Abstract

Coronavirus disease 2019 (COVID-19) outbreak, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), poses an unprecedented threat to public health and healthcare systems. It presents unusual pathophysiological effects mainly characterized by immune-inflammatory response and prothrombotic state causing acute respiratory distress syndrome and multiple organ failure. SARS-CoV-2 enters target cells after binding to the angiotensin-converting enzyme 2 (ACE2) receptor and therefore has a direct effect on the renin-angiotensin system (RAS). Apart from affecting numerous organs including lungs, heart, gastrointestinal system, spleen, brain and kidneys, the spike protein of SARS-CoV-2 could attack hematopoietic stem cells and hematopoietic progenitor cells in bone marrow (BM) microenvironment together with the precursor and mature blood cells. Within this hematopoietic viral spread context, it is crucial to search the clinicopathological correlations of COVID-19 in order to develop specific potential therapeutics against pleiotropic SARS-CoV-2 actions. Therefore, pharmacological disruption of the pathological cross-talk of local BM RAS and pulmonary RAS via administration of the tissue-RAS modulating agents such as soluble ACE2, angiotensin (1-7), TXA127 and MAS receptor agonists may prevent the clinical progression of the COVID-19 syndrome via reducing the hematopoietic virus propagation and systemic multi-organ spread.

Abstract

Objective: Long-term care facilities (LTCF) caring for chronically ill residents and older adults are high-risk centers for acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) outbreaks. The aim of this study was to report an outbreak of coronavirus disease 2019 (COVID-19) in a single LTCF located in Canakkale, Turkey, and describe symptoms and chronological aspects of the disease among both LTCF residents and staff. Materials-methods: This retrospective study analysis the demographic and clinical characteristics of SARS-CoV-2 infected LTCF residents and staff in a middle-sized LTCF located in Canakkale. Results: A total of 69/102 residents and 21/50 staff members of a single LTCF were infected with SARS-CoV-2. Residents exhibited both general and respiratory signs (37.7% fever, 31.9 % dyspnea, 29% weakness and fatigue, and 60.9% cough). ICU admission was significantly elevated in patients over >65 years old (p=0.005). All deaths (n=5) was observed in patients over 65 years (0.003). Conclusion: The increased rates of mortality among older LTCF residents justify mass screening in LTCF, and obliges responsible authorities to identify and exclude potentially infected staff and visitors and carry out proper infection prevention and control measures.

7. Retrospective Analysis of demographic and laboratory characteristics of patients with liver cirrhosis: A single center experience.

Abstract

Liver cirrhosis is a progressive, irreversible disease constituted by a decrease in normal hepatocytes, diffuse fibrosis, and regeneration nodules due to liver damage developed by the chronic inflammation process. This study aims to evaluate patients with liver cirrhosis who applied to Çanakkale Onsekiz Mart University (COMU) Hospital and met the criteria in terms of demographic, etiological, and laboratory values. Patient data were recorded from the hospital data recording system that was admitted to COMU hospital between January 1, 2013, and January 01, 2022. A total of 199 patients with liver cirrhosis were included in the study. Of these patients, 38.2% were female, and 61.8% were male. Their mean age was 63.43±10.73 years. When the patients were evaluated with regard to etiological distribution, the top three etiological causes of cirrhosis were cryptogenic, alcohol-induced, and HBV in 27.6%, 27.1%, and 25.1% of patients, respectively. In terms of complications, ascites were detected in 43.2% of the patients, esophageal varices in 56.3%, splenomegaly in 69.3% and hepatic encephalopathy in 7%. Over the years, the number of patients with cirrhosis due to viral hepatitis has decreased due to an increase in viral hepatitis vaccination campaigns and the spread of antiviral treatments. However, due to the rise in chronic alcohol use, alcohol appears more frequently as a significant risk factor for the etiology of liver cirrhosis. [ABSTRACT FROM AUTHOR]

Abstract

Amaç: Çoğu ülkede, yaşlı nüfusun acil servislere (AS’ler) giderek artan bir başvuru oranı vardır. Özellikle bu yaşlı hastalar, fonksiyonel ve yaşam kalitesini etkileyen birden fazla komorbiditeye sahip oldukları için genç hastalardan farklıdır. Bu çalışmanın amacı, Charlson komorbidite indeksinin (CCI) çok yaşlı hasta popülasyonunun kısa ve uzun vadeli prognozunu tahmin edip etmediğini belirlemektir.Gereç ve Yöntem: Çalışma, 2013 ve 2018 yılları arasında Çanakkale Onsekiz Mart Üniversitesi (ÇOMÜ) Hastanesine başvuran 85 yaş ve üstü hastaların acil servis (AS) ziyaretlerinin tanımlayıcı, retrospektif bir analiziydi. Hastaların demografik verileri CCI'ye göre analiz edildi. Değişkenlerin mortaliteyi etkileyip etkilemediğini belirlemek için Cox-regresyon analizleri yapıldı.Bulgular: Yaş ortalaması 86,96±2,49 olan 85 yaş ve üzeri (507 erkek, 635 kadın) toplam 1142 hasta çalışmaya dahil edildi. Çok değişkenli Cox regresyon analizine göre erkek cinsiyet, CCI ≥6 ve yoğun bakım ünitesine yatış, artan mortalite oranları ile anlamlı şekilde ilişkiliydi.Sonuç: CCI akut hasta olarak hastaneye yatırılan çok yaşlı hastalarda kısa ve uzun dönem prognozu tahmin eder. CCI, hastaneden taburculukta iyileşmenin bir göstergesi olarak, kabulde, çok yaşlı hastaların seçilmesinde kullanılabilir., Purpose: In most countries, there is an ever-increasing admission rate of the elderly population into emergency departments (EDs). In particular, these elderly patients differ from younger patients because they have multiple comorbidities that affect the functionality and quality of life. The goal of this study is to reveal whether the Charlson comorbidity index (CCI) foresee the short- and long-term prognosis of the super-elderly patient population. Materials and Methods: The study was a descriptive, retrospective analysis of emergency department (ED) admissions by patients over 85 years of age and admitted to the Canakkale Onsekiz Mart University (COMU) Hospital between 2013 and 2018. The demographic data of the patients were analyzed according to CCI. Cox-regression analyses were conducted to determine whether the variables affected mortality.Results: A total of 1142 patients aged 85 and older (507 men, 635 women) with a mean age of 86.96±2.49 were included in the study. According to the multivariable Cox regression analysis male gender, CCI ≥6 and ICU admission were significantly associated with increased mortality ratesConclusion: The CCI predicts short and long-term prognosis in acutely ill, hospitalized super-elderly patients. The CCI could be used to select super-elderly patients at admission as an indicator of improvement at hospital discharge.

Abstract

Background. Mortality due to Covid-19 and severe community-acquired pneumonia (CAP) remains high, despite progress in critical care management. We compared the precision of CURB-65 score with monocyte-to-lymphocyte ratio (MLR), neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR) in prediction of mortality among patients with Covid-19 and CAP presenting to the emergency department. Methods. We retrospectively analysed two cohorts of patients admitted to the emergency department of Canakkale University Hospital, namely (i) Covid-19 patients with severe acute respiratory symptoms presenting between 23 March 2020 and 31 October 2020, and (ii) all patients with CAP either from bacterial or viral infection within the 36 months preceding the Covid-19 pandemic. Mortality was defined as in-hospital death or death occurring within 30 days after discharge. Results. The first study group consisted of 324 Covid-19 patients and the second group of 257 CAP patients. The non- survivor Covid-19 group had significantly higher MLR, NLR and PLR values. In univariate analysis, in Covid-19 patients, a 1-unit increase in NLR and PLR was associated with increased mortality, and in multivariate analysis for Covid-19 patients, age and NLR remained significant in the final step of the model. According to this model, we found that in the Covid-19 group an increase in 1-unit in NLR would result in an increase by 5% and 7% in the probability of mortality, respectively. According to pairwise analysis, NLR and PLR are as reliable as CURB-65 in predicting mortality in Covid-19. Conclusions. Our study indicates that NLR and PLR may serve as reliable predictive factors as CURB-65 in Covid-19 pneumonia, which could easily be used to triage and manage severe patients in the emergency department. Abstract

Amaç: Ülseratif kolit (ÜK) hastalarında hastalık aktivitesini değerlendirmek için serum ve gayta belirteçleri sıklıkla kullanılmaktadır. Ancak bu belirteçlerden hiçbiri hastalık aktivitesini göstermede yeterli spesifite ve sensitiviteye sahip değildir. Bu sebeple ÜK hastalarında hastalık aktivitesini belirlemede kullanılabilecek basit, maliyet-etkin ve kolay uygulanabilir laboratuvar testlerine olan ihtiyaç her geçen gün artmaktadır. Bu çalışma ÜK hastalarında aktiviteyi tahmin etmede ortalama trombosit hacmi (MPV), platelet lenfosit oranı (PLO) ve nötrofil-lenfosit oranının (NLO) kullanışlı olup olmadığını belirlemek için planlandı. Bununla birlikte MPV, PLO ve NLO’nun ÜK’daki diğer inflamatuar belirteçlerle olan ilişkisinin varlığı da araştırıldı. Gereç ve Yöntem: Bu çalışmaya 63 aktif ÜK, 68 remisyondaki ÜK hastası ve 34 sağlıklı birey kontrol grubu olarak dahil edildi. Hastalık aktivitesi Truelove-Witts kriterlerine uygun olarak belirlendi. Hasta yaşı, cinsiyeti, inflamatuar belirteçler ve tam kan sayımı değerleri her hasta için kaydedildi. Sonuçlar: Aktif ÜK grubunda NLO ve PLO değerleri remisyondaki ÜK ve kontrol grubu ile kıyaslandığında belirgin bir şekilde yüksek idi. Aktif ve remisyondaki ÜK grubunda NLO ortalamaları sırasıyla 2,8±1,7 ve 2,1±1,3, PLO ortalamaları ise sırasıyla 177,1±85,5 ve 137,9±64,0 idi. MPV düzeyleri aktif ÜK grubunda remisyondaki ÜK grubuna gore anlamlı bir şekilde düşük idi (p=0,044). Korelasyon analizinde NLO ve PLO hem CRP ve hem ESR ile korele olarak saptandı. Tartışma: Bu çalışmada aktif ÜK hastalarında artmış NLO ve PLO ile azalmış MPV düzeyleri saptandı. Periferik kanda kolaylıkla ölçülebilecek MPV, NLO ve PLO gibi ölçümlerle ÜK hastalarında hastalık aktivitesi saptanabilir ve bulgu intestinal inflamasyonu değerlendirmede ek katkılar sağlayabilir.
