- Abstract
The study assessed the clinical response to add-on brivaracetam (BRV) in real-world practice by means of time-to-baseline seizure count methodology. Patients with focal epilepsy who were prescribed add-on BRV were identified. Primary endpoint was the time-to-baseline seizure count defined as the number of days until each patient experienced the number of focal seizures that occurred in the 90 days before BRV initiation. Subgroup analysis was performed according to levetiracetam (LEV) status (naive vs prior use). Three-hundred eighty-seven patients were included. The overall median time-to-baseline seizure count was 150 (95% confidence interval [CI] = 130-175) days. The median time-to-baseline seizure count was 198 (lower limit of 95% CI = 168) days for LEV-naive patients, 126 (95% CI = 105-150) days for patients with prior LEV use and withdrawal due to insufficient efficacy, and 170 (95% CI = 128-291) days for patients who discontinued LEV due to adverse events (P =.002). The number of prior antiseizure medications (adjusted hazard ratio [adjHR] = 1.07, 95% CI = 1.02-1.13, P =.009) and baseline monthly seizure frequency (adjHR = 1.004, 95% CI = 1.001-1.008, P =.028) were independently associated with the primary endpoint. Add-on BRV improved seizure control in LEV-naive and LEV-prior patients. The time-to-baseline seizure count represents an informative endpoint alongside traditional study outcomes and designs.

Introduction: In randomized controlled trials, add-on brivaracetam (BRV) reduced seizure frequency in patients with drug-resistant focal epilepsy. Most real-world research on BRV has focused on refractory epilepsy. The aim of this analysis was to assess the 12-month effectiveness and tolerability of adjunctive BRV when used as early or late adjunctive treatment in patients included in the BRIVAracetam add-on First Italian netwoRk Study (BRIVAFIRST). Methods: BRIVAFIRST was a 12-month retrospective, multicenter study including adult patients prescribed adjunctive BRV. Effectiveness outcomes included the rates of sustained seizure response, sustained seizure freedom, and treatment discontinuation. Safety and tolerability outcomes included the rate of treatment discontinuation due to adverse events (AEs) and the incidence of AEs. Data were compared for patients treated with add-on BRV after 1-2 (early add-on) and ≥ 3 (late add-on) prior antiseizure medications. Results: A total of 1029 patients with focal epilepsy were included in the study, of whom 176 (17.1%) received BRV as early add-on treatment. Summary: Background: Insertional deep digital flexor tendon (DDFT) lesions with concurrent injury to the flexor surface of the distal phalanx are uncommon and there is limited information regarding prognosis. Objectives: To provide descriptive data on horses with insertional DDFT injuries and determine whether there is any association between magnetic resonance imaging (MRI) findings, and outcome. Study design: Retrospective case series. Methods: Horses with lameness localised to the foot that underwent low field standing MRI between January 2015 and April 2019 were included. Injury of the DDFT, presence of short tau inversion recovery (STIR) hyperintensity at the flexor surface of the distal phalanx and concurrent lesions were recorded. Follow‐up information was obtained by telephone questionnaire. Results: Thirty‐two horses fulfilled the inclusion criteria. Thirty‐one limbs had DDFT insertional lesions on MRI with three limbs showing hyperintense STIR signal of the flexor surface of the distal phalanx without a tendon lesion. Seventeen DDFT lesions were graded as mild and 14 as severe. Thirteen limbs showed hyperintense STIR signal within the tendon and 15 limbs showed hyperintense STIR signal within the flexor surface of the distal phalanx at the insertion of the DDFT and/or the distal sesamoidean impar ligament (DSIL). Eight horses returned to previous athletic function, 8 returned to a lower level of athletic activity, 4 were retired, 9 were subjected to euthanasia and 3 were lost to follow‐up. No significant association was found between DDFT lesion severity, hyperintense STIR signal within the DDFT, hyperintense STIR signal at the flexor surface of the distal phalanx and outcome post‐injury. Main limitations: Small sample size and reliance on clients to provide accurate follow‐up information. Conclusions: Insertional DDF tendinopathy carries a guarded prognosis for return to previous athletic function. The presence of hyperintense STIR signal at the flexor surface of the distal phalanx did not appear to influence the prognosis. [ABSTRACT FROM AUTHOR]

Background: In randomized controlled trials, add-on brivaracetam (BRV) reduced seizure frequency in patients with drug-resistant focal epilepsy. Studies performed in a naturalistic setting are a useful complement to characterize the drug profile. Objective: This multicentre study assessed the effectiveness and tolerability of adjunctive BRV in a large population of patients with focal epilepsy in the context of real-world clinical practice. Methods: The BRIVAFIRST (BRIVAracetam add-on First Italian netwoRk STudy) was a retrospective, multicentre study including adult patients prescribed adjunctive BRV. Patients with focal epilepsy and 12-month follow-up were considered. Main outcomes included the rates of seizure‐freedom, seizure response (≥50% reduction in baseline seizure frequency), and treatment discontinuation. The incidence of adverse events (AEs) was also considered. Analyses by levetiracetam (LEV) status and concomitant use of strong enzyme-inducing antiseizure medications (EiASMs) and sodium channel blockers (SCBs) were performed. Results: A total of 1029 patients with a median age of 45years (33–56) was included. At 12 months, 169 (16.4%) patients were seizure-free and 383 (37.2%) were seizure responders. Summary: A 5‐year‐old Thoroughbred gelding with recent history of head trauma presented with multiple facial swellings, bilateral mucopurulent nasal discharge, neck pain, inappetence and depression. On computed tomographic examination, lesions within the pituitary fossa and structures adjacent to the right guttural pouch were identified. Soft tissue swelling was seen in the dorsal aspect of the right guttural pouch surrounding several cranial nerves, with fluid‐like material in the dependent portions of the right guttural pouch. A CSF sample revealed mild mixed pleocytosis and increased protein concentration. The horse had concurrent periapical disease of the 209‐cheek tooth and mild left sinusitis. The horse's demeanour deteriorated requiring euthanasia. Post‐mortem examination revealed a pituitary gland abscess. [ABSTRACT FROM AUTHOR]

Summary: A 17‐year‐old Irish Draught cross gelding with a history of chronic recurrent right hindlimb cellulitis was evaluated due to acute onset of severe lameness affecting this limb. Scintigraphic examination identified marked radiopharmaceutical uptake within the central and third tarsal bones and the lameness improved following tibial and peroneal nerve blocks. Distal intertarsal joint collapse was observed but the radiographic changes were considered insufficient to explain the lameness severity. Magnetic resonance imaging revealed marked bone pathology affecting both the central and third tarsal bones. A subcutaneous abscess developed on the dorsolateral aspect of the limb, but clear evidence of joint infection was not established. Computed tomography showed lytic lesions affecting mainly the third tarsal bone, so the animal was treated surgically for suspected osteomyelitis involving this bone by implantation of gentamicin impregnated polymethylmethacrylate. Shortly following surgery the horse developed gastric rupture and was euthanased. Osteonecrosis of the third tarsal bone was identified, but sepsis was not demonstrated to be the cause. It is hypothesised that osteonecrosis developed as a sequel to chronic osteomyelitis which may have responded to previous antimicrobial treatment. The report also highlights the importance of gastric disorders as a differential for discomfort in animals undergoing treatment for orthopaedic disease. [ABSTRACT FROM AUTHOR]

- Abstract
The European Space Agency's Planck satellite, launched on 14 May 2009, is the third-generation space experiment in the field of cosmic microwave background (CMB) research. It will image the anisotropies of the CMB over the whole sky, with unprecedented sensitivity (${{\Delta T}\over T}$ ~ 2 × 10-6) and angular resolution (~5 arcmin). Planck will provide a major source of information relevant to many fundamental cosmological problems and will test current theories of the early evolution of the Universe and the origin of structure. It will also address a wide range of areas of astrophysical research related to the Milky Way as well as external galaxies and clusters of galaxies. The ability of Planck to measure polarization across a wide frequency range (30-350 GHz), with high precision and accuracy, and over the whole sky, will provide unique insight, not only into specific cosmological questions, but also into the properties of the interstellar medium. This paper is part of a series which describes the technical capabilities of the Planck scientific payload. It is based on the knowledge gathered during the on-ground calibration campaigns of the major subsystems, principally its telescope and its two scientific instruments, and of tests at fully integrated satellite level. It represents the best estimate before launch of the technical performance that the satellite and its payload will achieve in flight. In this paper, we summarise the main elements of the payload performance, which is described in detail in the accompanying papers. In addition, we describe the satellite performance elements which are most relevant for science, and provide an overview of the plans for scientific operations and data analysis.

Summary: Radiographic technology has rapidly advanced over the last decade with the use of both computed radiography and digital radiography now being common in equine practice. Image quality is critical for optimal diagnostic accuracy, so identification of factors that negatively influence quality is vital. The most commonly encountered problems include positioning errors, exposure anomalies, movement artefacts, labelling errors and image processing faults. The aim of this review is to describe common radiographic faults that will allow the equine practitioner to recognise and learn how to prevent these issues, thus improving image quality. This will aid in improving diagnostic accuracy and will enhance radiation safety by reducing the number of repeat exposures required. [ABSTRACT FROM AUTHOR]

- Abstract
Cryogenic Low Noise Amplifiers, based on MMIC HEMT technology, require a careful packaging to reach optimal performance. Differences between modeled and measured performance can often be related to chip mounting details. In the framework of the development of new cryogenic LNAs, described in a companion paper, we have developed a specific packaging to host W-band cryogenic MMIC LNAs. We present here some of the main factors analyzed in the design and chip integration activities. In particular, mechanical and thermal modeling, LNA chip gluing and adhesive properties, sensitivity to components integration accuracy (i.e. deviation from the ideal orientation). Preliminary test results are also reported.

- Abstract
The European Space Agency's Planck satellite, launched on 14 May 2009, is the third-generation space experiment in the field of cosmic microwave background (CMB) research. It will image the anisotropies of the CMB over the whole sky, with unprecedented sensitivity ( ~ 2 × 10-6) and angular resolution (~5 arcmin). Planck will provide a major source of information relevant to many fundamental cosmological problems and will test current theories of the early evolution of the Universe and the origin of structure. It will also address a wide range of areas of astrophysical research related to the Milky Way as well as external galaxies and clusters of galaxies. The ability of Planck to measure polarization across a wide frequency range (30-350 GHz), with high precision and accuracy, and over the whole sky, will provide unique insight, not only into specific cosmological questions, but also into the properties of the interstellar medium. This paper is part of a series which describes the technical capabilities of the Planck scientific payload. It is based on the knowledge gathered during the on-ground calibration campaigns of the major subsystems, principally its telescope and its two scientific instruments, and of tests at fully integrated satellite level. It represents the best estimate before launch of the technical performance that the satellite and its payload will achieve in flight. In this paper, we summarise the main elements of the payload performance, which is described in detail in the accompanying papers. - Abstract
Reasons for performing study The distribution of lesions detected using magnetic resonance imaging ( MRI) of hind feet has not been reported. Objective To evaluate type and distribution of lesions detected using MRI in hind feet of lame horses and to compare the findings with those reported in front feet; to document follow-up information. Methods Horses were selected if hindlimb lameness was abolished by plantar nerve blocks performed at the base of the proximal sesamoid bones. Magnetic resonance images were acquired using either a low-field or a high-field magnet, and all images were evaluated retrospectively by both authors. A primary diagnosis was defined as the most likely cause of lameness, based also on clinical findings and results of other imaging techniques. If ≥3 structures were equally affected, the horse was classified as having multiple injuries. Follow-up information was collected by telephone questionnaire; outcome was classified as return to former level of athletic function, working at a lower level or retired. Results Thirty-eight horses were included. Primary collateral sesamoidean ligament ( CSL) injury was identified in 9 horses. Nine horses had multiple injuries, 8 had primary bone pathology, 5 had collateral desmopathy of the distal interphalangeal joint, one had primary deep digital flexor tendonitis, 2 had navicular bone pathology and 4 had lesions involving other structures within the foot. Seven horses were subjected to euthanasia shortly after diagnosis; 6 as a result of poor prognosis and one as a result of post anaesthetic complications, 4 of which had primary bone lesions, 2 multiple lesions and one distal interphalangeal joint pathology. Follow-up information was available for 24 horses. Eleven horses returned to previous performance and 2 to a lower level of performance; 9 horses were retired because of recurrent lameness. Five of 8 horses with CSL injury returned to previous performance and 2 to a lower level. Conclusions and potential relevance Collateral sesamoidean ligament injury and primary bone pathology are commonly observed on MRI examination of hind feet, and the prognosis is reasonable. Summary Reasons for performing study: The significance of distal border fragments of the navicular bone is not well understood. There are also no objective data about changes in thickness and proximal/distal extension of the palmar cortex of the navicular bone. Objectives: To describe the distribution of distal border fragments and their association with other radiological abnormalities of the navicular bone and describe the shape of the navicular bone in sound horses and horses with foot-related lameness, including navicular pathology. Methods: Sound horses had radiographs acquired as part of a prepurchase examination. Lame horses had forelimb lameness abolished by palmar nerve blocks performed at the base of the proximal sesamoid bones. Diagnosis was assigned prospectively based on results of local analgesia and all imaging findings. The thickness of the palmar cortex of the navicular bone and size of proximal/distal extensions were measured objectively. Other radiological abnormalities were evaluated subjectively and each navicular bone graded. Results: Fifty-five sound and 377 lame horses were included. All measurements were larger in lame compared with sound horses except the size of the distal extension of the palmar cortex. Fragments were observed in 3.6 and 8.7% of sound and lame horses respectively and in 24.1% of horses with a diagnosis of primary navicular pathology. There was an association between fragments and overall navicular bone grade, radiolucent areas at the angles of the distal border of the navicular bone and number and size of the synovial invaginations. Conclusions and potential relevance: The palmar cortex of the navicular bone was thicker in lame compared with sound horses. Distal border fragments were most frequent in horses with navicular pathology. Evaluation of changes in shape of the navicular bone may also be important for recognition of pathological abnormalities of the bone. [ABSTRACT FROM AUTHOR]

Summary Reasons for performing study: Distal border fragments of the navicular bone can be seen in lame and nonlame horses and their clinical significance remains open to debate. Objectives: To describe the magnetic resonance imaging (MRI) appearance of distal border fragments and the adjacent navicular bone. To investigate the relationship between fragments and other abnormalities of the navicular bone and the distal sesamoidean impar ligament (DSIL). Methods: Horses were included if pain causing forelimb lameness was localised to the foot and high-field MR images were acquired. The size and location of distal border fragments were recorded. Abnormalities in the adjacent navicular bone were graded to obtain a fragment grade. A total navicular bone grade was assigned. The DSIL was also graded. A Chi-squared test was used to test for associations between the presence of a fragment and specific lesions involving the distal border of the navicular bone, the total grade of the navicular bone, and the grade of the DSIL. Results: 427 horses were included and 111 fragments observed. There was a significant association between the presence of a fragment and the total navicular bone grade, osseous cyst-like lesions, increased number and size of the synovial invaginations of the distal border, increased signal intensity on fat suppressed images and size of distal border entheseophytes. Conclusions: There is an association between distal border fragments and other pathological MRI abnormalities of the navicular bone. Potential relevance: Distal border fragments are part of navicular disease, but their contribution to pain and lameness remains to be clarified. [ABSTRACT FROM AUTHOR]

Objective: Post-stroke epilepsy (PSE) is one of the most common causes of acquired epilepsy and accounts for about 10-15% of all newly diagnosed epilepsy cases. However, evidence about the clinical profile of antiseizure medications in the PSE setting is currently limited. Brivaracetam (BRV) is a rationally developed compound characterized by high-affinity binding to synaptic vesicle protein 2A. The aim of this study was to assess the 12-month effectiveness and tolerability of adjunctive BRV in patients with PSE treated in a real-world setting. Methods: This was a subgroup analysis of patients with PSE included in the BRIVAracetam add-on First Italian netwoRk Study (BRIVAFIRST). The BRIVAFIRST was a 12-month retrospective, multicentre study including adult patients prescribed adjunctive BRV. Effectiveness outcomes included the rates of seizure response (≥50% reduction in baseline seizure frequency), seizure‐freedom, and treatment discontinuation. Safety and tolerability outcomes included the rate of treatment discontinuation due to adverse events (AEs) and the incidence of AEs. Results: Patients with PSE included in the BRIVAFIRST were 75 and had a median age of 57 (interquartile range, 42-66) years. The median daily doses of BRV at 3, 6, and 12 months from starting treatment were 100 (100-150) mg, 125 (100-200) mg and 100 (100-200) mg, respectively. At 12 months, 32 (42.7%) patients had a reduction in their baseline seizure frequency by at least 50%, and the seizure freedom rates was 26/75 (34.7%). During the 1-year study period, 10 (13.3%) patients discontinued BRV. The reasons of treatment withdrawal were insufficient efficacy in 6 (8.0%) patients and poor tolerability in 4 (5.3%) patients. Adverse events were reported by 13 (20.3%) patients and were rated as mild in 84.6% and moderate in 15.4% of cases. Significance: Adjunctive BRV was efficacious and generally well-tolerated when used in patients with PSE in clinical practice. Adjunctive BRV can be a suitable therapeutic option for patients with PSE.

The first part of the article discusses the theorical and performative problems involved in the use of phrases said "tra sé" in musical numbers as reflected in eighteenth- end nineteenth-century treatises. The second part examines some examples taken from Jommelli's works, both serious and comic, in order to identify recurrent technics and features.

Cryogenic Low Noise Amplifiers, based on MMIC HEMT technology, require a careful packaging to reach optimal performance. Differences between modeled and measured performance can often be related to chip mounting details. In the framework of the development of new cryogenic LNAs, described in a companion paper, we have developed a specific packaging to host W-band cryogenic MMIC LNAs. We present here some of the main factors analyzed in the design and chip integration activities. In particular, mechanical and thermal modeling, LNA chip gluing and adhesive properties, sensitivity to components integration accuracy (i.e. deviation from the ideal orientation). Preliminary test results are also reported.

22 páginas, 16 figuras, 6 tablas.-- Planck Science Team: et al., The European Space Agency's Planck satellite, launched on 14 May 2009, is the third-generation space experiment in the field of cosmic microwave background (CMB) research. It will image the anisotropies of the CMB over the whole sky, with unprecedented sensitivity ( ~ 2 × 10-6) and angular resolution (~5 arcmin). Planck will provide a major source of information relevant to many fundamental cosmological problems and will test current theories of the early evolution of the Universe and the origin of structure. It will also address a wide range of areas of astrophysical research related to the Milky Way as well as external galaxies and clusters of galaxies. The ability of Planck to measure polarization across a wide frequency range (30-350 GHz), with high precision and accuracy, and over the whole sky, will provide unique insight, not only into specific cosmological questions, but also into the properties of the interstellar medium. This paper is part of a series which describes the technical capabilities of the Planck scientific payload. It is based on the knowledge gathered during the on-ground calibration campaigns of the major subsystems, principally its telescope and its two scientific instruments, and of tests at fully integrated satellite level. It represents the best estimate before launch of the technical performance that the satellite and its payload will achieve in flight. In this paper, we summarise the main elements of the payload performance, which is described in detail in the accompanying papers. In addition, we describe the satellite performance elements which are most relevant for science, and provide an overview of the plans for scientific operations and data analysis.
