment of Health Information Technology (HIT) systems by healthcare organizations has encountered many challenges, particularly in the field of telemetry concerning patient monitoring and its operational workflow. These challenges can add more layers of complexity when an unplanned software security patching is performed, affecting patient monitoring and causing disruption in daily clinical operations. This study is a reflection on what happened associated with software security patching and why it happened through the lens of an incident report to develop potential preventive and corrective strategies using qualitative analyses--inductive and deductive approaches. There is a need for such analyses to identify the underlying mechanism behind such issues since very limited research has been conducted on the study of software patching. The incident was classified as a "software functionality" issue, and the consequence was an "incident with a noticeable consequence but no patient harm", and the contributing factor was a software update, i.e., software security patching. This report describes how insufficient planning of software patching, lack of training for healthcare professionals, contingency planning on unplanned system disruption, and HIT system configuration can compromise healthcare quality and cause risks to patient safety. We propose 15 preventive and corrective strategies grouped under four key areas based on the system approach and socialtechnical aspects of the patching process. The key areas are (i) preparing, developing, and deploying patches; (ii) training the frontline operators; (iii) ensuring contingency planning; and (iv) establishing configuration and communication between systems. These strategies are expected to minimize the risk of HIT-related incidents, enhance software security patch management in healthcare organizations, and improve patient safety. Reservoir formation in a river system changes a lotic environment to more lacustrine conditions, with impacts throughout the ecosystem. In this study, a river reach containing typical salmonid riffle/run habitat was flooded to create a large, deep pool from June to September in each of 3 years. We test the hypothesis that juvenile Atlantic salmon (Salmo salar) with their preference for run/riffle habitats will respond to the transformation to a lentic environment by moving into adjacent lotic environments. Movements of juvenile Atlantic salmon were monitored using a combination of biotelemetry (radio‐ and passive integrated transponder‐tagging) and electrofishing. Results showed that no tracked fish moved away from the created pool habitat. Mass‐specific growth rates showed the created pool habitat resulted in net growth of juveniles. The results confirm that fish may not immediately (i.e., at least for an approximate 2 months) respond to rapid, large‐scale habitat alterations by moving to find similar habitat conditions outside the altered habitat. This is most probably related to plasticity of behavior and habitat use, and no change in biological conditions to a point that would negatively impact fish growth and survival, for example food availability, competition, or predation. The results also support the hypothesis that the relative importance of physical habitat variables is not universal among streams and populations, therefore limiting the value of applying standard habitat suitability criteria and use. Deciding when to depart on long‐distance, sometimes global, movements can be especially important for flying species. Adverse weather conditions can affect energetic flight costs and navigational ability. While departure timings and conditions have been well‐studied for migratory flights to and from the breeding range, few studies have focussed on flights within the non‐breeding season. Yet in some cases, overwintering ranges can be large enough that ecological barriers, and a lack of resting sites en route, may resist movement, especially in unfavorable environmental conditions. Understanding the conditions that will enable or prohibit flights within an overwintering range is particularly relevant in light of climate change, whereby increases in extreme weather events may reduce the connectivity of sites. We tracked 495 (n = 251 in 2019; n = 244 in 2020) overwintering red knots (Calidris canutus islandica) in the Dutch Wadden Sea and investigated how many departed towards the UK (on westward relocation flights), which requires flying over the North Sea. For those that departed, we used a resource selection model to determine the effect of environmental conditions on the timing of relocation flights. Specifically, we investigated the effects of wind, rain, atmospheric pressure, cloud cover, and migratory timing relative to sunset and tidal cycle, which have all been shown to be crucial to migratory departure conditions. Approximately 37% (2019) and 36% (2020) of tagged red knots departed on westward relocation flights, indicating differences between individuals' space use within the overwintering range. Red knots selected for departures between 1 and 2.5 h after sunset, approximately 4 h before high tide, with tailwinds and little cloud cover. However, rainfall and changes in atmospheric pressure appear unimportant. Our study reveals environmental conditions that are important for relocation flights across an ecological barrier, indicating potential consequences of climate change on connectivity. Wireless Body Area Network (WBAN) is an interconnection of tiny biosensors that are organized in/on several parts of the body. The developed WBAN is used to sense and transmit health-related data over the wireless medium. Energy efficiency is the primary challenges for increasing the life expectancy of the network. To address the issue of energy efficiency, one of the essential approaches i.e., the selection of an appropriate relay node is modelled as an optimization problem. In this paper, energy efficient routing optimization using Multiobjective-Energy Centric Honey Badger Optimization (M-ECHBA) is proposed to improve life expectancy. The proposed M-ECHBA is optimized by using the energy, distance, delay and node degree. Moreover, the Time Division Multiple Access (TDMA) is used to perform the node scheduling at transmission. Therefore, the M-ECHBA method is used to discover the optimal routing path for enhancing energy efficiency while minimizing the transmission delay of WBAN. The performances of the M-ECHBA are analyzed using life expectancy, dead nodes, residual energy, delay, packets received by the Base Station (BS), Packet Loss Ratio (PLR) and routing overhead. The M-ECHBA is evaluated with some classical approaches namely SIMPLE, ATTEMPT and RE-ATTEMPT. Further, this M-ECHBA is compared with the existing routing approach Novel Energy Efficient hybrid Meta-heuristic Approach (NEEMA), hybrid Particle Swarm Optimization-Simulated Annealing (hPSO-SA), Energy Balanced Routing (EBR), Threshold-based Energy-Efficient Routing Protocol for physiological Critical Data Transmission (T-EERPDCT), Clustering and Cooperative Routing Protocol (CCRP), Intelligent-Routing Algorithm for WBANs namely I-RAW, distributed energy-efficient two-hop-based clustering and routing namely DECR and Modified Power Line System (M-POLC). This paper presents a dual-band implantable antenna with coaxial probe feeding for wireless biotelemetry applications. The antenna features spiral patches, resulting in a compact size of 27 × 14 × 1.6 mm³. It can operate in two different frequency bands, 241–641 MHz and 1.17–2.06 GHz, providing coverage for the medical implant communication service (MICS) band and the industrial, scientific, and medical (ISM) band. This simple design offers improved return loss and higher bandwidths that are achieved by incorporating patch slots and shortening pins in spiral patches, representing a significant contribution to the field of dual-band antenna design for wireless biotelemetry systems. In the human body phantom model, the SAR values of 48.9 mW/kg and 1.19 W/kg are achieved, which satisfy the IEEE standard safety constraints. Background: Dexmedetomidine is a sedative drug with a wide safety margin. Case presentation: We present a case of accidental iatrogenic dexmedetomidine overdose in an adult patient during high-intensity focused ultrasound (HIFU) treatment. This is the first case report of an adult patient receiving an intravenous push of dexmedetomidine. Overdose resulted in severe oversedation, but symptoms receded spontaneously over time. AbstractWildlife tourism can benefit conservation of target species, however, it can have detrimental effects on animal behaviour and physiology. Whale shark Rhincodon typus tourism has seen recent rapid growth globally, but methods and regulations vary widely. Ningaloo Reef, Australia is considered “gold standard” whale shark tourism management due to legal regulation, strict enforcement, and high compliance. Rather than relying on observational data, we used biotelemetry to collect high-resolution data (20 Hz) on whale sharks’ movement behaviour in the presence or absence of tourists. Tourism encounters lasted an average of 62 min and swimming with tourists increased the activity levels of larger (> 7 m) but not smaller sharks. Given that activity levels positively correlate with energetic costs, it is likely the 18% increase seen in activity of large sharks would have incurred additional energetic costs. However, when considered as a proportion of daily energy requirements, these additional costs were only incurred for an average of 4% of a whale shark’s day. The tourism-induced impacts we found on the endangered whale sharks at this highly regulated tourism site would not have been apparent from purely observational studies, highlighting the utility of biotelemetry to quantify tourism-related impacts on wildlife. A previously healthy 27-year- old man was admitted to the acute neurology ward with events involving his face, throat and upper limb, which video telemetry later confirmed were refractory focal seizures. He also had progressive pyramidal features, dysarthria and ataxia. MR scans of the brain identified progressive bilateral basal ganglia abnormalities, consistent with Leigh syndrome. However, extensive laboratory and genetic panels did not give a unifying diagnosis. A skeletal muscle biopsy showed no histopathological abnormalities on routine stains. Sequencing of the entire mitochondrial genome in skeletal muscle identified a well-characterised pathogenic variant (m.10191T>C in MT-ND3; NC_012920.1) at 85% heteroplasmy in skeletal muscle. We discuss the clinical and molecular diagnosis of an adult presenting with Leigh syndrome, which is more commonly a paediatric presentation of mitochondrial disease, and how early recognition of a mitochondrial cause is important to support patient care. To inform and optimise a cochlear implant (CI) fitting software design through an analysis of big data to define array-specific comfort (C) level profiles, frequently-used MAP parameters, and the minimum number of Neural Response Telemetry thresholds (tNRT) needed to create an accurate profile. To evaluate the software's ease of use and completion time for AutoNRT®s. MAPs analysis. Clinical study evaluating software use in creating MAPs, addressing sound-quality issues and setting patient goals. MAPs (N = 39,885); CI recipients (N = 47) and clinicians (N = 19). Distinct C-level profiles were observed for lateral-wall, contour, and slim-modiolar electrode arrays. Default settings were used for most MAP parameters (13/16) except for Pulse Width, Rate, and Maxima. Nine tNRT measurements were required for an accurate C-level profile. Measurement-time of nine tNRTs via the new algorithm was comparable to five tNRTs using the previous algorithm. Nearly all (99%) clinical tasks were completed by clinicians with the first use of the software. Most CI recipients (79.5%) rated goal-setting as valuable. Custom Sound Pro fitting software developed based on big data analysis incorporates a guided fitting workflow and expected fitting ranges. Introduction: The increasing use of insertable cardiac monitors (ICMs) for long‐term continuous arrhythmia monitoring creates a high volume of transmissions and a significant workload for clinics. The ability to remotely reprogram device alert settings without in‐office patient visits was recently introduced, but its impact on clinic workflow compared to the previous ICM iteration is unknown. Methods: The aim of this real‐world study was to evaluate the impact of device reprogramming capabilities on ICM alert burden and on clinic workflow. Deidentified data was obtained from US patients and a total of 19 525 receiving a LINQ II were propensity score‐matched with 19 525 implanted with LINQ TruRhythm (TR) ICM based on age and reason for monitoring. Results: After reprogramming, ICM alerts reduced by 20.5% (p <.001). Compared with patients monitored with LINQ TR, patients with LINQ II had their device reprogrammed sooner after implant and more frequently during follow‐up. Adoption of remote programming was projected to lead to an annual total clinic time savings of 211 h per 100 ICM patients managed. Conclusion: These data suggest that utilization of ICM alert reprogramming has increased with remote capabilities, which may reduce clinic and patient burden for ICM follow‐up and free clinician time for other valuable patient care activities. The migratory behavior and spawning of Curimatá‐pacu (Prochilodus argenteus) were assessed in a free‐flowing remnant spanning approximately 450 km. This evaluation was conducted using radio‐tagging and egg sampling, with identification performed through metabarcoding techniques. Among the studied individuals, half migrated 100–365 km upstream, primarily in response to the initial rise in river discharge at the onset of the wet season. Conversely, the remaining fish did not exhibit upstream movement, indicating a partial migratory behavior. The presence of eggs exclusively in the headwaters suggests that breeding occurred solely among fish inhabiting the upstream sites. Background: Postoperative atrial fibrillation (POAF) is the most common arrhythmia following cardiac surgery (CS). It may occur between the 1st and the 4th postoperative day as acute POAF or between the 5th and the 30th as subacute (sPOAF). sPOAF is associated with higher thromboembolic risk, which consistently increase patients' morbidity. Neutrophil‐to‐lymphocyte ratio (NLR) is a low‐cost inflammatory index proposed as possible POAF predictor. Identification of patients' risk categories might lead to improved postoperative outcomes. Methods: The aim was to assess the incidence of sPOAF and to identify possible predictors in patients performing cardiovascular rehabilitation (CR) after CS. A single‐center cohort study was performed on 737 post‐surgical patients admitted to CR on sinus rhythm. Continuous monitoring with 12‐lead ECG telemetry was performed. We evaluated the predictive role of anamnestic, clinical, and laboratory data, including baseline NLR. Results: Subacute POAF was documented in 170 cases (23.1%). At the multivariate analysis, age (OR 1.03; p =.001), mitral valve surgery (OR 1.77; p =.012), acute POAF (OR 2.97; p <.001), and NLR at baseline (OR 1.13; p =.042) were found to be independent predictive factors of sPOAF following heart surgery. Conclusions: sPOAF is common after CS. Age, mitral valve procedures, acute POAF, and preoperative NLR were proved to increase sPOAF occurrence in CR. NLR is an affordable and reliable parameter which might be used to qualify the risk of arrhythmias at CR admission. Acoustic telemetry is a popular approach used to track many different aquatic animal taxa in marine and freshwater systems. However, information derived from focal studies is typically resource‐ and geography‐limited by the extent and placement of acoustic receivers. Even so, animals tagged and tracked in one region or study may be detected unexpectedly at distant locations by other researchers using compatible equipment, who ideally share that information. Synergies through national and global acoustic tracking networks are facilitating significant discoveries and unexpected observations that yield novel insight into the movement ecology and habitat use of wild animals. Here, we present a selection of case studies that highlight unexpected tracking observations or absence of observations where we expected to find animals in aquatic systems around the globe. These examples span freshwater and marine systems across spatiotemporal scales ranging from adjacent watersheds to distant ocean regions. These unexpected movements showcase the power of collaborative telemetry networks and serendipitous observations. Unique and unexpected observations such as those presented here can capture the imagination of both researchers and members of the public, and improve understanding of movement and connectivity within aquatic ecosystems. The pandemic highlighted a need to ensure appropriate care for patients with COVID who resided on a medical-surgical unit while also experiencing an acute mental health crisis. This paper proposes a slotted patch antenna with wide bandwidth covering ISM frequency band (2.40-2.48 GHz) for implantable biotelemetry applications. A homogeneous skin phantom (HSP) model proves the usability of the proposed antenna in in-body environments. At a resonance frequency of 2.42 GHz, the design shows an S11 parameter of -35.56 dB, a percentage impedance bandwidth of 66.6% (2-4 GHz), and the maximum peak gain of -24.80 dBi. To validate the simulated results, the designed antenna was fabricated and measured, showing good compliance with the expected results. To ensure tissue safety, a specific absorption rate (SAR) is simulated for the proposed antenna which satisfies the requirements of IEEE standards, with a value of 87.75 W/kg for 10 g of tissue. The proposed antenna shows a telemetry range of 11 and 6.3 m at 7 kbps and 100 kbps data rates, respectively. The key features of the proposed antenna include the following: miniaturization, good S parameters, wide bandwidth, low SAR, good telemetry range, and high gain.

Abstract Background The ability to detect the location of free-ranging sea turtles over time is desirable for ecological, conservation, and veterinary studies, but existing detection methods have limited sensitivity or longevity. Externally attached acoustic transmitters have variable, and sometimes short retention times for sea turtles. For several vertebrate taxa, surgically implanted acoustic transmitters have proven to be safe and effective for long-term detection; however, implanted transmitters have not yet been used for turtles. Results In this pilot study, INNOVASEA acoustic transmitters were surgically implanted subcutaneously in the pre-femoral region of fifteen hospitalized loggerhead sea turtles (Caretta caretta) that had been rehabilitated after stranding due to cold-stunning. Model V16-4H transmitters (estimated battery longevity = 2435 days) were implanted in turtles measuring ≥ 50 cm straight carapace length (SCL), and model V13-1H transmitters (estimated battery longevity = 1113 days) were implanted in turtles measuring 30–49 cm SCL. Incision healing was monitored over several months prior to release. Twelve turtles’ incisions healed without complication, on average, 55 days after surgery (median 47, range 41–100). Three turtles experienced incision complications, two of which healed after a second surgery, while the third required transmitter removal to promote healing. One of the fourteen implanted transmitters was confirmed to be dysfunctional prior to release, although it had been functional prior to implantation. To date, 100% of turtles released with functional acoustic transmitters (n = 13) have been detected a total of 915 times by 40 individual acoustic receivers off the coasts of Massachusetts, Rhode Island, New York, Virginia, North Carolina, and in southern New England offshore waters. Turtles with transmitters generated 5–235 detections (mean 70, median 43) on 1–13 individual acoustic receivers (mean 5, median 5) for periods of 3–400 days post-release (mean 118, median 87). Total detections and detection durations for these individuals are expected to increase over time due to anticipated transmitter battery longevity. Conclusions This study demonstrates that surgically implanted acoustic transmitters are effective for the detection of free ranging sea turtles, but refinement of surgical methodology is needed in light of the observed complications. Monitoring of healing is critical when evaluating novel surgical techniques in wildlife.
