Preparedness for bioterrorist attacks and early recognition of specific agents are essential for public health. Emergency departments may play an important role in this field. The large spectrum of bioterrorism involves not only disastrous terrorism with mass casualties, but also microevents using low technology but producing civil unrest, disruption, disease, disabilities, and death. It aims not only to cause mortality and morbidity, but also to lead to social and political disruption. Preparedness appears to be the most potent defense against possible bioterrorist events. In this article, we aim to create awareness against biological agents and underline the importance of emergency departments in this public health problem. © 2018 Ali Kemal Erenler et al.

Bu olgunun materyalini Veteriner Eğitim Hastanesine getirilen 10 yaşlı dişi Terrier ırkı bir köpek oluşturdu. Anamnezde;hastada anoreksi, konstipasyon varlığı ve 3 yıl önce mastektomi operasyonu geçirdiği bilgisi alındı. Klinik ve ultrasonografikdeğerlendirmeler hepatik kolestazı doğruladı. İki hafta süreli; Sefazolin, Enrofloksasin, Ursodeoksikolik asit ve destekleyici sıvısağaltımı ile tedaviye başlandı. Bir ay sonra hasta, ani başlayan anoreksi ve sarılık şikayetleriyle tekrar hastaneye başvurdu.Rutin kan analizleri ve ultrasonografi ile şiddetli kolestazis ve safra kesesinde genişleme belirlendi. Deneysel laparotomi ilesafra kanallarına bası yapan pankreatik tümör tespit edildi. Total tümör rezeksiyonu ve koleduedenostomi uygulandı.Histopatolojik değerlendirmeler ile pankreatik duktal adenokarsinom belirlendi. Kemoterapi ve post-operatif tedaviye onay verilmemesisonucu hasta ötenazi yapıldı. Bu olgu sunumuyla; şiddetli kolestazisi bulunan köpeklerde neoplastik kitle ihtimaliningöz ardı edilmemesinin önemi vurgulanmıştır, A-10 year-old, intact female terrier breed dog referred to Veterinary Teaching Hospital with the complaints of anorexiaand constipation. The mastectomy operation had been obtained 3 years ago accorrding to owner’s information. Clinicaland ultrasonographic assessments confirmed the hepatic cholestasis. Medical management with Cefazolin, enrofloxacin,ursodeoxycholic acid, and fluid therapy were initiated. The dog has referred back a month later with the complaints of abruptanorexia and icterus. Routine blood tests and control ultrasonography confirmed severe cholestasis and enlarged gall bladderwithout mass. Experimental laparotomy revealed pancreatic tumor compressing bile ducts. Total tumor resection and choleduedonostomyperformed. Pancreatic ductal adencarcinoma was proved by the histopathologic examination of the samples.According to owner’s unwilling to chemotherapy and post-operative treatment, dog was euthanized. In this case report; neoplasticmass possibility in dogs with severe cholestasis must be taken into consideration.

The aim of this study was to determine whether PACTOIR predict the thrombolytic therapy in the treatment of pulmonary embolism (PE). Materials and methods: This retrospective study was conducted by reviewing charts of 52 patients (aged >= 18 years) who were admitted to our emergency department (ED) with initial diagnosis of pulmonary embolism and confirmed diagnosis of PE by Computed Tomography Pulmonary Angiography (CPTA). The patients were divided into two groups as follows: Group 1 (n=14) consisted of the patients who received thrombolytic therapy and Group 2 (n=38) consisted of the patients who did not receive thrombolytic therapy. PACTOIR was calculated in both groups to estimate thrombus load. We evaluated the effectiveness of thrombolytic therapy by comparing the levels of D-dieter and Troponin I. Results: Mean value for PACTOIR, D-dimer and Troponin I levels were 56.4 (%), 6.21 (mg/L) and 0.35 (mu g-IL) in Group 1 whereas they were 20.46 (%), 220 (mg/L) and 0.11 (mu g/L) in Group 2, respectively. Methods: The prospective case-control study was conducted at Ondokuz Mayis University, Samsun, Turkey, from January 2008 to June 2009, and comprised adult patients with acute pulmonary embolism. A smaller group of healthy adults served as the control. Blood N-terminal pro-brain natriuretic peptide and a pelin levels were measured on admission to the Emergency Department. Bu çalışmanın amacı kene ısırması sonucu acil servise başvuran hastaları epidemiyolojik yönden değerlendirmektir. Kene ısırması şikâyeti ile acil servisimize başvuran erişkin hastaların (yaş≥18) dosya kayıtları geriye dönük olarak incelendi. Çalışmaya dâhil edilen hastaların, demografik özellikleri, klinik bulguları ve tetkik sonuçları değerlendirildi. Bu çalışmaya 263 hasta dâhil edildi. Hastaların ortalama yaşı 46,5±17,2, ve 139’u (%52,9) kadın idi. Hastalar en fazla haziran ayında (%41,4) başvurmuştu. Hastaların %57’si kırsal kesimde yaşıyordu. Kene ısırığı en fazla (%38) gövdede tespit edildi. Hastaların vücudundaki keneler % 63,9 oranında bir doktor tarafından çıkarılmıştı. Hastaların çoğunluğu ayaktan takip edilmişken, 33 tanesi hastaneye yatırılmıştı. Kene ısırması sonucu acil servisebaşvuran hastalar genellikle kırsal kesimde yaşamaktadır ve yaz mevsiminde başvurularartmaktadır. Acil servisler kene ısırması şikâyeti olan hastaların yönetiminde önemli yeresahiptir.

Türkiye’de yaygın olarak bulunan ve halk arasında boru çiçeği, kahkaha çiçeği, sihirbaz otu, şeytan elması olarak da bilinen ‘Datura stramonium’ bitkisi halk tarafından astım, diyare, noktüri, hemoroid, akne ve bölgesel ağrılarda faydalı olduğu düşüncesi ile yaygın olarak kullanılmaktadır. Bitkinin içeriğinde bulunan L-hiyosiyamin, atropin ve skopolamin zehirlenmesinden sorumlu olan maddelerdir. Yazımızda, antikolinerjik bulgularla acil servise başvuran, öykü ve fizik muayene ile Datura stramonium zehirlenmesi tanısı konulan bir olgu sunulmaktadır. Amacımız, antikolinerjik bulgularla acil servise başvuran olgularda Datura stramonium zehirlenmesinin de akılda tutulması gerektiğini vurgulamaktır, Datura stramonium is a widely abundant plant in Turkey. It is regarded as pipe plant, lough plant, sorcerer’s plant, or devil’s apple among the population. People may frequently use it against asthma, diarrhea, nocturia, hemorrhoids, acne, and regional pain symptoms. The plant contains L-hiyosiyamin and this molecule is responsible for the atropine and scopolamine intoxication findings. In this report, we present a case that was admitted to the emergency department with anticholinergic symptoms and diagnosed as Datura stramonium intoxication with history and physical examination findings. Bu olguda; altı yaşlı, Setter ırkı dişi (kısırlaştırılmış) bir köpekte lokalize benign adneksiyal tümör (Trikoblastoma) tanımlandı. Ankara Üniversitesi Veteriner Fakültesi Eğitim Araştırma Hastanesi'ne sahibi tarafından elden getirilen köpeğin klinik muayenesinde sağ üst gluteal bölgesinde, düzensiz yaklaşık 2.51.8 cm çapında, solid yapıda, sert kıvamlı, beyazımsı gri renkli bir kitle saptandı. Olgunun rutin kan tahlilleri ve idrar analizleri normal referans aralıklarında olduğu; mikrobiyolojik inceleme için ülseratif lezyonlu bölgeden alınan numunelerden herhangi bir patojen etken ya da etkenler saptanmadı. Lezyondan alınan punch biyopsi örneğinin histopatolojik incelemelerinde, olgu Trichoblastoma (benign adneksiyal tümör) olarak değerlendirildi. Tümör cerrahi olarak total eksize edildi. Herhangi bir nüks görülmedi ve takip edilen göğüs radyografileri sürecinde, cerrahi sonrası herhangi bir metastaz bulgusunun olmadığı görüldü. Konu ile ilgili yapılan literatür taramalarına ve klasik kaynaklara dayanarak, bu olgu, Ülkemizde, bir köpekte ilk defa rastlanan atipik lokalize Trichoblastoma olarak değerlendirildi., In this case; a 6-year-old, Setter breed female (neutered) localized benign adnexal tumor in a dog (Trikoblastoma) were detected. An irregular lesion on the upper right buttock limited solid construction, hard consistency, whitish gray mass complaints of 2.51.8 cm with a lesion in the dog complaint was referred to the owner of the Ankara University Veterinary Faculty of Research Hospital. Routin blood work and urine analyses were in normal reference ranges in this case. Samples taken from the lesion for microbiological examination revealed no pathogens. At punch biopsy of the lesion, it was defined histopathologically as Trichoblastoma. The tumor was surgically excised. Any recurrence noted after surgery with no evidence of metastases on follow up chest radiographs. The authors performed the literature on the subject as far as he knew, and based on classical sources, this case in our country are considered as atypical localized Trichoblasto encountered the first time in a dog.

BAYDIN, AHMET/0000-0003-4987-0878 WOS: 000362601500007 In this study, we aimed to determine the role of a novel peptide - nesfatin 1-in appetite control and define its anorectic mechanism in order to reveal whether it can be used in fight against obesity. We reviewed the articles in the literature investigating the structure, mechanism of effect and experimental usefulness of nesfatin-1. We entered the word "nesfatin" to medical databases and reviewed the literature. Administration of nesfatin-1 maintains a significant reduce in food intake. Its effectiveness in rats is proven. More and more, new researches are in progress to determine its exact mechanism of effect. Nesfatin-1 is a satiety agent. However, use of nesfatin-1 as an anti-obesity agent in humans remains an unclarified field and needs further investigations.

Objective: The aim of this study was to assess the demographic and clinical features, diagnostic methods and outcomes of patients diagnosed as acute pancreatitis in the emergency department (ED). Materials and Methods: This study was designed retrospectively by examining medical records of the patients (age≥18 years) who were diagnosed as acute pencreatitis in the ED, between January 2007 and January 2008. Results: A total of 61 patients (31 male [50.8%], mean age 60,6±15,4) diagnosed as acute pancreatitis were enrolled the study. Nausea and abdominal pain were common complaints on admission. Clinical severity was mild in 32 patients (61.5%) and was severe in 20 patients (38.5%). CT severity index (CTSI) was determined in these patients. The sensitivity, specificity, positive predictive value, negative predictive value were calculated for the CTSI>3 (90%, 97%, 95%, 94%) for identifying the severe pancreatitis. Fifty patients (%82) were hospitalized. There was a correlation between duration of hospitalization and CTSI in these patients (p=0.001, r=0.497). Conclusion: Early identification of clinical severity is crucial for planning of therapeutic strategies of acute pancreatits in the ED. Abdomen CT is an effective method in the diagnosis, assessment of clinical severity and complications of acute pancreatitis., Amaç: Bu çalışmanın amacı acil serviste (AS) akut pankreatit tanısı alan olguların demografik ve klinik özelliklerini, tanısal yaklaşımlarını ve klinik sonuçlarını tartışmaktır. Gereç ve Yöntemler: Ocak 2007 ve Ocak 2008 tarihleri arasında hastanemiz AS'inde akut pankreatit tanısı alan hastaların (yaş≥18) kayıtları geriye dönük olarak incelendi. Bulgular: Çalışmaya akut pankreatit tanısı alan 61 hasta (31 erkek [%50,8], ortalama yaş 60,6±15,4) dahil edildi. Başvuru anında karın ağrısı ve bulantı sık görülen şikayetler idi. Hastaların 32'sinde (%61,5) klinik hafif iken, 20'sinde (%38,5) şiddetli idi. Bu hastalarda BT şiddet skoru (BTŞS) değerlendirildi. BTŞS >3 değerinin şiddetli pankreatiti tanımadaki duyarlılık, özgüllük, pozitif öngörü değeri, negatif öngörü değeri sırasıyla (%90, %97, %95, %94) hesaplandı. Hastalardan 50'si (%82) hastaneye yatırılmıştı. Yatan hastaların BTŞS'si ile yatış süreleri arasında korelasyon mevcuttu (p=0.001, r=0.497). Sonuç: Acil serviste akut pankreatit tanısı alan hastalarda klinik şiddetin erken dönemde tanımlanması uygun tedavi stratejisinin belirlenmesine katkı sağlar. Batın BT akut pankreatit tanısında, klinik şiddetin ve komplikasyonların belirlenmesinde etkili bir yöntemdir.

Different organ dysfunctions may be seen in mushroom poisoning. However, acute pancreatitis is a rare complication of mushroom poisoning. Sixtysix years old women admitted to emergency department 24 h after wild mushroom ingestion with complaints of nausea, vomiting and abdominal pain. The epigastric discomfort was determined in initial examination. It was determined the elevated serum hepatic transaminases, amylase and lipase levels on admission. Computerized tomography of the abdomen showed minimal peripancreatic fluid. The patient was diagnosed as acute pancreatitis and mushroom poisoning and he was observed in emergency intensive care unit. Hepatic transaminases and pancreatic enzymes levels were progressively decreased and the clinical course was good during the observation period. The patient recovered without complication and was discharged from the hospital. Early symptoms of mushroom poisoning and acute pancreatitis are similar. Possibility of acute pancreatitis as well as other organ dysfunctions should be investigated in patients with mushroom poisoning. Early recognition and appropriate therapy for acute pancreatitis and mushroom poisoning may lead to an improved prognosis and complications., Mantar zehirlenmelerinde farklı organ yetersizlikleri görülebilir. Ancak akut pankreatit mantar zehirlenmesinin nadir bir komplikasyonudur. Altmış altı yaşında kadın hasta yabani mantarı yemesinden 24 saat sonra bulantı, kusma ve karın ağrısı yakınmaları ile acil servisimize başvurdu. Hastanın fizik muayenesinde epigastrik bölgede hassasiyet vardı. Başvuru anında serum hepatik transaminazlar, amilaz ve lipaz düzeylerinde yükseklik saptandı. Çekilen bilgisayarlı batın tomografisinde peripankreatik mayi görüldü. Hasta mantar zehirlenmesine ve akut pankreatit tanılarıyla acil yoğun bakım ünitesine yatırıldı. Takip süresince klinik durumu iyi olan hastanın hepatik transaminazları ve pankreatik enzim düzeyleri progresif olarak azaldı. Komplikasyon gelişmeyen hasta şifa ile taburcu edildi. Mantar zehirlenmesinin ve akut pankreatitin erken semptomları benzerdir. Mantar zehirlenmeli hastalarda diğer organ yetersizliklerinin yanı sıra akut pankreatit olasılığı düşünülmelidir. Akut pankreatit ve mantar zehirlenmesinin erken tanısı ve uygun tedavisi iyi prognozun sağlanması ve komplikasyonların önlenmesine katkı sağlayabilir.

Acute aortic dissection (AAD) is an uncommon clinical picture with a high mortality rate. Early diagnosis and treatment are important in decreasing the mortality. Clasically, AAD is characterized by a severe sudden onset pain of the chest, back, and abdomen. However, the patients with AAD may admit with a variety of different clinical findings as due to level of dissection in the aorta. In this study, our aim is to emphasize that the ADD rarely will cause gastrointestinal symptoms. Erken teşhis ve tedavi mortalitenin azaltılmasında önemlidir. Klasik olarak AAD ani ve şiddetli göğüs ağrısı, sırt ağrısı veya karın ağrısı ile karakterizedir. Ancak AAD'lu hastalar diseksiyonun bulunduğu bölgeye bağlı olarak çok farklı klinik bulgularla başvurabilirler. Bu çalışmadaki amacımız AAD'nun nadiren de olsa gastrointestinal semptomlara neden olabileceğini vurgulamaktır. Objectives: Diseases of cardiovascular system (DCVS) are the first cause of death of women and men among all the ethnical groups on the many countries of the world. Evaluation of the distribution and prevalence of the DCVS and several conditions of the cases with DCVS, who have applied to 112 emergency health services (EHS) in Samsun, have been aimed. Materials and Methods: The datas of this study were obtained from reports of 2004 annual studies of EHS of city health administration of Samsun. The patients which known as diseases of DCVS, were included to the study. This is a retrospective and a descriptive study. Results: Reports had been obtained from 9015 cases along a year. Of the cases, 19% had DCVS. Of the cases, 57% were women and 43% were men. The mean age of women and men were 61±13.6 and 61.9±14.2 years, respectively. When the cases were evaluated according to prediagnosis of them, it was detected hypertension (HT) as the most frequent kind of DCVS. 34.9 % patients had just HT. 15.5% patients had another disease together HT. The mean arrival time to the patients from station was 4.6±2.4 minutes. Conclusion: In the lights of evaluated datas (when studies of EHS were evaluated), it was detected that DCVS had high frequency. When the importance of early medical intervention is considered for DCVS and related health problems with high risk of mortality and disability, it is clearly seen priority and worthy of mobile emergency health services.©2008, Firat University, Medical Faculty., Amaç: Dünya'da birçok ülkede kardiyovasküler sistem hastalıkları (KVSH) tüm etnik gruplardaki erkek ve kadınlarda ölüm nedeni olarak ilk sırada yer almaktadır. Samsun ili 112 Acil Sağlık Hizmetleri'ne (ASH) yapılan başvurular içerisindeki KVSH'nın dağılımı, sıklığı ve olgulara ait bazı özelliklerin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Retrospektif ve tanımlayıcı tipteki bu çalışmanın verileri Samsun İli Sağlık Müdürlüğü 112 ASH Şube Müdürlüğü 2004 yılı kayıtlarından elde edildi. KVSH olarak tanımlanan olgular çalışma kapsamına alındı. Bulgular: Toplam 9015 olgunun %19'unu KVSH oluşturmaktaydı. Bu olguların %57'sini kadınlar ve %43'ünü erkekler oluşturmakta iken kadınların yaş ortalaması 61.0±13.6 yıl, erkeklerin yaş ortalaması 61.9±14.2 yıl idi. Olguların ön tanılarına göre dağılımına bakıldığında; en sık görülen KVSH'nın hipertansiyon (HT) olduğu saptandı. Sadece HT'si olan kişi sayısı %34.9, HT'nin yanı sıra diğer bir hastalığı olan kişi sayısı %15.5 idi. Çalışma kapsamına alınan olgulara istasyondan ulaşma zamanı 4.6 ± 2.4 dakika olarak saptandı. Sonuç: 112 ASH çalışmaları değerlendirildiğinde, KVSH'nın yüksek sıklığa sahip olduğu saptandı. Mortalite ve sekel riski yüksek olan KVSH ve benzeri sağlık sorunlarına erken tıbbi müdahalenin önemi göz önüne alındığında, mobil acil sağlık hizmetlerinin ne denli önemli ve öncelikli olduğu açıkça görülmektedir. ©2008, Fırat Üniversitesi, Tıp Fakültesi

High dose amitriptyline poisoning is very important condition among emergency department visits. Particularly more than one gram of amitriptilin exposure may cause mortality. Most of the deaths occur in several hours after taking amitripitilin. Twenty three year old woman patient applied our emergency department with a complaint of unconsciousness after three grams amitriptyline exposure. At the inital approach of the patient, it was seen that cardiopulmoner collaps and myoclonic seizure had been developed. At her blood gas analysis, there was profound metabolic asidosis. Endotracheal entubation and antiepileptic drugs were applied to the patient. Also bicarbonate was given for her profound metabolic asidosis. Because of the patient's acidosis did not improve with conservative treatment, hemoperfusion was applied to her. After two days follow up at intensive care unit, the patient recovered completely. As a conclusion, high dose amytriptilline poisoning is a serious case for emergency departments and can be fatal but appropriate evaluation and management of these cases at the emergency department can make patients recover rapidly., Yüksek doz amitriptilin alımına bağlı zehirlenme acil servise başvuru nedenleri arasında önemini korumaktadır. Özellikle 1 gramdan daha fazla alındığında, amitriptilin zehirlenmesi hayatı tehdit eden bir durum olmaktadır. Ölümlerin birçoğu alımdan sonraki ilk birkaç saat içinde meydana gelir. Yirmi üç yaşındaki bayan hasta 3 gram amitriptilin aldıktan sonra bilinç kaybı ile acil servisimize getirildi. Olguda kardiyopulmoner kollaps gelişmişti. Miyoklonik tarzda nöbet geçirmekteydi. Kan gazında derin asidozu mevcuttu. Endotrakeal entübasyon ve antiepileptik ilaçlar uygulandı. Asidozu nedeniyle bikarbonat verildi. Asidozunun düzelmemesi üzerine hemoperfüzyon uygulandı. Yoğun bakıma yatırılan hasta 2. gün tamamen düzeldi. Sonuç olarak amitriptilin yüksek doz alımları hayatı tehdit edici zehirlenmelere neden olabilmekle birlikte acil serviste uygulanacak doğru yaklaşımla olgular tamamen iyileşebilmektedir.
