Abstract

Undue alarm about the safety of oral contraceptives (OCs) containing desogestrel or gestodene was caused when the results of three unpublished reports (a multinational study by the World Health Organization [WHO] the Jick cohort study on record linkage and the Spitzer transnational study) revealed a mild association of these OCs with deep venous thrombosis and pulmonary embolism. The WHO cautioned that its results (risk of venous thrombosis of 3/100000 women/year among nonusers 8 in the levonorgestrel group and 17 with gestodene and desogestrel) should be interpreted in light of the relatively small number of women affected and that further studies would be necessary to confirm the findings. Spitzer likewise urged caution in interpreting the findings and noted that deep vein thrombosis is a less serious condition than heart attacks or stokes (which the newer progestogens were developed to protect against). Before the findings were published and submitted to peer review however UK authorities advised doctors to restrict prescribing OCs containing desogestrel or gestodene. The European Agency for Evaluation of Medicinal Products decided not to withdraw combined OCs using these progestins but requested that further studies be performed and reminded physicians of the known risk factors of venous thromboembolism and that the risk is greater with pregnancy than with use of any combined OC. German authorities imposed more stringent restrictions on the three manufacturers of the OCs including restricting first-time use to women over 30 and adding information to the packaging information. It is highly unusual for epidemiological studies to be reported in the press before peer review and it is hoped that scientific discussion following publication will end some of the confusion this event has caused.
