Amaç: Gonyoskopi aracılı translüminal trabükülotomi (GATT) cerrahisi uygulanmış hastaların sonuçlarının bildirilmesi ve başarıya etki eden prognostik faktörlerin araştırılmasıdır.Gereç ve Yöntem: Çalışmaya 2015-2018 yılları arasında GATT cerrahisi uygulanan 53 hastanın 53 gözü retrospektif olarak dahil edildi. En az 6 ay takipli açık açılı glokom hastaları dahil edildi. Hastaların preoperatif demografik özellikleri, oftalmolojik muayene bulguları, göz içi basıncı (GİB) değeri, antiglokomatöz medikasyon sayısı, glokom tipi, postoperatif GİB değerleri, komplikasyonlar, kullandıkları antiglokomatöz molekül sayısı kaydedildi. Bu parametrelerin cerrahi başarı ile ilişkisi araştırıldı.Bulgular: Ortalama takip süresi 13,7 aydı. Medyan cup/disk oranı 0,8; ortalama mean deviasyon -18,48 db, pattern standart deviasyon 7,79 db ve retina sinir lifi tabakası kalınlığı 59,27µ idi. Olguların ortalama GİB 25,6±6,2 mmHg'den 14,6±3,5 mmHg'ye düşmüştü. Ortalama antiglokomatöz ilaç sayısı 3,2±0,78'dan 1,2±1,3'e düşmüştü. Hedef GİB 18 mmHg olarak belirlendiğinde başarı oranı %81,1 idi. Katarakt cerrahi ile kombinasyonun başarıya olumlu katkısı saptanırken makrohifema gelişiminin olumsuz etkisi saptandı. Hedef GİB 15 mmHg olarak belirlendiğinde %60,4 olarak hesaplandı. Hiçbir faktörün başarıya etkisi olmadığı saptandı. İncelenen parametrelerin cerrahi başarı hızına etkileri Cox'un oransal hazard regresyon analizleri ile incelendiğinde hiçbir faktörün her iki hedef GİB değeri için de başarıya etkisi olmadığı saptandı.Sonuç: GATT cerrahisi ileri evre glokom olgularında başarıyla uygulanabilecek bir cerrahidir. Postoperatif makrohifema gelişiminin başarıya olumsuz katkıları vardır. Katarakt cerrahisi ile kombinasyonun başarıya etkisi ise tartışmalıdır. Glokomun evresi cerrahi başarıyı etkilememektedir. Purpose: To report the results of patients who underwent gonioscopy-assisted transluminal trabeculotomy (GATT) surgery and to investigate prognostic factors affecting the surgical success.Materials and Methods: Fifty-three eyes of 53 patients who underwent GATT surgery between 2015 and 2018 were enrolled retrospectively. Open-angle glaucoma patients with at least 6 months follow-up were included. Preoperative demographic characteristics, ophthalmologic examination findings, intraocular pressure (IOP) value, number of anti-glaucomatous medications, glaucoma type, postoperative IOP, complications, number of anti-glaucomatous molecules were recorded. The effect of these parameters on surgical success was investigated. Results: The average follow-up time is 13,7 months. The median cup / disk ratio was 0,8; mean mean deviation was -18,48 dB, pattern standard deviation was 7,79 dB and retinal nerve fiber layer thickness was 59,27μ. The mean IOP decreased from 25,6 ± 6,2 mmHg to 14,6 ± 3,5 mmHg. The mean number of anti-glaucomatous medication decreased from 3,2 ± 0,78 to 1,2 ± 1,3. When the target IOP was 18 mmHg, the success rate was 81,1%. There was a positive effect of the combination with cataract surgery and a negative effect of postoperative macrohyphema development on success rates. Success rate was 60,4% when the target IOP was 15 mmHg. None of the parameters affected the success rate. When the effects of the parameters on the surgical success rate were analysed by Cox proportional hazards regression analysis, it was found that none of the factors affected the success rates for both target IOP values.Conclusion: GATT surgery can be successfully performed in advanced glaucoma cases. Postoperative macrohyphema might negatively affect surgical success rates. The contrubution of combination with cataract surgery is controversial. Glaucoma stage does not seem to affect surgical success. 88
