Analizzare i sogni in psicoterapia è ancora una prassi attuale e utile? Per rispondere a una domanda così semplice, ma anche così complessa, devo fare riferimento alla mia esperienza come psicoterapeuta più che a quella di ricercatore (anche se una ha sempre influenzato l’altra). Assumendo questa prospettiva, ritengo utili alcune considerazioni sul significato dei sogni e sulla loro funzione all’interno della relazione terapeutica.

L’adolescente vive una serie di cambiamenti che lo impegnano sia sul piano corporeo (sviluppo puberale, modificazioni dell’aspetto fisico) che sul piano psicologico (aumento dell’importanza del mondo interno, passaggio da un pensiero concreto a uno ipotetico-deduttivo). Molte mutazioni si verificano anche nei confronti della famiglia (caduta dell’idealizzazione dei genitori, rifiuto dei rapporti di dipendenza, ricerca di maggiore autonomia) e in ambito sociale (dove si sperimentano nuovi ruoli nella scuola, nel gruppo degli amici e nei confronti dell’altro sesso).

The Perinatal Assessment of Paternal Affectivity (PAPA) (Baldoni et al., 2016, 2018) is a new self-report questionnaire for the screening of affective symptoms in fathers. It has been developed on recent research on perinatal affective disorders and assesses different dimensions of paternal affective problems: anxiety, depression, irritability/anger, relational and couple difficulties, somatic complaints, dangerous behaviors and addictions (smoking, alcohol, drugs, gambling, internet abuse, physical or sexual compulsive and risky behavior). It also considers some ethnic and socio-cultural influences.

I risultati degli studi evidence-based hanno confermato che diversi modelli di psicoterapia producono risultati ampiamente positivi, ma nessuna tecnica psicoterapeutica ha dimostrato una particolare superiorità rispetto alle altre. Un fattore significativamente correlato alla soddisfazione del paziente e al risultato finale di una psicoterapia sembra la qualità dell’alleanza terapeutica o Working Alliance e il paradigma dell’attaccamento è stato utilizzato come chiave di interpretazione e misura di questa dimensione. Nonostante i limiti metodologici e la variabilità delle dimensioni indagate, le ricerche hanno evidenziato che la sicurezza dell’attaccamento del terapeuta e del paziente influenzano positivamente la qualità della relazione, il processo terapeutico e l’esito della cura, mentre l’insicurezza, particolarmente di tipo preoccupato, è associata a una minore qualità e un’instabilità dell’alleanza e a una minore efficacia terapeutica. The results of the evidence-based research have confirmed that various models of psychotherapy produce very positive results, but no psychotherapeutic technique has shown a significant superiority compared to the others. A factor significantly related to patient satisfaction and to the final result of psychotherapy seems to be the quality of the therapeutic alliance or Working Alliance, and the attachment paradigm has been used as a key for interpretation and assessment of this dimension. Despite methodological limitations, and the variability of the investigated dimensions, research has shown that attachment security of the patient and the therapist positively influence the quality of the relationship, the therapeutic process and the outcome of the treatment, while insecurity in attachment, particularly preoccupied type, is associated with a lower quality and instability of the alliance and a decreased therapeutic efficacy.

Il paradigma dell’attaccamento offre una chiave di lettura molto utile per lo studio del comportamento di malattia e della vulnerabilità nei confronti delle patologie psichiche e fisiche. Questa problematica, comunque, non va intesa in modo lineare. Un pattern insicuro non può essere considerato di per sé causa di malattia, in quanto la sua funzione è quella di proteggere l’individuo adattandolo al meglio alle condizioni di pericolo. Piuttosto, seguendo una prospettiva biopsicosociale, l’attaccamento deve essere inteso come una sistema che interagisce con altri sistemi comportamentali (in particolare quello di accudimento e quello riproduttivo) e con livelli diversi di sistemi (genetici, biologici, psicologici, culturali, sociali, ambientali). L’interazione di questi sistemi e sottosistemi determina nel suo complesso le condizioni in cui un essere umano si considera e viene riconosciuto dalla società come ammalato, influenzando il modo di manifestare e valutare la sofferenza, lo sviluppo delle malattie e la possibilità di essere curato in modo adeguato.

The mentalizing processes allow a psychological representation and control of emotions (including related somatic states) and are fundamental for psycho-physiological regulation and for stress management. In all the life difficulties which may interfere with individual development assuming a traumatic value (illness, losses, family conflicts, splitting and divorces, neglect, abuses, dangerous physical experiences, social exclusion, serious economic problems) reflective functions tend to be impaired. In these cases, the representation of the individual self tends to be only somatic and the physiological reaction to stress is out of control. Research has shown that in these individuals somatic symptoms and abnormal illness behavior (hypochondriacal concerns, somatization disorders, functional medical syndromes) can appear, as well as an increased susceptibility to physical or mental illnesses. From the psychological point of view, in fact, these conditions are often characterized by a lack of impulse control (violent or aggressive behavior, panic attacks, fugues, antisocial behavior) or dissociative disorders (amnesia, fugues, depersonalization), expressions of an attempt to protect against traumatic consequences of mental tension. Moreover, subjects with reflexive difficulties tend to manifest compulsive and increasingly extreme behavior (smoking, drinking, taking drugs, eating too much or too little, driving too fast, extreme sports or sexual activities, gambling, excessive use of chat, video games and internet) in an attempt to regulate and control the disturbing emotions in a non-psychological way. The tendency to use these "external regulators of emotions" involves a greater vulnerability to psychic and somatic effects of traumatic experiences and is a feature of many medical illnesses and psychiatric disorders, particularly borderline personality disorders, eating disorder and addictions.

Procedures to assess attachment in school-age are mainly based on verbal responses and are carried out in conditions of relative security, implying some reliability and validity problems. In particular, the assessment in these children presents particular difficulties related to the specific characteristics of this stage of development—such as the different use of verbal and non-verbal communication and the particular display of facial expressions and behaviors related to their inner emotional states. The School-age Assessment of Attachment (SAA) (Crittenden, 1997-2005) is a semi-structured clinical interview that assesses self-protective strategies in children from six to thirteen years following the Dynamic-Maturational Model of attachment and adaptation (DMM). The procedure consists of a set of cards regarding increasing levels of threats that school-age children frequently face. The interview is usually audio recorded, and the classification of the transcript is based on the observation of the child’s behavior and a discourse analysis derived from the Adult Attachment Interview. Adding the video to the transcript improves the classification process by benefiting from para-verbal and non-verbal information— such as the display of false-positive affects in dismissing (Type A) children or the emphasis of negative feelings in preoccupied/coercive (Type C) children, and somatic response. An example of a videotaped SAA of a clinical case will be displayed using Doctor Notes® (Telpress International, 2015), an application originally developed for audiovisual analysis and classification of information that can provide specific tags and a set of buttons for SAA coding. SAA represents a bridge between infant observational procedures and verbal interviews with adults, evidencing the utility of integrating a careful behavioral analysis with the coding process. Using Dr. Notes, the process of transcription based on the video analysis allows a more accurate detection of para-verbal and non-verbal behavior offering researchers and clinicians more information for the attachment classification.

Research on perinatal psychopathology has extensively investigated maternal depression. In recent years, evidence is increasing about the role played by maternal anxiety, in terms of negative consequences on obstetrical and birth outcomes, maternal postnatal adjustment and infant development (Dunkel Schetter, Tanner, 2012). Concerning paternal perinatal psychopathology, only recent literature is showing a growing interest on the study of depression, while anxiety and illness behavior still needs to be further investigated. A prevalence of 20% for anxiety symptoms has been reported in men during their partner's pregnancy (Boyce et al., 2007), with a significant association with woman’s symptoms (Matthey et al., 2003). Besides, paternal prenatal anxiety has been suggested to represent a risk factor for men’s postnatal depression (Edwards et al., 2015). The aim of this talk is to deepen the understanding of perinatal anxiety and somatization in fathers. A brief summary of main findings from international literature on this topic will be presented. Also, data will be shown from two studies, conducted by the authors, on perinatal anxiety in fathers. Both studies focused on: 1) the assessment of perinatal anxiety in fathers from pregnancy to 3 months after birth; 2) the investigation of the relationship between paternal and maternal mood. Differences between the two studies were represented by the investigation on differents dimensions (psychological and physical) of anxiety, and the use of different instruments. The first study focused on stait and trait anxiety and the relationship with pregnancy specific anxieties in mothers. The second study explored general anxiety, somatization, and their relationship with maternal depressive symptoms. In conclusion, few studies have, until now, explicitly assessed paternal perinatal anxiety and illness behavior. This lack of data evidences the need to increase research in this area, as well as the to develop more specific instruments to assess perinatal affective disorders in fathers. During the perinatal period affective alterations in fathers, like Paternal Perinatal Depression, are very frequent, but occur differently than in women (Tuszyńska-Bogucka, & Nawra, 2014; Edward et al. 2015). In particular, depressive symptoms in fathers tend to be less severe, less definite, and often occur in comorbidity with anxiety disorders, alteration of illness behavior and behavioral acting outs like addictions or anger attacks. As a consequence of their indefinite clinical features, these problems are often under-assessed or undiagnosed (Musser et al. 2013). The Perinatal Assessment of Paternal Affectivity (PAPA) is a new questionnaire for the screening of high-risk fathers during the perinatal period. It is based on recent research on perinatal affective disorders and is the first instrument to assess these problems in fathers through many different dimensions: Anxiety, Depression, Hostility, Perceived stress, Relational difficulties, Somatic complaints, Dangerous behavior (driving, sport, at work), Addictions (smoking, alcohol, drugs, internet, gambling) and physiological alteration (sleep, eating). It is fast and simple to administer (one page, five minutes), and ethnic and socio-cultural influences are considered. In this pilot study a sample of 50 Italian fathers assessed during a gynecological consultation at the 8th month of pregnancy of their companion were considered. Besides the PAPA, all subjects were given the following questionnaires: CES-D, EPDS, SCL-90-R, Perceived Stress Scale (PSS) and Dyadic Adjustment Scale (DAS). The preliminary data analysis shows a significant association (P = 0.1) between the scales of Depression, Anxiety, Somatic complaints and Stress perception of the PAPA with the relative scales of CES-D, EPDS, SCL-90-R and PSS. The PAPA is a screening tool, and its goal is not an accurate diagnosis, but it provides a simple and practical guide for detecting fathers at high-risk of perinatal affective alteration, in order to foster a more in-depth diagnostic assessment, and a possible treatment. One crucial phenomenological aspect in patients with eating disorders is a distressing feeling of emptiness. A void that cannot be filled by any object, that corresponds to the absence of a mental function, the inability to think, to symbolically represent, to attribute meanings, to recognize and communicate fantasies and emotional states. This lack of mentalization is the consequence of the failure of parental reflective capacities and of dysfunctions of attachment and family relationships. A non-psychological way to adjust the mental states is to use external regulators of the emotions, that is, objects, experiences or behaviors that stimulate the body by changing the mental states and somatic functions that accompany them. Children and adults commonly use similar behaviors to calm down, get excited, overcome boredom or distraction, without activating mentalizing psychological processes. People who have difficulty in mentalizing, however, tend to implement, compulsively, intensive and continuous increasingly extreme behavior in an attempt to regulate and control emotions. The behaviors that more frequently assume this significance are smoking, drinking alcohol, taking drugs, overeating (as in bulimic crises or severe obesity) or eating too little (as in anorexia), or intensive, extreme and potentially dangerous physical or sexual activities. These behaviors enable control of disturbing emotional tensions that cannot properly be adjusted by thinking (anxiety, fear, depression, anger, sexual arousal, feelings of helplessness, of emptiness, boredom), as they solicit bodily reactions and opposite emotional tensions (excitement, fear, euphoria, physical pain, hunger, satiety, blunting). The compulsive tendency to use external regulators of emotion, however, represents an important risk factor that affects physical and mental health and is a feature of many psychopathologies, particularly borderline personality disorders, lack of impulse control, addiction and eating disorders.

Psychological trauma is configured as a set of experiences of which it is difficult to make sense. Exposure to hazardous conditions, however, does not necessarily imply a traumatic evolution. Psychological trauma is manifested when the situation cannot be addressed in such a way as to give a meaning to the event and maintain the danger under control. This may occur during a specific, sudden and intense episode, or, cumulatively, as a result of daily and constant experiences. The trauma affects the sense of self, as well as the sense that each of us gives to our existence. This semantic assignment process engages human beings for life and is different in childhood, adolescence and adulthood. The trauma can therefore be considered as a meeting between a demanding event, sometimes painful, and a mind, that has different possibilities to process it. It constitutes one of the most potent activators of emotions, and the body reacts as a sounding board. Research has amply demonstrated the physical consequences of traumatic experiences: abnormal illness behaviors (hypochondria, somatization disorders, functional syndromes), cardiovascular, metabolic or endocrine diseases, cancer and early death. In the past, in a psychoanalytic perspective, the bodily responses to trauma have been studied primarily as a symbolic expression of emotions and fantasies excluded from the mind, as in the case of hysterical conversion symptoms. Today, studies on attachment and mentalization, polyvagal theory and recent neuroscientific acquisitions have profoundly changed the understanding of the mechanisms that govern the emotion-body connection. We know that the chaotic emotional states resulting from trauma involve a dysregulation of emotions and arousal, with a somatoform dissociation which makes the individual more vulnerable to the effects of any subsequent dis-integrative trauma. The symposium will address these issues through theoretical reflections and presenting research data.

La gravidanza e la nascita di un figlio rappresentano per entrambi i genitori momenti di vulnerabilità psicologica nei quali la frequenza dei disturbi affettivi aumenta di quasi tre volte rispetto alla media della popolazione generale. In questo periodo anche i padri, come le proprie compagne, spesso manifestano problemi emotivi che possono influenzare negativamente la vita di coppia, la gravidanza, la salute psicologica della madre, la relazione tra questa e il bambino e lo sviluppo psicofisico del figlio. I disturbi affettivi paterni, però, tendono a manifestarsi in modo differente rispetto alla donna e, per quanto siano frequenti, sono spesso sottovalutati o non diagnosticati, oltre che trascurati dalla ricerca. Nonostante la letteratura scientifica sulla psicopatologia perinatale materna sia molto ampia (decine di migliaia di pubblicazioni negli ultimi 15 anni), infatti, gli studi sui disturbi psicologici legati alla paternità sono ancora oggi molto più rari. Riconoscere la manifestazione di disturbi depressivi nel padre durante il periodo perinatale è molto importante in quanto questi disturbi hanno conseguenze negative non solo per il paziente, ma per il benessere della madre e lo sviluppo del bambino. Però non è ancora chiaro se questa alterazione psicopatologica debba essere riconosciuta come un’entità clinica a sé stante in termini di sintomatologia fattori di rischio e comorbilità rispetto alla depressione maggiore (in effetti anche nei confronti della Depressione Perinatale Materna sono stati sollevati gli stessi dubbi). In attesa che il dibattito all’interno della comunità scientifica si chiarisca, la diagnosi di Depressione Perinatale Paterna può essere utilizzata per indicare nel loro complesso le alterazioni affettive del padre durante il periodo perinatale, piuttosto che riferirsi a una sindrome specifica.

Considerata alla luce degli studi etologici e di quelli sull’attaccamento, l’aggressività umana perde la propria connotazione negativa assumendo un significato utile per l’evoluzione e la sopravvivenza della specie. Dietro a un comportamento ostile, violento, apparentemente irrazionale o distruttivo, infatti, si possono individuare strategie difensive relative alla protezione dai pericoli, al proprio ruolo sociale e alle funzioni svolte nei confronti del partner e della prole. Tale visione concorda sostanzialmente con quella della psicoanalisi classica che vede l’aggressività come una pulsione innata, ma mentre per Freud l’aggressività è al servizio della pulsione di morte e deve essere repressa, per Lorenz è al servizio della vita. Seguendo questa prospettiva le ragioni di un comportamento aggressivo possono essere ricondotte a quattro ambiti fondamentali: 1) un’esperienza infantile di deprivazione materna e di mancanza di cure genitoriali; 2) un comportamento di protesta teso a evitare la separazione e ottenere protezione da parte della figura di attaccamento; 3) una carente capacità di mentalizzazione e un atteggiamento di difesa conseguenti a traumi, abusi, maltrattamenti o scarsa sensibilità genitoriale; 4) lo sviluppo di un attaccamento insicuro distanziante o preoccupato. Lo studio delle funzioni adattive svolte dai comportamenti aggressivi apre nuove prospettive nella psicoterapia non solo dei pazienti antisociali e violenti, ma di tutti quelli che manifestano problemi di controllo degli impulsi, di strutturazione del sé e di regolazione emozionale (attacchi di panico, disturbi di personalità), nei disturbi del comportamento di malattia (somatizzazioni, disturbi funzionali, ipocondria) e nei disturbi dissociativi e post traumatici.

During the perinatal period affective alterations in fathers, like Paternal Perinatal Depression, are very frequent (ranging from 2% to 31%, with a mean of 10.4 %), but occur differently than in women and they are often under-assessed or undiagnosed. Depressive symptoms in fathers tend to be less severe, less definite, and often occur in comorbidity with anxiety disorders, alteration of illness behavior and behavioral acting outs like anger attacks. Moreover, in the perinatal period the mother’s and father’s emotional states are linked and empirical research has found a significant correlation between maternal and paternal depression. Some research data will be presented: 1. Fathers whose partner have suffered from affective disorders during post-partum (maternity blues or post-partum depression) show anxiety (p= .02), depressive symptoms (p= .01) and worry about their own health and paternal role up to the fifth month of pregnancy (Baldoni et al. 2014); 2. If the male is anxious, depressed or hostile during the In Vitro Fertilization and Embryo Transfer Procedure (IVF-ET), women tend to manifest more severe affective disorders, anxiety and somatization independently of the success of the procedure (Baldoni et al. not published); 3. In researches using the CARE-Index (a video recorded procedure for the study of the parent-child relationship) depression, low sensitivity and insecure attachment forerunners in fathers negatively influence the psycho-motor development of the child (p= .01) (Baldoni et al. 2012). These results suggest that anxious or depressed fathers, or those with behavioral problems, may hamper the emotional equilibrium of their companion and the development of a good mother-child relationship. In these cases, a lack of a paternal protective function as “secure base” can foster an affective disorder in the mother and negatively influence the attachment relationships and the psychosomatic development of the child.

Le emozioni sono stati tensionali selezionati nel corso dell’evoluzione che favoriscono l’adattamento e la sopravvivenza svolgendo una funzione nella motivazione del comportamento animale. Hanno una base biologica e sono mediate dal sistema nervoso centrale, in particolare dal sistema limbico e dalle strutture sottocorticali. Negli esseri umani, però, le emozioni possono assumere il significato di manifestazioni psicologiche più complesse, denominate sentimenti o affetti, che implicano un’elaborazione cognitiva e un vissuto soggettivo conseguenti all’attività della neocorteccia e del sistema limbico (in particolare delle aree prefrontali e dell’amigdala). I sentimenti permettono la consapevolezza di uno stato emotivo, consentono di regolarlo e di manifestarlo intenzionalmente in modo verbale o non verbale. Ad esempio in una crisi di rabbia l’individuo può rendersi conto della propria emozione, definirla con un nome, metterla in relazione con esperienze passate, ipotizzare diverse modalità di manifestarla e valutarla dal punto di vista etico (ad esempio avvertendo un senso di colpa). Le emozioni sono alla base dei sentimenti, ma possono assumere un significato psicologico solo attraverso un processo di elaborazione che è stato denominato mentalizzazione. Il termine mentalizzazione si riferisce alla capacità di percepire se stessi e gli altri in termini di stati mentali, interpretando il comportamento come conseguenza di questi. Si tratta quindi di attribuire alle azioni un significato psicologico. Assumere un atteggiamento mentalizzante può costituire un notevole vantaggio in quanto permette di comprendere meglio le proprie reazioni emotive e di andare oltre le apparenze nel valutare il comportamento degli altri, interpretando con maggiore successo le loro intenzioni. I processi di mentalizzazione, inoltre, permettono una rappresentazione psicologica e un controllo delle emozioni (compresi gli stati somatici correlati) e risultano fondamentali per la regolazione psicofisiologica e nella gestione dello stress.

Le funzioni femminili nel ciclo vitale della famiglia, attraverso il ruolo di moglie e di madre, sono state molto studiate dalla ricerca psicologica, mentre quelle maschili sono state per lungo tempo trascurate. Il ruolo del padre, ovviamente, è stata molto valorizzata da Freud, che ha riconosciuto la sua importanza soprattutto riguardo ai processi connessi al complesso di Edipo, allo sviluppo dell’identità sessuale e di un modello di vita di coppia, all’interiorizzazione di un codice etico e morale e allo sviluppo del Super-Io. Però si sa ancora poco riguardo alle funzioni svolte dal padre al di fuori del periodo edipico. Per lungo tempo, inoltre, la maggior parte degli studi di psicologia dell’età evolutiva ha considerato il bambino nell’interazione con un solo genitore, studiando la relazione madre-bambino (e molto più raramente quella con il padre) in una prospettiva rigidamente diadica. I più recenti dati di ricerca, invece, suggeriscono che il padre è estremamente importante sin dai primi mesi di vita, ma il suo ruolo deve essere studiato non solo all’interno della relazione diretta con il figlio, ma anche in una prospettiva triadica che comprenda pure la madre. In questo modo possiamo riconoscere le funzioni fondamentali del padre in almeno tre diversi momenti del ciclo vitale della famiglia: la prima infanzia, il periodo edipico e l’adolescenza.Recentemente l’interesse scientifico per questo argomento è aumentato. Negli ultimi dieci anni, infatti, sono stati pubblicati decine di studi empirici sui disturbi depressivi perinatali paterni volti a valutarne la sintomatologia, l’epidemiologia, il legame con lo stato emotivo della madre e l’influenza sullo sviluppo del figlio . In particolare, le ricerche hanno evidenziato come nel periodo perinatale gli stati mentali di madri e padri siano significativamente correlati e che anche i padri possono soffrire di disturbi affettivi perinatali. Mancano però studi controllati sulla prevenzione e sul trattamento dei disturbi affettivi perinatali paterni e non è stata ancora indagata l’efficacia del counselling, delle psicoterapie e dei trattamenti farmacologici antidepressivi.

Una funzione maschile importante, per lungo tempo sottovalutata e solo recentemente oggetto di studio, è quella di proteggere la propria compagna offrendole una base sicura nei periodi in cui è maggiormente esposta a condizioni di potenziale pericolo fisico o psicologico. Questo compito si rivela particolarmente importante durante il periodo perinatale. La ricerca ha evidenziato che in questo periodo gli stati mentali dei genitori sono significativamente correlati e come anche il padre possa soffrire di disturbi affettivi simili alla depressione post-partum. I padri troppo ansiosi, depressi, assenti o quelli che manifestano alterazioni del comportamento di malattia (disturbi di somatizzazione, sindromi funzionali, ipocondria) o problemi comportamentali (aggressività, alcolismo, dipendenze patologiche), possono rappresentare una minaccia per l'equilibrio emotivo della compagna e per lo sviluppo di una buona relazione di attaccamento tra madre e figlio.
